RESUMEN
Objetivo: Evaluar la relación entre el índice de masa corporal (IMC) con la presión arterial en embarazadas durante su segundo trimestre de gestación en conjunción con variables socioeconómicas y demográficas. Métodos: Estudio transversal y descriptivo- correlacional en 168 embarazadas previo consentimiento informado; se aplicó encuesta Graffar Méndez-Castellano (Graffar M-C) y se recopilaron datos del embarazo. La muestra se estratificó empleando la gráfica de Atalah que evalúa el estado nutricional de gestantes y se midió presión arterial según protocolo. Resultados: Sobrepeso y obesidad representó 47,1 % de las gestantes; en aquellas sin pareja 60 % y en las que tenían pareja 45,2 %. En las pacientes con fetos femeninos el sobrepeso y obesidad fue 60,1 % y con fetos masculinos 35,5 %. La presión arterial sistólica (PS) y diastólica (PD) resultaron significativamente (P < 0,001) más elevadas en las gestantes ≥ 35 años. La PD fue más elevada en las gestantes sin pareja (P < 0,001). La PS fue mayor en el estrato II de Graffar M-C (P < 0,001) y la PD en el IV (P< 0,003). En gestantes con fetos femeninos, la PS fue mayor que las que tenían fetos masculinos (P < 0,02). Las pacientes con sobrepeso presentaron la mayor cifra tanto de PD (P < 0,0001) como de PS (P < 0,0001). Se encontró una correlación significativa y positiva entre los valores de PS con la edad materna e IMC. Conclusiones: El sobrepeso-obesidad, así como las gestantes sin pareja, con fetos femeninos y ≥ 35 años son factores a considerar en las gestantes con riesgo de hipertensión arterial.
ABSTRACT
Objectives: To assess the relationship between the body mass index (BMI) with the blood pressure in pregnant women during their second trimester of gestation in conjunction with socioeconomic and demographic variables. Methods: Transversal and descriptive-correlational study in 168 pregnant prior informed consent; applied survey Graffar Méndez-Castellano (Graffar M-C) and we collected data from the pregnancy. The sample was stratified using the graph of Atalah that evaluates the nutritional status of pregnant women and measured blood pressure according to Protocol. Results: Overweight and obesity represented 47.1 per cent of pregnant; in those without a partner 60 % and they did couple 45.2 %. Overweight and obesity was 60.1 % in patients with female fetuses with 35.5.% male fetuses. (PS) and diastolic (PD) systolic blood pressure were significantly (P < 0.001) higher in pregnant ≥ 35 years. The PD was higher in pregnant without a partner (P < 0.001). The PS was higher in the stratum II of Graffar M-C (P < 0.001) and the PD IV (P < 0.003). In pregnant women with female fetuses, the PS was greater than they did male fetuses (P < 0.02). Overweight patients presented the largest figure both PD (P < 0.0001) and PS (P < 0.0001). We found a significant and positive correlation between the values of PS with the maternal age and BMI. Conclusions: The overweight-obesity, as well as pregnant without a partner, with female foetuses and ≥ 35 years are factors to be considered in the pregnant women at risk of high blood pressure.
