Purpose The purpose of this study is to investigate musculoskeletal pain among eye care professionals. Methods This cross-sectional study was conducted using an electronic detailed questionnaire through Google Forms. The structured questionnaire was distributed through various social media platforms targeting eye care professionals. The study included currently practicing ophthalmologists (Consultants, Specialists, Residents), optometrists, and orthoptists. Results A total of 514 eye care professionals participated in the study. The majority were younger than 30 years old (43.8%), with more than half being males (51.8%) and ophthalmologists (55.2%). The prevalence of eye care professionals who were suffering from musculoskeletal pain was 66.7%. The prevalence was significantly higher among females (76.2%) and those over 50 years old (71.4%). Sixty-eight point three percent (68.3%) of participants who don't do running exercises and 92.2% of those with comorbidities suffer from pain. The prevalence of pain among eye care professionals who every week examine more than 150 patients is 72.4%, perform more than 20 surgeries is 85.7%, and conduct more than 20 laser treatment sessions is 100%. Conclusion Musculoskeletal pain is highly prevalent among eye care professionals. This is especially true among females and older adults (>50 years). Among different exercises, running is most protective against musculoskeletal pain. Comorbidities contribute significantly to developing pain.

BACKGROUND/PURPOSE: The aim of this report was to present the details of a case of culture-positive endophthalmitis in a silicone oil-filled eye. METHODS: This report includes the description of the preoperative, intraoperative, and postoperative findings of the case. PATIENT: A 73-year-old male patient presented to our emergency department with complaints and symptoms indicative of acute postvitrectomy endophthalmitis. RESULTS: Aqueous, silicone oil, and vitreous washout samples tested positive for Streptococcus pneumoniae after a culture test. CONCLUSION: Culture-positive endophthalmitis in a silicone oil-filled eye has rarely been described in literature. The reported cases were culture-negative cases, possibly because they were cases of acute inflammatory reactions to silicone oil. In the present case, an otherwise healthy patient developed acute endophthalmitis after pars plana vitrectomy plus silicone oil tamponade. The case was successfully managed with conventional silicone oil removal, antibiotic injection, and silicone oil reinjection.

Globally, it is known that smoking can cause a variety of diseases. Studies have shown that smoking not only causes heart and lung diseases but is also strongly related to ocular diseases which could lead to blindness. This study aims to assess the level of awareness of blindness related to smoking in young people in Saudi Arabia. This is a descriptive cross-sectional study conducted among the young population in Saudi Arabia. A self-administered questionnaire was distributed using an online platform that includes socio-demographic data, education level, smoking status, and knowledge of the harmful effect of smoking like lung and cardiac diseases, including blindness. The data were analyzed using Statistical Packages for Social Sciences (SPSS) version 26 (IBM Corp., Armonk, NY). A total of 655 young individuals responded to the survey. Among our subjects, the prevalence of smoking participants was (18.7%) more common among the age group 21-25 years, males, bachelor, or higher degrees and those living in the Central region (p<0.001). Participants were definitely or probably more aware that the most common condition associated with excessive smoking was lung cancer (99.5%), followed by heart disease (98.1%) and stroke (93%) while the least of them was melanoma (91.3%) and blindness (81%). This is the first study in Saudi Arabia to assess the level of awareness of the risks of blindness related to smoking in young people and disclosed limited knowledge of the effect of smoking on their vision. The effects of smoking on lung cancer, heart disease, stroke, and even melanoma received higher ratings than blindness. Smokers may be able to quit smoking and enhance their quality of life by better understanding the link between smoking and blindness, which suggests that appropriate action is required to enhance their awareness.

Background Diabetic retinopathy (DR) is one of the diabetic complications that leads to the loss of vision. Most diabetic patients will have DR that is varying in its severity under the effect of many factors such as type of diabetes, duration of diabetes, and poor glycemic control. This study aimed to assess the level of adherence of diabetic patients to diabetic retinopathy screening (DRS) and to identify the influencing factors of adherence among diabetic patients in Al Ahsa, Saudi Arabia. Methods A cross-sectional study was conducted via a self-administered questionnaire among diabetic patients who were attending the primary health-care centers in Al Ahsa, Saudi Arabia, from July to August 2021. This questionnaire included five sections: sociodemographic data, diabetic profile, assessment of the knowledge, attitude toward DRS, and barriers to DRS. Results A total of 397 diabetic patients were involved in the study. Diabetic ocular complications were reported among 35.3% of the patients. The most commonly reported diabetic eye complication was cataract (37.1%; 52) followed by retinopathy (36.4%). Among the participants, 32.2% had a good awareness level regarding DR. In addition, 46.9% of diabetic patients have DRS. The most reported barriers were having no visual or eye problems, difficulty getting an appointment, and the cost and lack of information about the screening procedure (50.9%, 50.1%, 42.1%, and 39.8%, respectively).  Conclusion Improvement of patients' knowledge is a significant step to enhancing adherence to DRS. The availability of screening programs and well-established ophthalmology clinics in primary healthcare centers in addition to trained physicians will help to overcome the barriers of DRS.

INTRODUCTION: Diabetes mellitus (DM) microvascular complications can impair corneal sensitivity and lacrimal gland functioning, leading to dry eye disease (DED). Hence, this study aimed to measure the prevalence and severity of DED symptoms, and the related risk factors, among the Saudi diabetic population. METHODS: This is a retrospective, cross-sectional, survey-based study which targeted Saudi adults (20 years and older) previously diagnosed with type 1 or type 2 DM. It was conducted in eight primary healthcare centers (PCHs) scattered around eight different provinces of Saudi Arabia (SA). The prevalence and severity of DED were measured by the Ocular Surface Disease Index (OSDI). RESULTS: The total study population was 389 subjects, of which 182 (46.8%) were males and 207 (53.2%) were females. The overall prevalence of DED was 51.7%. Among those, 20.3% of patients had mild dryness, 11.1% had moderate dryness, and 20.3% had severe dryness. Glycosylated hemoglobin (HbA1c) levels of 6.5% or higher proved to be an independent risk factor for the development of DED symptoms, 3.6-folds higher for HbA1c levels of 6.5% to 9% (AOR=3.573; p=0.001), and 2.3-folds higher for HbA1c levels higher than 9% (AOR=2.293; p=0.013). The long duration of diabetes did not show a significant association with manifesting DED symptoms (p=0.263). CONCLUSION: Half of the diabetic population complained of DED symptoms, compared to one-third of the previously studied general Saudi population. Patients with mild to moderate HbA1c elevation were more likely to report DED symptoms than those with severe elevation, which could be related to impaired corneal sensation. Therefore, a routine ophthalmological examination is recommended.

PURPOSE: The aim of this study was to report the intraoperative and postoperative complications of vitrectomy for epiretinal membrane (ERM) and macular hole (MH) performed by retinal fellows under direct faculty supervision compared with experienced faculty members. PATIENTS AND METHODS: A total of 271 eyes that underwent pars plana vitrectomy (PPV) for MH and ERM from January 2014 to December 2019 at King Khaled Eye Specialist Hospital were analyzed. PPV for ERM and MH was performed by vitreoretinal fellows and consultants. RESULTS: The outcome measures assessed were the intraoperative complications rates, such as posterior lens touch, retinal breaks (RBs), retinal detachments, and vitreous hemorrhage. Moreover, the postoperative complications and optical coherence tomography (OCT) changes were assessed upon a minimum follow-up of 6 months. The rate of iatrogenic RB was more common in the ERM than in the MH surgery (15.5% vs 11.2%). Fellows and consultants had a rate of 20.5% of RB during the ERM surgery and 14.6% during MH surgery, respectively. However, these differences were not statistically significant (p = 0.12 for MH and p = 0.236 for ERM). Postoperative OCT analysis revealed an MH closure rate of 72.2%, and complete removal of the ERM was achieved in 88.6% in cases performed by fellows, while consultants achieved 61.8% closure rate of MH, and 83.3% of the patients achieved complete removal of ERM. CONCLUSION: Macular surgery is overall a safe procedure and the complication rates between fellows and consultants are comparable. With proper supervision, vitreoretinal fellows can achieve equally high anatomical outcomes with few complications.

Obstructive sleep apnea (OSA) is characterized by frequent episodes of partial or complete obstruction of the airway during sleep causing repeated episodes of apnea. OSA is more prevalent in middle-aged and older adults. OSA is associated with numerous ocular manifestations, including retinal manifestations. Literature highlighted the clear association between OSA and numerous ocular conditions including glaucoma and papilledema. This comprehensive and narrative review aims to summarize up-to-date clinical research concerning the association of OSA and vascular conditions that affect the retina. OSA is associated with the central serous chorioretinopathy (CSC), retinal vein occlusion (RVO), hypertensive retinopathy (HTRP) and development of diabetic retinopathy (DR). Sympathetic activation, hypoxia, and hormonal dysregulation all lead to serious retinal vascular conditions that will worsen OSA patients' quality of life. It is important to refer patients with newly diagnosed OSA to an ophthalmology clinic for the appropriate test.

RESUMO

The purpose of the study is to report a case of peripheral exudative hemorrhagic chorioretinopathy (PEHCR), managed surgically with favorable visual outcome. A 66-year-old female presented with painless visual loss due to dense vitreous and subretinal hemorrhage extending from the far periphery to the macula. Pars plana vitrectomy (PPV) with subretinal tissue plasminogen activator (TPA) injection was performed resulting in good anatomical and visual outcome. PEHCR can present with severe visual loss. Surgical management with PPV and subretinal TPA injection might result in favorable anatomical and visual outcome.

PURPOSE: To evaluate the outcome of pars plana vitrectomy (PPV) and combined pars plana vitrectomy with scleral buckle (PPV-SB) in the primary pediatrics rhegmatogenous retinal detachment repair. DESIGN: A retrospective cohort study. METHODS: A retrospective study was conducted in patients under 18 years of age, who presented with rhegmatogenous retinal detachment at King Khaled Eye Specialist Hospital between January 2014 and October 2018. Primary measurable outcomes were single surgery success rate (SSSR) and rate of postoperative complications. RESULTS: A total of 122 eyes of 117 patients were included in the study; 80 eyes in PPV group and 42 eyes in PPV-SB group. Mean follow-up period after surgery was 18.2 months ±11 months. SSSR was 76.3% (n=61) and 66.7% (n=28) for PPV and PPV-SB, respectively. No statistically significant difference was found between the two methods of RRD repair in single surgery success rate with an OR of 1.3 (P=0.45). Complication rates were comparable in both groups in the last follow-up. CONCLUSION: In this series, PPV and PPV-SB have comparable results in regard to anatomical success and rate of postoperative complications. More complicated cases were selected to undergo PPV-SB upon surgeons' preference.

PURPOSE: The purpose of this study is to evaluate the use of and beliefs toward social media (SM) among Saudi ophthalmologists. METHODS: This was a cross-sectional descriptive study. A web-based, self-administered questionnaire was e-mailed to ophthalmologists within Saudi Arabia. RESULTS: Out of 392 recipients, 293 completed the survey (74.7% response rate). The vast majority (90.1%) used at least one platform for personal and professional purposes or either. The three most commonly used applications were Twitter (72.3%), Snapchat (65.9%), and Instagram (51.9%). The opinions of respondents with regard to the different aspects of SM varied widely. Around half (56.7%) believed that it was permissible for ophthalmologists to promote themselves on SM and 70.6% agreed that SM usage by health-care professionals should be regulated. Participants believe that SM had both a positive and a negative impact on their daily life and clinical practice. For example, 94 (32.1%) respondents stated that it improved their ability to educate patients, on the other hand, 80 (27.3%) reported that SM wastes quality time that can be spent on something more productive. CONCLUSION: Our results demonstrate an increased utilization of SM among Saudi ophthalmologists. The ethical aspects of SM usage were a matter of controversy among our respondents. This necessitates the establishment of guidelines by regulatory bodies and/or individual health-care organizations.

BACKGROUND: Ophthalmology residents strongly rely on digital technology in training. This characteristic may not be shared by their teachers, attending physicians. Therefore, the aim of this study was to describe the ownership and usage of mobile devices among Saudi ophthalmology residents and their attending physicians and to compare ownership and usage patterns between both groups. METHODS: A survey was conducted to determine the rates of ownership of mobile devices and the patterns of usage among Saudi ophthalmology residents and their attending physicians. The survey was sent to 305 eligible participants. RESULTS: The overall response rate was 81%. The mean age of residents and attendings was 27.4 and 48.6 years, respectively. The ownership of mobile phones was higher among attendings (1.21 ± 0.4 vs 1.36 ± 0.5, p = 0.02), whereas the ownership of tablets was higher among residents (1 ± 0.6 vs 0.7 ± 0.6, p = 0.01). Residents utilized mobile devices to access online educational resources more frequently compared to attendings. A statistically significant difference between residents and attendings was reported in the utilization of wikis (91% vs 46%), e-books (90% vs 54%), file sharing sites (84% vs 52%), and vodcasts (78% vs 58%). Both groups also differed with regards to reasons of utilization. While residents most commonly used mobile devices to answer clinical questions on demand (87%) and to acquire basic knowledge (84%), the main reasons of utilization cited by attendings were to look up controversial issues (77%) and to connect with peers (72%). CONCLUSION: The two studied groups differ from each other in the type of devices owned, reasons for usage, and frequency of utilization. The difference in behavior between educators and their students can lead to a generation gap that halts the progress of residents' training.

Intraocular pressure (IOP) reduction by medications, laser, or surgery remains the mainstay of treatment in glaucoma. However, the role of complementary and alternative medicine (CAM) in glaucoma has received great interest from both patients and ophthalmologists. Previous evidence suggests that diet, a major domain of CAM, can influence an individual's IOP level. Furthermore, certain dietary components have been linked to the incidence and progression of glaucoma. In this review, we aim to provide a summary of the current evidence regarding the role of obesity, certain dietary components, and dietary supplements in glaucoma.

PURPOSE: To evaluate the effectiveness of an educational video in increasing knowledge among glaucoma patients and to determine the factors that may influence a patient's level of knowledge. PATIENTS AND METHODS: This was a pre-post intervention study on adult glaucoma patients attending the outpatient service at King Khaled Eye Specialist Hospital. The intervention tested was a short educational video that was edited specifically for this study. All patients completed a pre-video and post-video knowledge questionnaire; moreover, sociodemographic and clinical characteristics were obtained. RESULTS: The total number of patients included was 196. The mean age of patients was 55.7±15.5 years. Overall, 55.1% were males, 29.6% were illiterate, 85.2% resided in an urban area, 62.8% had a low income, and 41.8% were unemployed. The mean pre-intervention knowledge score was 6 out of 17, and the post-intervention score was 11.1 (P≤0.001). Predictors of a poor knowledge score were old age (>60 years), female sex, illiteracy, rural residence, low income, unemployment, and a negative family history of glaucoma. CONCLUSION: The evaluated video intervention was effective in a short-term increase in knowledge among glaucoma patients. This tool may serve as an alternative to traditional educational methods.
