RESUMO

During critical moments, disaster and accident victims may not be able to request help from the emergency response center. This may happen when the victim's vehicle is located within a coverage hole in a wireless network. In this paper, we adopt an unmanned aerial vehicle (UAV) to work as an automatic emergency dispatcher for a user in a vehicle facing a critical condition. Given that the UAV is located within a coverage hole and a predetermined critical condition is detected, the UAV becomes airborne and dispatches distress messages to a communication network. We propose to use a path loss map for UAV trajectory design, and we formulate our problem mathematically as an Integer Linear Program (ILP). Our goals are to minimize the distress messages delivery time and the UAV's mission completion time. Due to the difficulty of obtaining the optimal solution when the scale of the problem is large, we proposed an efficient algorithm that reduces the computational time significantly. We simulate our problem under different scenarios and settings, and study the performance of our proposed algorithm.

RESUMO

The feasibility, safety, and high technical success of robotically assisted percutaneous coronary intervention (PCI) for the treatment of both simple and complex coronary disease has been demonstrated. As the current generation robotic platform is limited to a rapid exchange system, orbital or rotational atherectomy cannot be performed robotically. However, excimer laser coronary atherectomy is performed with a rapid exchange catheter but its feasibility during robotically assisted PCI is unknown. We report the successful use of laser atherectomy during two complex robotically assisted PCI procedures using the CorPath GRX robotic system.

BACKGROUND: The role of microbiota in the pathogenesis of ulcerative colitis (UC) has been increasingly recognized. However, most of the reports are from Western populations. In Middle Eastern countries, including Saudi Arabia, little is known about the role of microbiota. Therefore, our aim was to describe the bacterial microbiota profile and signature in pediatric UC in Saudi Arabia. METHODS: Twenty children with UC and 20 healthy controls enrolled in the study gave stool samples. Twenty rectal mucosal samples were taken from UC and 20 from non-UC controls. Inclusion criteria included newly diagnosed and untreated children and lack of antibiotic exposure for at least 6 months before stool collection was required for children with UC and controls. Bacterial deoxyribonucleic acid was extracted and sequenced using shotgun metagenomic analysis. Statistical analysis included Shannon alpha diversity metrics, Bray-Curtis dissimilarity, DESeq2, and biomarker discovery. RESULTS: The demographic characteristics were similar in children with UC and controls. There was a significant reduction in alpha diversity (P = 0.037) and beta diversity in samples from children with UC (P = 0.001). Many taxa were identified with log2 abundance analysis, revealing 110 and 102 species significantly depleted and enriched in UC, respectively. Eleven bacterial species' signatures were identified. CONCLUSIONS: In Saudi Arabian children with UC, we demonstrate a dysbiosis similar to reports from Western populations, possibly related to changes of lifestyle. Microbial signature discovery in this report is an important contribution to research, leading to the development of adjunctive non-invasive diagnostic options in unusual cases of UC.

Objectives: Adenomyomatosis (ADM) of the gallbladder is a benign condition, which is characterized by mucosal hyperplasia of the gallbladder and formation of intramucosal invagination through the thickened mucosal layer. The pathogenesis is unclear. This condition is rare in children. The aim of this publication is to present the case of a teenager with ADM of the gallbladder and review the pediatric literature on this topic. Methods: A 17-year-old female presented with severe postprandial right upper quadrant abdominal pains. The abdominal ultrasound revealed ADM of the gallbladder. Results: A curative laparoscopic cholecystectomy was performed. Since 1998, eleven of the 13 pediatrics cases reported with ADM of the gallbladder were symptomatic and a cholecystectomy was curative in all of them. Conclusion: ADM of the gallbladder should be considered in the differential diagnosis of recurrent right abdominal upper quadrant pains in pediatrics. Abdominal ultrasound is the best diagnostic procedure. In symptomatic patients, a cholecystectomy is curative.

OBJECTIVES: Quantify and characterize bacterial contamination of lead aprons in a high-volume catheterization laboratory and evaluate the efficacy of decontamination using an ultraviolet-C (UV-C) radiation system. BACKGROUND: Bacterial contamination and ineffective disinfection of personal protective equipment in medical centers pose potential health risks to patients and medical staff. The contamination burden of lead aprons and a reliable disinfection strategy are unknown. METHODS: Ten routinely used, unsterilized lead aprons from a high-volume catheterization laboratory were studied. Standard and bacteria-resistant outer fabrics were included. Swabbings from four locations on each apron (inner thyroid collar, chest, waist, and bottom border) were obtained at baseline and after a 15-minute decontamination cycle using the UV-C based DCab System (Nosocom Solutions). Colony counts, speciation, and antibiotic resistance were obtained from aerobic and anaerobic cultures. RESULTS: Baseline cultures grew ≥1 colony from 25 of 40 samples (62.5%; 310 colonies; 0-100 colonies/sample; 16 organisms), mainly skin and mouth flora without antibiotic resistance. Baseline growth was greatest from the thyroid collar and similar between different fabrics. UV-C reduced subsequent growth (7.8 ± 23.8 colonies overall vs 0.1 ± 0.3 colonies overall; P<.001), with all four isolates considered contaminants of laboratory handling. Colony counts were reduced in thyroid collar, chest, waist, nylon fabric, polyurethane fabric, and alternative bacteria-resistant fabric subgroups (all P<.05). CONCLUSIONS: Routinely used lead aprons in a high-volume catheterization laboratory were contaminated by non-pathogenic skin and mouth flora located predominantly on the thyroid collar. Disinfection using an automated UV-C based system is highly effective across different apron surface locations and fabric types.
