RESUMO

Compared to peripheral pain, trigeminal pain elicits higher levels of fear, which is assumed to enhance the interruptive effects of pain on concomitant cognitive processes. In this fMRI study we examined the behavioral and neural effects of trigeminal (forehead) and peripheral (hand) pain on visual processing and memory encoding. Cerebral activity was measured in 23 healthy subjects performing a visual categorization task that was immediately followed by a surprise recognition task. During the categorization task subjects received concomitant noxious electrical stimulation on the forehead or hand. Our data show that fear ratings were significantly higher for trigeminal pain. Categorization and recognition performance did not differ between pictures that were presented with trigeminal and peripheral pain. However, object categorization in the presence of trigeminal pain was associated with stronger activity in task-relevant visual areas (lateral occipital complex, LOC), memory encoding areas (hippocampus and parahippocampus) and areas implicated in emotional processing (amygdala) compared to peripheral pain. Further, individual differences in neural activation between the trigeminal and the peripheral condition were positively related to differences in fear ratings between both conditions. Functional connectivity between amygdala and LOC was increased during trigeminal compared to peripheral painful stimulation. Fear-driven compensatory resource activation seems to be enhanced for trigeminal stimuli, presumably due to their exceptional biological relevance.

OBJECTIVES: To assess the image quality of T2-weighted (T2w) magnetic resonance imaging of the prostate and the visibility of prostate cancer at 7 Tesla (T). MATERIALS & METHODS: Seventeen prostate cancer patients underwent T2w imaging at 7T with only an external transmit/receive array coil. Three radiologists independently scored images for image quality, visibility of anatomical structures, and presence of artefacts. Krippendorff's alpha and weighted kappa statistics were used to assess inter-observer agreement. Visibility of prostate cancer lesions was assessed by directly linking the T2w images to the confirmed location of prostate cancer on histopathology. RESULTS: T2w imaging at 7T was achievable with 'satisfactory' (3/5) to 'good' (4/5) quality. Visibility of anatomical structures was predominantly scored as 'satisfactory' (3/5) and 'good' (4/5). If artefacts were present, they were mostly motion artefacts and, to a lesser extent, aliasing artefacts and noise. Krippendorff's analysis revealed an α = 0.44 between three readers for the overall image quality scores. Clinically significant cancer lesions in both peripheral zone and transition zone were visible at 7T. CONCLUSION: T2w imaging with satisfactory to good quality can be routinely acquired, and cancer lesions were visible in patients with prostate cancer at 7T using only an external transmit/receive body array coil. KEY POINTS: • Satisfactory to good T2-weighted image quality of the prostate is achievable at 7T. • Periprostatic lipids appear hypo-intense compared to healthy peripheral zone tissue at 7T. • Prostate cancer is visible on T2-weighted MRI at 7T.

OBJECTIVE: The aim of this study was to assess the feasibility of first-pass contrast-enhanced renal MR angiography (MRA) at 7 T. METHODS: In vivo first-pass contrast-enhanced high-field examinations were obtained in eight healthy subjects on a 7-T whole-body MRI. A custom-built body transmit/receive radiofrequency (RF) coil and RF system suitable for RF shimming were used for image acquisition. For dynamic imaging, gadobutrol was injected intravenously and coronal unenhanced, arterial and venous data sets using a T1-weighted spoiled gradient-echo sequence were obtained. Qualitative image analysis and assessment of artefact impairment were performed by two senior radiologists using a five-point scale (5 = excellent, 1 = non-diagnostic). SNR and CNR of the perirenal abdominal aorta and both main renal arteries were assessed. RESULTS: Qualitative image evaluation revealed overall high-quality delineation of all assessed segments of the unenhanced arterial vasculature (meanunenhanced 4.13). Nevertheless, the application of contrast agent revealed an improvement in vessel delineation of all the vessel segments assessed, confirmed by qualitative (meanunenhanced 4.13 to meancontrast-enhanced 4.85) and quantitative analysis (SNR meanunenhanced 64.3 to meancontrast-enhanced 98.4). CONCLUSION: This study demonstrates the feasibility and current constraints of ultra-high-field contrast-enhanced renal MRA relative to unenhanced MRA.

As the magnetic field strength and therefore the operational frequency in MRI are increased, the radiofrequency wavelength approaches the size of the human head/body, resulting in wave effects which cause signal decreases and dropouts. Especially, whole-body imaging at 7 T and higher is therefore challenging. Recently, an acquisition scheme called time-interleaved acquisition of modes has been proposed to tackle the inhomogeneity problems in high-field MRI. The basic premise is to excite two (or more) different B 1+ modes using static radiofrequency shimming in an interleaved acquisition, where the complementary radiofrequency patterns of the two modes can be exploited to improve overall signal homogeneity. In this work, the impact of time-interleaved acquisition of mode on image contrast as well as on time-averaged specific absorption rate is addressed in detail. Time-interleaved acquisition of mode is superior in B 1+ homogeneity compared with conventional radiofrequency shimming while being highly specific absorption rate efficient. Time-interleaved acquisition of modes can enable almost homogeneous high-field imaging throughout the entire field of view in PD, T(2) , and T(2) *-weighted imaging and, if a specified homogeneity criterion is met, in T(1) -weighted imaging as well.

The development and analysis of three waveguides for the exposure of small biological in vitro samples to mobile communication signals at 900 MHz (GSM, Global System for Mobile Communications), 1.8 GHz (GSM), and 2 GHz (UMTS, Universal Mobile Telecommunications System) is presented. The waveguides were based on a fin-line concept and the chamber containing the samples bathed in extracellular solution was placed onto two fins with a slot in between, where the exposure field concentrates. Measures were taken to allow for patch clamp recordings during radiofrequency (RF) exposure. The necessary power for the achievement of the maximum desired specific absorption rate (SAR) of 20 W/kg (average over the mass of the solution) was approximately P(in) = 50 mW, P(in) = 19 mW, and P(in) = 18 mW for the 900 MHz, 1800 MHz, and 2 GHz devices, respectively. At 20 W/kg, a slight RF-induced temperature elevation in the solution of no more than 0.3 °C was detected, while no thermal offsets due to the electromagnetic exposure could be detected at the lower SAR settings (2, 0.2, and 0.02 W/kg). A deviation of 10% from the intended solution volume yielded a calculated SAR deviation of 8% from the desired value. A maximum ±10% variation in the local SAR could occur when the position of the patch clamp electrode was altered within the area where the cells to be investigated were located.

The sensitivity of proton MR Spectroscopic Imaging ((1)H-MRSI) of the prostate can be optimized by using the high magnetic field strength of 7 T in combination with an endorectal coil. In the work described in this paper we introduce an endorectal transceiver at 7 T, validate its safety for in vivo use and apply a pulse sequence, optimized for three-dimensional (3D) (1)H-MRSI of the human prostate at 7 T. A transmit/receive endorectal RF coil was adapted from a commercially available 3 T endorectal receive-only coil and validated to remain within safety guidelines for radiofrequency (RF) power deposition using numerical models, MR thermometry of phantoms, and in vivo temperature measurements. The (1)H-MRSI pulse sequence used adiabatic slice selective refocusing pulses and frequency-selective water and lipid suppression to selectively obtain the relevant metabolite signals from the prostate. Quantum mechanical simulations were used to adjust the inter-pulse timing for optimal detection of the strongly coupled spin system of citrate resulting in an echo time of 56 ms. Using this endorectal transceiver and pulse sequence with slice selective adiabatic refocusing pulses, 3D (1)H-MRSI of the human prostate is feasible at 7 T with a repetition time of 2 s. The optimized inter-pulse timing enables the absorptive detection of resonances of spins from spermine and citrate in phase with creatine and choline. These potential tumor markers may improve the in vivo detection, localization, and assessment of prostate cancer.

Radial waveguide set-ups are introduced as exposure devices for long-term experiments with large numbers of non-restrained animals exposed simultaneously. Methods are presented to ensure well-defined exposure conditions even for potentially overmoded waveguides and for the exposure of large groups of animals per cage. The proposed methods are applied for a four-generation study being performed on up to 2500 mice exposed to a generic UMTS test signal at prescribed averaged whole body specific absorption rates (SARs). The variation of the whole body SAR due to the movement of the mice inside the cage is calculated by using the finite-difference time-domain method and detailed animal models for selected configurations of the mice inside the cage for all stages of the study.

PURPOSE: The increasing spread of high-field and ultra-high-field MRI scanners encouraged a new discussion on safety aspects of MRI examinations. Earlier studies report altered acoustically evoked potentials. This finding was not able to be confirmed in later studies. In the present study transcranial magnetic stimulation (TMS) was used to evaluate whether motor cortical excitability may be altered following MRI examination even at field strength of 1.5 T. MATERIALS AND METHODS: In 12 right-handed male volunteers individual thresholds for motor responses and then the length of the post-excitatory inhibition (silent period) were determined. Subsequently the volunteers were examined in the MRI scanner (Siemens Avanto, 1.5 T) for 63 minutes using gradient and spin echo sequences. MRI examination was immediately followed by another TMS session and a third 10 minutes later. As a control condition, the 12 subjects spent one hour in the scanner without examination and one hour on a couch without the presence of a scanner. RESULTS: After MRI examination, the silent period was significantly lengthened in all 12 subjects and then tended to the initial value after 10 min. Motor thresholds were significantly elevated and then normalized after 10 minutes. No significant effects were found in the control conditions. CONCLUSION: MRI examination leads to a transient effect on motor cortical excitability indicated by elongation of the post-excitatory inhibition and to an increase in motor thresholds in some subjects. These effects do not seem to be associated with a static magnetic field.

This report outlines the characteristics of universal mobile telecommunications system (UMTS) signals and discusses the signal parameters with respect to their possible biological relevance in order to define a generic UMTS test signal (GUS) for experiments aiming at the investigation of biological effects of weak electromagnetic fields. The GUS includes features of a real UMTS signal and especially the characteristics of UMTS, which differ from those of already applied second generation mobile communication systems (GSM 900, DCS1800, PCS1900, IS-95). It has been specified on the basis of the recommendations of a working group of the German Forschungsgemeinschaft Funk (FGF) with a focus on the mechanisms of UMTS which are responsible for slow term signal contributions, i.e., low frequency variations of the radio frequency (RF) envelope, since the hypothetical possibility of biological relevance of weak electromagnetic fields is often attributed to time variations of the RF envelope with frequencies close to those of natural processes. In this respect, it is also shown that the mandatory power control loop in UMTS gives rise to very strong 1.5 kHz variations on the air interface. Based upon the concept of the GUS, a UMTS test signal generator (GUS6960S) is described.
