RESUMO

An eight-year-old bull mastiff dog underwent a craniotomy for surgical excision of an olfactory lobe meningioma. Rapidly progressive neurological deficits with cervical pain developed within the early postoperative period. Intraventricular and cervical subarachnoid space air accumulation (pneumorrhachis) was identified through magnetic resonance imaging and computed tomography. Repair of a dural defect using synthetic dura substitute resulted in gradual resolution of neurological signs attributable to the tension pneumocephalus and subarachnoid space pneumorrhachis. Regrowth of the meningioma was not observed. Postoperative intraventricular tension pneumocephalus and air accumulation within subarachnoid space are uncommon but life-threatening complications of intracranial surgery. Early diagnosis and treatment can result in a satisfactory outcome.

RESUMO

We perform a likelihood analysis of the constraints from accelerator experiments and astrophysical observations on supersymmetric (SUSY) models with SU(5) boundary conditions on soft SUSY-breaking parameters at the GUT scale. The parameter space of the models studied has seven parameters: a universal gaugino mass [Formula: see text], distinct masses for the scalar partners of matter fermions in five- and ten-dimensional representations of SU(5), [Formula: see text] and [Formula: see text], and for the [Formula: see text] and [Formula: see text] Higgs representations [Formula: see text] and [Formula: see text], a universal trilinear soft SUSY-breaking parameter [Formula: see text], and the ratio of Higgs vevs [Formula: see text]. In addition to previous constraints from direct sparticle searches, low-energy and flavour observables, we incorporate constraints based on preliminary results from 13 TeV LHC searches for jets + [Formula: see text] events and long-lived particles, as well as the latest PandaX-II and LUX searches for direct Dark Matter detection. In addition to previously identified mechanisms for bringing the supersymmetric relic density into the range allowed by cosmology, we identify a novel [Formula: see text] coannihilation mechanism that appears in the supersymmetric SU(5) GUT model and discuss the role of [Formula: see text] coannihilation. We find complementarity between the prospects for direct Dark Matter detection and SUSY searches at the LHC.

We perform a likelihood analysis of the minimal anomaly-mediated supersymmetry-breaking (mAMSB) model using constraints from cosmology and accelerator experiments. We find that either a wino-like or a Higgsino-like neutralino LSP, [Formula: see text], may provide the cold dark matter (DM), both with similar likelihoods. The upper limit on the DM density from Planck and other experiments enforces [Formula: see text] after the inclusion of Sommerfeld enhancement in its annihilations. If most of the cold DM density is provided by the [Formula: see text], the measured value of the Higgs mass favours a limited range of [Formula: see text] (and also for [Formula: see text] if [Formula: see text]) but the scalar mass [Formula: see text] is poorly constrained. In the wino-LSP case, [Formula: see text] is constrained to about [Formula: see text] and [Formula: see text] to [Formula: see text], whereas in the Higgsino-LSP case [Formula: see text] has just a lower limit [Formula: see text] ([Formula: see text]) and [Formula: see text] is constrained to [Formula: see text] in the [Formula: see text] ([Formula: see text]) scenario. In neither case can the anomalous magnetic moment of the muon, [Formula: see text], be improved significantly relative to its Standard Model (SM) value, nor do flavour measurements constrain the model significantly, and there are poor prospects for discovering supersymmetric particles at the LHC, though there are some prospects for direct DM detection. On the other hand, if the [Formula: see text] contributes only a fraction of the cold DM density, future LHC [Formula: see text]-based searches for gluinos, squarks and heavier chargino and neutralino states as well as disappearing track searches in the wino-like LSP region will be relevant, and interference effects enable [Formula: see text] to agree with the data better than in the SM in the case of wino-like DM with [Formula: see text].

OBJECTIVE: There are limited data relating glycemic control to medical costs among patients with diabetes. The goal of this study was to examine the potential impact of improved glycemic control on selected short-term complications of diabetes and associated costs in a managed care setting. RESEARCH DESIGN AND METHODS: Using a retrospective cohort design and automated databases from January 1994 to 30 June 1998, adult members of the Fallon Clinic who were diagnosed with diabetes were identified and assigned to one of three study groups based on each patient's mean HbA1c level: good control (<8%), fair control (8-10%), and poor control (> 10%) groups. Inpatient (hospital or skilled nursing facility) admissions for selected acute (short-term) complications, represented by selected infections, hyperglycemia, hypoglycemia, and electrolyte disturbances, and the associated medical charges were evaluated across the three HbA1c groups. Multivariate analyses were used to control for differences in several potential confounding factors among the study groups. All findings were expressed on a 3-year basis. RESULTS: Of 2,394 patients with diabetes, approximately 10% (251) had at least one inpatient stay for a short-term complication, accounting for 447 admissions. Over 3 years, the adjusted rate of inpatient treatment ranged from 13 per 100 patients with good glycemic control to 16 per 100 patients with fair glycemic control and 31 per 100 patients with poor glycemic control (P < 0.05). The corresponding mean adjusted charges were approximately $970, $1,380, and $3,040, respectively Among the 30% of the study population with long-term diabetic complications, the results were more marked; the adjusted admissions per 100 patients (mean charges) were estimated to be 30 ($2,610), 38 ($3,810), and 74 ($8,320) over 3 years for patients with an HbA1c of <8, 8-10, and > 10%, respectively. CONCLUSIONS: In typical practice, better glycemic control is associated with a reduced rate of admission for selected short-term complications and, therefore, reduced medical charges for these complications over a 3-year period. The potential short-term economic benefits are important to consider when making decisions regarding the adoption and use of new interventions for the management of diabetes.

Different mechanisms operate in various regions of the MSSM parameter space to bring the relic density of the lightest neutralino, [Formula: see text], assumed here to be the lightest SUSY particle (LSP) and thus the dark matter (DM) particle, into the range allowed by astrophysics and cosmology. These mechanisms include coannihilation with some nearly degenerate next-to-lightest supersymmetric particle such as the lighter stau [Formula: see text], stop [Formula: see text] or chargino [Formula: see text], resonant annihilation via direct-channel heavy Higgs bosons H / A, the light Higgs boson h or the Z boson, and enhanced annihilation via a larger Higgsino component of the LSP in the focus-point region. These mechanisms typically select lower-dimensional subspaces in MSSM scenarios such as the CMSSM, NUHM1, NUHM2, and pMSSM10. We analyze how future LHC and direct DM searches can complement each other in the exploration of the different DM mechanisms within these scenarios. We find that the [Formula: see text] coannihilation regions of the CMSSM, NUHM1, NUHM2 can largely be explored at the LHC via searches for [Formula: see text] events and long-lived charged particles, whereas their H / A funnel, focus-point and [Formula: see text] coannihilation regions can largely be explored by the LZ and Darwin DM direct detection experiments. We find that the dominant DM mechanism in our pMSSM10 analysis is [Formula: see text] coannihilation: parts of its parameter space can be explored by the LHC, and a larger portion by future direct DM searches.

We present a frequentist analysis of the parameter space of the pMSSM10, in which the following ten soft SUSY-breaking parameters are specified independently at the mean scalar top mass scale [Formula: see text]: the gaugino masses [Formula: see text], the first-and second-generation squark masses [Formula: see text], the third-generation squark mass [Formula: see text], a common slepton mass [Formula: see text] and a common trilinear mixing parameter A, as well as the Higgs mixing parameter [Formula: see text], the pseudoscalar Higgs mass [Formula: see text] and [Formula: see text], the ratio of the two Higgs vacuum expectation values. We use the MultiNest sampling algorithm with [Formula: see text]1.2 [Formula: see text] points to sample the pMSSM10 parameter space. A dedicated study shows that the sensitivities to strongly interacting sparticle masses of ATLAS and CMS searches for jets, leptons [Formula: see text][Formula: see text] signals depend only weakly on many of the other pMSSM10 parameters. With the aid of the Atom and Scorpion codes, we also implement the LHC searches for electroweakly interacting sparticles and light stops, so as to confront the pMSSM10 parameter space with all relevant SUSY searches. In addition, our analysis includes Higgs mass and rate measurements using the HiggsSignals code, SUSY Higgs exclusion bounds, the measurements of [Formula: see text] by LHCb and CMS, other B-physics observables, electroweak precision observables, the cold dark matter density and the XENON100 and LUX searches for spin-independent dark matter scattering, assuming that the cold dark matter is mainly provided by the lightest neutralino [Formula: see text]. We show that the pMSSM10 is able to provide a supersymmetric interpretation of [Formula: see text], unlike the CMSSM, NUHM1 and NUHM2. As a result, we find (omitting Higgs rates) that the minimum [Formula: see text] with 18 degrees of freedom (d.o.f.) in the pMSSM10, corresponding to a [Formula: see text] probability of 30.8 %, to be compared with [Formula: see text] in the CMSSM (NUHM1) (NUHM2). We display the one-dimensional likelihood functions for sparticle masses, and we show that they may be significantly lighter in the pMSSM10 than in the other models, e.g., the gluino may be as light as [Formula: see text]1250 [Formula: see text] at the 68 % CL, and squarks, stops, electroweak gauginos and sleptons may be much lighter than in the CMSSM, NUHM1 and NUHM2. We discuss the discovery potential of future LHC runs, [Formula: see text] colliders and direct detection experiments.

Pneumatic otoscopy was performed on 81 healthy full-term babies during the first 72 hours of life and at routine well-baby follow-up visits. Only 14 of 115 (12%) of the eardrums visualized during the first three days of life moved briskly to insufflation as compared with 29 of 65 (44%) and 50 of 71 (71%) by 3 weeks and 10 weeks of age, respectively. A pink color was noted in 38 of 115 (33%) and redness in five of 115 (4%) of the eardrums examined during the first 72 hours of life, whereas 64 of 71 (90%) of the tympanic membranes appeared gray by age 10 weeks. Tympanic membrane dullness, decreased light reflex, and diminished translucence occurred in greater than 90% of the infants during the first three days of life and declined to 26% or less by 4 months of age. The data suggest that limited mobility, changes in color, poor luster, and relative opacity of the tympanic membrane occur in healthy neonates and may reflect physiologic changes unique to the newborn period.

The amount of positive pressure delivered during pneumatic otoscopy was recorded in 53 healthy pediatric patients. The pressures generated ranged from 338 to 1134 mm H2O. The mean pressure introduced was 748 mm H2O with the bulb and 502 mm H2O through the mouthpiece. The mean intraobserver variation in pressure delivered to each patient was 199 and 125 mm H2O with these two methods of insufflation, respectively. These data document the wide variation in pressures generated with the pneumatic otoscope and suggest that the feasibility of modifying this instrument into a standardized, objective pneumatic system should be explored.

In summary, it is apparent that discussions on substance abuse should begin at the prenatal visit and extend through adolescence. Educational efforts and preventive strategies must be given highest priority. Signs and symptoms may be subtle, and pediatricians should have a high index of suspicion, particularly for high-risk individuals. Practitioners also must be willing to routinely address other important psychosocial-medical issues that are closely correlated with the use of drugs and alcohol. Laboratory assessment and formal substance abuse screening instruments should be used under selected circumstances. Following diagnosis, referral to the appropriate resources and coordination of follow-up services are important roles for the primary care clinician.

OBJECTIVE: To document opinions and decision-making strategies of young adults regarding abortions for young women. METHODS: Eighty-nine male and 215 female college students completed a questionnaire on abortion during routine visits to their university health center. RESULTS: Among the salient findings, most respondents took a pro-abortion stance for girls under 18 in cases of rape (92% of students), incest (90%), or danger to the girl's health (90%). A much lower priority was given for abortion in cases of fetal abnormalities (55% of students), economic hardship (51%), or for girls who were married (55%). Abortion was considered acceptable regardless of circumstances by 46% of students and never a good idea by 18%. The outcome of unplanned pregnancies for minors should be decided by the girl (90% of students), partner (55%), parents (29%), and state or federal law (8%). Abortions for minors should require parental notification (45%) or consent (33%). Although only one student felt illegal abortions were safe, 19% would seek this kind of abortion and 4% of females would try to cause their own miscarriage if abortions were outlawed in the United States. CONCLUSIONS: The vast majority of young adults believed that girls under 18 should retain the right to decide the outcome of their pregnancy and should not be subjected to governmental restrictions. The highest priority for abortion was given to girls who had been victimized or whose health was at risk. Although nearly all respondents are aware of the hazards of criminal abortions, many would resort to such unsafe practices if legal alternatives were no longer available.

Unexplained lower abdominal pain in young women can present a challenge for even the most experienced clinicians. Although the cause is usually benign and self-limited, occasionally a serious underlying disorder exists. Clinicians should have an organized approach for diagnosis and management in an effort to avoid any unnecessary tests or referrals. The most important elements of the evaluation are thorough history, careful physical, and sequential follow-up as needed. Selective use of the laboratory and radiographic studies should be considered on an individual basis. This paper describes four disorders in adolescent girls that may present with pain in the lower abdominal region and closely resemble pathologic conditions within the gynecologic tract. Practitioners who provide services to young women should be aware of these entities so that the correct diagnosis is established as soon as possible and appropriate therapy initiated in a timely fashion.

Pityriasis rosea is a common disorder of unknown etiology that may simulate a number of other dermatologic entities. Secondary syphilis should always be ruled out, particularly in adolescents. Pruritus occurs more frequently than is generally appreciated. The herald patch may not always be present. Secondary lesions occasionally occur distal to the elbows and knees and on the face. Over the more proximal parts of the body the secondary lesions will characteristically align themselves with the long axis following the skin lines of cleavage. The natural course is that of spontaneous resolution within 6 to 8 weeks. Sequelae are rare and usually medically insignificant. Management with oral antipruritics and/or topical steroids may be necessary to relieve the itching.

Fifty young women were given 126 telephone appointments as a supplement to follow-up care in a university hospital adolescent clinic. Eighty (64%) calls were placed on the appropriate day, 71 (89%) of which were within 20 minutes of the assigned time. Patients with one or two clinic visits honored 39 of 52 (75%) of their phone appointments, whereas those with 11 or more visits kept only 8 of 17 (47%). Telephone appointments scheduled directly in the clinic were kept on 66 of 98 (67%) occasions as compared to 14 of 28 (50%) for those made over the phone. Sixty-four of 93 (69%) calls were made when the interval from the time the phone appointments were given to date of appointment was 7 days or less, while 16 of 33 (48%) responded when this period was 8 days or longer. The data document the compliance of adolescent girls with telephone appointments and suggest that this technique may be a useful adjunct for monitoring patients requiring close medical follow-up.

Most pediatricians do not screen for family problems as part of routine adolescent health care. This study documents fears, worries, and concerns about family issues expressed by 147 teenagers on a confidential questionnaire during consecutive initial visits to an adolescent medicine clinic in a university hospital setting. Among the salient findings, 44% had thought about running away from home, 42% reported having been subjected to some form of abuse, and 33% had felt like hurting themselves or someone else. Twenty-eight percent reported conflict in their home, 27% were having problems with their family, and 20% were concerned about their parent's relationship. In addition, 27% worried about the physical or mental health of family members and 22% believed that a family member had a problem with alcohol or other drugs. Twelve percent did not have a confidant with whom to discuss their innermost concerns. The data demonstrate a high frequency of stressful and anxiety-provoking family situations as reported by adolescents on a confidential questionnaire. Pediatricians who wish to provide comprehensive services to their young adult patients must be prepared to discuss such issues on a routine basis.

Loss of previously attained developmental milestones in an infant is often associated with central nervous system tumor, neuromuscular disease, or an inborn metabolic error. An infant with developmental regression and involuntary movements who was found to be vitamin B12 deficient on the basis of unrecognized maternal vitamin B12 deficiency is described. The infant had a dramatic neurologic recovery after receiving vitamin B12. The case and a review of similar cases is presented.

Parents' opinions regarding anticipatory guidance for adolescents have not been adequately investigated. In this study, 932 parents of adolescents completed a computerized questionnaire listing 30 common psychosocial-medical concerns of adolescence. Respondents were asked how important it is for private pediatricians to discuss these topics with their teenagers during regular checkups. Ten items were rated important by > or = 90% of parents, 22 items by > or = 80%, and 29 items by > or = 66%. Heeding parents' expectations and concerns, pediatricians should incorporate discussions of psychosocial-medical issues as a meaningful component of routine adolescent health care.

In some patients, primary molars with no permanent successor can be retained for an indefinite time; however, if occlusal relationships necessitate intraocclusal augmentation, cast ceramic onlays can provide an excellent solution. This paper reports the case of a teenager in whom two cast ceramic onlays were bonded with resin to retained mandibular primary molars, to create an esthetic and functional occlusion.

Decisions regarding the treatment of enamel pits and fissures, carious or not, can be difficult for the dentist; exceptional diagnostic acumen is required. In years gone by, teeth with Class I carious lesions were always restored with silver amalgam material; now, however, various treatment options are available to the clinician. Resin bonding, glass-ionomer systems, and visible light-curing technology have irrevocably changed restorative dentistry. This article reviews six methods for restoring teeth with Class I carious lesions or sealing those with susceptible pits and fissures.

RESUMO

One of the most gratifying composite resin bonding procedures for patients and dentists alike is closure of unwanted and unsightly interdental spaces. Various methods of performing such bonding have been described. The most modern composite resins are strong, tooth-colored materials that can exactly simulate the appearance of normal tooth structure and reliably stay in place for many years. A method of augmenting the mesiodistal coronal dimension of anterior teeth without mechanical preparation of the enamel described.

RESUMO

This study compared the effects of immediate self-correction, delayed self-correction, and no correction on the acquisition and maintenance of multiplication facts by a fourth-grade student with learning disabilities. Data from daily and maintenance tests indicated that both correct response rate and accuracy were higher when self-correction was immediate rather than delayed or absent.
