RESUMO

INTRODUCTION: Universities have the ability to bring science and comprehensive care to remote regions. This can be done through the creation of rural clerkships during the training of health professionals. METHODS: Report of students' experiences during their rural clerkships in Brazil. RESULTS: Rural clerkships made contact possible between students from different areas of health, such as medicine, nutrition, psychology, social assistance, and nursing. This multidisciplinary team expanded the possibilities of care in the region, which often suffers from a shortage of healthcare professionals. DISCUSSION: Students noticed that the use of management and treatment guided by evidence-based medicine was more common in their university than in rural facilities. The relationship between students and local health professionals provided discussions and application of new scientific evidence and updates. Due to the greater number of students and residents of the multi-professional health team, it was possible to initiate health education, integrated case discussions, and territorialization projects. Areas with untreated sewage and a high local concentration of scorpions were identified, which allowed a targeted intervention. The students noticed the numerous differences between the tertiary care they were used to at medical school and the access to health and resources in a rural area. Collaboration between educational institutions and rural areas with scarce resources makes it possible to exchange knowledge between students and local professionals. In addition, these rural clerkships expand the possibilities for care for local patients and make it possible to carry out projects related to health education.

RESUMO

BACKGROUND: Mental disorders are the leading cause of disability for youth worldwide. However, there is a dearth of validated, brief instruments to assess mental health in low- and middle-income countries (LMIC). We aimed to facilitate identification of mental disorders in LMIC contexts by adapting and validating measures of internalizing and externalizing disorders for adolescents in Mozambique, an LMIC in southeastern Africa. METHODS: We selected instruments with good support for validity in high-income and other LMIC settings: the Patient Health Questionnaire Adolescent (PHQ-A), Generalized Anxiety Disorders 7 (GAD-7), and Strengths and Difficulties Questionnaire (SDQ). Instruments were adapted by local and international mental health specialists followed by cognitive interviews (n = 48) with Mozambican adolescents. We administered the instruments along with the Miniature International Neuropsychiatric Interview for Children and Adolescents (MINI-KID)to 485 adolescents aged 12-19 years attending two secondary schools in Maputo City, Mozambique. One week later, we re-administered instruments to a randomly selected sample of 49 adolescents. RESULTS: Participants were 66.2% (n = 321) female and the average age was 15.9 (S.D = 1.7).Internal consistency (alpha = 0.80, PHQ-A; 0.84, GAD-7; 0.80, SDQ) and test-retest reliabilty (ICC = 0.74, PHQ-A; 0.70, GAD-7; 0.77, SDQ) were acceptabe for the PHQ-A, GAD-7, and the full SDQ. The SDQ internalizing subscale showed poor test-retest reliability (ICC = 0.63) and the SDQ externalizing subscale showed poor internal consistency (alpha = 0.65). All instruments demonstrated good sensitivity and specificity (> 0.70). Youden's index identified optimal cutoff scores of 8 for the PHQ-A, 5 for the GAD-7, 10 for the SDQ internalizing and 9 for the SDQ externalizing subscales, though a range of scores provided acceptable sensitivity and specificity. CONCLUSIONS: Our data supports reliability and validity of the PHQ-A, GAD-7, and SDQ instruments for rapidly assessing mental health problems in Mozambican adolescents. Use of these tools in other contexts with limited specialists may asist with expanding mental health assessment. Specific instrument and cutoff selection should be based on screening goals, treatment resources, and program objectives.

RESUMO

Superficial lymphadenectomy is an easy-to-perform and cost-effective routine technique. Despite its simplicity, it remains underutilized in veterinary medicine, with most practitioners being oncological surgeons. This study aims to enhance accessibility to the surgical procedure by providing anatomical representations of superficial lymphadenectomy in the carcasses of dogs and cats. A preliminary study involving two canines and two felines was conducted, with each group comprising a dog and a cat. Group A was designated to superficial lymphadenectomy techniques to create an illustrated step-by-step procedure, while group B underwent anatomical dissection to expose lymph nodes and their adnexa. The approach to superficial lymph nodes in dogs and cats is simple, allowing for the demonstration of superficial lymphadenectomy techniques in the corpses of dogs and cats without complications. This includes the dissection and presentation of anatomical structures adjacent to the lymph nodes. In conclusion, the techniques applied to subjects in groups A and B proved effective, successfully demonstrating and excising all superficial lymph nodes in the corpses of dogs and cats. These findings suggest that the developed set of techniques developed for lymph node excision holds promise for safe and effective application in live animals.

The objective of this review is to expose the main genetic changes and genetic polymorphisms that may or may not be associated with greater susceptibility to reduced survival of dental implants and, consequently, to their loss. Case-control studies that fully portrayed the specific types of genetic polymorphisms that may be associated with dental implant failure were included by searching in the PubMed, Scopus, and Web of Science databases from 2010 to 2023. The following descriptors and their combinations in English were used to search for articles: "dental implants," "bone genes," "genetics," "polymorphism genetics," "genetic risk factor," and "interleukin." The initial search resulted in 107 results (PubMed n = 47, Scopus n = 14, and Web of Science n = 46). After a manual search, reviewing each article's title and abstract, and excluding duplicates, systematic reviews, and literature reviews, 30 articles were selected. After reading these 30 articles in full, 18 studies that did not describe the specific genetic polymorphism in relation to dental implant survival were excluded. Therefore, 12 articles were included in this review. The genetic polymorphisms of interleukin (IL)-1A, IL-1B, IL-4, IL-6, IL-10, IL-1 receptor antagonist, tumor necrosis factor-α, receptor activator of nuclear factor kappa B legend, and cluster of differentiation 14 were analyzed in the included studies. In seven of the studies, a statistically significant correlation between genetic polymorphisms and dental implant failure was observed. Of the polymorphisms studied, IL-1A (-899), IL-1B (+3954), and IL-4 (+33) showed a greater association with dental implant loss.

RESUMO

Psychiatric disorders are the number one cause of disability in adolescents worldwide. Yet, in low- and middle-income countries (LMIC), where 90% of adolescents reside, mental health services are extremely limited, and the majority do not have access to treatment. Integration of mental health services within primary care of LMICs has been proposed as an efficient and sustainable way to close the adolescent mental health treatment gap. However, there is limited research on how to effectively implement integrated mental health care in LMIC. In the present study, we employed Implementation Mapping to develop a multilevel strategy for integrating adolescent depression services within primary care clinics of Maputo, Mozambique. Both in-person and virtual approaches for Implementation Mapping activities were used to support an international implementation planning partnership and promote the engagement of multilevel stakeholders. We identified determinants to implementation of mental health services for adolescents in LMIC across all levels of the Consolidated Framework for Implementation Research, of which of 25% were unique to adolescent-specific services. Through a series of stakeholder workshops focused on implementation strategy selection, prioritization, and specification, we then developed an implementation plan comprising 33 unique strategies that target determinants at the intervention, patient, provider, policy, and community levels. The implementation plan developed in this study will be evaluated for delivering adolescent depression services in Mozambican primary care and may serve as a model for other low-resource settings.

RESUMO

This study aimed to evaluate the clinical, epidemiological, and histopathological aspects of canine breast tumors at the Federal Rural University of Rio de Janeiro Veterinary Hospital (VH-UFRRJ) between April 2017 and October 2018. The study included 137 female canine dogs with mammary neoplasia who underwent a mastectomy. The animals were evaluated using a standard form that guided anamnesis and physical examination to assess the tumor's epidemiology, development, evolution, treatment choice, and disease prognosis. Furthermore, laboratory and imaging tests were performed on the animals to search for changes that suggested metastasis. The average age of the female dogs was 10 years, and mixed-breed dogs (33%) were the most affected, followed by poodles (21%). The inguinal (79%) and caudal abdominal (64%) breasts were the most affected. Approximately 26% of the animals were castrated, 32% had already reproduced throughout their lives, 30% had pseudocyesis, and only 8% used contraceptive methods. The majority were classified as stage I (33%) and stage III (39%) according to the Tumor-Node-Metastasis (TNM) Classification System of Malignant Tumors. In 95 female dogs, 191 nodules were histopathologically evaluated, and 62% of these animals had at least one malignant tumor. Statistically, there was a direct association between tumor size and malignancy. However, other associations, such as age at diagnosis and the degree of malignancy, were not confirmed. Therefore, it can be concluded that tumor size is an important prognostic factor, with tumors >3 cm having an approximately 70% chance of being malignant.

This randomized clinical trial aimed to evaluate different fluid therapy protocols associated with inhalational or total intravenous anesthesia in the cardiorespiratory stability of bitches with sepsis subjected to a surgical procedure to control the infectious focus. Thirty-two bitches diagnosed with pyometra and sepsis and treated at the University Veterinary Hospital between 2018 and 2019 were recruited. After admission, diagnosis, clinical, and laboratory evaluation, patients were randomly distributed into the following groups: propofol 5 (P[5]: preoperative restrictive fluid therapy-5mL/kg/h and intravenous general anesthesia); propofol 10 (P[10]: preoperative liberal fluid therapy-10mL/kg/h and intravenous general anesthesia); and isoflurane 5 (I[5]: preoperative restrictive fluid therapy-5mL/kg/h and inhalational general anesthesia). Lactate on admission (LAC1) and release (LAC2), heart rate (HR), respiratory rate (RR), systolic blood pressure (SBP), rectal temperature (RT), oxygen saturation (SpO2), and carbon dioxide extraction rate (EtCO2) were analyzed at PRE, T10, T20, T30, T40, T50, TEXT, and TDIS. Clearance of 20% of lactate occurred in 18 dogs, with the P[10] group displaying the best performance. There was no statistical difference in vasopressor requirements among the groups. Liberal fluid therapy showed greater cardiovascular stability than restrictive therapy in the perioperative period. Regarding general anesthesia, isoflurane showed greater cardiorespiratory stability than propofol during anesthetic maintenance. In conclusion, although the three proposed protocols are safe and there is no difference in their superiority, some observed changes may be relevant and considered when it is possible to individualize the therapy for the patient.

This retrospective study evaluated the impact of the clinical presentation of pyometra on the morbidity and mortality of the affected bitches. The medical records of 47 bitches surgically treated for pyometra between May and December 2017 were evaluated. The admission data were analyzed to classify the animals according to the clinical presentation of pyometra, presence of systemic inflammatory response syndrome (SIRS) and organic dysfunctions (cardiovascular, neurological, renal, and hemostatic). The outcomes were recorded for up to 14 days. The overall mortality rate was 13%. Open and closed pyometra were identified in 62% (n= 29) and 38% (n= 18) of the cases, respectively. The SIRS was present in 44 (94%) bitches, and 27 (57%) had at least one organ dysfunction. The most prevalent dysfunction was hemostatic, followed by neurological, cardiovascular, and renal dysfunction. The clinical presentation of pyometra and the presence of SIRS did not correlate with each other, or with the presence of organ dysfunction and mortality. However, cumulative organ dysfunction (r=0.532, P=<0.001) and the presence of neurological (r= -0.371; P=0.012) and renal dysfunction (r=0.303; P=0.041) were correlated with mortality. Thus, the identification of organ dysfunction through physical examination and routine laboratory analysis proved to be more effective in identifying and predicting the critically ill patients than the SIRS criteria or clinical presentation of the disease.

Different anesthetic combinations are used for orchiectomy in cats. This study aimed to evaluate the anesthetic and cardiopulmonary effects on the physiological variables of ketamine (10 mg/kg), midazolam (0.2 mg/kg) and methadone (0.3 mg/kg), combined with local anesthesia, in cats undergoing orchiectomy (n = 19 cats). The time for lateral recumbency, degree of sedation, muscle relaxation and nociception were recorded preoperatively. The propofol rescue dose was recorded. The time to head up and quality of recovery were evaluated postoperatively. The time for lateral recumbency was 5 ± 2 minu-tes. Fifteen minutes after the administration of the ketamine-midazolam-methadone combination, a greater sedative effect, muscle relaxation and less response to noxious stimulation were observed. Propofol was administered to twelve cats under local anesthesia, at a total dose of 1.5 ± 0.8 mg/kg. Surgery was started 28 ± 5 minutes after the administration of ketamine--midazolam-methadone combination. There were no differences in the physiological variables evaluated over the other evalu-ation times (p > 0.05). The recovery quality scores were adequate, and the time to head up was 51 ± 10 minutes. Under the conditions of this study, the ketamine-midazolam-methadone combination did not allow local anesthesia for orchiectomy. Many cats required propofol rescue prior to surgery. This combination promoted minimal changes in physiological variables and prolonged anesthetic recovery.(AU)

Neurological disorders are frequent in small animal veterinary clinics and studies giving regional justification for such disorders are essential so that veterinarians can be better prepared. The objective of this retrospective study was to describe the clinical and epidemiological aspects of dogs with neurological disorders treated at the Veterinary Hospital of Federal Rural University of Rio de Janeiro, between August 2017 and April 2018. The study describes the location of the lesions and their etiology according to the acronym VITAMIN-D, as well as the distribution of cases according to sex, breed and age. One hundred and twenty-two (122) dogs were included, of which 58% (n=71) were males and 42% (n=51) females. The age distribution ranged from two months to 18 years old; 16% were less than two years old, 50% between two and seven years old and 34% were more than seven years old. The most affected breeds were Mixed Breed dogs (43%), Poodles (9%) and Dachshunds (7%). The diagnoses were confirmed in 54 animals (44%) and in 68 (56%) remained presumptive. The most common location was the spinal cord (61%), where the T3-L3 region was the most affected (35%), followed by multifocal (20%) and forebrain (10%). The most frequent categories were degenerative (24%), inflammatory/infectious (23%) and traumatic (20%) diseases. Intervertebral disc disease was the most observed, corresponding to 21.3% of all cases. The results showed that the neurological diseases in this Veterinary Hospital were more prevalent in young male adults with spinal cord diseases of degenerative nature.(AU)

Accelerated population aging is in force and requires vigilance in health models, especially in Primary Care (PC). However, due to the intense population demand for the use of health services, it is inevitable to use other policies and programs such as: home care services. Care is performed through a multidisciplinary team, using actions that seek promotion, prevention, and rehabilitation, to positively interfere in the client's quality of life. Thus, it is necessary the performance of nursing professionals, who have a theoretical-practical basis, offering an individualized and continuous care. Thus, the study aims to identify how home nursing care is provided in the case of elderly people with injury pressure. This is a narrative literature review, carried out in LILACS, VHL and BDENF databases, through the search strategy "Elderly's Health" AND "Pressure Injury" AND "Home Care Services". The search returned 17 articles, and after applying the eligibility criteria (full articles available, published between 2016 and 2021, Portuguese language) 6 articles were analyzed for the final composition of the sample. There was a lack of specific actions directed to the user individualities, lack of training on the part of health professionals, lack of guidance and health education to caregivers, insufficiencies of material resources and services provided, needs for actions aimed at prevention, since the cost to services would be lower than carrying out recovery and rehabilitation strategies.

Background: Urethral tumors are uncommon in canines, represented predominantly by transitional cell carcinoma, marked by aggressive behavior associated with short life expectancy. Definitive diagnosis is achieved by histopathological analysis. Surgery associated with chemotherapy is the main therapeutic alternative. The aim of this paper is to report a case of primary transitional cell carcinoma of the urethra in a bitch, submitted to surgical treatment associated with conventional adjuvant chemotherapy and metronomic chemotherapy, achieving survival of 21 months, to date. Case: A 12-year-old bitch mixed breed was admitted at the Veterinary Hospital of Federal Rural University of Rio de Janeiro (UFRRJ), manifesting hematuria and urinary incontinence for one month. Physical examination indicated that vital parameters were within normal limits. Laboratory tests of blood count and serum biochemistry, urinalysis, culture, urinary antibiogram, and abdominal ultrasound were performed. The ultrasound image suggested a neoplastic urethral process. Because of the suspicion of neoplasm, a thoracic X-ray was performed, showing no evidence of metastasis. Computed tomography (CT) of the abdominal region was performed, revealing an expansive lesion in the urethra with loss of definition of the walls and urethral lumen extending to the level of the pelvic floor, measuring about 2.9 x 1.4 x 1.2 cm. After preanesthetic exams, the animal underwent resection and surgical anastomosis of the affected urethral region. The sectioned tissue was sent for histopathological analysis, with diagnosis of transitional cell carcinoma. Adjuvant chemotherapy was performed using carboplatin at a dose of 250 mg/m², intravenously, every 21 days for 6 sessions. After completing the protocol, abdominal ultrasonography was performed again, which showed a cicatricial process in the urethral region of the surgical excision, with no sign of tumor recurrence. A metronomic chemotherapy protocol was then started with cyclophosphamide at a dose of 15 mg/m², daily for 6 months, with periodic oncological follow-up. At the end of the period, the animal remained under periodic follow-up with ultrasound exams performed at 2-month intervals, and has been free from recurrence for 21 months until now. Discussion: Urethral neoplasms account for 0.5 - 2% of all canine tumors, and are represented mostly by transitional cell carcinomas. Clinical signs of urinary obstruction and hematuria are the most common, as reported in the patient in question. The definitive diagnosis is by histopathological examination. However, imaging tests such as computed tomography are important in the initial investigation and surgical planning. The indicated treatment is surgical resection of the mass with subsequent adjuvant chemotherapy or radiotherapy. The chemotherapy protocol associating cisplatin and piroxicam can achieve remission rates of 70%, but this association has a high nephrotoxic effect. In the present case, cisplatin was replaced by carboplatin in order to reduce the nephrotoxic effects of chemotherapy. The metronomic protocol using cyclophosphamide was used as maintenance therapy, at the end of the conventional chemotherapy protocol. Survival achieved was longer than that described in the literature in cases of transitional cell carcinomas in the urethral region. Therefore, multimodal therapy using surgery associated with conventional and metronomic chemotherapy, is an option in canines with urethral carcinoma.

RESUMO

Background: Fibrosarcomas are malignant neoplasms of mesenchymal origin and can have different symptoms depending on the species, age, location and etiopathogenesis. Intestinal tumors in domestic cats are common and the small intestine is the most common site; however, fibrosarcomas are rare in the intestine of all animal species. This work reports intestinal fibrosarcoma in 2 domestic cats and aims to clarify and present information concerning this neoplastic type in the gastrointestinal tract of this species. Cases: We report 2 cases of intestinal fibrosarcoma in domestic felines (Felis catus). Cat 1. A 14-year-old female Persian breed, domestic cat, was taken to the Feline Sector of the Veterinary Hospital of Small Animals (HVPA) of the Federal Rural University of Rio de Janeiro (UFRRJ). The main complaint was chronic constipation and rectal prolapse. The clinical examination revealed an ulcerated mass, measuring 4.0 cm x 1.7 cm. Cat 2. A 10-year-old female undefined breed, domestic cat, was taken to the private clinic. The main complaint was diarrhea with bloody and rectal prolapse. The clinical examination revealed nodule measuring 2.5 cm in diameter. The surgical option decided upon was to use the rectal pull-through technique in both animals. The patients had no trans-surgical or postoperative complications. The material collected during the surgical interventions was analyzed macroscopically and fixed in 10% buffered formalin for 24 h and then sent to the Histopathology Laboratory of the Pathological Anatomy Sector (SAP) at UFRRJ for the cat 1 and in private laboratory for the cat 2. After fixation, it was cleaved for routine microscope exam using Hematoxylin and Eosin (HE) stains and for the histochemical method of Masson's Trichrome staining technique. Complementary immunohistochemistry tests and electron microscopy were also performed. The patients were followed up clinically, showing complete remission of the clinical signs and survival for approximately 1 year after the neoplastic resection. Discussion: There are few reports of intestinal fibrosarcomas in veterinary medicine, therefore, little is known about racial predilection, age, sex or biological behavior. As far as these authors know, this is the 6th and 7th report of this neoplasm with a primary site in the large intestine in this species. The morphological diagnosis of fibrosarcoma is relatively simple, whereas, in some cases the differential diagnosis for tumors of the peripheral nerve sheath, leiomyosarcomas and gastrointestinal stromal tumor (GIST) can be extremely difficult. The immunohistochemistry technique in these cases may not be particularly useful. The fibrosarcoma diagnosis was also confirmed by electron microscopy since no evidence was found that could lead to a neuronal origin, thus excluding tumors such as neurofibrosarcoma and schawnoma, corroborating the immunohistochemical examination. The surgical management of tumor resection with wide safety margins (minimum 2 cm) remains the "gold standard" therapy for dealing with fibrosarcomas since they have a low response rate to chemotherapy and radiotherapy and the use of these therapies as an adjuvant is controversial. The advantages of this technique are related to the surgical time, simplicity, easy access and reduction in the risk of abdominal contamination. Histopathological, immunohistochemistry and electron microscopy evaluations were sufficient to enable the diagnosis of an intestinal fibrosarcoma in both cats. The occurrence of this neoplasm with intestinal involvement in the feline species is rare; therefore, this description is important as it provides information about epidemiology, associated signs, differential diagnoses, biological behavior, treatment and prognosis.

RESUMO

The inverted ovariosalpingohysterectomy (OSH) technique involves initial uterine removal, followed by ovarian disinsertion. The objective of this study was to compare the inverted technique with the traditional technique, characterized by initial removal of the ovaries followed by uterine resection, for evaluating the performance of the inverted OSH technique in cats. For this, 24 healthy, female cats, aged between 7 months and 5 years, with a body condition score of up to 6 points (9-point scale), divided into two equal groups: the traditional technique (TT) and the inverted technique (IT). Procedures were performed by the same veterinarians (surgeon and their assistant) as well as semi-quantitative analysis of the techniques (questionnaire related to the ease of uterine and ovarian manipulation as well as of intestinal and urinary vesicle manipulation) and descriptive qualitative analysis (technical report with the opinion of surgeons and an external evaluator). In addition, the total surgical time and positive pain response were evaluated, indicated by the changes in vital parameters observed by the anesthesiologist during surgery. Inverted technique it promoted greater ovarian exposure, allowing uterine cranial flexion to perform the ligatures, thus facilitating their manipulation, hemostasis, and section of the pedicle. Free the uterus from its ligaments to the abdominal cavity allowed greater mobilization of the conjoint. Both techniques required short operation time (average time of 16’± 1.321’ in the IT group and 15’± 0.525’ in the TT group). The presence of algetic stimulus was similar in the techniques, three animals in each group suffered analgesic rescue.

Background: Anal atresia is a congenital malformation, which often affects calves, and is related to the imperforation of the membrane that separates the endoderm of the posterior intestine from the ectodermal anal membrane. It is commonly associated with other congenital malformations and skeletal anomalies. The clinical signs generally appear in the first days of the animal's life, due to a retention of feces. The diagnosis is clinical and is based on observation, anamnesis and a physical examination of the animal. The only viable treatment is surgical. This paper aims to report 6 cases of anal atresia in bovine calves, 4 males and 2 females that were successfully treated surgically. Cases: This work reports 6 cases of anal atresia in 4 male calves and 2 female calves. Three presented total atresia (type II), one partial (type I) and in both female calves, anal atresia and rectovaginal fistula (type IV) were observed. The animals were all of undefined race. Five of the cases were from northern Tocantins, 4 males and 1 female (anal atresia with rectovaginal fistula), and 1 female (anal atresia with rectovaginal fistula) was from Valença, RJ. All animals were born active, by eutocic/natural birth, and assumed a quadrupedal position followed by the first feeding as normal. They were aged between 2 days and 6 months, and had a clinical history of abdominal distention and difficulty or inability defecating, and the females both also had a rectovaginal fistula, all cases compatible with anal atresia. Based on the patient's history and clinical examination, surgical treatment for anal reconstruction was decided upon. Postoperative treatment consisted of enrofloxacin [2.5 mg/kg - intramuscularly (IM), once daily (SID), for 5 days] and fluxinin meglumine [1.1 mg/kg - IM, SID, for 3 days] ; as well as a healing ointment which was applied to the area of the surgical wound, every 12 h, for 7 days. There were no trans-surgical complications. The animals showed progressive recovery after anal reconstruction and the stitches were removed in all cases on the 10th postoperative day, with no postoperative complications and no recurrence of any clinical signs from that moment on. Discussion: The study of congenital and hereditary changes enables the identification of their origins, can help prevent new cases and, some of them, are open to economically viable treatment and/or correction that can improve the well-being of the animal and prevent economic losses due to death or animal sacrifice, as reported in the present study. Anal atresia is the most common congenital defect of the lower gastrointestinal tract in calves, being an isolated abnormality, or associated with other malformations, especially of the distal spinal column such as the absence of a tail (perosomus acaudatus), as one of the animals in this study. The clinical signs and physical examination are sufficient to establish the diagnosis, as demonstrated in this report, which is usually made in newborn animals, due to the lack or difficulty in defecation associated with no anal orifice and/or swelling in the perineal region. The treatment of choice for anal atresia is surgical, in order to construct an anal neo-orifice and thus avoid endotoxemic shock as well as providing relief and well-being for the animals. As observed in this study, when anal atresia is diagnosed early, and surgical treatment is properly instituted, the prognosis is favorable. The surgery is considered of low complexity, quick and it can be carried out in the field. Thus, from a commercial point of view, considering the costs of the procedures and the value of the calf at the end of weaning, such treatments are beneficial to the owners. In addition, the surgical treatment is essential for animal health and welfare in cases of anal atresia.

RESUMO

Background: The ovariosalpingohysterectomy (OSH) is one of the most performed surgeries in bitches often used in population control. Surgical techniques that decrease the chances of operative complications and cause less nociceptive stimulation and acute pain to the patient are highly sought after by veterinarians. We evaluated the inverted OSH technique (IT), which, after celiotomy, begins with the ligature execution, transection of the uterine body and subsequent ligation and ovarian disinsertion. The purpose of this investigation was to analyze aspects of IT execution in bitches, such as visceral manipulation, access to the uterus and ovaries, and surgical time compared to the traditional technique. Materials, Methods & Results: Inverted OSH was compared to the so-called traditional OSH technique, characterized by the initial removal of the ovaries followed by uterine section. The OSH techniques were performed in 24 healthy animals, with a body condition score of up to six (on a nine-point scale) in dogs aged between seven months and five years. All dogs were from the Birth Control Program of Dogs and Cats of the UFRRJ and were distributed into two groups: traditional technique (TT) and inverted technique (IT). Surgeries and anesthesia were carried out by the same veterinarian crew (surgeon, surgical assistant, and anesthesiologist). OSH techniques were evaluated by the surgeon and surgical assistant using semiquantitative analysis (multiple-choice questionnaire with closed questions that pointed scores related to the ease of uterine and ovarian manipulation, as well as to the level of manipulation of the intestinal and urinary vesicle) and qualitative descriptive methods (report of techniques with the opinion of surgeons and an external evaluator-veterinarian surgeon who attended surgeries). Also, the total surgical time and the indication of a positive analgesic response according to changes in the vital parameters observed by the anesthesiologist during specific moments of the operation were

RESUMO

Background: Flexural deformities are anatomical deviations, in varying degrees, of one or more joints, and may have a congenital or acquired origin. Congenital contracture of the deep digital flexor tendon (DDFT) affects newborn calves and results in hyperflexion of the metacarpal-phalangeal joint, which in severe cases means that the animal must support its own weight on its fetlock joints. The aim of this study is to report the rapid and successful result of applying bilateral total tenotomy technique on a newborn bovine that had been diagnosed with severe bilateral DDFT contracture in the thoracic limbs. Case: A 3-week-old male bovine with a history of difficulty in maintaining a quadrupedal position was attended at the Veterinary Hospital of the Federal University of Tocantins. The animal had severe bilateral locomotor alterations in the thoracic members to the point that he walked on his fetlock joints. The physical examination revealed clinical parameters within the normal range. However, the locomotor system examination showed severe flexor deformity in the bilateral metacarpal-phalangeal joints, and the limbs were being supported on the dorsal face of the fetlock joints, which presented ulcerations, and was suggestive of a shortening of the DDFT. Based on the patient’s history and clinical examination, as well as the severity of the tendon contractures, surgical treatment using the DDFT bilateral total tenotomy technique was decided upon. Postoperative treatment consisted of 2.5 mg/kg of enrofloxacin intramuscularly (IM), SID, for 5 days and 0.5 mg/kg meloxicam via IM, SID, for 3 days; as well as a dressing (cotton, medical bandage and a PVC mold) on the thoracic limbs to provide support and allow the animal to..
