RESUMO

Pleural effusion in patients with liver cirrhosis and intractable ascites is well known, but hepatic hydrothorax in the absence of ascites is a rare complication. We present the case of a 43-year old male, with a medical history of liver cirrhosis due to hepatitis C virus, who was admitted to the Pneumology Clinic for dyspnoea, worsening of general status and chronic asthenia. The pleural effusion, revealed on physical and laboratory examinations, persisted despite the therapy with diuretics and the frequent thoracocentesis. The thoracostomy followed by pleurodesis also failed. The pecularity of this case was the presence of refractory hydrothorax in the absence of ascites.

RESUMO

We present a 34 year old patient, intravenous drug user, hospitalized with fever, distortion of general status, dry irritating cough, abdominal colicative pains, and we established the diagnosis of HIV infection advanced stage/AIDS; his antecedents revealed (August 2000) abdominal tuberculosis not treated during the last 3 months. He presented a pneumonia with Pneumocystis carinii during hospitalization. Death was due to a colon perforation with secundary peritonitis. Miliary tuberculous lesions in liver, spleen and colon were revealed at necropsy and cytomegalovirus was identified in necrotic samples also.

RESUMO

The Wegener's granulomatosis is characterized by the involvement of the upper airways, the lower airways and the late involvement of the kidneys. The authors are presenting in this article, the case of a 44 years old man, which presented the three conditions mentioned above, with an early kidney involvement and who died due to a major complication of the disease: the intravascular disiminated coagulation and a multivisceral vascular involvement, despite the correct and early treatment.

RESUMO

Primary pleuro-pulmonary non-Hodgkin lymphomas are very rare tumors, possibly under diagnosed, with a very long, frequently asymptomatic evolution. The authors present the diagnostic difficulties of such pathology, in a particular case, which was diagnosed only by open biopsy and immunohistochemical analysis of tissue fragments. Chemotherapy and radiotherapy determined a favorable course of the disease.
