In this study, representative urban and peri-urban Indian food baskets have been studied for the presence of toxic and essential elements. The concentration of target toxic and essential elements was used to estimate dietary intakes (EDIs) and health risks. Across all food matrices, toxic elements like Cd and Pb were dominant. The highest concentrations of the target elements were found in vegetables, with Cd, Pb, and Ni being beyond permissible limits of the Food and Agriculture Organization of the United Nations and the World Health organization (0.05 mg/kg, 0.1 mg/kg, and 1.5 mg/kg, respectively) in okra, spinach, and cauliflower. The sum of concentrations of the toxic elements (As, Ni, Hg, Cr, Cd, Pb) in vegetables had a range of 0.54-12.08 mg/kg, the highest sum was found in spinach (median 12.08 mg/kg), followed by okra (median 1.68 mg/kg). The EDI was observed for vegetables with a contribution as high as 92% for Cd. Dairy products were found with the highest loading for Ni with a dietary intake of 3.1 mg/kg/day for adults and twice as much for children. Carcinogenic risk for Ni was the highest and found above the threshold for all food categories, as was the case with As. Cumulative carcinogenic and non-carcinogenic risks were mostly contributed by milk and vegetables, in particular, spinach.

Mineral bottled water packed in three polymers viz., virgin polyethylene terephthalate (PET), recycled PET, and low-density polyethylene (LDPE) were investigated for the occurrence, migration, and health risk of phthalic acid esters (PAEs) at 25 °C, 35 °C, and 45 °C. The average concentration of six USEPA priority PAEs in refrigerated water samples was highest in recycled PET> LDPE > virgin PET. The highest leaching was seen at 45 °C after 2 days for LDPE water packets with ∑6PAEs amounting to 64,300 ng/L. Similarly, for recycled PET, the highest migration was seen at 45 °C after seven days (3,800 µg/L). Bis 2-ethyl hexyl phthalate (DEHP) and di-n-butyl phthalate (DnBP) were the predominant plasticizers from PET bottles and LDPE water packets, respectively. Predicted concentration after three weeks based on best fit obtained through the polynomial model for PET bottles was seen higher than the recommended limit suggested by USEPA (6 µg/L) and WHO (8 µg/L).

Improper handling of plastic waste and related chemical pollution has garnered much attention in recent years owing to the associated detrimental impacts on human health and the environment. This article reports an overview of the main interlinkages between persistent organic pollutants (POPs) and plastic in the waste management system of India. Both plastics and POPs share certain common traits such as persistence, resistance to biological degradation, and the ability to get transported over long distances. Throughout the processes of production, consumption, and disposal, plastics interact with and accumulate POPs through several mechanisms and end up co-existing in the environment. Plastic waste can undergo long-range transport through rivers and the oceans, break down into microplastics and get transported through the air, or remain locked in waste dump yards and landfills. Over time, environmental processes lead to the leaching and release of accumulated POPs from these plastic wastes. Plastic recycling in the Indian informal sector including smelting, scrubbing, and shredding of plastic waste, is also a potential major POPs source that demands further investigation. The presence of POPs in plastic waste and their fate in the plastic recycling process have not yet been elucidated. By enhancing our understanding of these processes, this paper may aid policy decisions to combat the release of POPs from different waste types and processes in India.

OBJECTIVE. To effectively combine functional MRI (fMRI), diffusion tensor tractography (both guided by neuronavigation) along with cortical stimulation (CS) for surgery of eloquent cortex (EC) lesions. MATERIALS AND METHOds. Fifteen patients with lesions adjacent to the eloquent motor and sensory cortex were included. Preoperative fMRI and diffusion tensor imaging were performed and then integrated into the neuronavigation system. Intraoperative CS of sensory/motor cortex was performed to localize the EC under awake condition and this was correlated with areas active on fMRI utilizing neuronavigation. For excision of the deeper structures, CS, and tractography guided by neuronavigation were utilized. RESULTS. A total of 127 cortical sites were evaluated with CS in 15 patients. The overall sensitivity, specificity, and accuracy of fMRI were 79%, 85%, and 82%, respectively, keeping the areas positive on CS as a referential parameter. Tractography helped in resecting the deeper areas of the tumor, but was not very accurate due to brain shift. However, it was useful in roughly assessing the deeper areas close to the long tracts. The risk of developing persistent neurological deficits was 6%. Pathologies included gliomas in ten patients, cavernous malformation in two patients, meningioma in one patient, and focal cortical dysplasia and Dysembryonic neuroepithelial tumor in one patient each. Near total excision was achieved in 7/10 (> 95% excision) gliomas and a total excision in all others lesions. CONCLUSIONS. Lesions directly over the EC present a special surgical challenge. The challenge lies in excising these lesions without producing any deficits. These goals may be achieved better by combined use of multimodal neuronavigation (fMRI and tractography) and intraoperative mapping with CS under awake conditions.

The behavior of clinical preceptors plays a key role in the learning process of students. This study explored preceptors' perceptions of the clinical teaching-learning process of final-year undergraduate nursing students. After ethical permission was obtained, a 30-item questionnaire was administered to 76 preceptors at Sultan Qaboos University Hospital in 2011. Most preceptors had more than 10 years of experience. The majority (87%) of preceptors rated students' response to constructive feedback positively, and 75% evaluated students' professional behaviors and communication positively. Several factors were identified as interfering with preceptorship, including lack of protected time and heavy workload of preceptors, poor correlation of theory and practice, and lack of interest in direct patient care by students. Most preceptors identified factors that facilitate preceptorship as students' commitment to direct patient care and consistent shift duty with the assigned preceptor during the entire clinical rotation. These results may help faculty appreciate preceptors' views and acknowledge areas of success, as well as areas that need improvement.

Two strains (JC85(T) and JC108) of Gram-stain-negative, motile bacteria were isolated from endolithic beach sand samples on an oligotrophic medium. Based on the 16S rRNA gene sequence analysis, both strains were identified as belonging to the genus Rhizobium. Strain JC108 had 16S rRNA gene sequence similarity of 100 % with Rhizobium pusense NRCPB10(T) and formed a cluster with this strain. Strain JC85(T) had 96.9 % 16S rRNA gene sequence similarity and was 18 % related (based on DNA-DNA hybridization) to Rhizobium borbori DN316(T). With other strains of the genus Rhizobium, the 16S rRNA gene sequence similarity was less than 96.3 %. Strain JC85(T) could tolerate up to 3 % salinity, fix N(2), was resistant to ampicillin (10 µg) and was positive for catalase and oxidase. The major fatty acid was C(18 : 1)ω7c (69 %) with minor amounts of C(19 : 0) cyclo ω8c (8.9 %), C(16 : 0) (6.9 %), C(12 : 0) (5.7 %) and C(19 : 1)ω7c/C(19 : 1)ω6c (2.2 %). Polar lipids of strain JC85(T) include two unidentified aminophospholipids (APL1,2), two unidentified phospholipids (PL1,2), phosphatidylcholine and four unidentified lipids (L1-4). Q-10 is the major quinone of strain JC85(T). Based on polyphasic taxonomic analysis, strain JC85(T) represents a novel species for which, the name Rhizobium subbaraonis JC85(T) is proposed. The type strain is JC85(T) ( = DSM 24765(T) = KCTC 23614(T)).

Bacterial infections have a large impact on public health. Through this study, we report on the development of complementary split-ring resonators (CSRR) supplemented by functionalized nanoparticles to detect bacteria in the aqueous medium. Iron oxide (Fe3O4) nanoparticles were functionalized with amino groups using (3-aminopropyl) triethoxysilane (APTES) to form (APTES@Fe3O4) nanoparticles, which have a specific affinity towards the bacterial species. This affinity was evaluated using theEscherichia coli (E. coli)andStaphylococcus aureus (S. aureus)bacterial species. The resonant sensor was tuned at 430 MHz and the CSRR sensor bed was further activated using APTES@Fe3O4nanoparticles. Bacterial detection was studied over a range of concentrations from 2.66 × 109cells to 2.66 × 108cells. The sensor actively responded to small changes in bacterial concentration, showing an overall shift in resonance frequency of âˆ¼44 MHz (∼40 MHz/cell count) forE. coliand âˆ¼55 MHz (50.43 MHz/cell count) forS. aureus. Dextran sulphate and Chitosan were used as the references. The magnetic character of the conjugated system exhibited strong interaction of the bacterial species with APTES@Fe3O4, justifying the high selectivity towards these species. This demonstrates the feasibility of a sensitive, fast, portable device, against the traditionally used time-consuming bio-assays.

Endocrine-disrupting chemicals (EDCs) such as phthalic acid esters (PAEs) and bisphenol A (BPA) are the most widely used plastic additives in polymeric materials. These EDCs are ubiquitously distributed in the environment. Hence selected PAEs and BPA were investigated in twenty-five food types and drinking water (supply and packaged) from the metropolitan city, Delhi, and the peri-urban areas of a non-metropolitan city, Dehradun. Except cabbage and orange, the sum of thirteen PAEs (∑13PAEs) and BPA in all the other food types were significantly higher in Delhi over Dehradun (p < 0.01). Highest mean ∑13PAEs (665 ng/g) and BPA (73 ng/g) were observed in cottage cheese and potatoes, respectively followed by fish (PAEs - 477 ng/g, BPA - 16 ng/g). Supply water from the west zone of Delhi was found to contain the highest concentration of BPA (309 ng/L) and ∑13PAEs (5765 ng/L) with the dominance of diethyl phthalate (DEP). Based on the compositional profile and compound-wise principal component analysis, environmental contamination and food processing were attributed as significant sources of most priority PAEs in food samples. Di-ethyl hexyl phthalate (DEHP) was over 100-fold higher in the bottled water from local brands than composite bottled water samples. Packaging material was identified as a source for di-n-butyl phthalate (DnBP) in packaged food. This study observed the highest estimated daily dietary intake (EDI) in the high-fat-containing food products viz., cottage cheese, and fish from north Delhi. High bioaccumulation of BPA can be a possible reason for elevated EDI in vegetables and local fish of Delhi. Unlike Dehradun, EDI for ∑13PAEs and BPA was slightly higher for the non-vegetarian adult when compared to the vegetarian adult. DEHP and DnBP exhibited the highest estimated estrogenic potential for bottled water from local brands. Dietary exposure due to six priority PAEs contamination in food stuffs was two to four-fold higher in Delhi than Dehradun for adult man and woman.

RESUMO

Endocrine-disrupting chemicals (EDCs) are a prime concern for the environment and health globally. Research shows that in developing countries such as India both the environment and human populations are severely exposed to EDCs and consequently experience rising incidents of adverse health effects such as diabetes and cancers. In this paper, we discuss the current EDC management approach in India, critically assess its limitations, and describe opportunities for potential improvements. Foremost, current EDC management actions and interventions in India are fragmented and outdated, and far behind the modern and comprehensive approaches adopted in the European Union and other developed countries. Strong and well-planned actions are required on various fronts of science, policy, commerce, and public engagement. These actions include the adoption of a dedicated and modern regulatory framework for managing EDCs, enhancing capacity and infrastructure for EDC monitoring in the environment and human population, employing public-private partnership programs for not only managing EDCs but also in the sectors that indirectly contribute toward the mismanagement of EDCs in the country, and raising awareness on EDCs and promoting health-preserving consumption habits among the public. As India hosts a large proportion of the global human population and biodiversity, the success or failure of its actions will substantially affect the direction of global efforts to manage EDCs and set an example for other developing countries.

Surveillance of SARS-CoV-2 and organic tracers (OTs) were conducted in the community wastewater of Chennai city and the suburbs, South India, during partial and post lockdown phases (August-September 2020) as a response to the coronavirus disease 2019 (COVID-19) pandemic. Wastewater samples were collected from four sewage treatment plants (STPs), five sewage pumping stations (SPSs) and at different time intervals from a suburban hospital wastewater (HWW). Four different methods of wastewater concentrations viz., composite (COM), supernatant (SUP), sediment (SED), and syringe filtration (SYR) were subjected to quantitative real time-polymerase chain reaction (qRT-PCR). Unlike HWW, STP inlet, sludge and SPS samples were found with higher loading of SARS-CoV-2 by SED followed by SUP method. Given the higher levels of dissolved and suspended solids in STPs and SPSs over HWW, we suspect that this enveloped virus might exhibit the tendency of higher partitioning in solid phase. Cycle threshold (Ct) values were < 30 in 50% of the HWW samples indicating higher viral load from the COVID-19 infected patients. In the STP outlets, a strict decline of biochemical oxygen demand, >95% removal of caffeine, and absence of viral copies reflect the efficiency of the treatment plants in Chennai city. Among the detected OTs, a combination of maximum dynamic range and high concurrence percentage was observed for caffeine and N1 gene of SARS-CoV-2. Hence, we suggest that caffeine can be used as an indicator for the removal of SARS-CoV-2 by STPs. Our predicted estimated number of cases are in line with the available clinical data from the catchments. Densely distributed population of the Koyambedu catchment could be partly responsible for the high proportion of estimated infected individuals during the study period.

Endocrine-disrupting chemicals (EDCs) in diet are a health concern and their monitoring in food has been introduced in the European Union. In developing countries, EDC dietary exposure data are scarce, especially from areas perceived as pollution hotspots, including industrialized countries like India. Several persistent organic pollutants (POPs) act as EDCs and pose a pressure to human health mainly through dietary exposure. In the present study, a range of organochlorine pesticides (OCPs), polychlorinated biphenyls (PCBs), polybrominated diphenyl ethers (PBDEs), dioxins and furans were measured in several food items collected from Indian urban (Delhi) and peri-urban (Dehradun) areas. Food basket contamination data were used to estimate EDC dietary exposure and compare it with that of the average European population estimated from available monitoring data. All the target contaminants were found in most food items, especially in dairies and meat products. OCPs were the main contributers to the measured EDC contamination. Food supplied to Delhi's markets had higher EDC contamination than that supplied to the peri-urban market in Dehradun. Despite lax compliance and control measures, Indian dietary exposure of OCPs and PBDEs were comparable with that of Europe and were lower for PCBs and dioxins. Higher meat consumption in Europe only partly explained this pattern which was driven also by the higher EDC residues in some European food items. A substantial part of endocrine disrupting potential in the diet derives from food and animal feeds internationally traded between developed and developing countries. With increasingly globalized food systems, internationally harmonized policies on EDC content in food can lead to better protection of health in both these contexts.

Detection of pesticide residues from fruits and vegetables is of significant importance to ensuring human health and environmental safety. An efficient and tunable shape-selective synthesis of Ag/CeO2 nanostructures as an active flexible SERS substrate for the detection of thiram on an apple surface via a paste, peel off, and paste again process was performed. The well-controlled formation of silver assembled CeO2 microspheres constituting nanospheres and nanospindles with an average size of approximately 56 and 32 nm with anisotropic structures has been confirmed through morphological and crystallographic analysis. Interestingly, CeO2 (111) was strongly anchored in the Ag (111) matrix, which provides a more adequate pathway for rapid ion-electron transportation, as observed from the structural and chemical composition analysis. The detection of thiram on the surface of an apple using our proposed nanospindle SERS active substrate achieves a wide detection range from 10-2 to 10-9 M with a correlation coefficient of 0.9929 and a low detection limit of 27 nM at S/N = 3. In addition, the charge transfer mechanism between the Ag/CeO2 nanostructures and thiram molecules has also been proposed. We believe that the present work could provide novel ways to develop SERS active substrates for highly efficient onsite detection of pesticides on fruits in the near future.

BACKGROUND: Oral cancer is the most frequent type of cancer of the head and neck area, with squamous cell carcinoma (SCC) being the most common single entity. Worldwide, oral cancer accounts for 2%-4% of all cancer cases, the prevalence being highest in India. Lymph node metastases occur in about 40% of patients with oral cancer. Clinically, their manifestations are hidden in rates of 15% to 34%. More accurate imaging techniques can reduce the risk of undiagnosed metastasis. Ultrasonography has gained wide acceptance as a diagnostic aid in the evaluation of reactive and metastatic lymph nodes. The present study is an attempt to assess the earliest evaluation of the cervical lymph nodes by ultrasound-guided fine-needle aspiration cytology (FNAC). METHODOLOGY: A descriptive diagnostic evaluation study was carried out to find out the sensitivity and specificity of ultrasound-guided FNAC in detecting metastasis to cervical lymph nodes from oral SCC in the Department of Oral Medicine and Radiology, Government Dental College, Thiruvananthapuram, in collaboration with the Department of Imageology, Regional Cancer Centre, Thiruvananthapuram during the time period from July 2015 to September 2016. RESULTS: A total of 112 patients with histologically proven oral SCC having palpable lymph nodes were evaluated clinically and ultrasonographically. In this study, sensitivity and specificity of >90% were obtained for ultrasonographic criteria such as the long axis to short axis ratio <2, the absence of hilum, heterogeneous architecture, and altered vascularity in the evaluation of metastatic lymph nodes. The diagnostic yield in the detection of metastatic lymph nodes was much higher in the ultrasonographic examination. CONCLUSION: Ultrasound-guided FNAC offers an opportunity to enhance patient prognosis through early detection and a specific diagnosis (92.5%) when compared to clinical examination (78.6%) in the current study.

Human-induced land use changes like agriculture and urban development occur commonly at the cost of natural wetlands, which require immediate attention and sustained efforts from urban planners and concerned governments. In view of the East Kolkata Wetlands (EKW), India, this paper has examined the trend, causes, extent and impacts of landscape transformation during the last few decades. Hence we aim to present a review of several studies in and around EKW and suggests a sustainable model for EKW. This 'Ramsar site of International Importance' is one of the world's largest wastewater-fed aquaculture systems where the city sewage is recycled for pisciculture and agriculture, and acts as flood resistance for the city of Kolkata. Rapid encroachment in the EKW due to unplanned development has caused various environmental, social and economic impacts. The key ecological impacts found in the study included loss of faunal and floral diversity, eco-toxicological impacts, etc. In terms of biodiversity loss, while the number of bird species in the EKW decreased from 248 in 1960s to 72 in 2005, about 59% fish species are near threats, vulnerable and endangered. Reported persistent organic pollutants (POPs) around the EKW region were found to pose deleterious impact on environment and ambient biota. Several studies have evidenced that the open burning of solid waste in the dumping ground of Dhapa is a potential source of polychlorinated biphenyls (PCBs) and polychlorinated dibenzo dioxin/furans (PCDD/Fs). Further the ecological transformation of the EKW focussing on policy development and regulatory compliance issues has been studied in this paper. The aspects of encroachment and wetland shrinkage triggered by economic drivers like urban sprawl have been analyzed. The paper presents adaptive co-management as the future roadmap for action to curb further degradation of the wetlands, and adopt an integrated approach of environmental, political, physical, economic and institutional aspects. It is envisaged that the adaptive co-management model will contribute to long-term sustainable sustenance of this precious ecosystem.

BACKGROUND AND AIMS: Disparity in spread of spinal anesthesia is a known complication in scoliosis patients. Our primary aim was to compare this disparity based on Cobb Angle and thorocolumbar spine curvature. Secondary aim was to calculate the appropriate lateral angulation of the spinal needle from midline for successful lumbar puncture. MATERIALS AND METHODS: All poliomyelitis patients with scoliosis posted for lower limb orthopedic contracture release surgeries were enrolled into Group A (Cobb Angle <50°), Group B (Cobb Angle >50°), and on thoracolumbar curve into Group R (Right), Group L (Left). Group A, B, R, and L were studied for bilateral spread of spinal anaesthesia. Lateral angle of the spinal needle from midline was noted with Goniometer in groups A and B. Statistical analysis was done using unpaired t test and Chi-square test. RESULTS: Failures in subarachnoid block (SAB) (unilateral anaesthesia/inadequate/patchy block) was significant in Group B (P = 0.033). Segmental disparity in bilateral spread of spinal anaesthesia was significant in Group R with P value of 0.042. Approximate lateral angle for needle in Group A was (4.1 ± 2.45) and in Group B was (9.14 ± 2.45). CONCLUSIONS: The study showed that there was a strong correlation between right-sided thoracolumbar curve and the spread of spinal anesthesia.

Electronic waste (e-waste) has emerged as a global environmental problem because of its massive production volume and un-structured management policy. Since the rate of e-waste accumulation is startling and the combinatorial effects of toxicants are complex, we have investigated six phthalic acid esters (PAEs), bis (2-ethylhexyl) adipate (DEHA)), bisphenol A (BPA), sixteen polycyclic aromatic hydrocarbons (PAHs) and eight heavy metals (HMs) in the surface soil of e-waste recycling workshops and nearby open dumpsites in four metropolitan cities of India viz., New Delhi (north), Kolkata (east), Mumbai (west) and Chennai (south). Average concentration of ∑16PAHs (1259 ng/g), ∑6PAEs (396 ng/g), BPA (140 ng/g) and ∑8HM (1288 mg/kg) in the informal e-waste recycling sites were higher than ∑16PAHs (1029 ng/g), ∑6PAEs (93 ng/g), BPA (121 ng/g) and ∑8HM (675 mg/kg) in dumpsites. Almost 50-90% of BPA, bis (2-ethylhexyl) phthalate (DEHP), ∑7carcPAHs and copper (Cu) were from e-waste sites predominantly from metal recovery sites (EWR). Extensive combustion of e-waste particularly in the EWR sites at New Moore market and Pudupet in Chennai and Wire Lane, Kurla of Mumbai can explain the segregation of diethyl phthalate (DEP), benzyl butyl phthalate (BBP) and carcinogenic PAHs in the first principal component (PC-1). Copper and lead along with highly abundant plasticizers like DEHP, dibutyl phthalate (DBP) and BPA were loaded in PC-2. Combined impact of burning the plastic cables in e-waste and acid leaching process especially at Mandoli in New Delhi might have driven this result. Loading of chrysene, DEHA and low molecular weight (LMW) PAHs mostly in dumpsite soil might have resulted from incomplete combustion of dumped e-waste. Copper was found to exhibit the highest pollution estimated by geo-accumulation index (Igeo). Maximum estimated carcinogenic risk for adults via dermal contact was due to copper, followed by chromium, lead and nickel.

Pharmaceuticals, personal care products (PPCPs), and artificial sweeteners (ASWs) are contaminants of emerging concern commonly found in the aquatic environments. In India, studies reporting environmental occurrence of these contaminants are scarce. In this study, we investigated the occurrence and distribution of 15 PPCPs and five ASWs in the river and groundwater (used untreated as drinking water) at several sites along the Ganges River. Based on the measured groundwater concentrations, we estimated the life-long human health risk from exposure to PPCPs through drinking. In addition, we estimated the risk of exposure to PPCPs and ASWs in the river water for aquatic organisms. The sum of detected PPCPs in the river water ranged between 54.7-826 ng/L, with higher concentrations in the severely anthropogenically influenced middle and lower reaches of the Ganges. The highest concentration among the PPCPs in the river water was of caffeine (743 ng/L). The sum of detected ASWs in river water ranged between 0.2-102 ng/L. Similar to PPCPs, the sum of ASWs in the river water was higher in the middle and lower reaches of the Ganges. In groundwater, the sum of detected PPCPs ranged between 34-293 ng/L, whereas of ASWs ranged between 0.5-25 ng/L. Negligible risk for humans was estimated from PPCPs in the drinking groundwater sources along the Ganges River, whereas moderate risks to PPCPs and ASWs (namely: caffeine, sulfamethoxazole, triclocarban, triclosan, and sucralose) were estimated for aquatic organisms in the Ganges River.

BACKGROUND: Peripheral arterial disease is a significant problem worldwide. In developing countries such as India, the increased incidence of smoking and other forms of nicotine intake has resulted in a large proportion of young individuals with Buerger's disease. The results of surgical and endovascular treatment for this condition have not been very rewarding. Hence, we focused on providing alternative therapies. Neovascularization by autologous bone marrow mononuclear cell transplantation is being tried as an alternative therapeutic option. We have reviewed our series of patients who underwent autologous bone marrow mononuclear cell transplantation during the last 2 years. METHODS: We enrolled 38 patients who were chosen to undergo autologous bone marrow mononuclear cell transplantation for nonreconstructible Buerger's disease. We injected the bone marrow mononuclear cells into the calf muscles of the affected limbs in 36 patients. We monitored ulcer healing, ankle-brachial index (ABI), and transcutaneous oximetry (TcPo(2)) level. RESULTS: No procedurally related complications occurred, although one injected sample of bone marrow aspirate later revealed infestation with Strongyloides stercoralis. Two patients were seropositive on the Venereal Disease Research Laboratory test and were not injected with the bone marrow mononuclear cells. Three patients (12%) underwent major amputations
