RESUMO

Convection-based renal replacement therapies (RRTs) have the potential to improve patient outcomes when compared to diffusion-based RRT such as hemodialysis (HD), but have limited clearance rates. We propose and characterize multipoint dilution hemofiltration (MPD-HF), a purely convective blood purification technology which removes the fundamental filtration limit associated with convective RRT resulting in clearance rates on par with HD. In MPD-HF, filtration of liquid and solutes occurs along the length of the hollow fibers that convey the blood, and substitution fluid is pushed into the fibers at multiple points along their length. Since multiple filtration and dilution steps are contained within one pass of the blood through the hollow fiber, the fraction of fluid that can be filtered may be increased to allow a high clearance rate that removes a wide range of toxins. In vitro tests yielded an average steady-state filtrate fraction of 68%, exceeding commercial HDF cartridge filtrate fractions by a factor of approximately 3. The molecular weights of molecules cleared spans up to the cutoff of 66 kDa for albumin.

RESUMO

Understanding the cortical activity patterns driving dexterous upper limb motion has the potential to benefit a broad clinical population living with limited mobility through the development of novel brain-computer interface (BCI) technology. The present study examines the activity of ensembles of motor cortical neurons recorded using microelectrode arrays in the dominant hemisphere of two BrainGate clinical trial participants with cervical spinal cord injury as they attempted to perform a set of 48 different hand gestures. Although each participant displayed a unique organization of their respective neural latent spaces, it was possible to achieve classification accuracies of ~70% for all 48 gestures (and ~90% for sets of 10). Our results show that single unit ensemble activity recorded in a single hemisphere of human precentral gyrus has the potential to generate a wide range of gesture-related signals across both hands, providing an intuitive and diverse set of potential command signals for intracortical BCI use.

BACKGROUND AND OBJECTIVES: Limitations in manpower in health care facilities, both in civilian and military settings, can severely affect patient safety as well as overall outcomes. Regional anesthesia via neural blockade is an effective means of managing uncontrolled acute pain, which has been associated with cardiopulmonary, endocrine, immunologic, and hematologic derangement in addition to the development of potentially life-threatening coagulopathy. We have designed a remote-controlled injection device that may expedite the performance of regional nerve blocks in these situations. METHODS: This work examines how the device affects the ability of the operator to act independently with respect to various block component times by statistically comparing device-assisted blockade with usual or clinically relevant techniques. The classic or two-person technique was compared with the foot-controlled technique. RESULTS: The results validated the hypothesis that the novel mechanism of performing a nerve block is not inferior to the classic technique with regard to the specified endpoints within our experimental design. CONCLUSION: This confirmation indicates that the use of this device may be feasible when the use of another technique could be cumbersome, or otherwise untenable.

Real-time evaluation of novel prosthetic control schemes is critical for translational research on artificial limbs. Recently, two computer-based, real-time evaluation tools, the target achievement control (TAC) test and the Fitts' law test (FLT), have been proposed to assess real-time controllability. Whereas TAC tests provides an anthropomorphic visual representation of the limb at the cost of confusing visual feedback, FLT clarifies the current and target locations by simplified non-anthropomorphic representations. Here, we investigated these two approaches and quantified differences in common performance metrics that can result from the chosen method of visual feedback. Ten able-bodied and one amputee subject performed target achievement tasks corresponding to the FLT and TAC test with equivalent indices of difficulty. Able-bodied subjects exhibited significantly (p <0.05) better completion rate, path efficiency, and overshoot when performing the FLT, although no significant difference was seen in throughput performance. The amputee subject showed significantly better performance in overshoot at the FLT, but showed no significant difference in completion rate, path efficiency, and throughput. Results from the FLT showed a strong linear relationship between the movement time and the index of difficulty (R2 = 0.96), whereas TAC test results showed no apparent linear relationship (R2 = 0.19). These results suggest that in relatively similar conditions, the confusing location of virtual limb representation used in the TAC test contributed to poorer performance. Establishing an understanding of the biases of various evaluation protocols is critical to the translation of research into clinical practice.
