RESUMO

The exponential progress of image editing software has contributed to a rapid rise in the production of fake images. Consequently, various techniques and approaches have been developed to detect manipulated images. These methods aim to discern between genuine and altered images, effectively combating the proliferation of deceptive visual content. However, additional advancements are necessary to enhance their accuracy and precision. Therefore, this research proposes an image forgery algorithm that integrates error level analysis (ELA) and a convolutional neural network (CNN) to detect the manipulation. The system primarily focuses on detecting copy-move and splicing forgeries in images. The input image is fed to the ELA algorithm to identify regions within the image that have different compression levels. Afterward, the created ELA images are used as input to train the proposed CNN model. The CNN model is constructed from two consecutive convolution layers, followed by one max pooling layer and two dense layers. Two dropout layers are inserted between the layers to improve model generalization. The experiments are applied to the CASIA 2 dataset, and the simulation results show that the proposed algorithm demonstrates remarkable performance metrics, including a training accuracy of 99.05%, testing accuracy of 94.14%, precision of 94.1%, and recall of 94.07%. Notably, it outperforms state-of-the-art techniques in both accuracy and precision.

Hydrogen bonds are the path through which protons and hydrogen atoms can be transferred between molecules. The relay mechanism, in which H-atom transfer occurs in a sequential fashion along hydrogen bonds, plays an essential role in many functional compounds. Here we use the scanning tunnelling microscope to construct and operate a test-bed for real-space observation of H-atom relay reactions at a single-molecule level. We demonstrate that the transfer of H-atoms along hydrogen-bonded chains assembled on a Cu(110) surface is controllable and reversible, and is triggered by excitation of molecular vibrations induced by inelastic tunnelling electrons. The experimental findings are rationalized by ab initio calculations for adsorption geometry, active vibrational modes and reaction pathway, to reach a detailed microscopic picture of the elementary processes.

Water clusters are assembled and imaged on Cu(110) by using a scanning tunneling microscope. Water molecules are arranged along the Cu row to form "ferroelectric" zigzag chains of trimer to hexamer. The trimer prefers the chain form to a cyclic one in spite of the reduced number of hydrogen bonds, highlighting the crucial role of the water-substrate interaction in the clustering of adsorbed water molecules. On the other hand, the cyclic form with maximal hydrogen bonds becomes more favorable for the tetramer, indicating the crossover from chain to cyclic configurations as the constituent number increases.

A 64-year-old woman with Parkinson is disease had a severe resting tremor that was not completely relieved by right-sided gamma knife thalamotomy (GKT). We performed bilateral staged thalamic deep brain stimulation (DBS) and compared the right and left ventral intermediate nucleus (Vim) of the thalamus including the frequency of single units recorded with microelectrodes, and also the somatotopical distribution of kinaesthetic cells (Ki). The average frequency of units for the presumed left Vim exceeded that of the right (22.6 +/- 19.2 Hz vs. 14.3 +/- 8.8 Hz). Regarding the somatotopic distribution of Ki, the receptive field for the leg, which is usually situated in the dorsolateral Vim, was more widely scattered in the right Vim than the non-lesioned left side. Our findings raise the possibility that the specific properties of the neurons changed due to partial coagulation by GKT within both the coagulated and the surrounding thalamic lesions.

RESUMO

We have studied the ζ-phase of solid oxygen using the generalized gradient approximation in the density functional approach. Calculations of total energies and pressures have been carried out for the prototype of diatomic ζ-phase and other hypothetical monoatomic crystal structures. The diatomic phase was found to be stable over a wide range of pressure (100-2000 GPa). The stacking of molecular layers is discussed in comparison with the available experimental data.

RESUMO

As Iraq faces the challenge of securing a sustainable resolution to the current violence, the burden of mental illness is likely to increase dramatically. The impact of Saddam Hussein's dictatorship, the Iran-Iraq war, U.S.-led economic sanctions, the Persian Gulf wars, and the U.S. invasion and subsequent violent insurgency have devastated Iraq's governmental and social infrastructure. Health care delivery across sectors has suffered greatly. During the reconstruction phase, the United States and coalition forces allocated resources to restructure Iraq's health care system. Many multinational organizations, governments, and policy makers had the political will as well as the financial and human resources to greatly influence Iraq's mental health program. However, the lack of an existing mental health plan stifled these efforts. Applying Kingdon's model for policy development, which includes political analysis, problem defining, and proposal drafting, the authors describe the development of Iraq's current mental health policy.

RESUMO

We report on a family in which a male infant had the asplenia syndrome, a younger brother had the polysplenia syndrome, and their father had situs inversus totalis. The occurrence of the asplenia and the polysplenia syndromes in a sibship of the present family and in two other previously reported sibships indicates that the two syndromes are causally and pathogenetically related to each other. If it is assumed that the father had an incomplete form of the polysplenia complex, then the condition in this family either is an autosomal dominant trait or is multifactorially determined.

We describe a 6 month-old girl with a 49, XX-XXX chromosome constitution. The patient had a characteristic round face, a low hairline, hypertelorism, epicanthus, a long philtrum, high-arched palate, short and webbed neck, small hands and feet, clinodactyly of the fifth fingers, overlapping toes, and separation between the first and the second toes. She also had atrial septal defect and patent ductus arteriosus complicated by myocarditis which exacerbated the course of her congestive heart failure. Psychomotor development was retarded with opisthotonoid posture, axial hypotonia, and with a borderline abnormal EEG. A densitometric, transmission analysis on X-linked polymorphic DNA-fragments of the Southern blots of the patient and the parents, using P20/MspI and pERT87-1/XmnI as probe/enzyme combinations, showed that the pentasomy X had resulted from 3 successive nondisjunctions at maternal meiosis. Clinical manifestations among 22 previously reported penta X syndrome patients are also reviewed.

Tumours consisting of a mixture of mature adipose and smooth muscle tissues, including those designated lipoleiomyomas, fibrolipoleiomyomas and myolipomas, are exceedingly rare, but most often occur in the uterine corpus. We describe here a case of such a tumour arising in the right round ligament of a 44-year-old woman. The tumour, which measured approximately 20x15x10 cm, was well encapsulated and did not involve the intrapelvic organs. Intricate mixtures of adult adipose tissue and bland smooth muscle exhibited no cellular atypia or nuclear mitotic figures, and there was little vascular proliferation. We diagnosed the lesion as a myolipoma of soft tissue with dual differentiation, and have found only 13 cases of this tumour including our own in the English literature. The present tumour is the first reported in the round ligament. Although this tumour is rare, its recognition is important for the avoidance of erroneous diagnoses.

RESUMO

We describe a modified Hamilton microsyringe that allows extracellular recording of neuronal activity and subsequent injections. It is assembled with a Hamilton removable needle and a syringe for injection, a Teflon-coated tungsten wire for recording, and polyimide tubing as a sheath. The device is inexpensive and easy to handle in anatomical and physiological experiments in awake monkeys.

The characteristics of the precentral neuron activity related to ipsilateral movements were studied while the monkey was performing finger, wrist and arm movements on either side. Out of 197 task-related neurons, 134 discharged in association with contralateral movements, but not with any one of 3 ipsilateral movements. Fifty neurons discharged with bilateral movements. Thirteen neurons discharged in association with ipsilateral movements (ipsi-neurons). Ten were recorded from the trunk or shoulder area of the motor cortex and were accompanied by contraction of those muscles by intracortical stimulation (ICS). The remaining 3 were related to elbow or wrist, but no ipsi-neurons were related to finger muscle contractions. In ipsilateral task performance, 7 ipsi-neurons discharged in association with finger and/or wrist movements in addition to arm movement. Five others were associated with arm movement. The last one discharged with wrist movement. Most of the units showed similar response to contralateral movement. Ipsi-neurons were classified into two groups. One group was recorded around the sulcus precentralis superior, had the lower threshold current and was mostly associated with finger, wrist and arm movements. The other was recorded in the rostral motor cortex, and had the higher threshold current and was related to arm movement. Among 185 neurons to which pyramidal tract stimulation was delivered, 2 out of the 80 PTNs and 11 out of the 105 non-PTNs were ipsi-neurons. EMGs were recorded from various muscles involved in the forelimb movements. Arm and finger muscles showed no activity when the monkey used the ipsilateral hand, while most of the shoulder and trunk muscles showed tonic or moderate transient changes in the activity during the ipsilateral tasks. The ipsi-neuron activity was discussed in consideration with EMGs.

The relationships between lesion location and clinical outcome following posteroventral pallidotomy for Parkinson's disease were studied. Forty-four patients were operated forty-six times and studied with neurological and psychological examinations before and after pallidotomy. Lesion location was confirmed using films with a coagulation electrode which were X-rayed during the operation. Changes of intelligence were observed in the patients with anteromedial lesions. Wearing-off phenomenon in four patients and dopa-induced dyskinesia in three patients were not improved following pallidotomy in twenty patients with severe wearing-off and dyskinesia. Lesions in the patients with no improvement of wearing off were located more lateral and those in the patients with sustained severe dyskinesia were located more dorsal in the internal part of the globus pallidus. It may be concluded that clinical outcome is related to lesion location.

During three different motor tasks of finger, wrist and arm movements on either side, 80 pyramidal tract neuron (PTN) activities were recorded in the monkey motor cortex. They were divided into three groups; PTNs related to contralateral movement (contra-PTNs), those related to ipsilateral and contralateral movement (bilateral-PTNs) and those related to ipsilateral movement (ipsi-PTNs). The latency histogram of the antidromic activation was similar for contra-PTNs as well as ipsi- and bilateral-PTNs in the fast PTN group, but most of slow PTNs appeared among contra-PTNs. Intracortical microstimulation (ICMS) was delivered to correlate muscular contraction with PTN activity. Most of slow PTNs were related to proximal muscular contraction and PTNs related to proximal muscles appeared more in ipsi- and bilateral-PTNs than in contra-PTNs.

The cells of origin of ipsilateral, prefrontal cortical projections to the post-arcuate area were studied in rhesus monkeys with the horseradish peroxidase (HRP) method. Following injections of HRP into the dorsal portion of the post-arcuate area, retrogradely labeled cells appeared in the arcuate area dorsal to the principal sulcus, while, following injections into its ventral portion, labeled cells appeared in the arcuate area ventral to the principal sulcus. They were pyramidal cells and lay mainly in the III layer, but small numbers of them were also found in the IV and V layers. In addition, HRP labeled cells were seen ipsilaterally in the banks of the cingulate sulcus.

RESUMO

A cortical motor region that represented the cutaneous muscles on the back was identified on the medial wall of the frontal lobe in the macaque monkey. In this region, neurons responded to somatosensory stimuli such as light touch or squeezing of the back skin, and intracortical microstimulation elicited contraction of the back skin. Such a region was located primarily on the dorsal bank of the cingulate sulcus, corresponding to the dorsal cingulate motor area.

During a single-step visual tracking task of monkeys, parametric changes of the wrist extension-flexion movement and related discharge rate changes of pyramidal tract neurons (PTNs) of hand-arm motor area were studied. The task consisted of preparatory, precontraction, contraction and target periods. If the displacement amplitude was changed from narrow (10-20 degrees) to moderate (40 degrees) range, peak velocity, peak acceleration and contraction period increased linearly but precontraction period decreased slightly. In 61 movement-related PTNs, no linear relationships were found between PTN discharge rate during precontraction or contraction period and displacement amplitude, velocity, acceleration, precontraction period or contraction period. In less than 20% of PTNs, however, correlations between PTN discharge rate during precontraction period and velocity or acceleration were found in the moderate range task. It occurred less frequently in narrow range task. It is said in a visual tracking task that PTN activity is not dependent upon factors related to the task parameters, such as velocity, acceleration. Possible related factors were discussed.

The neuronal activity in the motor cortex of the rhesus monkey was investigated in three different tasks performed with finger, wrist, and arm movements. A total of 125 neuronal activities were analysed. They were classified into five groups in terms of muscular contractions provoked by intracortical stimulation; neurons related to contractions of finger, wrist, elbow, shoulder, or trunk muscles. The neuronal activities in three tasks performed with finger, wrist, or arm movements were investigated for each group. Most of the neurons related to the contractions of elbow, shoulder, or trunk muscles were associated solely with arm movement. Smaller numbers of neuronal activities changed their firing frequencies in association with two or three tasks. Neurons related to the contractions of finger and wrist muscles showed various firing patterns in the three tasks; some responded to a single task with wrist or arm movement, while others changed their activities in association with more than one task. The presence of multi-task related neurons is discussed with respect to the multisegmental termination of corticospinal axons in the spinal cord.

CS-1170 was administered to 9 cases of the pediatric field. The clinical effect was good in 6 cases, fair in 2, and poor in one. The doses ranged from 80 mg/kg to 100 mg/kg, and an intravenous injection of 300 mg/kg was given in only one case. In the poor case, continuous instillation of 100 mg/kg was given. In the 2 fair cases, the patients with septicemia received 80 mg/kg and 300 mg/kg, respectively. There was no improvement in clinical findings, but the bacteria alone disappeared. As side effects, vasalgia due to intravenous injection was observed in 2 cases, but it disappared by the intravenous injection of 500 mg dissolved in 10 ml. No case showed eruption, diarrhea, or acidophilia. GOT or GPT increased in no case either.

Four adults and a child undergoing surgery with sevoflurane anesthesia developed acute pulmonary edema immediately after anesthesia. Prior to development of pulmonary edema all patients exhibited severe arterial hypertension and tachycardia. Their episodes of circulatory changes were, we believed, caused by the local injection of epinephrine for hemostasis in 2 patients (9 y.o. child, 45 y.o. man) and the intrusion of painful surgical stimuli in one patient (67 y.o. man). Circulatory changes in these three patients were treated by increasing the inspired sevoflurane concentration. We, however, speculate that the increase in inspired sevoflurane decreased the cardiac output and that the resulting increase in pulmonary wedge and capillary pressures was caused by an abrupt increase of arterial blood pressure, followed by a rapid increase in afterload due to cardiac suppression from the high concentration of sevoflurane. In 2 patients (74 y.o. man, 61 y.o. woman) arterial hypertension occurred during endotracheal extubation after sevoflurane anesthesia. Because of fast uptake and elimination of sevoflurane due to a low blood/gas partition coefficient, a fast awakening in the latter 2 patients, may be responsible for the abrupt increase in arterial blood pressure. In conclusion, it should be noted that pulmonary edema may be involved when severe circulatory changes occur in a patient undergoing sevoflurane anesthesia.

We report the anesthetic management of a 4-yr-old boy with Simpson-Golabi-Behmel syndrome. This syndrome is an X-linked condition characterized by pre- and postnatal overgrowth with visceral and skeletal anomalies. Affected males have a distinctive facial appearance with wide nasal bridge, anteverted nostrils, wide-open mouth, enlarged tongue, and large protruding maxilla and jaw. Although it had been speculated preoperatively that airway management would be complicated by the macroglossia, there was no difficulty in endotracheal intubation in the present patient. Preparations for difficult airway management should be made by the anesthesiologist before induction of anesthesia.
