High levels of work stress are prevalent today, and the underlying working conditions need to be tackled urgently. In this study, our aim was to identify the range of factors that employees themselves perceive as hindrances to the flow of work, that is, hindrance stressors. We analysed the open-ended questionnaire responses of 4766 employees working in the health care sector using semi-automated content analysis. We then used more detailed conventional content analysis to compare the responses of the groups that reported high (n = 1388) and low (n = 833) levels of subjective stress. Finally, we interpreted and categorised the stressors raised by the respondents from the viewpoint of controllability, to shed light on where to target interventions. The main hindrance stressors reflected inadequate staffing, work overload, time pressure, and management-related issues, of which the responses revealed concrete examples. Interruptions and problems related to cooperation and instructions were also commonly mentioned. The respondents in the high stress group emphasised work overload and issues related to management and clients. Our results suggest that the major hindrances to daily work are beyond employees' control and require decisions and resources at the level of supervisors, managers, directors, and policymakers. Future studies on work stress should explore the controllability of common stressors in more detail and include the appraisal of controllability in explanatory models. Avoiding overemphasis of psychological coping and instead targeting harmful working conditions and the organisational actors who can influence these could make workplace stress management interventions more effective.

BACKGROUND: Cognitively straining conditions such as disruptions, interruptions, and information overload are related to impaired task performance and diminished well-being at work. It is therefore essential that we reduce their harmful consequences to individual employees and organizations. Our intervention study implements practices for managing the cognitive strain typical to office work tasks and working conditions in offices. We will examine the effects of a cognitive ergonomics intervention on working conditions, workflow, well-being, and productivity. METHODS/DESIGN: The study is a stratified cluster randomized trial. The clusters are work units, for example, teams or offices. The four participating organizations entered a total of 36 clusters, and we invited all 1169 knowledge employees of these units to participate. We randomly allocated the clusters into an intervention group (cognitive ergonomics) or an active control group (recovery supporting). We invited an additional 471 participants to join a passive control group only for baseline and follow-up measurements, with no intervention. The study consists of a baseline survey and interviews and observations at the workplace, followed by an intervention. It starts with a workshop defining the specific actions for the intervention implementation stage, during which we send task reminder questionnaires to all employees to support behaviour change at the individual and team levels. The primary outcome measure is perceived frequency of cognitive strain from working conditions; the secondary outcome measures include subjective cognitive load, well-being, workflow/productivity, and cognitive stress symptoms. Process evaluation uses the quantitative and qualitative data obtained during the implementation and evaluation phases. The baseline measurements, intervention phase, and end-of-treatment measurements are now complete, and follow-up will continue until November 2019. DISCUSSION: There is a need to expand the research of cognitive strain, which poses a considerable risk to work performance and employee well-being in cognitively demanding tasks. Our study will provide new information about factors that contribute to such strain. Most importantly, the results will show which evidence-based cognitive ergonomic practices support work performance in knowledge work, and the project will provide concrete examples of how to improve at work. OBJECTIVE: To examine the association between work stress and cognitive performance. METHODS: Cognitive performance of a total of 99 women (mean age = 47.3 years) working in hospital wards at either the top or bottom quartiles of job strain was assessed using validated tests that measured learning, short-term memory, and speed of memory retrieval. RESULTS: The high job strain group (n = 43) had lower performance than the low job strain group (n = 56) in learning (P = 0.025), short-term memory (P = 0.027), and speed of memory retrieval (P = 0.003). After controlling for education level, only the difference in speed of memory retrieval remained statistically significant (P = 0.010). CONCLUSIONS: The association found between job strain and speed of memory retrieval might be one important factor explaining the effect of stress on work performance.

Expertise effects in music were studied in a new task: the construction of mental representations from separate fragments. Groups of expert musicians and non-musicians were asked to recall note patterns presented visually note by note. Skill-level, musical well-formedness of the note patterns and presentation mode were varied. The musicians recalled note patterns better than the non-musicians, even though the presentation was visual and successive. Furthermore, only musicians' performance was affected by musical well-formedness of the note patterns when visual gestalt properties, verbal rehearsability, and familiarity of the stimuli were controlled. Musicians were also able to use letter names referring to notes as efficiently as visual notes, which indicates that the better recall of musicians cannot be explained by perceptual visual chunking. These results and the effect of skill level on the distribution of recall errors indicate that the ability to chunk incoming information into meaningful units does not require that complete familiar patterns are accessible to encoding processes, yet previous knowledge stored in long-term memory affects representation construction in working memory. The present method offers a new reliable tool, and its implications to the research on construction of representations and musical imagery are discussed.
