RESUMO

Type material and additional collections of 11 taxa of Gautieria described in Europe and North Africa have been studied, namely G. dubia, G. graveolens, G. morchelliformis var. globispora, G. morchelliformis var. magnicellaris, G. morchelliformis var. morchelliformis, G. morchelliformis var. stenospora, G. otthii, G. pseudovestita, G. retirugosa, G. trabutii and G. villosa. At the same time, morphological and genetic studies on recent and herbarium collections from several European countries have been carried out. This enabled clarification of sections within Gautieria and differentiation of 28 taxa, of which 21 are new to science. However, the deeper relationships and nomenclature changes related to the phylogenetic position of the genus Gautieria within Gomphaceae will not be addressed in this study because they would require a more complete molecular analysis together with that of related genera, e.g., Gomphus, Turbinellus, and the four subgenera of Ramaria. In addition, a lectotype for G. villosa var. villosa and reference specimens for G. graveolens and G. morchelliformis var. morchelliformis are selected, and the new combination G. morchelliformis var. dubia is proposed. Detailed descriptions, macro- and microphotographs and distribution maps of all taxa are provided, as well as extensive information on their ecology, chorology and phylogeny. We theoretically propose an all-optical scheme for generation of isolated electron pulses with subfemtosecond durations for ultrafast electron microscopy and diffraction. The scheme is based on simultaneous longitudinal and transverse momentum modulation of freely propagating electrons at two distinct frequencies via ponderomotive interaction with tailored light fields of three phase-controlled laser pulses in vacuum. After a drift distance, an attosecond electron pulse train is formed with the individual pulses displaced in the transverse direction. Spatial filtering of the electron beam by an aperture leads to isolation of an individual attosecond pulse with a duration of <100 as. Subfemtosecond isolated electron pulses will enable direct space-time imaging of attosecond electronic dynamics in atoms, molecules, and solids with atomic spatial resolution, allowing us to directly observe phenomena such as electron tunneling or electron-electron interaction in strong laser fields of few-cycle laser pulses.

OBJECTIVES: The aim of this study was to evaluate the correlation between the change in heart strains and the success rate of Cardiac Resynchronization Therapy (CRT) optimization. We further explored the benefit of speckle tracking for CRT. METHODS: In this prospective cohort study, CRT-Ds were implanted to 60 patients. 3 months later, the response was evaluated. In the non-responders, optimization based on speckle tracking was performed. The AV interval was optimized with respect to the quality of left ventricle filling and the VV interval was optimized with respect to heart strains. After a further three months, the optimization success was evaluated. RESULTS: Thirty-nine patients responded well to the initial CRT. The response was independent of etiology; the subsequent optimization was however more successful in dilated cardiomyopathy (DCM) (8 out of 9) than in ischemic heart disease (IHD) patients (3 out of 10 responded). The ejection fraction increase and area strain were the best predictors of NYHA improvement. CONCLUSION: AV and VV optimization in patients who do not respond well to initial CRT seems to have better results in patients suffering from DCM. Speckle tracking (specifically A-strain) may be used to guide CRT optimization (Tab. 2, Fig. 3, Ref. 22).

We report on a simple approach for the compression of few-cycle laser pulses generated in an ultrafast laser oscillator to a duration corresponding to 1.7 cycles of near-infrared light (compression factor of 1.44) by nonlinear spectral broadening in diamond and subsequent dispersion compensation using chirped mirrors. After the spectral broadening, the pulse spectrum spans over almost an octave (580-1000 nm at the -10 dB level). The pulses are compressed by broadband-chirped mirrors and a wedge pair to a duration of 4.5 fs measured by spectral phase interferometry for direct electric-field reconstruction (SPIDER). The properties of the broadened spectrum and their modelling by numerical solution of a 1D nonlinear Schrödinger equation show that the main source of spectral broadening is self-phase modulation, whereas stimulated Raman scattering does not play a significant role.

Atomic motion dynamics during structural changes or chemical reactions have been visualized by pico- and femtosecond pulsed electron beams via ultrafast electron diffraction and microscopy. Imaging the even faster dynamics of electrons in atoms, molecules, and solids requires electron pulses with subfemtosecond durations. We demonstrate here the all-optical generation of trains of attosecond free-electron pulses. The concept is based on the periodic energy modulation of a pulsed electron beam via an inelastic interaction, with the ponderomotive potential of an optical traveling wave generated by two femtosecond laser pulses at different frequencies in vacuum. The subsequent dispersive propagation leads to a compression of the electrons and the formation of ultrashort pulses. The longitudinal phase space evolution of the electrons after compression is mapped by a second phase-locked interaction. The comparison of measured and calculated spectrograms reveals the attosecond temporal structure of the compressed electron pulse trains with individual pulse durations of less than 300 as. This technique can be utilized for tailoring and initial characterization of suboptical-cycle free-electron pulses at high repetition rates for stroboscopic time-resolved experiments with subfemtosecond time resolution.

Periodontal disease is a common oral disease. Inflammatory and immune responses to oral microorganisms initiate the development of periodontitis. Cigarette smoking is an important environmental risk factor for periodontitis. Another important inflammatory mediator is nitric oxide (NO). NO modulates vascular tone, microvascular permeability, leukocyte migration and oxidative activity, contributing to the direct killing of microorganisms. Several polymorphisms of the NOS3 gene have been detected, which may alter gene expression and NO synthesis. The aim of this study was to examine the association between the NOS3 rs1799983 and rs2070744 polymorphisms and periodontal disease. This study enrolled 200 patients with periodontal diseases (130 were non-smokers and 70 were smokers) and 160 control subjects (126 were non-smokers and 34 were smokers). Among the patients with periodontal disease, we observed a statistically increased frequency of patients with the CT genotype (TC vs. TT; 95%CI 1.83, OR 1.16-2.88, P = 0.011). There was a statistically significant increased frequency of CT genotype carriers among non-smoking patients with periodontal disease as compared with non-smoking controls, whereas there were no statistically significant differences between smoking patients with periodontal disease and smoking control subjects. The results of our study suggest an association between the NOS3 rs2070744 polymorphism and periodontal disease.

OBJECTIVES: In diabetic patients, there is a discrepancy in guidelines for ankle-brachial index (ABI) screening for peripheral arterial disease (PAD). While diabetes organizations suggest the value of upper limit of normal ABI to be 1.3, cardiologists recommend 1.4. Also, guidelines recommend using the higher value of ankle pressure (HAP) but multiple recent studies propose the opposite (LAP). METHODS: In this prospective study, we performed ABI measurements in 62 diabetic patients. Results were calculated by comparing higher and lower values of ankle pressure to those of duplex ultrasound (stenosis ≥ 50 % was considered PAD). Special attention was paid to patients with high and non-measurable ABI. RESULTS: LAP ABI appears to be a preferable method for PAD screening in diabetics. The upper cut-off value of 1.4 yielded better results with sensitivity of 93 % and negative predictive value of 91 %. No limbs with ABI between 1.3 and 1.4 with significant stenosis were found. However, using HAP for the upper cut-off captured additional PAD patients. PAD was abundant among patients with high or non-measurable ABI. CONCLUSIONS: LAP should be used for assessing low ABI (cut-off 0.9) while HAP for detecting the abnormally high ABI. The preferable high ABI cut-off is 1.4. Condition with abnormally high or non-measurable ABI should be considered as PAD (Tab. 3, Ref. 22).

We report on a theoretical and experimental study of the energy transfer between an optical evanescent wave, propagating in vacuum along the planar boundary of a dielectric material, and a beam of sub-relativistic electrons. The evanescent wave is excited via total internal reflection in the dielectric by an infrared (λ = 2 µm) femtosecond laser pulse. By matching the electron propagation velocity to the phase velocity of the evanescent wave, energy modulation of the electron beam is achieved. A maximum energy gain of 800 eV is observed, corresponding to the absorption of more than 1000 photons by one electron. The maximum observed acceleration gradient is 19 ± 2 MeV/m. The striking advantage of this scheme is that a structuring of the acceleration element's surface is not required, enabling the use of materials with high laser damage thresholds that are difficult to nano-structure, such as SiC, Al2O3 or CaF2.

The aim of this study was to compare the utility of BACTEC™ Mycosis-IC/F (Mycosis), BACTEC™ Plus Aerobic/F (Aerobic), and BACTEC™ Plus Anaerobic/F (Anaerobic) media in the detection of fungi from simulated (obtained by the inoculation of tested media first with sterile sheep's blood and subsequently with one of 60 clinical yeast isolates) and clinical blood samples, taken during routine diagnostic examination in two hospitals. All tested strains grew on Mycosis as well as Aerobic bottles, and the time to detection obtained for Mycosis was significantly shorter (p < 0.05). The largest differences in the time to positivity was found for Candida glabrata and Cryptococcus neoformans, when Mycosis preceded Aerobic in 20-48 h (mean 35.5 h) and 0.7-64 h (mean 24 h), respectively. On the contrary, C. krusei were detected earlier in Aerobic media. In clinical samples, the detection of C. glabrata was also significantly faster in Mycosis than in Aerobic (29.22 ± 11.48 h compared to 86 ± 40 h). The media complement each other and, in 45% of clinical examination sets, a single positive medium was noted (25% in Mycosis and 19% in Aerobic). The study proved that both Aerobic and Mycosis media serve as the correct condition for the culture of fungi and that they varied significantly in the detection time of clinically important species. This result could suggest that the simultaneous use of Aerobic as well as Mycosis media may improve the time of diagnosis in many patients, especially those infected with C. glabrata or C. neoformans.

We report on the time-resolved coherent anti-Stokes Raman spectroscopy of phonon dephasing in micro- and nanocrystalline diamond films. The dephasing times T(2) were found to be dependent on the morphology of diamond films (average size of crystals and content of nondiamond carbon phase) and changed from 0.7 to 1.72 ps. The dephasing times were found to be temperature independent in the range 10-295 K. In addition to diamond Raman active phonon mode at 1332 cm(-1), we investigated also the dynamics of a broad Raman peak at 1530 cm(-1) which is present in samples with higher content of nondiamond sp(2) hybridized carbon phase. This peak was found to be homogenously broadened with very fast dephasing (T(2)~50 fs).

PURPOSE OF THE STUDY: Injury to the anterior cruciate ligament (ACL) frequently results in the development of knee instability. This is managed by ACL reconstruction using autogenous or allogeneic grafts. This study presents a comprehensive evaluation of the patients with ACL reconstruction using a tendon allograft harvested from the anterior tibial muscle (ATM). MATERIAL AND METHODS: The group under study comprised 53 patients, 33 men and 20 women, with unilateral ACL reconstruction while the other knee was intact. The average age was 32.4 years (17 to 51) and the average follow-up was 33,4 months (21 to 53). Clinical evaluation included the Tegner and Lysholm scores and questionnaire-based satisfaction rating. Functional assessment included measurements of the range of motion and thigh circumference. Knee stability was determined using the pivot shift test, Lachman test and one-leg hop test. ACL laxity was measured on a GNRB® arthrometer. Placement of the tibial and the femoral tunnel on radiographs was assessed by the method of Harner. RESULTS: All patients reported their willingness to undergo the surgery again. The average pre-operative Tegner score improved from 7.17 (4-10) to 6.45 (2-10; p = 0.0001). The average post-operative Lysholm score was 87 points (47-100). At final follow-up, the full range of motion was recorded in 39 patients (74%), while the remaining patients exhibited only insignificant restriction of knee motion in comparison with the pre-operative status. The results of the Lachman test were negative in 38 patients (71.7%) while the pivot shift test was negative in 52 patients (98.1%). In the one-leg hop test, 44 patients (83%) were able to jump on the treated leg for a distance longer than 90% of the distance done on the healthy leg. The GNRB test at a force of 134N showed an anterior shift of the proximal tibia by 2.2 mm (0.1-9.3) on the average. The tibial tunnel was placed in zone B in 48 patients (90.6%) and the femoral tunnel was most often located to zone D (n = 47; 88.7%). DISCUSSION: The choice of tissue for ACL reconstruction is still being discussed. In some centres an ATM tendon allograft is used only in revision surgery while, in other centres, it is also employed in primary procedures. Recent studies on ACL surgery suggest that outcomes are relatively independent of the material used for reconstruction. In view of evidence-based medicine, more well-conducted multicentre clinical trials are needed to optimise indications for allograft ACL reconstruction. CONCLUSIONS: The ACL reconstruction using an ATM tendon allograft showed good clinical outcome at a minimum follow-up of 21 months. The method can be recommended not only for revision ACL reconstruction but also in primary reconstructions particularly in elderly patients. The present-day technology of producing allografts maintains tissue biomechanical characteristics and reduces risks for transmission of infection to a minimum.

Background: Wound healing complications affect numerous patients each year, creating significant economic and medical challenges. Currently, available methods are not fully effective in the treatment of chronic or complicated wounds; thus, new methods are constantly sought. Our previous studies showed that a peptide designated as PDGF2 derived from PDGF-BB could be a promising drug candidate for wound treatment and that RADA16-I can serve as a release system for bioactive peptides in wound healing. Based on that, in this work, we designed a new self-assembling hydrogel RADA-PDGF2, connecting both peptides by a sequence specific for neutrophil elastase, and evaluated its activity in wound healing. Methods: The physicochemical properties of the designed scaffold were analyzed using transmission electron microscopy, atomic force microscopy, cryoSEM microscopies, and circular dichroism spectroscopy. The enzymatic cleavage was performed using human neutrophil elastase and monitored using high-performance liquid chromatography and MS spectroscopic techniques. The aforementioned techniques (HPLC and MS) were also used to assess the stability of the peptide in water and human plasma. The biological activity was analyzed on human skin cells using a colorimetric XTT test, collagen synthesis evaluation, and a migration assay. The biocompatibility was analyzed with LDH cytotoxicity assay and flow cytometric analysis of activation of immune cells. Finally, RADA-PDGF2 activity in wound healing was checked in a mouse dorsal skin injury model. Results: The analysis showed that RADA-PDGF2 can self-assemble, form a hydrogel, and release a bioactive sequence when incubated with human elastase. It shows pro-proliferative and pro-migratory properties and accelerates wound closure in the mouse model compared to RADA16-I. In addition, it is not cytotoxic to human cells and does not show immunogenicity. RADA-PDGF2 seems to be a promising drug candidate for wound management.

Restenosis/re-occlusion remains a frequent complication in the first year after percutaneous transluminal angioplasty (PTA). In this study, association of nuclear receptor related 1 protein (Nurr1) haplotypes to the restenosis/re-occlusion rate after femoropopliteal PTA was investigated. Patients (n = 142) with disabling claudication or critical limb ischaemia, who had undergone technically successful femoropopliteal PTA, were prospectively followed up by vascular ultrasound imaging 12 months after the procedure. Nurr1 haplotypes 2 and 3 were associated significantly with the restenosis/re-occlusion rate (adjusted odds ratio 1.6, 95% confidence interval (CI) 1.1-2.3 and 2.0, 1.3-2.8, respectively) on univariate analysis.

BACKGROUND: The incidence of gingival overgrowth among renal transplant patients treated with cyclosporine A ranges from 13% to 84.6%, and the overgrowth is not only esthetic but also a medical problem. We studied the determination of association between TGF-ß1 (TGFB1) gene polymorphism and gingival overgrowth in kidney transplant patients medicated with cyclosporin A. METHODS: Eighty-four kidney transplant patients with gingival overgrowth and 140 control transplant patients without overgrowth were enrolled into the case control study. TGFB1 polymorphism was determined using the PCR-RFLP assay for +869T > C in codon 10 and +915G > C in codon 25 as well as TaqMan real-time PCR assays for promoter -800G>A and -509C > T SNPs. RESULTS: In kidney transplant patients suffering from gingival overgrowth, mean score of gingival overgrowth was 1.38 ± 0.60, whereas in control subjects it was 0.0. The patients with gingival overgrowth were characterized by similar distribution of TGFB1 genotypes and allele in comparison to subjects without gingival overgrowth. Among 16 potentially possible haplotypes of TGFB1 gene, only four were observed in the studied sample of kidney transplant patients: G_C_T_G, G_T_C_G, G_C_C_C, and A_C_T_G, with similar frequency in patients with and without gingival overgrowth. CONCLUSION: No association between the TGFB1 gene polymorphism and gingival overgrowth was revealed in kidney transplant patients administered cyclosporine A.

Small quantities of highly purified granulocytic pyrogen have been separated from contaminating proteins by disc electrophoresis in polyacrylamide gel. The biologically active material thus isolated was shown to be electrophoretically homogeneous at pH 9 and pH 3.8. Earlier work on the chemical properties of the pyrogen molecule has been extended to include: (a) estimation of its molecular weight by gel filtration; (b) demonstration of free sulfhydryl groups essential for its biological activity; and (c) evidence that it is not inactivated by exhaustive extraction with ethanolether or n-heptane.

We report on investigation of nonlinear optical phenomena in nanocrystalline diamond prepared by microwave plasma enhanced chemical vapour deposition. We observed the upconverted photoluminescence, the second and the third harmonic generation and Z-scan signal. The value of the third order nonlinear susceptibility was estimated. Our results show that nonlinear optical properties of nanocrystalline diamond have many features of the bulk diamond affected to some extent by the presence of grain boundaries.

BACKGROUND AND OBJECTIVE: Gingival enlargement frequently occurs in transplant patients receiving immunosuppressive drugs. It was hypothesized that gingival enlargement associated with cyclosporine use results from reduced degradation of extracellular matrix in the gingiva. Matrix metalloproteinase-3 (MMP-3) is involved in biodegradation of the extracellular matrix, and its inhibition may contribute to an abnormal accumulation of fibronectin and proteoglycans, which are MMP-3 substrates. The aim of this study was to investigate whether an association exists between MMP-3 genotypes and gingival enlargement in kidney transplant patients medicated with cyclosporine A. MATERIAL AND METHODS: Sixty-four unrelated kidney transplant patients suffering from gingival overgrowth, as well as 111 control transplant patients without gingival overgrowth, were enrolled in the study. Gingival overgrowth was assessed 6 mo after transplantation. During the post-transplant period all patients were given cyclosporine A as a principal immunosuppressive agent. MMP-3 polymorphism was determined using a PCR restriction fragment length polymorphism assay. RESULTS: In kidney transplant patients suffering from gingival overgrowth the mean gingival overgrowth score was 1.35 +/- 0.57, whereas in control subjects the mean gingival overgrowth score was 0.0. The distribution of MMP-3-1178A/dupA alleles among all kidney transplant patients, as well as in the two study subgroups, did not differ significantly from Hardy-Weinberg equilibrium. The frequency of the MMP-3-1171A/A genotype (28.1% for gingival overgrowth vs. 26.1% for controls) and of the MMP-3-1171dupA/dupA genotype (32.8% for gingival overgrowth vs. 22.5% for controls) was similar for both study groups. The risk of gingival overgrowth was lowest among patients carrying the MMP-3-1171A/dupA genotype (odds ratio 0.52), but this did not differ markedly from the other genotypes. CONCLUSION: No association between MMP-3 gene polymorphism and gingival overgrowth was revealed in kidney transplant patients administered cyclosporine A.

BACKGROUND: The outcome of percutaneous transluminal angioplasty (PTA) of peripheral arterial lesions is influenced by several factors, including the haemodynamic conditions. Our study tested: (a) whether infrapopliteal run-off after completed PTA influenced the time course of restenosis/reocclusion of the femoropopliteal arterial segment, and (b) whether worsening of infrapopliteal run-off influenced the long-term femoropopliteal patency after PTA. PATIENTS AND METHODS: Among 245 patients treated by femoropopliteal PTA we enrolled 176 patients who consented to regular follow-up. Concomitant infrapopliteal PTA was performed whenever feasible. The technical success of PTA and the patency of calf arteries were assessed by angiography. Infrapopliteal run-off was scored by a modification of the Society for Vascular Surgery criteria. The treated patients' limbs were divided into a group with good infrapopliteal run-off and a group with compromised run-off. Follow-up examination of the femoropopliteal arterial segment was performed by vascular ultrasonography (US) 1, 6 and 12 months after PTA, and an adverse outcome was defined by a > or = 50 % stenosis, i.e., at least doubling of the maximal systolic velocity, or occlusion - evidenced by the absence of flow. The patency of calf arteries was re-assessed by US 12 months after PTA. RESULTS: One month after femoropopliteal PTA 19 / 83 (23 %) of patients with compromised run-off developed the combined end-point of restenosis or reocclusion in comparison to 10 / 93 (11 %) with good run-off (p = 0.03). After 6 months the incidence of restenosis/reocclusion had increased in both groups at an approximately equal rate, but the differences were no longer significant: 39 / 80 (49 %) in the compromised run-off group vs. 36 / 83 (43 %) in the good run-off group after 6 months, p = 0.49, and 42 / 73 (57 %) vs. 38 / 73 (52 %) after 12 months, p = 0.51. However, in patients' limbs with good periprocedural run-off that deteriorated into compromised run-off in the year after PTA, femoropopliteal restenosis/reocclusion occurred more often than in limbs which retained good run-off: 10 / 14 (71 %) vs. 18 / 51 (35 %), p = 0.02. CONCLUSIONS: Compromised postprocedural infrapopliteal run-off predisposes to early restenosis/reocclusion after femoropopliteal PTA. Deterioration of infrapopliteal run-off in the year after femoropopliteal PTA is accompanied by worsening of long-term femoropopliteal patency.

RESUMO

The patients without ventricular arrhythmias with markers of high risk of sudden cardiac death are indicated for ICD implantation today. Last generation of ICD systems are equipped with high capacity batteries, with many automatic functions, capabilities of data sending and possibilities of prediction of worsening of heart failure. Nowadays ICD systems offers not only elimination of the risk of sudden cardiac death but reduction of symptoms of chronic heart failure through the resynchronization therapy, too.

OBJECTIVE: The aim of this study was an analysis of patients with ICD implanted in primary prevention (PP) in long-term follow-up and comparison with data obtained in secondary prevention patients (SP). PATIENTS AND METHODS: We assessed 73 patients with PP ICD and 138 patients with SP ICD. Males were in majority with 88% in PP group and 78% in SP group. The average age was 63 years. Majority of patients suffered from coronary artery disease. The mean ejection fraction (LV EF) was 28% in PP patients and 38% in SP patients. The number of affected coronary arteries and medication were similar in both groups. We compared the occurrence of intercurrent diseases, malignant arrhythmias, inappropriate therapies, hospital readmissions and patients' survival. RESULTS: PP patients had lower LV EF ejection fraction (p < 0.001), higher number of revascularized arteries (p < 0.001) and lower occurrence of inappropriate therapies and arrhythmic storms (p < 0.001). There was higher number of patients with diabetes (p = 0.009), dyslipidemia (p < 0.001) and cerebral artery disease (p = 0.017) among those in PP group. Renal insufficiency was related to a higher risk of death. CONCLUSIONS: Patients with PP ICD implantation have lower LVEF, and more intercurrent diseases then patients with ICD implanted form SP reasons. Their myocardium is more often revascularized and the occurrence of inappropriate therapy is low. Arrhythmic storm is a typical feature of patients with ICD implanted in secondary prevention.
