OBJECTIVE: The study aimed to investigate the effects of the level of contraction during isometric shoulder abduction at different abduction angles on muscle thickness and stiffness of the shoulder girdle in asymptomatic individuals. DESIGN: Measurement properties study. SETTING: Biomechanics and motion analysis lab. PARTICIPANTS: Twenty individuals volunteered to participate in this study. MAIN OUTCOME MEASURE: The subjects were tested for morphological and mechanical properties, expressed by thickness and stiffness of the supraspinatus tendon and muscle, and upper trapezius muscle. Moreover, acromiohumeral distance was also evaluated using B-mode ultrasound and shear-wave elastography. INTERVENTION: The thickness and stiffness of the supraspinatus and upper trapezius muscle were assessed at 3 angles of abduction (0°, 60°, and 90°) and 3 levels of contraction (0%, 10%, and 20% of the maximal voluntary isometric contraction) using ultrasonography with shear-wave imaging. Moreover, the acromiohumeral distance was measured to establish the occupation ratio during passive movement. RESULTS: The supraspinatus and upper trapezius muscle thickness and stiffness were significantly greater at 60° shoulder abduction compared with 0°, and 90° compared with 60°, as well as significantly greater at 20% maximal voluntary isometric contraction compared with 0% and 10% maximal voluntary isometric contraction. Thickness and stiffness were significantly greater in the supraspinatus compared with the upper trapezius muscle at all 3 angles of shoulder abduction for all 3 level of contractions. The acromiohumeral distance decreased significantly from 0° to 60° and from 60° to 90°. CONCLUSION: Morphological and mechanical properties of the supraspinatus and upper trapezius muscles depended on the relative level of muscle contraction and the angle of shoulder abduction.

PURPOSE: This study assessed the effects of upper-body rowing exercise on cardiorespiratory fitness, traditional cardiometabolic risk factors, and vascular health in individuals with spinal cord injury (SCI). METHODS: Seventeen male and female adults with chronic (> 1 yr) motor-complete and incomplete SCI (level of injury: C4-L3) were randomized to control (CON, n = 9) or exercise (UBROW, n = 8). Participants in UBROW performed 12-week, 3 weekly sessions of 30-min upper-body ergometer rowing exercise, complying with current exercise guidelines for SCI. Cardiorespiratory fitness ([Formula: see text]O2peak), traditional risk factors (lipid profile, glycemic control) as well as inflammatory and vascular endothelium-derived biomarkers (derived from fasting blood samples) were measured before and after 6 (6W) and 12 weeks (12W). Brachial artery resting diameter and flow-mediated dilation (FMD) were determined by ultrasound as exploratory outcomes. RESULTS: UBROW increased [Formula: see text]O2peak from baseline (15.1 ± 5.1 mL/kg/min; mean ± SD) to 6W (16.5 ± 5.3; P < 0.01) and 12W (17.5 ± 6.1; P < 0.01). UBROW increased resting brachial artery diameter from baseline (4.80 ± 0.72 mm) to 12W (5.08 ± 0.91; P < 0.01), with no changes at 6W (4.96 ± 0.91), and no changes in CON. There were no significant time-by-group interactions in traditional cardiometabolic blood biomarkers, or in unadjusted or baseline diameter corrected FMD. Explorative analyses revealed inverse correlations between changes (∆12W-baseline) in endothelin-1 and changes in resting diameter (r = - 0.56) and FMD% (r = - 0.60), both P < 0.05. CONCLUSION: These results demonstrate that 12 weeks of upper-body rowing complying with current exercise guidelines for SCI improves cardiorespiratory fitness and increases resting brachial artery diameter. In contrast, the exercise intervention had no or only modest effects on traditional cardiometabolic risk factors. The study was registered at Clinicaltrials.gov (N-20190053, May 15, 2020).

Musculoskeletal modeling uses metabolic models to estimate energy expenditure of human locomotion. However, accurate estimation of energy expenditure is challenging, which may be due to uncertainty about the true energy cost of eccentric and concentric muscle contractions. The purpose of this study was to validate three commonly used metabolic models, using isolated isokinetic concentric and eccentric knee extensions/flexions. Five resistance-trained adult males (25.6 ± 2.4 year, 90.6 ± 7.5 kg, 1.81 ± 0.09 m) performed 150 repetitions at four different torques in a dynamometer. Indirect calorimetry was used to measure energy expenditure during these muscle contractions. All three models underestimated the energy expenditure (compared with indirect calorimetry) for up to 55.8% and 78.5% for concentric and eccentric contractions, respectively. Further, the coefficient of determination was in general low for eccentric contractions (R2 < 0.46) indicating increases in the absolute error with increases in load. These results show that the metabolic models perform better when predicting energy expenditure of concentric contractions compared with eccentric contractions. Thus, more knowledge about the relationship between energy expenditure and eccentric work is needed to optimize the metabolic models for musculoskeletal modeling of human locomotion.

Studies have reported sex-based differences in conduit artery function, however little is known about possible sex-based differences in microvascular function, and possible influence of muscle group. Blood-oxygen-level-dependent (BOLD) MR images acquired during ischemia-reperfusion assess the reactive hyperemic response in the microvasculature of skeletal muscle. We tested the hypothesis that women have greater microvascular reactivity, reflected by faster time-to-peak (TTP) and time-to-half-peak (TTHP) of the BOLD response, across all lower leg muscles. In healthy, young men (n = 18) and women (n = 12), BOLD images of both lower legs were acquired continuously during 30 s of rest, 5 min of cuff occlusion and 2 min of reperfusion, in a 3 T MR scanner. Segmentation of tibialis anterior (TA), soleus (SO), gastrocnemius medial (GM), and the peroneal group (PG) were performed using image registration, and TTP and TTHP of the BOLD response were determined for each muscle. Overall, women had faster TTP (p = 0.001) and TTHP (p = 0.01) than men. Specifically, women had shorter TTP and TTHP in TA (27.5-28.4%), PG (33.9-41.6%), SO (14.7-19.7%) and GM (15.4-18.8%). Overall, TTP and TTHP were shorter in TA compared with PG (25.1-31.1%; p ≤ 0.007), SO (14.3-16%; p ≤ 0.03) and GM (15.6-26%; p ≤ 0.01). Intra class correlations analyses showed large variation in absolute agreement (range: 0.10-0.81) of BOLD parameters between legs (within distinct muscles). Faster TTP and TTHP across all lower leg muscles, in women, provide novel evidence of sex-based differences in microvascular function of young adults matched for age, body mass index, and physical activity level.

This study compared metabolic power (MP) and time trial (TT) running performance between Adidas Adizero Adios (AAA) and Nike VaporFly 4% (NVP). Thirty-seven runners completed three laboratory sessions and two field sessions (n=30). After familiarization (visit 1), participants completed eight 6-min treadmill running bouts (four with each shoe, counterbalanced) at their preferred pace, and MP was assessed using indirect calorimetry (visits 2 and 3). During visits 4 and 5, participants completed two outdoor TTs (~3.5 km) in NVP and AAA (counterbalanced). Compared with AAA, NVP exhibited superior MP (NVP: median=13.88 (Q1-Q3=12.90-15.08 W/kg; AAA: median=14.08 (Q1-Q3=13.12-15.44 W/kg; z=-4.81, p<.001, effect size=.56) and TT (NVP=793±98 s; AAA=802±100 s, p=.001; effect size=.09). However, there was no relationship between changes in MP and changes in TT between shoes (r=.151 p=.425, 95% confidence interval=[-.22; .48]). Our results demonstrate that NVP, compared with AAA, improves MP and TT in recreational runners. The lack of correlation between changes in MP and TT indicates that factors other than improved MP contribute to faster short-distance TT with NVP.

The purpose was to investigate differences in reliability of running economy measurements between familiar and unfamiliar shoes. Thirty-seven runners were included who all ran in familiar and unfamiliar running shoes while running economy was measured at steady state using a treadmill. Each participant was tested on two different visits (three sessions in total), with two trials in each of the three shoe conditions completed at each visit. Coefficient of variation, standard deviation of differences, and limits of agreement of running economy were used to quantify the repeatability (within-visit variation) and reproducibility (between-visit variation). The coefficient of variation showed a marginal difference in reproducibility across shoe conditions, whereas no differences were seen in coefficient of variation, standard deviation of the differences, or limits of agreement for repeatability across shoes. All three shoe conditions showed greater repeatability than reproducibility for running economy, and enhanced repeatability at visit 3 compared to visit 2. Our results indicate that familiarisation to shoes might not be needed for reliable measurements of running economy. Based on our results, when evaluating benefits in running shoes we suggest that running economy be assessed within the same day. Further, our data suggest a beneficial effect of using multiple familiarisation sessions if small differences between shoe conditions are expected.

OBJECTIVE: We recently demonstrated that upper-body rowing exercise (UBROW) improved aerobic fitness in individuals with spinal cord injury (SCI), with no effect on traditional cardiometabolic risk factors. Here, we tested the hypothesis that the exercise-induced increase in aerobic fitness was maintained at 6-month (6M) follow-up. DESIGN: Six-month follow-up. SETTING: University/hospital. PARTICIPANTS: Seventeen wheelchair-dependent participants with SCI. INTERVENTIONS: 12-week of exercise training (UBROW) or control (CON). OUTCOME MEASURES: Aerobic fitness (POpeak and V̇O2peak), body composition, blood pressure, and blood biomarkers of cardiometabolic risk were assessed at 6M follow-up and compared to baseline (BL) and immediately post-intervention (12-week). Minutes of mild, moderate, and heavy intensity leisure time physical activity (LTPA) were assessed by self-report. RESULTS: Fourteen participants returned at 6M follow-up (CON, n = 6; UBROW, n = 8). In UBROW, POpeak (median (Q1-Q3)) increased from BL (70 W (37-84)) to 12-week (77 W (58-109), P = 0.01) and 6M follow-up (81 W (51-96), P = 0.01), with no difference between 12-week and 6M follow-up (P = 0.21). Similarly, V̇O2peak increased from BL (15.4 ml/kg/min (10.5-19.4)) to 12-week (16.6 ml/kg/min (12.8-21.3), P = 0.01) with no difference between 12-week and 6M follow-up (16.3 ml/kg/min (12.9-19.7), P = 0.74). No differences were found in CON for either POpeak (P = 0.22) or V̇O2peak (P = 0.27). There were no changes over time in traditional cardiometabolic risk factors or for minutes of different LTPA intensities. CONCLUSION: We demonstrate that improvements in aerobic fitness are maintained for at least six months after completion of a 12-week exercise intervention, supporting the use of periodic exercise interventions to boost aerobic fitness level in individuals with SCI.Trial registration: ClinicalTrials.gov identifier: NCT04390087..

STUDY DESIGN: Exploratory clinical investigation. OBJECTIVES: To assess the feasibility, acceptability, and preliminary efficacy of upper-body rowing exercise adapted to wheelchair users with spinal cord injury (SCI). SETTING: University exercise laboratory. METHODS: Eight individuals with SCI exercised on a rowing ergometer modified for wheelchair users (REMW), three times weekly, for up to 30 min per session. Participants completed feasibility and acceptability questionnaire (1-5 Likert scale), and the Wheelchair Users Shoulder Pain Index (WUSPI) before and after six weeks of exercise. Average power output (POAVG), distance rowed, percent peak heart rate (%HRpeak), and rating of perceived exertion (RPE) (6-20 scale) were monitored throughout the 18 exercise sessions and analyzed to evaluate preliminary efficacy of the exercise modality. RESULTS: All eight participants completed the study (97% adherence). Participants rated the exercise high on the feasibility and acceptability scale; median (interquartile range) = 5.0 (4.0-5.0), where higher numbers indicated greater feasibility. Shoulder pain was reduced by 21% yet not significantly different from baseline (p = 0.899). Physiological measures (%HRpeak = 80-83%; RPE = 15.0-16.0) indicated a high cardiovascular training load. From week 1 to week 6, POAVG and distance rowed increased by 37 and 36%, respectively (both p ≤ 0.001). CONCLUSIONS: Data from six weeks of exercise on the REMW suggests that upper-body rowing is a feasible and acceptable exercise modality for wheelchair users with SCI. Session data on %HRpeak, RPE, and shoulder pain indicate that REMW evoked moderate to vigorous intensity exercise without exacerbation of shoulder pain. Future research is required to quantify potential training-induced changes in cardiorespiratory fitness.

INTRODUCTION: Cardiovascular and metabolic diseases are a growing concern for individuals with spinal cord injury (SCI). Physical inactivity contributes to cardiometabolic morbidity and mortality in the SCI population. However, previous studies have shown mixed results regarding the effects of exercise on cardiometabolic risk factors in individuals with SCI. This discrepancy could be influenced by insufficient exercise stimuli. Recent guidelines recommend 30 min of moderate-to-vigorous intensity aerobic exercise, three times per week, for improvement in cardiometabolic health in individuals with SCI. However, to date, no studies have implemented an exercise intervention matching the new recommendations to examine the effects on cardiometabolic risk factors. Therefore, the primary objective of this study is to determine the effects of 12 weeks of wheelchair user-modified upper-body rowing exercise on both traditional (constituents of the metabolic syndrome) and novel (eg, vascular structure and function) cardiometabolic risk factors in manual wheelchair users with SCI. METHODS AND ANALYSIS: A randomised controlled trial will compare 12 weeks of upper-body rowing exercise, 30 min three times per week, with a control group continuing their normal lifestyle. Outcome measurements will be performed immediately before (baseline), after 6 weeks (halfway), 12 weeks of training (post) and 6 months after the termination of the intervention period (follow-up). Outcomes will include inflammatory (eg, C reactive protein) and metabolic biomarkers determined from venous blood (with serum fasting insulin as primary outcome), body composition, arterial blood pressure, cardiorespiratory fitness level, brachial artery vascular structure and function and autonomic nervous system function. ETHICS AND DISSEMINATION: This trial is reported to the Danish Data Protection Agency (J.nr. 2019-899/10-0406) and approved by the Committees on Health Research Ethics in The North Denmark Region on 12 December 2019 (J.nr. N-20190053). The principal investigator will collect written informed consent from all participants prior to inclusion. Irrespective of study outcomes, the results will be submitted to peer-reviewed scientific journals for publication. TRIAL REGISTRATION NUMBER: NCT04390087.

In this study, we tested the hypotheses that unaccustomed eccentric exercise (ECC) would reduce the elastic modulus and dynamic stiffness of the upper trapezius muscle and that these changes would correlate with increases in muscle thickness, reflecting muscle edema. Shear wave elastography was used to measure elastic modulus, dynamic stiffness was assessed using myotonometry, and muscle thickness was measured using ultrasonography. All measurements were performed at four locations over the upper trapezius before and 24 h after a single bout of ECC. Fourteen healthy participants (11 males and 3 females; 23.2 ± 3.0 years; height 175.1 ± 10.4 cm; body mass 73.8 ± 11.3 kg) took part in the study. Overall, ECC resulted in decreased elastic modulus (from 45.8 ± 1.6 to 39.4 ± 1.2 kPa, p < 0.01) and dynamic muscle stiffness (from 369.0 ± 7.3 to 302.6 ± 6.0 N/m, p < 0.01). Additionally, ECC resulted in increased muscle thickness (from 6.9 ± 0.4 to 7.3 ± 0.4 mm, p < 0.01). Spatial changes (across the four locations) were found for elastic modulus, stiffness and thickness. No significant correlations were found between changes in measures of muscle stiffness, or between changes in stiffness and changes in thickness. In conclusion, the present pilot study showed that ECC altered biomechanical muscle properties, reflected by decreased elastic modulus and dynamic muscle stiffness 24 h after ECC.

BACKGROUND: The prognosis for men with non-aggressive prostate cancer is good, and several studies have investigated the impact of lifestyle changes including physical activity and diet on the prognosis. Despite positive results in animal studies and a few human interventions with whole-grain rye on markers of prostate cancer progression, the feasibility of trials investigating such dietary changes in combination with physical activity remains largely unstudied. The primary aim was to investigate the feasibility of an intervention with high whole-grain rye intake and vigorous physical activity for 6 months in men diagnosed with prostate cancer. METHODS: In total, 26 men (53-72 years) recently diagnosed with non-aggressive prostate cancer and on active surveillance, were enrolled in 2011-2012 and randomly assigned to an intervention or a control group. The intervention included 170 g/day of whole-grain rye and 3 × 45 minutes/week of vigorous physical activity. The duration of the intervention was 6 months and end of follow-up 12 months after baseline. Clinic visits were scheduled at baseline and 3, 6 and 12 months after baseline. Compliance with the intervention was evaluated by diaries, food frequency questionnaires, biomarkers, and heart rate monitor data. The effect of the intervention was evaluated by linear multiple regression analysis. RESULTS: In the intervention group, the mean daily intake of whole-grain rye measured from diaries was 146 g (SD: 19) for the first 3 months and 125 g (SD: 40) for the last 3 months of the intervention. The median level (5th and 95th percentiles) of vigorous physical activity was 91 (17, 193) min/week for the first 3 months and 66 (13, 259) min/week for the last 3 months. No recordings of physical activity were done for the control group. Aerobic fitness (VO2 peak) increased in the intervention group compared to the control group after the intervention. No effects were found on other cardio-metabolic outcomes or prostate cancer progression. CONCLUSIONS: The lifestyle intervention appeared feasible for 6 months among Danish men and the results are encouraging for conducting full-scale studies, where the impact of whole-grain rye and vigorous physical activity on prostate cancer progression and metabolic parameters can be evaluated. BACKGROUND: A prostate cancer diagnosis affects the patient and his spouse. Partners of cancer patients are often the first to respond to the demands related to their husband's illness and thus are likely to be the most supportive individuals available to the patients. It is therefore important to examine how spouses react and handle their husband's prostate cancer diagnosis. OBJECTIVE: The aim of this study was to explore how the prostate cancer diagnosis and the participation in their partners' behavioral lifestyle intervention program influenced the spouses' life, their relationship with their partner, and how they handle the situation. METHODS: Interviews were recorded with 8 spouses of potential low-risk prostate cancer patients on active surveillance as part of a clinical self-management lifestyle trial. RESULTS: We identified 3 phases that the spouses went through: feeling insecure about their situation, coping strategies to deal with these insecurities, and feeling reassured. CONCLUSIONS: The framework of a clinical trial should include mobilizing spousal empowerment so that they can take on an active and meaningful role in relation to their husband's disease. The observations here substantiate that the framework of active surveillance in combination with a lifestyle intervention in 1 specific prostate cancer clinical trial can mobilize spousal empowerment. IMPLICATIONS FOR PRACTICE: Creating well-designed clinical patient programs that actively involve the spouse appears to promote empowerment (meaning, self-efficacy, positive impact, and self-determination) in spouses. Spousal participation in clinical patient programs can give spouses relief from anxieties while recognizing them as a vital support for their husband.

PURPOSE: Noninvasive imaging of oxygen uptake may provide a useful tool for the quantification of energy expenditure during human locomotion. A novel thermal imaging method (optical flow) was validated against indirect calorimetry for the estimation of energy expenditure during human walking and running. METHODS: Fourteen endurance-trained subjects completed a discontinuous incremental exercise test on a treadmill. Subjects performed 4-min intervals at 3, 5, and 7 km·h (walking) and at 8, 10, 12, 14, 16, and 18 km·h (running) with 30 s of rest between intervals. Heart rate, gas exchange, and mean accelerations of ankle, thigh, wrist, and hip were measured throughout the exercise test. A thermal camera (30 frames per second) was used to quantify optical flow, calculated as the movements of the limbs relative to the trunk (internal mechanical work) and vertical movement of the trunk (external vertical mechanical work). RESULTS: Heart rate, gross oxygen uptake (mL·kg·min) together with gross and net energy expenditure (J·kg·min) rose with increasing treadmill velocities, as did optical flow measurements and mean accelerations (g) of ankle, thigh, wrist, and hip. Oxygen uptake was linearly correlated with optical flow across all exercise intensities (R = 0.96, P < 0.0001; V˙O2 [mL·kg·min] = 7.35 + 9.85 × optical flow [arbitrary units]). Only 3-4 s of camera recording was required to estimate an optical flow value at each velocity. CONCLUSIONS: Optical flow measurements provide an accurate estimation of energy expenditure during horizontal walking and running. The technique offers a novel experimental method of estimating energy expenditure during human locomotion, without use of interfering equipment attached to the subject.
