RESUMO

Alpha-melanocyte stimulating hormone (α-MSH) reduces food intake in birds and mammals. The objective of this experiment was to determine effects of α-MSH on food and water intake, and hypothalamic c-Fos immunoreactivity and appetite-associated factor mRNA in Japanese quail (Coturnix japonica), a species that has not undergone the same artificial selection for growth-related traits as the chicken. At 7days post-hatch, 3-h-fasted quail were intracerebroventricularly (ICV) injected into the lateral ventricle with 0 (vehicle), 0.5, 5, or 50pmol of α-MSH and food and water intake were recorded at 30min intervals for 180min. In the second and third experiment, quail were injected with 50pmol α-MSH and hypothalami were collected at 1h to determine c-Fos immunoreactivity and mRNA abundance, respectively. At 30min, quail injected with 5 or 50pmol of α-MSH ate and drank less than vehicle-injected quail. Quail injected with 50pmol ate less for the entire duration of the experiment and drank less than vehicle-injected quail for 120min post-injection. Hypothalamic expression of agouti-related peptide and DOPA decarboxylase were greater in vehicle- than α-MSH-injected quail, whereas melanocortin receptor 4 (MC4R) mRNA was greater in α-MSH- than vehicle-injected birds. Alpha-MSH injection was associated with more c-Fos immunoreactive cells in the ventromedial hypothalamus (VMH) and paraventricular nucleus (PVN) of the hypothalamus. Results suggest that the anorexigenic effect of α-MSH is conserved among avians and that effects in quail are associated with the VMH and PVN and involve MC4R.

Background: The bony facial trauma score (BFTS) is a rubric used to assess the severity of facial trauma. Objectives: To compare the BFTS with relevant clinical outcomes while controlling for medical comorbidities and polytrauma. Methods: A retrospective review of facial trauma patients evaluated between 2017 and 2022 was conducted. While controlling for medical comorbidities and polytrauma, multivariate regression models were used to assess the relationship between BFTS and outcome variables such as length of stay (LOS) and malocclusion. Results: In total, 176 patients were included in the analysis. The average age was 36.5 years (standard deviation [SD] of 16.8), and 68.8% were male. The most common mechanism of injury was blunt force (92.6%) and the mean BFTS was 10.73 (SD of 11.05). BFTS was found to correlate with the following (p < 0.05): total LOS and ICU LOS, malocclusion, likelihood of requiring multiple surgeries, and diplopia. Conclusion: The BFTS is significantly correlated with multiple outcome variables while controlling for medical comorbidities and polytrauma.

OBJECTIVES: To evaluate the rate of reoperation in patients without sagittal plane malalignment who underwent percutaneous screw fixation of a valgus-impacted femoral neck fracture. DESIGN: Retrospective case series. SETTING: Two Level 1 academic trauma centers. PATIENTS AND INTERVENTION: Two hundred seven patients >50 years of age with valgus-impacted femoral neck fractures treated with at least 3 large-diameter (>6.5 mm) cancellous screws from 2013 to 2019. Patients were excluded if there was a sagittal plane fracture deformity. MAIN OUTCOME MEASUREMENTS: The primary outcome was reoperation. Secondary outcomes considered "major complications" included: avascular necrosis, varus collapse or implant cutout, nonunion, deep infection, and hematoma requiring reoperation. Surgical fixation strategies (screw configuration, aim) and implant type (partial vs. fully threaded cancellous screws) were secondarily compared. RESULTS: Average patient age was 77 years, and median clinical follow-up was 658 days. Thirty-one patients (15%) required reoperation, and the major complication rate was 17.3% (36 complications in 33 patients). Logistic regression analysis demonstrated a higher risk of reoperation with constructs consisting of all partially threaded screws (17.0%) compared with the use of at least 1 fully threaded screw (7.5%) when an inverted triangle configuration was used (odds ratio, 2.50; 95% CI, 0.81-7.77). CONCLUSIONS: This study demonstrated a relatively high rate of reoperation and major complications in patients with valgus-impacted femoral neck fractures without sagittal malalignment treated with in situ percutaneous screw fixation. OBJECTIVE: This systematic review and meta-analysis seeks to characterize the rate of malignant progression among patients with laryngeal dysplasia treated with photoangiolytic laser and compare to prior systematic reviews of conventional surgical approaches. METHODS: OVIDMedline, Pubmed, Cumulative Index to Nursing and Allied Health Literature (CINAHL), and Google Scholar were searched, including terms related to patients with vocal fold dysplasia who were treated by angiolytic laser ablation. Some articles already known to authors or identified through hand searching were included. RESULTS: Six articles with 155 cases were included. Two studies used potassium titanyl phosphate exclusively, one solely used the pulsed dye laser, and three studies utilized both laser types during the study period. The pooled overall mean of malignant progression for patients with laryngeal dysplasia treated with photoangiolytic laser was 12%, as calculated by conducting a meta-analysis of single arm proportion. CONCLUSION: Laryngeal dysplasia is a premalignant lesion which confers a risk of progression to malignancy. After biopsy to establish the diagnosis there are multiple surgical techniques available for treatment with the goal of lesion eradication and voice preservation. In our review, there is a low malignant transformation rate for patients treated via with photoangiolytic laser.

OBJECTIVE: While Autoimmune Associated Vocal Fold Lesions (AaVFLs) have been described in many reports, there is no consensus on best practices in management. The purpose of this systematic review is to clarify the characteristics and treatment of dysphonia in the setting of AaVFLs. STUDY DESIGN: Systematic review METHODS: Pubmed and OVIDMedline and Google Scholar were searched, including terms related to (1) Vocal fold/cord, rheumatoid node/nodule, bamboo nodes/nodules, laryngeal deposits/nodes/nodules and (2) Autoimmune diseases/syndromes, connective tissue disease. RESULTS: Twenty-one studies with 83 patients diagnosed with AaVFLs were included. AaVFLs occurred predominantly in females in the 4th or 5th decade of life, with an overall mean age of 39.8 (SD = 12.8). Autoimmune or connective tissue disease was established prior to presentation to an otolaryngologist in 75.9% (44/58) of patients. Bilateral lesions were present in 83.8% (57/68) of patients. Treatment modalities included medical therapy alone (28.1%), voice therapy alone (17.5%), surgical treatment alone (7.0%), combination of medical and voice therapy (33.3%), and combination of surgical, medical and voice therapy (7.0%). All patients treated with voice therapy had voice improvement; lower rates were seen with solo medical (4/14 improved, 28.6%) or surgical therapy (3/6 improved, 50%). CONCLUSION: AaVFLs occur predominantly in women in their 30's to 50's and are associated with a variety of autoimmune conditions. A significant number of patients (25%) present to the Otolaryngologist without an established autoimmune diagnosis. While treatment outcomes are not robustly reported, a significant number of patients with AAVFLs treated with voice therapy alone or voice therapy in combination with other treatment modalities (medical or surgical) experience subjective improvement in voice quality and function.

Here, a 3D printed multiplexed competitive migration assay is reported for characterizing a chemotactic response in the presence of multiple spatially distributed chemoattractants. The utility of the assay is demonstrated by examining the chemotactic response of human glioblastoma cells to spatially opposing chemotactic gradients of epidermal growth factor (EGF) and bradykinin (BK). Competitive migration assays involving spatially opposing gradients of EGF and BK that are optimized in the absence of the second chemoattractant show that 46% more glioblastoma cells migrate toward EGF sources. The migration velocities of human glioblastoma cells toward EGF and BK sources are reduced by 7.6 ± 2.2% and 11.6 ± 6.3% relative to those found in the absence of the spatially opposing chemoattractant. This work provides new insight to the chemotactic response associated with glioblastoma-vasculature interactions and a versatile, user-friendly platform for characterizing the chemotactic response of cells in the presence of multiple spatially distributed chemoattractants.
