RESUMO

New strategies are needed to improve HIV testing rates in Tanzania, particularly among adult men. We sought to investigate if HIV oral self-testing would increase HIV testing uptake in Tanzanian rural community homes. The study design was a prospective community-randomized pilot study, in two matched villages with similar characteristics (intervention and control villages) Before data collection, we trained village health workers and research assistants for one week. We recruited male and female adults from 50 representative households in each of two villages in eastern Tanzania. We collected data at baseline and we followed-up the enrolled households after a one-month period. There was a high interest in testing for HIV, with all participants from both arms (100%; n = 259) reporting that they would like to test for HIV. After the one-month follow-up, overall, 66.1% (162/245) of study participants reported to have tested for HIV in both arms. In the intervention arm, 97.6% (124/127) reported that they tested for HIV versus in the control arm, 32.2% (38/118) tested for HIV, p-value < 0.001. In Tanzania, we found that availability of HIV self-testing was associated with an enormous increase in HIV testing uptake in a rural population.

OBJECTIVES: The aim of this study was to investigate the minimum iodine delivery rate (IDR) and contrast media (CM) volume required for diagnostic contrast enhancement of 350 HU (Hounsfield units) in the ascending aorta at different kV settings. METHODS: Dynamic computed tomography acquisitions from 70 to 150 kV were performed in a circulation phantom. First, injections with IDR ranging from 0.1 to 2.0 g I/s were tested for each kV. In the second part, the IDR was held constant, whereas the CM volume was reduced from 50 to 10 mL. Diagnostic aortic peak enhancement for each kV was compared using the Kruskal-Wallis test. P < 0.05 was considered statistically significant. RESULTS: The mean aortic peak enhancement for all diagnostic IDRs was 368.7 ± 11.1 HU. Diagnostic IDRs returned similar aortic peak enhancement values for all protocols (all P ≥ 0.18). For the second part of the study, a diagnostic enhancement was yielded by using a minimum of 30 mL of CM for 110 kV, 25 mL for 100 and 90 kV, and 15 mL for 80 and 70 kV. CONCLUSION: Our study suggests that a differentiated approach reducing the CM volume for tube voltages of less than 120 kV and increasing the IDR for higher kV settings seems to be the most effective approach.

RATIONALE AND OBJECTIVES: To evaluate the diagnostic accuracy of a prototype noncontrast, free-breathing, self-navigated 3D (SN3D) MR angiography (MRA) technique for the assessment of coronary artery anatomy in children with known or suspected coronary anomalies, using CT angiography (CTA) as the reference standard. MATERIALS AND METHODS: Twenty-one children (15 male, 12.3 ± 2.6 years) were prospectively enrolled between July 2014 and August 2016 in this IRB-approved, HIPAA-compliant study. Patients underwent same-day unenhanced SN3D-MRA and contrast-enhanced CTA. Two observers rated the visualization of coronary artery segments and diagnostic confidence on a 3-point scale and assessed coronary arteries for anomalous origin, as well as interarterial and intramural course. Sensitivity, specificity, positive (PPV) and negative predictive values (NPV) of SN3D-MRA for the detection of coronary artery abnormalities were calculated. Interobserver agreement was assessed using Intraclass Correlation Coefficients (ICC). RESULTS: Fourteen children showed coronary artery abnormalities on CTA. The visualization of coronary segments was rated significantly higher for CTA compared to MRA (p <0.015), except for the left main coronary artery (p = 0.301), with good to excellent interobserver agreement (ICC = 0.62-0.94). Diagnostic confidence was higher for CTA (p = 0.046). Sensitivity, specificity, PPV, and NPV of MRA were 92%, 92%, 96%, and 87% for the detection of coronary artery anomalies, 85%, 85%, 74%, and 92% for high origin, 71%, 92%, 82%, and 87% for interarterial, and 41%, 96%, 87%, and 80% for intramural course. CONCLUSIONS: Noncontrast SN3D-MRA is highly accurate for the detection of coronary artery anomalies in pediatric patients while diagnostic confidence and coronary artery visualization remain superior with CTA.

RESUMO

Myocardial T1-mapping has become feasible over the past decade as emerging technological magnetic resonance imaging advances enable increasingly rapid and reliable acquisition techniques. A variety of T1-mapping sequences are in development, with most allowing for the acquisition of a single-slice T1-map in a single breath-hold. The development of these protocols has spurred investigation into a wide range of potential clinical uses, including the characterization of cardiomyopathies. Although native T1-mapping provides superior tissue characterization, postcontrast T1-mapping is still indispensable for extracellular volume quantification. Ongoing and future studies should investigate the reliability, reproducibility, accuracy, and precision of available techniques and establish disease-specific reference values for T1-mapping-based parameters.

PURPOSE: The aim of this study was to prospectively evaluate the technical feasibility of a noncontrast magnetic resonance angiography (MRA) protocol using investigational prototype self-navigated 3D (SN3D) radial whole-heart and quiescent-interval single-shot (QISS) pulse sequences regarding their potential in planning transcatheter aortic valve replacement (TAVR). MATERIALS AND METHODS: Five patients (76±7 y) with severe aortic valve stenosis and prior computed tomographic angiography (CTA) for TAVR planning and 10 healthy volunteers (28±6 y) underwent noncontrast MRA on a 1.5 T system. A SN3D whole-heart acquisition was used to assess the aortic root anatomy. Evaluation of the aortoiliac access route was performed with both SN3D and QISS. Measurements were compared among SN3D, QISS, and CTA using a paired t test or 1-way analysis of variance. Image quality ratings and contrast-to-noise ratios (CNR) were analyzed using Mann-Whitney U tests. Interobserver agreement was evaluated using Cohen's κ. RESULTS: The combined SN3D and QISS protocol provided a 10.1±1.6-minute acquisition time. TAVR-relevant evaluation was technically feasible in healthy volunteers. All measurements showed good agreement with CTA in patients (all P>0.098). SN3D and QISS produced similar image quality both in volunteers and in patients (all P>0.122). There was no difference in qualitative ratings between MRA and CTA (all P>0.119). Interobserver agreement was good for MRA (κ=0.71 to 0.76) and excellent for CTA (κ=0.82 to 0.84). Thoracic SN3D provided a similar CNR compared with CTA (P=0.117). CTA yielded higher CNR in the abdominopelvic region compared with QISS (P=0.006). CONCLUSIONS: A noncontrast MRA protocol combining SN3D and QISS acquisitions for the assessment of cardiac and vascular access route anatomy has technical feasibility for TAVR planning.

BACKGROUND AND OBJECTIVES: Cognition in ESRD may be improved by kidney transplantation, but mechanisms are unclear. We explored patterns of resting-state networks with resting-state functional magnetic resonance imaging among patients with ESRD before and after kidney transplantation. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Thirty-seven patients with ESRD scheduled for kidney transplantation and 22 age-, sex-, and education-matched healthy subjects underwent resting-state functional magnetic resonance imaging. Patients were imaged before and 1 and 6 months after kidney transplantation. Functional connectivity of seven resting-state subnetworks was evaluated: default mode network, dorsal attention network, central executive network, self-referential network, sensorimotor network, visual network, and auditory network. Mixed effects models tested associations of ESRD, kidney transplantation, and neuropsychological measurements with functional connectivity. RESULTS: Compared with controls, pretransplant patients showed abnormal functional connectivity in six subnetworks. Compared with pretransplant patients, increased functional connectivity was observed in the default mode network, the dorsal attention network, the central executive network, the sensorimotor network, the auditory network, and the visual network 1 and 6 months after kidney transplantation (P=0.01). Six months after kidney transplantation, no significant difference in functional connectivity was observed for the dorsal attention network, the central executive network, the auditory network, or the visual network between patients and controls. Default mode network and sensorimotor network remained significantly different from those in controls when assessed 6 months after kidney transplantation. A relationship between functional connectivity and neuropsychological measurements was found in specific brain regions of some brain networks. CONCLUSIONS: The recovery patterns of resting-state subnetworks vary after kidney transplantation. The dorsal attention network, the central executive network, the auditory network, and the visual network recovered to normal levels, whereas the default mode network and the sensorimotor network did not recover completely 6 months after kidney transplantation. Neural resting-state functional connectivity was lower among patients with ESRD compared with control subjects, but it significantly improved with kidney transplantation. Resting-state subnetworks exhibited variable recovery, in some cases to levels that were no longer significantly different from those of normal controls.

To develop a quantitative T1-mapping-based synthetic inversion recovery (IRsynth) approach to calculate the optimal inversion time (TI0) for late gadolinium enhancement (LGE) imaging. Prospectively enrolled patients (n = 130, 58 ± 16 years) underwent cardiac MRI on a 1.5T system including Look-Locker TI-scout (LL), modified LL IR (MOLLI)-based T1-mapping, and LGE acquisitions. Patients were randomized into two groups: LL group (TI-scout followed T1-mapping) or MOLLI group (T1-mapping followed TI-scout). In both groups, the second acquisition was used to determine the TI0 for LGE. IRsynth images were generated from T1-maps between TI = 200-400 ms in 5 ms increments. Image quality was rated on a 3-point scale and the remote/background signal intensity ratio (SIR) was calculated. In the LL group (n = 53), the TI-scout-based TI0 was significantly shorter compared to IRsynth [230 ms (219-242) vs. 280 ms (263-297), P < 0.0001]. The TI0 used for LGE was set 30-40 ms longer [261 ms (247-276), P < 0.0001] than the TI-scout-based TI0, resulting in a TI0 ~ 20 ms shorter than what was obtained by IRsynth (P = 0.0156). In the MOLLI group (n = 63), IRsynth-based TI0 was significantly longer than the TI-scout-based TI0 [298 ms (262-334) vs. 242 ms (217-267), P = 0.0313]. The quality of myocardial nulling was rated higher [2.4 (2.2-2.5) vs. 2.0 (1.8-2.1), P = 0.0042] and the remote/background SIR was found to be more optimal (1.6 [1.1-2.1] vs. 2.6 [1.8-3.3], P = 0.0256) in the MOLLI group. T1-based IRsynth selects TI0 for LGE more accurately than conventional TI-scout imaging. IRsynth improves TI0 selection by providing excellent visualization of the representative image contrast for LGE images, reducing operator dependence in LGE acquisition.

RESUMO

The survival rate of patients with congenital heart disease (CHD) has dramatically improved over the last 2 decades because of technological and surgical advances in diagnosis and treatment, respectively. The vast majority of CHD patients are, in fact, amenable to treatment by either device closure or surgery. Considering the wide spectrum of surgical procedures and complex native and derived anatomy, continuous and detailed follow-up is of paramount importance. Cardiac magnetic resonance and cardiac computed tomography angiography are the cornerstones of diagnosis and follow-up of CHD, allowing for comprehensive noninvasive assessment of the heart, coronary tree, and intrathoracic great vessels, along with both morphological and functional evaluation. The aim of this pictorial review is to provide an overview of the most common CHDs and their related surgical procedures as familiarity with the radiological findings of grown-up congenital heart disease patients is crucial for proper diagnostic and follow-up pathways.

OBJECTIVE: We investigated the impact of iterative beam-hardening correction (IBHC) with advanced modeled iterative reconstruction (ADMIRE) of ultra-low radiation-dose tin filtered (Sn100 kVp) CACS acquisitions on image quality, calcium quantification, and risk classification. METHODS: CT data of 60 patients (55% male, age 62.3 ± 9.8years) who underwent a 3rd generation dual-source CACS scan using a prospectively ECG-triggered 100 kVp sequential acquisition protocol with tin filtration (Sn100 kVp) were reconstructed using IBHC with filtered back projection (FBP) and ADMIRE with strength levels of three and five. Image noise was calculated and Agatston scores were derived from all reconstructions. Image noise, Agatston score categories, and percentile-based cardiac risk categorization of the respective reconstruction techniques were compared. RESULTS: The mean estimated radiation dose equivalent of CACS acquisitions in the study population was 0.20 ± 0.07 mSv. Mean image noise significantly decreased with ADMIRE compared to FBP (both p < 0.0001). Agatston scores derived from the respective reconstructions were significantly different in a paired comparison (median [25th and 75th percentile]): FBP 34.7 [1.9, 153.6], ADMIRE 3 28.6 [1.1, 134.5], ADMIRE 5 22.7 [0.3, 116.8]; both p < 0.0001). Agatston score categories and cardiac risk categorization showed excellent agreement of ADMIRE 3 and ADMIRE 5 with FBP (ĸ = 0.92 [0.86-0.98] and ĸ = 0.86 [0.79-0.94]; ĸ = 0.94 [0.87-1.00] and ĸ = 0.91 [0.83-0.99]; however, modest cardiac risk reclassifications of 3% and 7% for ADMIRE 3 and ADMIRE 5, respectively, were observed. CONCLUSION: Iterative reconstruction using IBHC ADMIRE in low voltage, ultra-low dose CACS with tin filtration significantly decreased image noise. However, it also reduced Agatston scores compared to FBP, which may have an impact on subsequent cardiac risk classification, although risk reclassification occurred only in a modest number of subjects.

OBJECTIVES: To investigate the diagnostic accuracy of CT coronary artery calcium scoring (CACS) with tin pre-filtration (Sn100kVp) using iterative beam-hardening correction (IBHC) calcium material reconstruction compared to the standard 120kVp acquisition. BACKGROUND: Third generation dual-source CT (DSCT) CACS with Sn100kVp acquisition allows significant dose reduction. However, the Sn100kVp spectrum is harder with lower contrast compared to 120kVp, resulting in lower calcium score values. Sn100kVp spectral correction using IBHC-based calcium material reconstruction may restore comparable calcium values. METHODS: Image data of 62 patients (56% male, age 63.9±9.2years) who underwent a clinically-indicated CACS acquisition using the standard 120kVp protocol and an additional Sn100kVp CACS scan as part of a research study were retrospectively analyzed. Datasets of the Sn100kVp scans were reconstructed using a dedicated spectral IBHC CACS reconstruction to restore the spectral response of 120kVp spectra. Agatston scores were derived from 120kVp and IBHC reconstructed Sn100kVp studies. Pearson's correlation coefficient was assessed and Agatston score categories and percentile-based risk categorization were compared. RESULTS: Median Agatston scores derived from IBHC Sn100kVp scans and 120kVp acquisition were 31.7 and 34.1, respectively (p=0.057). Pearson's correlation coefficient showed excellent correlation between the acquisitions (r=0.99, p<0.0001). Agatston score categories and percentile-based cardiac risk categories showed excellent agreement (ĸ=1.00 and ĸ=0.99), resulting in a low cardiac risk reclassification of 1.6% with the use of IBHC CACS reconstruction. Image noise was 24.9±3.6HU in IBHC Sn100kVp and 17.1±3.9HU in 120kVp scans (p<0.0001). The dose-length-product was 13.2±3.4mGycm with IBHC Sn100kVp and 59.1±22.9mGycm with 120kVp scans (p<0.0001), resulting in a significantly lower effective radiation dose (0.19±0.07mSv vs. 0.83±0.33mSv, p<0.0001) for IBHC Sn100kVp scans. CONCLUSION: Low voltage CACS with tin filtration using a dedicated IBHC CACS material reconstruction algorithm shows excellent correlation and agreement with the standard 120kVp acquisition regarding Agatston score and cardiac risk categorization, while radiation dose is significantly reduced by 75% to the level of a chest x-ray.
