OBJECTIVE.: To assess the association between the air pollutants exposure on markers of oxidative stress and lung function in schoolchildren with and without asthma from Salamanca and Leon Guanajuato, Mexico. MATERIALS AND METHODS: We realized determinations of oxidative stress biomarkers and lung function tests in 314 schoolchildren. Information of air pollutants (O3, SO2, CO, PM2.5 and PM10) were obtained from monitoring stations and multiple linear regression models were run to assess the association. RESULTS: An increase of 0.09 pmol in conjugated dienes was observed by exposure to PM10 lag 1 in asthmatics from Salamanca (p<0.05). The exposure to O3 during the same day increased the concentration of Lipohydroperoxides in 4.38 nmol in asthmatics of Salamanca, as well as in 2.31 nmol by exposure to PM10 lag 2 (p<0.05). The forced vital capacity decreased by 138 and 203 ml in children without asthma, respectively, due to exposure to carbon monoxide (p<0.05). CONCLUSIONS: Exposure to air pollutants increase oxidative stress and decreased lung function in schoolchildren, with and without asthma.


OBJETIVO: Evaluar la asociación entre la exposición a contaminantes atmosféricos y marcadores de estrés oxidativo, por un lado, y la función pulmonar, por el otro, en escolares, con y sin asma, de las ciudades de Salamanca y León, en Guanajuato, México. MATERIAL Y MÉTODOS: Se realizaron determinaciones de marcadores de estrés oxidativo y pruebas de función pulmonar en 314 escolares, y se obtuvo información sobre contaminantes atmosféricos (ozono, dióxido de azufre, monóxido de carbono y partículas menores de 2.5 µm y menores de 10 µm) de las estaciones de monitoreo correspondientes. Para evaluar la asociación se corrieron modelos de regresión lineal múltiple. RESULTADOS: Con un día de retraso a la exposición a partículas menores de 10 µm (PM10), se observó un incremento de 0.09 pmol en los dienos conjugados entre niños asmáticos de Salamanca (p<0.05). La exposición a ozono durante el mismo día incrementó la concentración de lipo-hidroperóxidos en 4.38 nmol entre asmáticos de Salamanca, así como en 2.31 nmol por la exposición a PM10 para dos días de retraso (p<0.05). La capacidad vital forzada disminuyó 138 y 203 ml en niños sin asma, respectivamente, por la exposición a monóxido de carbono (p<0.05). CONCLUSIONES: La exposición a contaminantes atmosféricos incrementa el estrés oxidativo y disminuye la función pulmonar en escolares con y sin asma.

BACKGROUND: The impact of air pollution on the respiratory system has been estimated on the basis of respiratory symptoms and lung function. However; few studies have compared lung inflammation in healthy and asthmatics children exposed to high levels of air pollution. The aim of the study was to elucidate the modulatory effect of air pollution on Cysteinyl-leukotrienes (Cys-LTs) levels in exhaled breath condensate (EBC) among healthy and asthmatic children. METHODS: We performed a cross-sectional comparative study. Children between 7-12 years of age, asthmatics and non-asthmatics, residents of a city with high levels of PM10 were included. In all cases, forced spirometry, Cys-LTs levels in EBC, and the International Study of Asthma and Allergies in Childhood questionnaire were evaluated. We also obtained average of PM10, CO, SO2 and O3 levels during the period of the study by the State Institute of Ecology. RESULTS: We studied 103 children (51 asthmatics and 52 non-asthmatics). Cys-LTs levels were higher in asthmatics than in non-asthmatics (77.3 ± 21.6 versus 60.3 ± 26.8 pg/ml; p = 0.0005). Also, Cys-LTs levels in children with intermittent asthma were lower than in children with persistent asthma (60.4 ± 20.4 versus 84.7 ± 19.2 pg/ml; p = 0.0001). In the multiple regression model, factors associated with levels of Cys-LTs were passive smoking (ß = 13.1, p 0.04) and to be asthmatic (ß = 11.5, p 0.03). CONCLUSIONS: Cys-LTs levels are higher in asthmatic children than in healthy children in a contaminated city and its levels are also associated with passive smoking.

Gene-specific methylation has been related with transcriptional/translational consequences in different cells; also, this epigenetic modification is affected by environmental exposures. In previous studies, CYP2E1 activity in toluene-exposed workers was decreased compared to controls, however, CYP2E1 promoter methylation levels did not show significant differences. Here, we compared gene-specific methylation levels at the 5'UTR region, in a subset of workers whom already participated in two former studies, compared to controls. METHODS: DNA was obtained from whole blood in five different groups: occupationally exposed to a mixture of volatile organic compounds (VOC): high levels (n = 19); low levels (n = 19) and very low levels (n = 17), toluene-exposed workers (n = 19) and control group (n = 19). We performed PCR-pyrosequencing at the 5'UTR region from four genes: CYP2E1, IL-6, SOD1 and TNF-α. RESULTS: In participants exposed to high levels of a VOC mixture, we found significant differences: lower methylation levels for IL-6, and higher methylation levels for TNF-α compared to controls. In toluene-exposed workers, we found significant, lower methylation levels for CYP2E1 compared to controls. CONCLUSION: Lower methylation levels at the 5'UTR region from CYP2E1 in toluene exposed-workers, suggests that this epigenetic modification could represent a functional correlate regarding enzymatic activity, as a response to toluene biotransformation.

OBJECTIVE: To estimate autonomic balance and pulmonary function in obese women according to their baseline weight and weight gain during pregnancy. DESIGN: A cohort study. SETTING: Hospital of Gynecology and Obstetrics in Leon, Mexico. POPULATION: One hundred and seventy-eight healthy pregnant women (88 obese and 90 non-obese). METHODS: At first visit of pregnancy and in the third trimester blood pressure, anthropometric measurements, glucose, insulin, and leptin levels were evaluated and spirometry, oximetry, and 60 minutes electrocardiograph monitoring were performed. MAIN OUTCOME MEASURES: Heart rate variability and pulmonary function. RESULTS: Blood pressure levels, glucose, insulin, and leptin levels were higher in obese women, whereas total power of heart rate variability was lower in this group than in non-obese women at the beginning of pregnancy. In the third trimester, the standard deviation of all the normal R-R intervals was lower in obese women, whereas blood pressure and glucose levels remain higher. In the multiple regression analysis, the change in forced expiratory volume at 1 second/forced vital capacity (FEV1/FVC) ratio and leptin levels independently of weight gain were associated to low-frequency/high-frequency (LF/HF) index at third trimester (R2=0.21; p<0.001 for the model) only in obese women. CONCLUSIONS: LF/HF index at third trimester of pregnancy is associated with increase in leptin levels and decrease in FEV1/FVC in obese women independently of weight gain.

OBJECTIVE: to compare in postmenopausal women with osteoporosis or osteopenia the effect of hydrochlorate of raloxifene versus hormone replacement therapy on the mammary density. METHOD: a double blind, randomized clinical trial was conducted in 57 postmenopausal women older than 55 years with osteoporosis or osteopenia. Participants were randomly allocated in two groups: the intervention group (IG) received hydrochlorate of raloxifene and the control group (CG) received hormone replacement therapy during 6 months. A mammography to evaluate the change of the mammary density, according to the BIRADS criteria, was made before and after the treatment. RESULTS: twenty-eight patients received hormone replacement therapy and 29 hydrochlorate of raloxifene. Eight (28.5 %) patients of the IG and one (3.0 %) of the CG showed an increased mammary density; whereas in none of IG patients and in four (13.7%) of the CG, the mammary density decreased (p = 0.01). CONCLUSIONS: the hydrochlorate of raloxifene in postmenopausal women with osteoporosis or osteopenia does not increase the mammary density.

OBJECTIVE: to determine the effect of the epinephrine-lidocaine anaesthetic solutions on blood pressure in healthy children subjected to short dentistry procedures. METHODS: we performed a prospective, observational and analytical study in 39 children with 7 and 8 years of age, who were subjected to dentistry treatment of short duration in a dentistry service. The blood pressure was measured in three times: previous to the infiltration of lidocaine to 2 % with epinephrine; 10 minutes later; at the end of procedure. RESULTS: the systolic blood pressure, like diastolic and the average increased after 10 minutes of the infiltration with the anesthetic-vasoconstrictor solution, but this difference was not significant (p = 0.39, 0.14 and 0.40). We observed an increase of the systolic (6.0 mm Hg, 95 % CI = 4.6-7.5, F = 14.0, p = 0.0001), diastolic (9.9 mm Hg, 95 % CI = 7.3-12.5; F = 26.0, p = 0.0001) and average arterial tension (7.3 mm Hg, 95 % IC = 5.8-8.8; F = 23, p = 0.0001) to the 10 minutes after the treatment. CONCLUSIONS: the use of the epinephrine-lidocaine anaesthetic solutions does not have significant effect on the blood pressure of children subjected to short dentistry procedures.

INTRODUCTION: adequate nutrition in adolescence is important for growth and development. There are environmental factors that cannot be avoided, such as exposure to heavy metals through natural sources such as water. Arsenic is a metalloid that can cause damage to health (alterations in nutritional status, diabetes, cancer) and it has been found in concentrations higher than those allowed in drinking water. OBJECTIVE: to measure the effect of vitamin and mineral supplementation on the nutritional status and urinary excretion of arsenic in adolescents exposed to this metal through drinking water. MATERIAL AND METHODS: an observational, follow-up study of a cohort was conducted to assess the efficacy of vitamin and mineral supplementation on supplementation in 45 adolescents exposed to arsenic in drinking water, who were given a daily multivitamin supplement for four weeks. Weekly nutritional status and arsenic levels in urine and drinking water were evaluated. RESULTS: the basal nutritional intake was low for proteins, fiber, folic acid, vitamin B2, B6, B12, E, C, selenium and iron, increasing their consumption through the supplement during the intervention and with an increase of approximately 1 g/dl of hemoglobin in all participants. At the end of the intervention, there was an increase in fat-free mass and a decrease in the percentage of body fat. In relation to the urinary excretion of arsenic, the biggest elimination of this metalloid was observed from the first week of intervention (35.91 µg/g Cr [IC 95% = 23.2-74.8 µg/g Cr]), which was statistically significant compared to basal levels of urinary arsenic (43.2 µg/g Cr [IC 95% = 30.8-117.6 µg/g Cr]) (p < 0.05), with an average water consumption with As of 96.2 ± 7.5 µg/l. CONCLUSION: four weeks of supplementation with vitamins and minerals in the adolescent population studied improved nutritional status and increased metalloid excretion significantly in the first and second week after intervention.

Existen factores ambientales que no pueden evitarse, como la exposición a metales pesados a través de fuentes naturales como el agua. El arsénico es un metaloide que puede causar un daño a la salud (alteraciones del estado nutricio, diabetes, cáncer) y ha sido encontrado en concentraciones superiores a las permitidas en el agua de consumo.Objetivo: medir el efecto de una suplementación de vitaminas y minerales sobre el estado nutricio y la excreción urinaria de arsénico en adolescentes expuestos a este metal a través de agua de consumo.Material y métodos: se realizó un estudio de intervención y longitudinal para la valorar la eficacia de la suplementación de vitaminas y minerales sobre la suplementación en 45 adolescentes, expuestos a arsénico en agua de consumo, a quienes se dio un suplemento multivitamínico diariamente durante cuatro semanas. Por otro lado, en cuanto a la excreción urinaria de arsénico, se observó mayor eliminación de este metal (35,91 µg/g Cr [IC 95% = 23,2-74,8 µg/g Cr]) desde la primera semana de intervención, la cual fue estadísticamente significativa en comparación con los niveles basales de arsénico urinario (43,2 µg/g Cr [IC 95% = 30,8-117,6 µg/g Cr]) (p < 0,05), con un consumo promedio de agua con As de 96,2 ± 7,5 µg/l.Conclusión: la suplementación con vitaminas y minerales de cuatro semanas en la población de adolescentes estudiada mejoró el estado nutricio y aumentó la excreción del metaloide de manera significativa en la primera y segunda semana postintervención.

Aim of the Study. To compare the effect of glossopharyngeal nerve block with topical anesthesia on the tolerance of patients to upper gastrointestinal endoscopy. Methods. We performed a clinical trial in one hundred patients undergoing upper gastrointestinal endoscopy. Subjects were randomly assigned to one of the following two groups: (1) treatment with bilateral glossopharyngeal nerve block (GFNB) and intravenous midazolam or (2) treatment with topical anesthetic (TASS) and intravenous midazolam. We evaluated sedation, tolerance to the procedure, hemodynamic stability, and adverse symptoms. Results. We studied 46 men and 54 women, from 17 to 78 years of age. The procedure was reported without discomfort in 48 patients (88%) in the GFNB group and 32 (64%) in the TAAS group; 6 patients (12%) in GFNB group and 18 (36%) in TAAS group reported the procedure as little discomfort (χ (2) = 3.95, P = 0.04). There was no difference in frequency of nausea (4% in both groups) and retching, 4% versus 8% for GFNB and TASS group, respectively (P = 0.55). Conclusions. The use of glossopharyngeal nerve block provides greater comfort and tolerance to the patient undergoing upper gastrointestinal endoscopy. It also reduces the need for sedation.

The forced vital capacity decreased by 138 and 203 ml in children without asthma, respectively, due to exposure to carbon monoxide (p<0.05). Conclusions: Exposure to air pollutants increase oxidative stress and decreased lung function in schoolchildren, with and without asthma.

BACKGROUND: Recent studies have reported an increase in the prevalence of obesity and metabolic syndrome in children and adolescents. However, few have focused how diabetes mellitus and metabolic syndrome together in parents can influence on obesity and metabolic disturbances in offspring. OBJECTIVE: To know the risk obesity and metabolic disturbance in children, adolescents, and young adults whose parents have diabetes mellitus and metabolic syndrome. METHODS: A comparative survey was made in healthy children of parents with diabetes mellitus and metabolic syndrome compared with offspring of healthy parents. We performed anthropometry and evaluated blood pressure, glucose, total cholesterol, HDL cholesterol, and triglycerides levels in plasma. We registered parent antecedents to diabetes mellitus and metabolic syndrome and investigated the prevalence of overweight, obesity, and metabolic disturbances in offspring. RESULTS: We studied 259 subjects of 7 to 20 years of age. The prevalence of overweight and obesity was 27% and 37%, respectively. The highest proportion of BMI >95th of the entire group was found in offspring with both diabetic parents. Glucose and total cholesterol levels were lower in the group with healthy parents compared with the group with diabetic mother and metabolic syndrome but with healthy father. HDL cholesterol was higher in the group with both healthy parents than in the group with diabetic mother and metabolic syndrome but healthy father. CONCLUSIONS: The offspring of parents with diabetes plus metabolic syndrome showed higher proportion of variables related to metabolic syndrome compared with healthy parents.
