Background and Objectives: Attentional bias (AB) for addictive substances is a feature of attention found in individuals with substance misuse or diagnosed with substance use disorders. When AB exists, the attention of the addicted individual may be quickly oriented to cues related to the addictive substance or be maintained on these cues for a longer time. AB toward opioids was found in Western samples of smokers with chronic noncancer pain. The level of AB was dose-responsive. However, similar studies in the Taiwanese population are lacking. This study compared the patterns of AB for opioid analgesics in Taiwanese participants with chronic noncancer pain to that of individuals without pain. This study aimed to investigate if AB toward opioids is presented in Taiwanese heavy smokers who are on long-term opioid therapy for pain control. Materials and Methods: Participants were grouped into chronic noncancer pain smokers, chronic pain nonsmokers, and smokers without pain, according to smoking habits and whether or not on long-term opioid therapy for pain control. Each participant completed demographic questionnaires, mood scales, and the opioid-related visual probe task. Differences in AB among the groups were compared using a three-way analysis of covariance controlling for daily cigarette consumption. Results: Chronic noncancer pain smokers (n = 17) and chronic pain nonsmokers (n = 16) displayed more severe levels of depression, anxiety, and pain, compared to smokers without pain (n = 28). Only did chronic pain nonsmokers show significant AB for opioid cues that were displayed for a short time. Analysis on reaction time found that smokers without pain consistently responded faster to the tasks. No difference in reaction time was found between the pain groups. Conclusions: The current study did not fully replicate findings from studies that were based in Western countries. Formulary availability and regulatory limitations might have affected patient's perception of prescription opioids in Taiwan. However, chronic pain nonsmokers exhibited initial orientation toward opioid-related cues when daily cigarette consumption was accounted for. According to previous research, this AB for shortly displayed opioid cues can be associated with the expectation of pain relief. The current finding also indicated general psychomotor retardation in individuals who were on long-term use of opioids.

Density functional theory calculations were employed to investigate the Pd-catalyzed regio-selective hydroallylations of alkynes with allylborons: cooperation of Cu(OAc)2 and dppe resulting in 1,4-dienes while combination of AdCO2H and PCy3 leading to 1,5-dienes. A unified rationalization mechanism called "Lewis-acid-base-interaction promoted deprotonation/3,3-rearrangement" was proposed. Compared with the commonly reported metathesis pathway to only afford the metal-allyl intermediate, in the newly established mechanism, an additional Brønsted acid (as an initiator of the Pd0 oxidative addition) is generated by the interaction of the allylboron (Lewis acid) B atom with the nBuOH (Lewis base) O atom, and subsequent 3,3-rearrangement ensures the thermodynamic feasibility of the reaction. In addition, it was found that excess Cu(OAc)2 plays two potential roles in the oxidative addition/alkyne insertion: (i) the participation of one AcO- of Cu(OAc)2 ensures a large orbital overlap between the migrating H and Pd atoms, facilitating the formal AcO-H cleavage and (ii) the extra (OAc)2Cu···O(carboxyl) σ-coordination indirectly contributes to the (Me)C≡C(Ph) insertion into the Pd-H bond. Further analysis showed that the origin of the regioselectivity is closely related to the employed phosphorus ligand. These revealed results, which have been overlooked in the previous documents, would aid the development of new related catalytic reactions.

PURPOSE: During COVID-19 pandemic, the authorization of emergent usage of new vaccine has raised suspicions and doubts about potential adverse events related to vaccination. Among the reported adverse events related to ChAdOx1/nCoV-19 vaccine, facial paralysis did not have an incident rate higher than natural occurrence like mRNA vaccines. However, temporal association between vaccination and facial palsy have been documented in several studies. Here, we report a case of an otherwise healthy 23-year-old Taiwanese female who experienced prolonged headache since the second day postvaccination and developed facial palsy on the tenth day. CASE REPORT: A 23-year-old Taiwanese female who was previously healthy experienced intermittent right side throbbing headache, general malaise, myalgia and fever. Headache, transient ear pain and right scalp numbness developed in the next few days but quickly resolved. On day ten after vaccination, signs of facial palsy on the right side of her face was noticed. The results of brain Magnetic Resonance Imaging (MRI) with contrast displayed no abnormality. Facial stimulation and blink reflex tests were compatible with right facial neuropathy. CONCLUSION: Reactivation of latent herpes virus has been suggested as one of the possible mechanisms underlying the phenomenon, but the causal pathophysiology related to the symptom needs further validation. Moreover, in the event of facial palsy post-vaccination, alternative diagnoses such as Guillain-Barre syndrome (GBS), Ramsey-Hunt syndrome, Lyme disease, trauma, central nervous system infection (CNS) infection, or stroke should also be considered.

Background and Objectives: Local anesthetics administered via epidural catheters have evolved from intermittent top-ups to simultaneous administration of continuous epidural infusion (CEI) and patient-controlled epidural analgesia (PCEA) using the same device. The latest programmed intermittent epidural bolus (PIEB) model is believed to create a wider and more even distribution of analgesia inside the epidural space. The switch from CEI + PCEA to PIEB + PCEA in our department began in 2018; however, we received conflicting feedback regarding workload from the quality assurance team. This study aimed to investigate the benefits and drawbacks of this conversion, including the differences in acute pain service (APS) staff workload, maternal satisfaction, side effects, and complications before and after the changeover. Materials and Methods: Items from the APS records included total delivery time, average local anesthetic dosage, and the formerly mentioned items. The incidence of side effects, the association between the duration of delivery and total dosage, and hourly medication usage in the time subgroups of the CEI and PIEB groups were compared. The staff workload incurred from rescue bolus injection, catheter adjustment, and dosage adjustment was also analyzed. Results: The final analysis included 214 and 272 cases of CEI + PCEA and PIEB + PCEA for labor analgesia, respectively. The total amount of medication and average hourly dosage were significantly lower in the PIEB + PCEA group. The incidences of dosage change, manual bolus, extra visits per patient, and lidocaine use for rescue bolus were greater in the PIEB + PCEA group, indicating an increased staff workload. However, the two groups did not differ in CS rates, labor time, maternal satisfaction, and side effects. Conclusions: This study revealed that while PIEB + PCEA maintained the advantage of decreasing total drug doses, it inadvertently increased the staff burden. Increased workload might be a consideration in clinical settings when choosing between different methods of PCEA.

Tumor-targeted and stimuli-activatable nanosensitizers are highly desirable for cancer theranostics. However, designing smart nanosensitizers with multiple imaging signals and synergistic therapeutic activities switched on is challenging. Herein, we report tumor-targeted and redox-activatable nanosensitizers (1-NPs) for sono-photodynamic immunotherapy of tumors by molecular co-assembly and redox-controlled disassembly. 1-NPs show a high longitudinal relaxivity (r1 =18.7±0.3 mM-1 s-1 ), but "off" dual fluorescence (FL) emission (at 547 and 672 nm), "off" sono-photodynamic therapy and indoleamine 2,3-dioxygenase 1 (IDO1) inhibition activities. Upon reduction by glutathione (GSH), 1-NPs rapidly disassemble and remotely release small molecules 2-Gd, Zn-PPA-SH and NLG919, concurrently switching on (1) dual FL emission, (2) sono-photodynamic therapy and (3) IDO1 inhibition activities. After systemic injection, 1-NPs are effective for bimodal FL and magnetic resonance (MR) imaging-guided sono-photodynamic immunotherapy of orthotropic breast and brain tumors in mice under combined ultrasound (US) and 671-nm laser irradiation.

An iron-based catalytic system was developed for the cross-coupling of 1-bromoalkynes with terminal alkynes to selectively generate unsymmetrical 1,3-butadiynes in water under air. It was found that a combination of 1-bromoalkynes derived from less acidic terminal alkynes with more acidic counterparts would greatly enhance yields and selectivity for unsymmetrical 1,3-butadiynes. The reaction was also applicable for the synthesis of unsymmetrical 1,3,5-hexatriynes through coupling of 1-bromoalkynes and trimethylsilyl-protected 1,3-butadiynes in a one-pot manner.

A facile access to highly fused tetracyclic indeno-1,2-benzothiazines has been established via a Rh(III)-catalyzed C-H bond activation and intramolecular annulation cascade between sulfoximides and all-carbon diazo indandiones. This strategy is characterized by the fact that the diazo coupling partners do not require preactivation, along with its high efficiency, broad substrate generality, and facile transformation. Particularly, the highly conjugated tetracyclic products demonstrate good optical properties and can easily enter cells to emit bright fluorescence for live cell imaging.

Intravenous thrombolytic therapy is recommended within 3 hours of onset of acute ischemic stroke. However, stroke mimics create challenges because of time pressures. We describe a case of an undiagnosed glioma, a rarely reported condition, that was nearly treated with thrombolytic therapy. A 71-year-old man presented to the emergency department with sudden left gaze preference, right-sided hemiplegia, and global aphasia, which suggested a large infarction in the left hemisphere. The thrombolytic protocol was started at once. However, noncontrast computed tomography (CT) of the brain, CT angiography, and CT perfusion were essentially normal. Later, magnetic resonance imaging of the brain demonstrated a diffuse intracerebral lesion. The patient was found to have an undiagnosed diffuse glioma, whose initial neurologic symptoms mimicked acute stroke within 3 hours of onset. Cerebral neoplasms are an absolute contraindication for thrombolysis therapy. Several brain tumors are rarely seen on a brain CT scan. The mismatch between neurologic examination and brain perfusion imaging may suggest a stroke mimic in some cases.

Pd-catalyzed alkoxycarbonylation of alkynes provided a redox-neutral method to selectively access branched/linear α,ß-unsaturated monoesters and 1,4-dicarboxylic acid diesters. Herein, a systematic computational study was performed to elucidate the mechanism and origin of ligand-controlled chemo- and regioselectivities. It is found that the catalytic cycle, including hydrometallation, carbon monoxide insertion, and methanolysis, is more likely than that involving palladium alkoxycarbonyl intermediates. Both hydrometallation and methanolysis stages are important to determine the chemo- and regioselectivities. Hydrometallation proceeds via anti-Markovnikov-selective migratory insertion or Markovnikov-selective ligand-participated electrophilic addition. A flexible bidentate phosphine ligand slows down migratory insertion due to the stronger trans effect of the CO ligand but accelerates the ligand-participated electrophilic addition by adopting better orbital orientations. On the other hand, a ligand-participated mechanism and an unrevealed mechanism involving ketene intermediates can promote methanolysis, whereas ligands with large bite angles or bulky substituents are detrimental to methanolysis. On the basis of these mechanistic foundations, the influence of the flexibility, basicity, bite angle, and steric hindrance of ligands on chemo- and regioselectivities was clarified. The present study provided more universal and deeper mechanistic insights into Pd-catalyzed alkoxycarbonylation reactions and shed light on the superior regulation performance of the bifunctional pyridyl-containing phosphine ligands.

A traceless approach to quinolin-4(1H)-one scaffolds through Rh(III)-catalyzed redox-neutral [3+3] cyclization of N-nitrosoanilines with cyclopropenones has been achieved. This protocol features short reaction time and atom-economical combination without extra additives, which can be further applied in the construction of privileged heterocyclic compounds in pharmaceutical chemistry.

Circular RNA (circRNA) is a group of RNA family generated by RNA circularization, which was discovered ubiquitously across different species and tissues. However, there is no global view of tissue specificity for circRNAs to date. Here we performed the comprehensive analysis to characterize the features of human and mouse tissue-specific (TS) circRNAs. We identified in total 302 853 TS circRNAs in the human and mouse genome, and showed that the brain has the highest abundance of TS circRNAs. We further confirmed the existence of circRNAs by reverse transcription polymerase chain reaction (RT-PCR). We also characterized the genomic location and conservation of these TS circRNAs and showed that the majority of TS circRNAs are generated from exonic regions. To further understand the potential functions of TS circRNAs, we identified microRNAs and RNA binding protein, which might bind to TS circRNAs. This process suggested their involvement in development and organ differentiation. Finally, we constructed an integrated database TSCD (Tissue-Specific CircRNA Database: http://gb.whu.edu.cn/TSCD) to deposit the features of TS circRNAs. This study is the first comprehensive view of TS circRNAs in human and mouse, which shed light on circRNA functions in organ development and disorders.

Density functional theory (DFT) calculations have been performed to study the Pd-catalyzed C-H functionalization of (E)-N-methoxy cinnamamide (E1), which selectively provides the α-C-H activation products (EP as minor product and its C═C rotation isomer ZP' as major product). Three crucial issues are solved: (i) The detailed mechanism leading to ZP' is one issue. The computational analyses of the mechanisms proposed in previously experimental and theoretical literature do not seem to be consistent with the experimental findings due to the high barriers involved. Alternatively, we present a novel oxidation/reduction-promoted mechanism featuring the Pd(0) → Pd(II) → Pd(0) transformation. The newly proposed mechanism involves the initial coordination of the active catalyst PdL2 (L = t-BuCN) with the C═C bond in EP, followed by the oxidative cyclization/reductive decyclization-assisted C═C double-bond rotation processes resulting in ZP' and regeneration of PdL2. (ii) The origin of the product E/Z selectivity is the second issue. On the basis of the calculated results, it is found that, at the initial stage of the reaction, EP is certainly completely generated, while no ZP' formation occurred. Once E1 is used up, EP immediately acts as the partner of the new catalytic cycle and sluggishly evolves into ZP'. A small amount of generated ZP' would reversibly transform to EP due to the higher barrier involved. (iii) The intrinsic reasons for the regioselectivity are the third issue. The calculated results indicate that the regioselectivity for α-C-H activation is mainly attributed to the stronger electrostatic attraction between the α-C and the metal center.

A water-soluble PdCl2(NH3)2/cationic 2,2'-bipyridyl system was found to be a highly efficient catalyst for Stille coupling of aryl iodides and bromides with organostannanes. The coupling reaction was conducted at 110 °C in water, under aerobic conditions, in the presence of NaHCO3 as a base to afford corresponding Stille coupling products in good to high yields. When aryltributylstannanes were employed, the reactions proceeded smoothly under a very low catalyst loading (as little as 0.0001 mol %). After simple extraction, the residual aqueous phase could be reused in subsequent runs, making this Stille coupling economical. In the case of tetramethylstannane, however, a greater catalyst loading (1 mol %) and the use of tetraethylammonium iodide as a phase-transfer agent were required in order to obtain satisfactory yields.

BACKGROUND: The development of internet technology has greatly increased the ability of patients with chronic obstructive pulmonary disease (COPD) to obtain health information, giving patients more initiative in the patient-physician decision-making process. However, concerns about the quality of website health information will affect the enthusiasm of patients' website search behavior. Therefore, it is necessary to evaluate the current situation of Chinese internet information on COPD. OBJECTIVE: This study aims to evaluate the quality of COPD treatment information on the Chinese internet. METHODS: Using the standard disease name "" ("chronic obstructive pulmonary disease" in Chinese) and the commonly used public search terms "" ("COPD") and "" ("emphysema") combined with the keyword "" ("treatment"), we searched the PC client web page of Baidu, Sogou, and 360 search engines and screened the first 50 links of the website from July to August 2021. The language was restricted to Chinese for all the websites. The DISCERN tool was used to evaluate the websites. RESULTS: A total of 96 websites were included and analyzed. The mean overall DISCERN score for all websites was 30.4 (SD 10.3; range 17.3-58.7; low quality), no website reached the maximum DISCERN score of 75, and the mean score for each item was 2.0 (SD 0.7; range 1.2-3.9). There were significant differences in mean DISCERN scores between terms, with "chronic obstructive pulmonary disease" having the highest mean score. CONCLUSIONS: The quality of COPD information on the Chinese internet is poor, which is mainly reflected in the low reliability and relevance of COPD treatment information, which can easily lead consumers to make inappropriate treatment choices. The term "chronic obstructive pulmonary disease" has the highest DISCERN score among commonly used disease search terms. It is recommended that consumers use standard disease names when searching for website information, as the information obtained is relatively reliable.

Background: In volleyball, the jump serve is a crucial and commonly used serving technique. Nonetheless, the angular momentum developed during the jump serve remains unexplored. The objectives of the current study were to determine the angular momentum manifesting during the airborne phase of the jump serve and to analyse the correlations between the angular momentum variables and arm swing speed. Methods: Three-dimensional coordinate data were obtained during the jump serves of 17 professional male volleyball players. Correlation and linear regression analyses were used to identify the angular momentum variables linked to the arm swing speed at ball impact (BI). Results: The arm swing speed at BI exhibited significant correlations with the peak angular momentum of the attack arm (r = 0.551, p = 0.024), non-attack arm (r = 0.608, p = 0.011), non-attack leg (r = -0.516, p = 0.034), forearm (r = 0.527, p = 0.032), and hand (r = 0.824, p < 0.001). A stepwise regression model (R2 = 0.35, p = 0.043) predicted arm swing speed based on the peak angular momentum of the non-attack leg, forearm, and hand. Conclusions: The study results suggest that during the arm-acceleration phase, (1) increasing angular momentum with the non-attack leg helps maintain aerial body balance, thereby enhancing arm swing execution, and (2) controlling the magnitude and timing of the force exerted by the elbow and wrist is crucial for effectively transmitting angular momentum, contributing to an increase in arm swing speed.

The delivery of ecosystem services (ESs), particularly in urban agglomerations, faces substantial threats from impending future climate change and human activity. Assessing ES bundles (ESBs) is critical to understanding the spatial allocation and interactions between multiple ESs. However, dynamic projections of ESBs under various future scenarios are still lacking, and their underlying driving mechanisms have received insufficient attention. This study examined the Beijing-Tianjin-Hebei urban agglomeration and proposed a framework that integrates patch-generating land use simulation into three shared socioeconomic pathway (SSP) scenarios and clustering analysis to assess spatiotemporal variations in seven ESs and ESBs from 1990 to 2050. The spatial trajectories of ESBs were analyzed to identify fluctuating regions susceptible to SSP scenarios. The results indicated that (1) different scenarios exhibited different loss rates of regulating and supporting services, where the mitigation of degradation was most significant under SSP126. The comprehensive ES value was highest under SSP245. (2) Bundles 1 and 2 (dominated by regulating and supporting services) had the largest total proportion under SSP126 (51.92 %). The largest total proportion of Bundles 4 and 5 occurred under SSP585 (48.96 %), with the highest provisioning services. The SSP126 scenario was projected to have the least ESB fluctuation at the grid scale, while the most occurred under SSP585. (3) Notably, synergies between regulating/supporting services were weaker under SSP126 than under either SSP245 or SSP585, while trade-offs between water yield and non-provisioning services were strongest. (4) Forestland and grassland proportions significantly affected carbon sequestration and habitat quality. Climatic factors (precipitation and temperature) acted as the dominant drivers of provisioning services, particularly water yield. Our findings advocate spatial strategies for future regional ES management to address upcoming risks.

Objective: To investigate effect of flow speeds on the upper limb muscular activity of butterfly swimmers training in a flow environment. A comparison of kinematic characteristics and muscular activity of upper limbs were made when the swimmers training with different flow speeds in a swimming flume. The purpose was to provide a basis for scientifically formulating special swimming training advice for athletes' training in flow environment. Methods: Ten youth female butterfly swimmers participated in the study with the speed of 70%, 80%, and 90% level of their max speeds. A stroke cycle was divided into four phases (entry, pull, push, and recovery). The kinematic parameters of upper limbs (stroke rate, stroke length, duration of each phase in a stroke cycle) and muscular activity (onset timing, integrated electromyography (iEMG), contribution ratio) of four muscles (Biceps brachii (BB), Triceps brachii (TB), Pectoralis major (PM), and Latissimus dorsi (LD)) were collected and analyzed in different stroke phases. Results: There was no significant difference between stroke rate and stroke length with different flow speeds. There were significant differences among the duration of the four stroke phases. The entry phase had the longest duration, the pull phase had the shortest duration, the push phase was longer than the recovery phase, and the recovery phase was shorter than the entry phase. The BB and PM were activated significantly earlier at 90% of target speed than at 80% of target speed, while the TB was activated significantly later than other two speeds. The muscular contribution ratio of the PM was highest in the pull phase and lowest in the pushing phase. The muscular contribution ratio of the BB was significantly lower in the pushing phase than in other three stroke phases. The muscular contribution of the TB was significantly higher in the recovery phase than in other three stroke phases. The muscular contribution ratio of the LD was highest in the pushing phase, and it was significantly higher in pushing phase and recovery phase than in pull phase. Conclusions: (1) When butterfly athletes training with 70%, 80% and 90% of their max speed in a flow environment, it didn't make significant differences between the kinematic or muscle activation characteristics of the upper limbs movement except the muscle onset timing. (2) Stroke phase was the main factor of the duration and the muscle contribution ratio during butterfly arm stroke for young athletes.

BACKGROUND: It is rare for newly diagnosed (de novo) or newly treated acute myeloid leukemia (AML) complicated with thrombotic complications, especially combined arterial and venous thrombosis. METHODS: We reported a 13-year-old boy diagnosed with AML and leukocytosis, who developed right femoral vein and right dorsal artery thrombosis during chemotherapy. After treatment with low molecular weight heparin, diosmin, and alprostadil, symptoms were relieved. Unfortunately, the child suffered from coagulopathy afterward, which was unexpectedly caused by vitamin K deficiency. RESULTS: After supplementation with vitamin K and prothrombin complex concentrate, coagulation function recovered. CONCLUSION: For childhood AML patients with high thrombotic risks, close monitoring during anticoagulant treatment was necessary. Concomitantly, we should be alert to past medication history and combined medication use, especially those that may lead to vitamin K deficiency, secondary bleeding, and coagulation disorders. Rational use of antibiotics, anticoagulants, and antitumor drugs must be guaranteed.

BACKGROUND: Lower limb heat exposure (LLHE) is a promising strategy for the daily management of cardiovascular health because of its non-pharmaceutical advantages. To support the application of this strategy in cardiovascular protection, we examined its impact on the global hemodynamic environment. METHODS: Skin blood flow (SBF) of eight locations on the lower limbs was measured before and after LLHE (40 °C and 44 °C) in ten healthy subjects by using a laser Doppler flowmeter. A closed-loop model of circulation uses changes in SBF to quantify the influence of LLHE on the blood flow of the arterial trunk (from ascending aorta to the femoral artery) and visceral branches (coronary, celiac, renal, and mesenteric arteries). RESULTS: The SBF in all locations tested on the lower limbs increased significantly (p<0.001) with LLHE and a 3.39-fold and 7.40-fold increase in mean SBF were observed under 40 °C and 44 °C conditions, respectively. In the model, the peak (3.9-25.1%), end-diastolic (13.7-107.3%), and mean blood flow (8.5-86.5%) in the arterial trunk increased with the increase in temperature, but the retrograde flow in the thoracic aorta and abdominal aorta Ⅰ increased at least twice in the diastolic period. Furthermore, LLHE also increased the blood flow of the visceral branches (2.5-20.7%). CONCLUSION: These findings suggest that LLHE is expected to be a daily strategy for enhancing the functions of both the arterial trunk and visceral arteries, but the increased blood flow reversal in the thoracic and abdominal aortas warrants further investigation.
