RESUMO

Light absorption enhancement (Eabs) of black carbon (BC) is a key factor in global climate models and is impacted by brown carbon (BrC) and the lensing effect of coatings. We conducted an in-depth field study on Eabs for ambient aerosols at a monitoring point in Shanghai, China, by real-time aerosol optical property monitoring and high-performance liquid chromatography/diode array detector/quadrupole-time-of-flight mass spectrometry (HPLC/DAD/Q-ToF-MS) analysis. The results showed Eabs at λ = 530 nm caused by the lensing effect was about 1.39 ± 027, accounting for 18.84% of the total light absorption. In this study, BrC is classified as soluble BrC (soluble in both water and methanol) or insoluble BrC (insoluble in both water and methanol). Soluble BrC accounted for 13.68 ± 11.15% of the total aerosol light absorption. For the first time, we concluded that insoluble BrC can contribute more than 60 and 97% of total aerosol and BrC light absorption in port cities, respectively. The molecular analysis of soluble BrC identified N-containing aromatic compounds (4-nitrophenol, 4-nitrocatechol, methyl nitrophenol, methyl nitrocatechols, and nitro-1-naphthol) commonly observed in biomass burning emissions or biomass burning-impacted atmospheres. A series of components (C16H26O3S, C17H28O3S, C18H30O3S, and C19H32O3S) were determined to be emissions from nearby cargo ships filled with heavy fuel oil (HFO), which further confirmed that insoluble BrC emitted from cargo ships could be the largest contributor to Eabs. This study confirms the global significance of evaluating HFO used in port cities in climate models. The control measures of cargo ship emission should be considered for the related environmental and health issues in port cities.

The molecular characteristics of organic aerosols (OAs) in heavily polluted areas affected by coal combustion (CC) were investigated. In terms of relative abundance, the total nitrogen-containing organic compounds (NOC) accounted for about 61%-68% of all molecules detected in methanol-soluble organic carbon (MSOC) by LC - Q-TOF - MS. More than 85% of the CHON- formulas are nitro-aromatic compounds, which are generally considered to be secondary organic compounds, as evidenced by the lower degree of overlap of these substances in the atmospheric samples and CC samples. Some polycyclic aromatic compounds with 4 N and 1-2O and very low H/C and O/C ratio produced by CC are unstable and easily react to form compounds with higher degrees of saturation. Almost all of the CHON+ homologues detected in the CC samples were also found in the atmospheric samples, indicating that the large amount of CHON+ compounds produced by CC are stable during atmospheric processes. The CHN+ compounds produced by CC contain a certain amount of highly unsaturated compounds, among which 1 N-containing polycyclic aromatic hydrocarbons (1 N-PAHs) is stable in atmosphere and can serve as markers of CC.

BACKGROUND: Percutaneous transhepatic cholangiography drainage (PTCD) effectively treats biliary obstruction. However, patients must maintain the drainage tube after hospital discharge, which may interfere with daily life and work, potentially causing psychological distress. Postoperative rehabilitation is crucial, and strengthened nursing interventions can shorten recovery time. AIM: The aim was to evaluate an inpatient model to shorten rehabilitation duration and improve quality of life after PTCD. METHODS: A total of 118 patients with malignant obstructive jaundice who were admitted to our hospital between May 2018 and January 2021 were included and divided into observational (with therapy) and control (no therapy) groups of 59 each. RESULTS: The observational group had fewer hospitalization days than the control group. The complication, the PTCD fixed-tube prolapse, and tube-related admission rates within 3 mo after PTCD were significantly lower in the observation group than in the control group (P < 0.05). The fatigue, pain, nausea, vomiting, pruritus, emaciation, and fever scores after PTCD decreased in both groups compared with the scores before PTCD (P < 0.05). The quality of life scores after the intervention were higher in the observation than in the control group (P < 0.05). CONCLUSION: The model promoted rehabilitation after PTCD, reduced post-PTCD complications, and the tube-related admissions in the 3 mo after the procedure, and improved the quality of life.

BACKGROUND: Epidemiological studies have found that the prevalence of chronic heart failure in China is 0.9%, the number of people affected is more than 4 million, and the 5-year survival rate is even lower than that of malignant tumors. AIM: To determine the impact of WeChat platform-based health management on severe chronic heart failure patients' health and self-management efficacy. METHODS: A total of 120 patients suffering from chronic heart failure with cardiac function grade III-IV, under the classification of the New York Heart Association, were admitted to our hospital in May 2017. In January 2020, they were divided into two groups: A control group (with routine nursing intervention) and an observation group (with WeChat platform-based health management intervention). Changes in cardiac function, 6-min walking distance (6MWD), high-sensitivity cardiac troponin (hs-cTnT), and N-terminal pro B-type natriuretic peptide (NT-proBNP) were detected in both groups. The Self-Care Ability Scale (ESCA) score, Minnesota Living with Heart Failure Questionnaire score, and compliance score were used to evaluate self-management ability, quality of life, and compliance of the two groups. During a follow-up period of 12 mo, the occurrence of cardiovascular adverse events in both the groups was counted. RESULTS: The left ventricular ejection fraction, stroke output, and 6MWD increased, and the hs-cTnT and NT-proBNP decreased in both the groups, as compared to those before the intervention. Further, cardiac function during the 6MWD, hs-cTnT, and NT-proBNP improved significantly in the observation group after intervention (P < 0.05). The scores of self-care responsibility, self-concept, self-care skills, and self-care health knowledge in the observation group were higher than those of the control group before intervention, and their ESCA scores were significantly improved after intervention (P < 0.05). The Minnesota heart failure quality of life (LiHFe) scores of physical restriction, disease symptoms, psychological emotion, social relations, and other items were decreased compared to those of the control group before intervention, and the LiHFe scores of the observation group were significantly improved compared to those of the control group (P < 0.05). With intervention, the compliance scores of rational diet, regular medication, healthy behavior, and timely reexamination were increased, thereby leading to the compliance scores of the observation group being significantly improved compared to those of the control group (P < 0.05). During the 12 mo follow-up, the incidence rates of acute myocardial infarction and cardiogenic rehospitalization in the observation group were lower than those of the control group, and the hospitalization time in the observation group was shorter than that of the control group, but there was no significant difference between the two groups (P > 0.05). CONCLUSION: WeChat platform-based health management can improve the self-care ability and compliance of patients with severe chronic heart failure, improve the cardiac function and related indexes, reduce the occurrence of cardiovascular adverse events, and enable the avoidance of rehospitalization.
