INTRODUCTION: Bariatric surgery (BS) produces superior weight loss compared to non-surgical interventions. However, studies suggest bariatric patients who have undergone gastric-bypass surgery have an increased risk of developing new onset substance use disorder (SUD) or suffer negative outcomes after surgery. As such, many bariatric programs consider alcohol/ illicit drug misuse a contraindication to BS. The purpose of this systematic review was to investigate weight loss outcomes, post-surgery substance use patterns and other morbidity/mortality in BS patients with a history of substance use/SUD. METHODS: Studies were identified by searching Ovid Medline(R), Embase, and PsychInfo. We included all study types investigating humans of any age/sex who had undergone any BS procedure with data regarding substance use before and/or after surgery. Outcome measures included metabolic outcomes and psychiatric outcomes after bariatric surgery in patients reporting substance use prior to bariatric surgery and substance use patterns after bariatric surgery. RESULTS: Fifty-eight studies were included in the review. Studies reporting weight loss after BS did not demonstrate an association between substance use and negative weight loss outcomes. Several studies reported a significant portion of participants having new onset or increased substance use after BS. Factors associated with new onset or increased substance use/SUD after BS included the type of surgery, a history of SUD, a family history of SUD, coping skills/life stressors, age, male sex and alcohol sensitization after surgery. CONCLUSION: Substance use history does not appear to influence weight loss after BS, however it may contribute to increased substance use after BS. Clinicians should ensure valid screening tools when assessing BS candidates for substance use history and ensure long term follow-up care post-operatively.

Huntington disease (HD), an autosomal dominant disorder involving HTT, is characterized by chorea, psychiatric illness and cognitive decline. Diagnosis and age of onset depend on the degree of expansion of the trinucleotide CAG repeat within the gene. The prevalence of HD is known for Europeans but has not been studied in the Israeli population. Between 2006 and 2011 we diagnosed in our adult genetics clinic ten HD probands, nine of whom were Caucasus Jews (CJ) (Azerbaijani), and one Ashkenazi Jewish. We performed haplotype analysis to look for evidence of a founder mutation, and found that of the nine CJ, eight shared the same haplotype that was compatible with the A1 haplogroup. We calculated the coalescence age of the mutation to be between 80 and 150 years. Ninety percent of our HD patients are CJ, as are 27% of the HD patients in Israel, although the CJ comprise only 1.4% of the Israeli population. Our findings suggest a higher prevalence of HD among CJ compared to the general Israeli population and are consistent with a recent founder mutation. We recommend a higher degree of suspicion for HD in CJ with subtle clinical findings.

Activity was recorded from 343 units of rostral, mid, and posterolateral thalamus following a conditioned click stimulus (CS). Over 40% of units responded with increased discharge in cats conditioned to blink to the CS. Twenty-nine units with short latency (less than 40 ms) responses were injected intracellularly with phaseolus lectin and identified morphologically; 83% had long, thick primary dendrites with smaller secondary branches. Almost half (46%) had larger somata than the largest previously described thalamic neurons of this morphologic classification. The results suggest that a previously unidentified class of oversized cells is likely to contain many short latency, auditory responsive units. A substantial number of these cells (36%) projected extrathalamically into the internal capsule, and thus may constitute a new auditory pathway between thalamus and cortex.

RESUMO

A grating-pair device is useful for producing negative group velocity dispersion of temporal optical signals. When high dispersion is needed or large bandwidth is involved, higher-order dispersion terms become significant. The grating triplet is a new approach to create dispersion that can produce negative or positive group-velocity dispersion without higher-order dispersion terms.

The auditory function of units in the mid- (lateralis dorsalis and centrolateral nuclei) and posterolateral (lateralis posterior-pulvinar complex) thalamus of cats was assessed during performance of conditioned eye blink responses (CRs) elicited discriminatively by a forward-paired, 70 dB click conditioned stimulus (CS) as opposed to a backward-paired, 70 dB hiss discriminative stimulus (DS). Discharges in response to the CS or DS were found in over 40% of units tested in each area, with onset latencies as short as 28-32 msec in mid-thalamus and 14 msec in posterolateral thalamus. The results provide evidence that both mid- and posterolateral thalamic regions of cats contain sufficient numbers of auditory responsive neurons to be considered part of the auditory system functionally. Patterns of activity changed after conditioning discriminative responses to the click CS. In mid-thalamus, the ratio of CS-evoked activity to baseline activity increased relative to levels found before conditioning. This increase was attributable to an increase in the magnitude of response to the CS. In posterolateral thalamus, an increase in the signal: noise ratio of activity in response to the CS was also found after conditioning, but this increase depended in large part on a decrease in the rate of baseline firing. Posterolateral thalamic units had substantially higher mean rates of baseline firing than mid-thalamic units before any conditioning sessions were begun.(ABSTRACT TRUNCATED AT 250 WORDS)

Short-latency auditory-responsive units were found in the rostral thalamus of cats during performance of conditioned eyeblink responses (CRs) elicited discriminatively by a forward-paired, 70-dB-click conditioned stimulus (CS) as opposed to a backward-paired, 70-dB-hiss discriminative stimulus (DS). Discharges in response to the CS or DS were found in 57% of 138 units tested. Forty-one percent of units responding to the CS did so at latencies of less than 40 msec. After conditioning a discriminative CR to click CS, an increase in the ratio of CS-evoked activity to baseline activity was found relative to that before conditioning. This increase was attributable, in part, to a decrease in baseline activity and, in part, to an increase in the magnitude of response to the CS. These responses preceded early components of the electromyographically measured motor responses with latencies sufficient to contribute to initiation of the movement. After acquisition of the CR, the proportion of CS responsive units also increased. We conclude that this area of the thalamus, a region thought to support thalamocortical reverberatory activity, also functions to transmit short-latency auditory signals. Our evidence further suggests that this region may participate in the elicitation of conditioned responses by specific auditory stimuli and in discrimination between auditory stimuli of different significance.

RESUMO

Optical signal processing can be done with time-lens devices. A temporal processor based on chirp-z transformers is suggested. This configuration is more compact than a conventional 4-ƒ temporal processor. On the basis of implementation aspects of such a temporal processor, we did a performance analysis. This analysis leads to the conclusion that an ultrafast optical temporal processor can be implemented.
