RESUMO

Spontaneous left ventricular pseudoaneurysms are very rare and can have catastrophic consequences if unrecognized. A case of combined spontaneous left ventricular aneurysm and pseudoaneurysm in Behcet's disease (BD) has been reported. The case emphasizes advanced techniques for percutaneous closure of the defects with the use of an ex-vivo three-dimensional cardiac printed model as a tool to facilitate the procedure.

Background: Cardioneuroablation (CNA) targeting ganglionated plexi has shown promise in treating vasovagal syncope. Only radiofrequency ablation has been used to achieve this goal thus far. Objective: The purpose of this study was to investigate the utility of cryoballoon ablation (CBA) of the pulmonary veins (PVs) as a potential simplified approach to CNA. Methods: We report our observations of autonomic modulation in a series of 17 patients undergoing CBA for atrial fibrillation and our early experience using CBA of the PVs in 3 patients with malignant vagal syncope. In 17 patients undergoing CBA of AF, sinus cycle length was recorded intraprocedurally after ablation of individual PVs. Results: The most pronounced shortening of the sinus cycle length was observed after isolation of the right upper PV, which was ablated last. Reduced sinus node recovery time and atrioventricular (AV) nodal effective refractory period were observed after CBA. Resting heart rate was elevated by 6-7 bpm after CBA and persisted during 12-month follow-up. CBA of the PVs was performed in 3 patients with recurrent vagal syncope mediated by sinus arrest (n = 2) and AV block (n = 1). In all patients, isolation of the right upper PV resulted in marked shortening of sinus cycle length. During follow-up of 178 ± 43 days (134-219 days), CNA resulted in abolition of pauses, bradycardia-related symptoms, and syncope in all patients. Conclusion: CBA of the PVs (particularly the right upper PV) may be a predictable anatomic CNA approach in patients with refractory vagal syncope due to sinus arrest and/or AV block and may warrant systematic investigation as a tool to perform CNA.

Objective: The social determinants of health (SDoH) account for 80%-90% of modifiable contributors to health outcomes for chronic diseases such as heart failure. Knowledge gaps exist on how SDoH influences hospitalization rates in women with heart failure. Our aim was to evaluate the relationship between the baseline SDoH status of women with heart failure with subsequent all-cause and cardiovascular hospitalization. Methods: This is a prospective observational longitudinal cohort study of women diagnosed with heart failure with 6-month follow-up. The subjects completed SDoH assessment by the Institute of Medicine. Monthly follow-ups were performed to assess for hospitalization events. Results: A total of 92 patients with at least 1 follow-up clinic visit were included. The mean age was 66 ± 15 years and 80% had nonischemic cardiomyopathy as the etiology of heart failure. New York Heart Association (NYHA) Classifications I-II were the most common (n = 66, 71.8%). In total, 51 patients (55.4%) had overall high-risk SDoH (4 or more SDoH domains at risk). By the 6-month follow-up, 22 (23.9%) patients were hospitalized for any cause; 8 patients (8.7%) were hospitalized for cardiovascular causes. There were no deaths. In multivariate logistic regression analysis, the high-risk SDoH group had a higher odds ratio for all-cause hospitalization (OR 5.31, 95% CI 1.59-17.73). In addition, Kansas City Cardiomyopathy Questionnaire 12-item (KCCQ-12) scores, surrogate for quality of life, were worse in the high-risk SDoH group. Conclusion: SDoH adversely impacts hospitalizations and quality of life in women with heart failure. Future efforts for screening and interventions should evaluate the SDoH at all levels, including the individual health care provider, institutional, and national levels.

RESUMO

Takotsubo syndrome (TTS) is an increasingly recognized heart disease that was initially regarded as a benign condition, but since has proven to cause irreversible myocardial damage, resembling that of acute coronary syndrome (ACS). The etiology of TTS is still uncertain but may be associated with catecholamine elevations during times of emotional or physical stress. Catecholamines are also understood to have prothrombotic properties, which could lead to ACS. With these similarities, differentiating these two pathologies can be difficult, especially when TTS and ACS occur simultaneously.
