RESUMO

BACKGROUND: Martinique shares with the other Caribbean countries specific public health issues, particularly in the diagnostic and therapeutic management of cancer patients. Mutualization of human and material resources by promoting cooperation is the most appropriate response to the challenges of the health systems of the Caribbean territories. Through the French PRPH-3 program, we propose to set up a collaborative digital platform adapted to the specificities of the Caribbean to strengthen professional links and skills in oncofertility and oncosexology and reduce inequalities in access to reproductive and sexual health care for cancer patients. METHODS: Within the context of this program, we have developed of an open-source platform based on a Learning Content Management System (LCMS), with an operating system developed by UNFM for low speed internet. LO libraries have been created and interaction between trainers and learners were done in asynchronous mode. This training management platform is based on: a TCC learning system (Training, Coaching, Communities); a web-hosting with pedagogical engineering appropriate to low bandwidth; a reporting system and a responsibility for processing. RESULTS: We have carried out a flexible, multilingual and accessible digital learning strategy functionality called e-MCPPO according to low-speed internet ecosystem. In close connection with the e-learning strategy we conceived (i) a multidisciplinary team; (ii) an appropriate training program for expert health professionals and (iii) a responsive design. DISCUSSION AND CONCLUSION: This low-speed web-based infrastructure allows communities of experts to cooperate in creating, validating, publishing and managing academic learning content. The self-learning modules provide the digital layer for each learner to extend their skills. Learners, as well as trainers, would gradually take ownership of this platform and encourage its promotion. Innovation in this context is both technological (low-speed Internet broadcasting, free interactive software) and organizational (moderating educational resources). This collaborative digital platform is unique in its form and content. This challenge could contribute to the digital transformation of the Caribbean ecosystem for capacity building in this specifics topics.

OBJECTIVES: To identify the drivers and challenges of successful nutrition programme implementation in a multisectoral, community-level approach to improve infant and young child feeding (IYCF) practices in northern Burkina Faso. DESIGN: A qualitative study was conducted in 2019 through (i) individual interviews with key informants from five different sectors (health, agriculture, environment, livestock and education) and association staff, agents and community leaders and (ii) focus groups with mothers of children under the age of 2 years. SETTING: Three health districts in the northern region of Burkina Faso implemented a multisectoral community nutrition programme to improve IYCF practices. PARTICIPANTS: Forty-seven implementing actors and twenty-four beneficiary mothers. RESULTS: Factors influencing successful implementation include community participation; sector commitment and involvement; the existence of nutrition champions; capacity building; the integration of interventions; micronutrient powder distribution; the introduction of nutrition-sensitive interventions, such as the promotion of the consumption of orange-fleshed sweet potatoes; improved food production and small livestock rearing and the effective coordination of actors and complementary funding. The main challenges of the implementation of multisectorality are low participation among nutrition-sensitive sectors, a tendency for siloed work among sectors, scheduling conflicts, high actor mobility, differences in the target population by sector, a lack of technical skills among community workers, insufficient financial resources, low geographic convergence and coverage of beneficiaries, a lack of a multisectoral monitoring mechanism and accountability and insecurity. CONCLUSIONS: Strengthening sector participation, identifying a common targeting strategy and mobilising financial resources have the potential to significantly reduce barriers and improve the quality of implementation.

BACKGROUND: In the French West-Indies, few studies have been performed on fertility and sexual problems in cancer survivors, which are frequent and recurring issues reported by surveys on unmet needs. Additionally, mutualizing human and material resources and promoting cooperation through a collaborative platform are the most appropriate response to complex health pathways in the Caribbean territories. Implementation of such a collaborative platform will help to launch a strategic Caribbean partnership to transfer theoretical and technical skills and care standards in oncofertility and oncosexuality. METHODS: We propose to set up a collaborative digital platform to strengthen, from the French expertise, Cuban health professionals' knowledge, know-how, and skills in oncofertility and oncosexuality. The project will be coordinated by a coordinating, scientific, and supervisory committee, and the main activities will include: 1. Theoretical training in e-learning adapted to low-speed Internet. 2. Practical training in fertility preservation and sexual rehabilitation. 3. Digital multidisciplinary consultation meetings for medical decisions to be taken for complex clinical cases. The platform will benefit from a recurrent evaluation, by the two cancer registries of Martinique and Cuba, with the following performance indicators: number of Cuban professionals trained, number of professionals sensitized, hourly volumes (or number of training courses provided), satisfaction of trained professionals, number of e-RCPs carried out online and number of missionaries supported. These indicators will be set up and analyzed by the registers. This project meets the Cuban and French health policies (cancer plans and national sexual health strategies) and will be implemented in liaison with the Health Agencies of both countries and the Embassy of France in Cuba. DISCUSSION: This project aims to provide support through bilateral exchanges to improve reproductive and sexual health in Cuba's cancer patients. This collaboration will be based on a long-lasting French expertise and a solid Cuban health system. Consequently, this collaborative digital platform will contribute to data collection for cancer surveillance, and the two participating countries will ultimately be identified in the Caribbean as having centers of competence and excellence in oncofertility and oncosexuality with care standards.

OBJECTIVES: This study aims to analyze how graduates, coming from low income Francophone countries, of the Master in International Health and Nutrition Policies program at the Senghor University are a new human capital bringing an original contribution to the global development of African communities. METHODS: A secondary analysis of qualitative findings responded to the research question about the evidences of the impact of this new human capital over the community development of countries where the graduates worked. Findings also revealed graduates' potential to contribute to future endeavors in global health. The analysis was conceptually framed by the population health promotion model and the dyad of human and social capital. Findings were analyzed applying the thematic analysis method focusing on four themes: profile of human capital, impacts and results, review of contextual conditions and expected results. RESULTS: Accounts of 70 graduates indicated an emergence of a new profile of human capital due the reinforcement of their qualities, valorization of their assets and consolidation of their skills. As multi-level knowledge brokers, the graduates brought an original contribution to the African development by enhancing individuals' competencies. CONCLUSIONS: Graduates contributed to all social actors by socializing knowledge they acquired, as well as by integrating themselves in social relations at all levels resulting in the mobilization and promotion of individuals' competencies. Thus, graduates reinforced the positive actions of the existing human capital.

INTRODUCTION: The factors leading to exclusion of a part of the population from mutual health insurance were studied by examining the profile of members, the social dynamics and the mode of functioning of this type of health insurance. METHODS: A cross-sectional survey was conducted in Parakou; Benin by selecting 50 patients from three healthcare centres selected at random from the ten "Réseau des Soins Coordonnés" centres. Individual interviews and focus groups were conducted with mutual health insurance beneficiaries and non-beneficiaries. RESULTS: Thirty-seven of the 50 patients did not have any mutual health insurance and 35 of them were under the age of 35 years with a female predominance (23 out of 37) with a low monthly income (less than CFA30,000). Factors such as group dynamics, the feeling of belonging to a group and confidence in mutual insurance management committees were related to the presence or absence of mutual health insurance. CONCLUSION: In the absence of universal health insurance coverage in Benin, mutual health insurance constitutes almost the only solution for populations, but few people take out this insurance. Consequently, the contribution of mutual health insurance in this region of Benin to access to healthcare remains marginal, as the most deprived populations cannot afford this insurance.

OBJECTIVE: To examine educational practices in type 2 diabetes among health professionals. METHODS: A cross-sectional study took place from June to September 2012 in Bamako and Ouagadougou. The study population consisted of all health professionals responsible for diabetes care in secondary and tertiary public health facilities. RESULTS: A total of 78 participants were interviewed. Doctors and nurses accounted for 77% and 23% respectively. The median number of patients seen in consultation was 10 per week (range 1-100). The median age of experience in diabetes care for health professionals was 5 years (range: 1-25 years). Sixteen participants reported giving advice for cooking. Of the 62 that did not, the reasons given were mainly: non-competence to give such advice (60/62); lack of time (35/62) and useless counseling (5/62). For out-of-home meals, recommendations included: taking a meal to work (60/77); avoiding meals outside the home (30/77); avoid alcohol and oils at parties (10/77). CONCLUSION: The recommendations made to patients come down to information and advice. As a result, the data collected lacks an educational strategy that aims to render the patient more independent by facilitating his adherence to counseling and improving his quality of life.

The central role played by calcium ion in biological systems has generated an interest for its potential implication in human malignancies. Thus, lines of research, on possible association of calcium metabolism regulation with tumorigenesis, implying disruptions and/or alterations of known molecular pathways, have been extensively researched in the recent decades. This paper is a critical synthesis of these findings, based on a functional approach of the calcium signaling toolkit. It provides strong support that this ubiquitous divalent cation is involved in cancer initiation, promotion, and progression. Different pathways have been outlined, involving equally different molecular and cellular structures. However, if the association between calcium and cancer can be described as constant, it is not always linear. We have identified several influencing factors among which the most relevant are (i) the changes in local or tissular concentrations of free calcium and (ii) the histological and physiological types of tissue involved. Such versatility at the molecular level may probably account for the conflicting findings reported by the epidemiological literature on calcium dietary intake and the risk to develop certain cancers such as the prostatic or mammary neoplasms. However, it also fuels the hypothesis that behind each cancer, a specific calcium pathway can be evidenced. Identifying such molecular interactions is probably a promising approach for further understanding and treatment options for the disease.

Objectives: To study the determinants of psychoactive substance use among street teenagers living in Ouagadougou.Methods: Qualitative and quantitative cross-sectional study with street teenagers living on the Zogona campus in Ouagadougou, in September 2012. A focus group was then formed with eight members of the group.Results: The median age was 16 years (range: 12 to 20 years). The various psychotropic agents were tobacco, alcohol, gasoline, glue sniffing and cannabis. Seventeen of the 31 teenagers had already been admitted to a rehabilitation centre and none of them attended school at the time of data collection. However, 12 street teenagers had previously attended school. The main reasons for the presence of these teenagers in the street were "poverty in the family", "death of the parents", "Koranic school". The main reasons reported by the teenagers for substance use were "to give oneself courage", "to relieve hunger", "to be like the others", "to be accepted by the group", "to protect oneself from the cold". The main determinants of substance abuse were social exclusion, group membership, and group identification.Conclusion: Psychoactive substance use is an integral part of the life of street teenagers and is a major obstacle to social integration.

OBJECTIVE: To evaluate the vulnerability of male prisoners to HIV, risk behaviour and access to prevention. METHODS: This cross-sectional descriptive study was conducted in july and August 2012 in Ouagadougou Prison in Burkina Faso. Two trained investigators collected data by means of individual interviews in the prison visiting room using a questionnaire administered to male inmates 18 years and older, imprisoned for more than three months. Two focus groups were conducted with prison guards and healthcare personnel. RESULTS: A total of165 male prisoners were interviewed. The mean prison sentence was 19 months, the median age of the inmates was 28years and 45% of them were illiterate. About4% of male prisoners reported having had homosexual relations during their imprisonment. However, data indicate underreporting and denial of homosexual behaviour by prisoners. 49% of prisoners shared razors or razorblades in prison. None of the interviewees reported injected drug use or tattoos in prison. The majority (84%) of prisoners had a good knowledge of HIVjAIDS and 6% were aware of the risk of sexually transmitted infections. Only 5% of prisoners had had a screening test during their stay in prison. CONCLUSION: Prison conditions, homosexual behaviour and absence of condoms in prison accentuate the vulnerability of prisoners to HIV j AIDS. Implementation of a prevention programme and management HIV-positive prisoners would help to reduce significantly the risk of HIV transmission in prison.
