RESUMO

DNA becomes single stranded (ssDNA) during replication, transcription, and repair. Transiently formed ssDNA segments can adopt alternative conformations, including cruciforms, triplexes, and quadruplexes. To determine whether there are stable regions of ssDNA in the human genome, we utilized S1-END-seq to convert ssDNA regions to DNA double-strand breaks, which were then processed for high-throughput sequencing. This approach revealed two predominant non-B DNA structures: cruciform DNA formed by expanded (TA)n repeats that accumulate in microsatellite unstable human cancer cell lines and DNA triplexes (H-DNA) formed by homopurine/homopyrimidine mirror repeats common across a variety of cell lines. We show that H-DNA is enriched during replication, that its genomic location is highly conserved, and that H-DNA formed by (GAA)n repeats can be disrupted by treatment with a (GAA)n-binding polyamide. Finally, we show that triplex-forming repeats are hotspots for mutagenesis. Our results identify dynamic DNA secondary structures in vivo that contribute to elevated genome instability.

SynTEF1, a prototype synthetic genome reader/regulator (SynGR), was designed to target GAA triplet repeats and restore the expression of frataxin (FXN) in Friedreich's ataxia patients. It achieves this complex task by recruiting BRD4, via a pan-BET ligand (JQ1), to the GAA repeats by using a sequence-selective DNA-binding polyamide. When bound to specific genomic loci in this way, JQ1 functions as a chemical prosthetic for acetyl-lysine residues that are natural targets of the two tandem bromodomains (BD1 and BD2) in bromo- and extra-terminal domain (BET) proteins. As next-generation BET ligands were disclosed, we tested a select set with improved physicochemical, pharmacological, and bromodomain-selective properties as substitutes for JQ1 in the SynGR design. Here, we report two unexpected findings: (1) SynGRs bearing pan-BET or BD2-selective ligands license transcription at the FXN locus, whereas those bearing BD1-selective ligands do not, and (2) rather than being neutral or inhibitory, an untethered BD1-selective ligand (GSK778) substantively enhances the activity of all active SynGRs. The failure of BD1-selective SynGRs to recruit BRD4/BET proteins suggests that rather than functioning as "epigenetic/chromatin mimics," active SynGRs mimic the functions of natural transcription factors in engaging BET proteins through BD2 binding. Moreover, the enhanced activity of SynGRs upon cotreatment with the BD1-selective ligand suggests that natural transcription factors compete for a limited pool of nonchromatin-bound BET proteins, and blocking BD1 directs pan-BET ligands to more effectively engage BD2. Taken together, SynGRs as chemical probes provide unique insights into the molecular recognition principles utilized by natural factors to precisely regulate gene expression, and they guide the design of more sophisticated synthetic gene regulators with greater therapeutic potential.

Background: Surgical site infections are common and expensive infections that can cause fatalities or poor patient outcomes. To prevent these infections, antibiotic prophylaxis is used. However, excessive antibiotic use is related to higher costs and the emergence of antimicrobial resistance. Objectives: The present meta-analysis aimed to compare the effectiveness of a single dosage versus several doses of antibiotics in preventing the development of surgical site infections. Methods: PubMed was used to find clinical trials evaluating the effectiveness of a single dosage versus several doses of antibiotics in avoiding the development of surgical site infections. The study included trials that were published between 1984 and 2022. Seventy-four clinical trials were included in the analysis. Odds ratios were used to compare groups with 95% confidence intervals. The data were displayed using OR to generate a forest plot. Review Manager (RevMan version 5.4) was used to do the meta-analysis. Results: Regarding clean operations, there were 389 surgical site infections out of 5,634 patients in a single dose group (6.90%) and 349 surgical site infections out of 5,621 patients in multiple doses group (6.21%) (OR = 1.11, lower CI = 0.95, upper CI = 1.30). Regarding clean-contaminated operations, there were 137 surgical site infections out of 2,715 patients in a single dose group (5.05%) and 137 surgical site infections out of 2,355 patients in multiple doses group (5.82%) (OR = 0.87, lower CI = 0.68, upper CI = 1.11). Regarding contaminated operations, there were 302 surgical site infections out of 3,262 patients in a single dose group (9.26%) and 276 surgical site infections out of 3,212 patients in multiple doses group (8.59%) (OR = 1.11, lower CI = 0.84, upper CI = 1.47). In general, there were 828 surgical site infections out of 11,611 patients in a single dose group (7.13%) and 762 surgical site infections out of 11,188 patients in multiple doses group (6.81%) (OR = 1.05, lower CI = 0.93, upper CI = 1.20). The difference between groups was not significant. Conclusion: The present study showed that using a single-dose antimicrobial prophylaxis was equally effective as using multiple doses of antibiotics in decreasing surgical site infections.

Pelvic fractures are an increasingly common injury seen in the older population and represent a significant burden of morbidity and mortality in this age group, as well as a large financial burden on the health service. It is well established that early fixation of femoral neck and acetabular fractures improves outcomes and increases the chances of patients returning close to their premorbid functional baseline. However, fixation of fragility fractures of the pelvis is less well established in current practice. There has been recent development of novel stabilisation techniques for unstable pelvic fractures, designed to tackle the difficulties associated with fixation in poor bone quality, along with medical trials of parathyroid hormone analogue treatment. However, it is still current practice to manage nearly all fragility fractures of the pelvis conservatively. In this article, we consider whether the development of surgical stabilisation techniques for pelvic fragility fractures may have the potential to improve the well-described morbidity and mortality associated with them.

BACKGROUND: Both hypertension and dyslipidemia are considered as major modifiable risk factors of cardiovascular diseases (CVDs), and their prevalence in Egypt has increased in recent years. Evidence-based systematic evaluation of data on hypertension and dyslipidemia is critical for effective patient-centric management to reduce the overall risk of CVDs in Egypt. This semi-systematic review aimed to quantify and identify data gaps in the prevalence and distribution of patient journey touchpoints including awareness, screening, diagnosis, treatment, adherence, and control of hypertension and dyslipidemia to provide the basis for research prioritization, practice guidance, and health care reforms in Egypt. MAIN BODY: Structured search was conducted on MEDLINE and Embase to identify articles published in English between January 2010 and December 2019 that reported key patient journey touchpoints in hypertension and dyslipidemia management. Unstructured search was conducted on public or government websites with no date restriction. Data from all sources were extracted and presented descriptively. In total, 22 studies published between 1995 and 2020 on hypertension and dyslipidemia were included in the final analyses. The prevalence of hypertension in Egypt ranged from 12.1 to 59%. Studies reported awareness (37.5% and 43.9%), diagnosis (42% and 64.7%), treatment (24% and 54.1%), and adherence to antihypertensive medication (51.9%) to be low. Furthermore, the percentage of patients who had their blood pressure controlled ranged from 8 to 53.2%. The prevalence of dyslipidemia varied in the general population (range 19.2-36.8%) but was higher in patients with acute coronary syndrome (ACS) (50.9% and 52.5%) and coronary artery disease (58.7%). A national report indicated that 8.6% of the general population was screened for dyslipidemia; however, no data was available on the diagnosis and treatment rates. Among ACS patients, 73.9% were treated for dyslipidemia. Data indicated low levels of medication adherence (59%) among dyslipidemia patients, with overall low control rates ranging from 5.1 to 34.4% depending on CVD risk in populations including ACS patients. CONCLUSION: Data on patient journey touchpoints of hypertension and dyslipidemia are limited in Egypt, indicating the need for more systematic and high-quality evidence-based studies covering different aspects of patient-centric management for better management of CVD and its risk factors.

BACKGROUND: Negative pressure therapy plays an important role in the healing process of different types of wounds. AIM OF THE STUDY: this study aimed to compare between modified and traditional method of vacuum wound closure for chronic wounds. SUBJECTS: Thirty participants with chronic ulcers completed the study, their ages ranged from 47 to 65 years, from Benha city, divided randomly into two groups, it was carried out from December 2017 to March 2019. METHODS: The first group treated by the traditional method of vacuum assisted wound closure for 8 weeks; dressing was changed 3 times weekly, at 125 mmHg. The mode of 2 min off and 5 min on was used. The second group treated by the modified method by switching off the device at the fifth week and applying traditional dressing for one week then ended by three weeks of vacuum wound closure. Assessment was performed before and after the study. Student t-test was used for analysis. RESULT: Paired t-test showed a significant improvement in wound surface area as well as volume for pre and post treatment in each group (p < 0.05). On the other hand independent t-test revealed a non-significant difference between the two groups pre as well as post treatment (p > 0.05). CONCLUSION: There is no significant difference between the two programs on healing of chronic ulcers. CLINICAL TRIAL REGISTRATION NO: NCT03701152 - at Clinicaltrials.gov.

Viruses are known to be etiologically related to several types of human cancer. In several published studies, viruses such as human mammary tumor virus, human papillomaviruses, Epstein-Barr virus, human cytomegalovirus, herpes simplex virus, and measles virus have been implicated in the pathogenesis of a subset of breast cancers (BC). However, these studies have produced conflicting results, causing considerable controversy. In this context, recent demonstration of elevated levels of mutagenic antiviral enzyme APOBEC3B in a majority of BCs is a highly significant development, as it provides a possible mechanism for development of large numbers of mutations (kataegis) that characterize many of the BCs. It has also provided further impetus for revaluating the role of viruses in the pathogenesis of BC.

-Online learning has become vital and fundamental for learning success in higher education institutions to cope with various learning methodologies or crises such as COVID-19. Therefore, this study investigates factors influencing the academic learning performance among diploma and bachelor's degree students who utilize online e-learning systems in Jordan. Using a conceptual model developed from an extensive previous literature review, the study used a quantitative research methodology to collect data via survey questionnaires. Using PLS-SEM, the empirical analysis of 846 questionnaires collected from Jordanian students demonstrates that task-technology fit and students' satisfaction positively influence learning performance and contribute significantly to student academic success using online learning systems. While the quality of system, service, and information are critical determinants of task-technology fit, student interaction, and instructor interaction are critical determinants of student satisfaction. The study highlights the importance of user-friendly interfaces, effective communication channels, collaboration, and positive learning environments. The findings provide practical insights for universities, online learning system designers, administrators, and instructors to enhance student-learning outcomes. Additionally, system providers can use these findings to address essential successful tools and features needed to create effective and efficient online learning environments. This study highlights practical implications that support higher education institutions' efforts to enhance their online e-learning systems quality and improve student-learning outcomes while helping system providers address essential successful tools and features needed.

We present a new category of quinoxaline di-N-oxides (QdNOs) containing amino acid side chains with dual antituberculosis and antileishmanial activity. These compounds were synthesized by combining a regioselective 2,5-piperazinedione opening and a Beirut reaction and were screened for their activity against Mycobacterium tuberculosis and the promastigote and amastigote forms of representative species of the Leishmania genus. Most QdNOs exhibited promising antitubercular activity with IC50 values ranging from 4.28 to 49.95 µM, comparable to clinically established drugs. Structure-activity relationship analysis emphasized the importance of substituents on the aromatic ring and the side chain. Antileishmanial tests showed that some selected compounds exhibited activity comparable to the positive control miltefosine against promastigotes of Leishmania amazonensis and Leishmania donovani. Notably, some compounds were found to be also more potent and less toxic than miltefosine in intracellular amastigote assays against Leishmania amazonensis. The compound showing the best dual antitubercular and leishmanicidal profile and a good selectivity index, 4h, can be regarded as a hit compound that opens up new opportunities for the development of integrated therapies against co-infections.

Laser-induced breakdown spectroscopy (LIBS) is a remarkable elemental identification and quantification technique used in multiple sectors, including science, engineering, and medicine. Machine learning techniques have recently sparked widespread interest in the development of calibration-free LIBS due to their ability to generate a defined pattern for complex systems. In geotechnical engineering, understanding soil mechanics in relation to the applications is of paramount importance. The knowledge of soil unconfined compressive strength (UCS) enables engineers to identify the behaviors of a particular soil and propose effective solutions to given geotechnical problems. However, the experimental techniques involved in the measurements of soil UCS are incredibly expensive and time-consuming. In this work, we develop a pioneering technique to estimate the soil unconfined compressive strength using artificial intelligent methods based on the spectra obtained from the LIBS system. Decision tree regression (DTR) and support vector regression learners were initially employed, and consequently, the adaptive boosting method was applied to improve the performance of the two single learners. The prediction power of the established models was determined using the standard performance evaluation metrics such as the root-mean-square error, CC between the predicted and actual soil UCS values, mean absolute error, and R2 score. Our results revealed that the boosted DTR exhibited the highest coefficient of correlation of 99.52% and an R2 value of 99.03% during the testing phase. To validate the models, the UCS values of soils stabilized with lime and cement were predicted with an optimum degree of accuracy, confirming the models' suitability and generalization strength for soil UCS investigations.

Biomass-derived activated carbons have gained significant attention as electrode materials for supercapacitors (SCs) due to their renewability, low-cost, and ready availability. In this work, we have derived physically activated carbon from date seed biomass as symmetric electrodes and PVA/KOH has been used as a gel polymer electrolyte for all-solid-state SCs. Initially, the date seed biomass was carbonized at 600 °C (C-600) and then it was used to obtain physically activated carbon through CO2 activation at 850 °C (C-850). The SEM and TEM images of C-850 displayed its porous, flaky, and multilayer type morphologies. The fabricated electrodes from C-850 with PVA/KOH electrolytes showed the best electrochemical performances in SCs (Lu et al. Energy Environ. Sci., 2014, 7, 2160) application. Cyclic voltammetry was performed from 5 to 100 mV s-1, illustrating an electric double layer behavior. The C-850 electrode delivered a specific capacitance of 138.12 F g-1 at 5 mV s-1, whereas it retained 16 F g-1 capacitance at 100 mV s-1. Our assembled all-solid-state SCs exhibit an energy density of 9.6 Wh kg-1 with a power density of 87.86 W kg-1. The internal and charge transfer resistances of the assembled SCs were 0.54 and 17.86 Ω, respectively. These innovative findings provide a universal and KOH-free activation process for the synthesis of physically activated carbon for all solid-state SCs applications.

Offshore hydrogen production through water electrolysis presents significant technical and economic challenges. Achieving an efficient hydrogen evolution reaction (HER) in alkaline and natural seawater environments remains daunting due to the sluggish kinetics of water dissociation. To address this issue, we synthesized electrocatalytic WO3-x@CdS1-x nanocomposites (WCSNCs) using ultrasonic-assisted laser irradiation. The synthesized WCSNCs with varying CdS contents were thoroughly characterized to investigate their structural, morphological, and electrochemical properties. Among the samples tested, the WCSNCs with 20 wt % CdS1-x in WO3-x (Wx@Sx-20%) exhibited superior electrocatalytic performance for hydrogen evolution in a 1 M KOH solution. Specifically, the Wx@Sx-20% catalyst demonstrated an overpotential of 0.191 V at a current density of -10 mA/cm2 and a Tafel slope of 61.9 mV/dec. The Wx@Sx-20% catalysts demonstrated outstanding stability and durability, maintaining their performance after 24 h and up to 1000 CV cycles. Notably, when subjected to natural seawater electrolysis, the Wx@Sx-20% catalysts outperformed in terms of electrocatalytic HER activity and stability. The remarkable performance enhancement of the prepared electrocatalyst can be attributed to the combined effect of sulfur vacancies in CdS1-x and oxygen vacancies in WO3-x. These vacancies promote the electrochemically active surface area, enhance the rate of charge separation and transfer, increase the number of electrocatalytic active sites, and accelerate the HER process in alkaline and natural seawater environments.

The presence of chloride in reinforced concrete can cause severe damage to the strength and durability of buildings and bridges. The detection of chloride in concrete structures at early stages of the corrosion buildup process is, therefore, very important. However, detection of chlorine in trace amounts in concrete is not a simple matter. A dual-pulsed laser-induced breakdown spectrometer (LIBS) has been developed at our laboratory for the detection of chloride contents in reinforced concrete by using two atomic transition lines of neutral chlorine (Cl I) at 594.8 and 837.5 nm. A calibration curve was also established by using standard samples containing chloride in known concentration in the concrete. Our dual-pulsed LIBS system demonstrated a substantial improvement in the signal level at both wavelengths (594.8 and 837.5 nm). However, the new atomic transition line at 594.8 nm shows a significant improvement compared to the line at 837.5 nm in spite of the fact that the relative intensity of the former is 0.1% of the latter. This weak signal level of the 837.5 nm transition line of chlorine can be attributed to some kind of self-absorption process taking place in the case of the concrete sample.

OBJECTIVE: The study assessed the developmental potential of germinal vesicle (GV) oocytes subjected to in vitro maturation (IVM) after prematuration culture with cilostamide (a phosphodiesterase-3 inhibitor) and the impact of cilostamide exposure on the morphology of meiosis II (MII) oocytes and subsequent embryo quality. METHODS: In total, 994 oocytes were collected from 63 patients. Among 307 GV oocytes, 140 oocytes were selected for the experimental group and 130 oocytes for the control group. The denuded GV-stage oocytes were cultured for 6 hours with cilostamide in the experimental group and without cilostamide in the control group. After 6 hours, the oocytes in the experimental group were washed and transferred to fresh IVM medium. The maturational status of the oocytes in both groups was examined at 26, 36, and 48 hours. Fertilization was assessed at 18 hours post-intracytoplasmic sperm injection. Embryo quality was assessed on days 3 and 5. RESULTS: In total, 92.1% of the oocytes remained in the GV stage, while 6.4% converted to the MI stage (p<0.01) after cilostamide exposure. In both groups, more MII oocytes were observed at 36 hours (25.8% vs. 21.5%) than at 26 hours (10.8% vs. 14.6%) and 48 hours (13% vs. 7.9%) (p>0.05). With the advent of cilostamide, blastocyst quality was better in the experimental group than in the control group (p<0.05). CONCLUSION: Cilostamide effectively blocked nuclear maturation and promoted cytoplasmic growth. Prematuration culture with cilostamide enabled synchronization between cytoplasmic and nuclear maturity, resulting in better blastocyst outcomes.

In this study, the samples of the ZnFe2O4 (ZFO) spinel ferrites nanoparticles (SFNPs), Co0.5Ni0.5Ga0.01Gd0.01Fe1.98O4 (CNGaGdFO) SFNPs and (Co0.5Ni0.5Ga0.01Gd0.01Fe1.98O4)x/(ZnFe2O4)y (x:y = 1:1, 1:2, 1:3, 2:1, 3:1 and 4:1) (CNGaGdFO)x/(ZFO)y spinel ferrite nanocomposites (NC) have been synthesized by both sol-gel and Green pulsed laser ablation in liquid (PLAL) approaches. All products were characterized by X-ray powder diffraction (XRD), scanning and transmission electron microscopies (SEM and TEM), elemental mappings and energy dispersive X-ray spectroscopy (EDX). It was objected to tune the magnetic properties of a soft spinel ferrite material with a softer one by mixing them with different fractions. Some key findings are as follows. M-H investigations revealed the exhibition of ferrimagnetic phases for all synthesized samples (except ZnFe2O4) that were synthesized by sol-gel or PLAL methods at both 300 K and 10 K. ZnFe2O4 ferrite NPs exhibits almost paramagnetic feature at 300 K and glass-like phase at very low temperatures below 19.23 K. At RT analyses, maximum saturation magnetization (MS) of 66.53 emu/g belongs to nanocomposite samples that was synthesized by sol-gel method and x:y ratio of 1:3. At 10 K analyses, MS,max = 118.71 emu/g belongs to same nanocomposite samples with ratio of 1:3. Maximum coercivities are 625 Oe belonging to CNGaGdFO and 3564 Oe belonging to NC sample that was obtained by sol-gel route having the 3:1 ratio. Squareness ratio (SQRs = Mr/MS) of NC sample (sol-gel, 4:1 ratio) is 0.371 as maximum and other samples have much lower values until a minimum of 0.121 (laser, 3:1) assign the multi-domain wall structure for all samples at 300 K. At 10 K data, just CNGaGdFO has 0.495 SQR value assigning single domain nature. The maximum values of effective crystal anisotropy constant (Keff) are 5.92 × 104 Erg/g and 2.4 × 105 Erg/g belonging to CNGaGdFO at 300 K and 10 K, respectively. Further, this sample has an internal anisotropy field Ha of 1953 Oe as largest at 300 K. At 10 K another sample (sol-gel, 3:1 ratio) has Ha,max of 11138 Oe which can also be classified as a soft magnetic material similar to other samples. Briefly, most magnetic parameters of NCs that were synthesized by sol-gel route are stronger than magnetic parameters of the NCs that were synthesized by PLAL at both temperatures. Some NC samples were observed to have stronger magnetic data as compared to magnetic parameters of Co0.5Ni0.5Ga0.01Gd0.01Fe1.98O4 NPs at 10 K.

BACKGROUND: De novo malignancy is a worrying complication after kidney transplantation; the type of which may vary due to factors such as the prevalence of viral infection and race. Kaposi sarcoma used to be the most common malignancy among our patients constituting more than one-third of cancers. Nevertheless, we noticed that Kaposi sarcoma has not been observed for a long period. Therefore, we conducted this study to explore such observation. METHODS: Data of all kidney transplant recipients were retrieved and retrospectively analyzed. Their total number was 3126 patients. Their mean age was 28.71 ± 10.97 years and of them, 823 (26.3%) were females. The pattern of Kaposi sarcoma throughout the last decade as well as the preceding three decades was studied. The possible relation between the disappearance of Kaposi sarcoma and three paradigm shifts in our practice, namely the use of mTOR inhibitors, steroid-free regimen and CMV prophylaxis was explored. RESULTS: Since 2010, no new cases of Kaposi sarcoma have been observed. In addition, patients who have been transplanted after 2006 did not develop such malignancy. Patients who received CMV prophylaxis and/or were maintained on mTOR inhibitor or steroid-free regimens have not developed Kaposi sarcoma. Moreover, CMV prophylaxis had a statistically significant difference when compared to a homogenous group without CMV prophylaxis. However, Kaplan-Meier analysis of patients of the three policies and their counterpart groups showed comparable results. CONCLUSION: Kaposi sarcoma, which was previously the most common malignancy, is no longer observed for almost a decade among our kidney transplant recipients. m-TOR inhibitors, steroid-free regimen and CMV prophylaxis policy are possible contributing factors. Nevertheless, only CMV prophylaxis policy had a statistically significant relation to the disappearance of Kaposi sarcoma.

INTRODUCTION: Diarrhea is one of the major complications occurring in over 90% of human immunodeficiency virus (HIV)-infected individuals in developing countries. Coccidian group of parasitic infections remain the standout opportunistic pathogens in many parts of the world. AIM: The objective was to understand the profile of diarrheagenic parasites in HIV/AIDS patients along with analysis of the changing trends in the profile of parasitic diarrhea with special context to coccidian parasitic infections. METHODOLOGY: A cross-sectional study was performed at "ID CENTRE FOR NORTHEAST," Shillong, from January 2014 to October 2017. Stool samples collected were observed microscopically for parasites both on direct and concentrated stool samples under ×10 and ×40 magnification. Modified acid-fast staining was used for the detection of coccidian parasites. All statistical analyses were performed using IBM SPSS software, Version 24.0. RESULTS: The prevalence of intestinal parasitic infections was 40.99%, coccidian parasitic infection accounted for 85.13% of total intestinal parasitic infections. Cryptosporidium parvum was the most common cause of diarrhea (70.64%), followed by Cystoisospora belli (23.81%) and Cyclospora spp. (5.55%). Trend analysis of coccidian etiology during the study revealed a significant rise in the positivity of Cryptosporidium spp. and a decrease in the Cystoisosporiasis belli infection. The common noncoccidian parasites identified include hookworm (8.1%) followed by Ascaris lumbricoides (4.7%). CONCLUSION: The magnitude of parasitic infections is considerably high among the HIV/AIDS patients in Northeast India, and it is essential for screening and periodic monitoring of all the HIV patients for coccidian parasites by stool microscopy.

Transparent and amorphous yttrium (Y)/Sialon thin films were successfully fabricated using pulsed laser deposition (PLD). The thin films were fabricated in three steps. First, Y/Sialon target was synthesized using spark plasma sintering technique at 1500 °C in an inert atmosphere. Second, the surface of the fabricated target was cleaned by grinding and polishing to remove any contamination, such as graphite and characterized. Finally, thin films were grown using PLD in an inert atmosphere at various substrate temperatures (RT to 500 °C). While the X-ray diffractometer (XRD) analysis revealed that the Y/Sialon target has ß phase, the XRD of the fabricated films showed no diffraction peaks and thus confirming the amorphous nature of fabricated thin films. XRD analysis displayed that the fabricated thin films were amorphous while the transparency, measured by UV-vis spectroscopy, of the films, decreased with increasing substrate temperature, which was attributed to a change in film thickness with deposition temperature. X-ray photoelectron spectroscopy (XPS) results suggested that the synthesized Y/Sialon thin films are nearly homogenous and contained all target's elements. A scratch test revealed that both 300 and 500 °C coatings possess the tough and robust nature of the film, which can resist much harsh loads and shocks. These results pave the way to fabricate different Sialon doped materials for numerous applications.

BACKGROUND AND AIM: Insulin resistance and consequent hyperinsulinemia are common among patients with polycystic ovary syndrome (PCOS). Ethnicity and dietary habits affect insulin levels. There is little published information from India on insulin levels in PCOS patients. Thus the present study aimed to determine the insulin response to oral glucose in women with PCOS and healthy women. METHODS: In a case-control study design, women with PCOS and lean healthy women without a family history of diabetes mellitus underwent oral glucose tolerance testing. Samples were collected at 0, 1 and 2 h after glucose ingestion. RESULTS: Two hundred and eighty-five women with PCOS and 27 lean healthy young women were enrolled into the study. The mean age of controls was 22.8 +/- 4.5 years (range 15-32 years) and their mean body mass index (BMI) was 19.7 +/- 2.6 kg/m(2). Mean blood glucose at 0, 1 and 2 h was 88.2 +/- 7.2, 115.5 +/- 25.5 and 91.8 +/- 20.5 mg/dl, respectively. Corresponding plasma insulin levels were 5.8 +/- 1.1, 32.7 +/- 26.5 and 14.6 +/- 9.6 mIU/l. Peak insulin levels were seen at 1 h and these came down to less than 40% of the peak value by 2 h. Glucose/insulin ratio at 0, 1 and 2 h was 15.6 +/- 3.1, 7.0 +/- 3.1 and 11.4 +/- 7.0. Homeostasis model assessment of insulin resistance (HOMA-IR) was 1.2 +/- 0.2. The age of the PCOS women ranged from 15 to 40 years (mean 23.4 +/- 6.2 years) and their BMI ranged from 16.4 to 50.4 kg/m(2) (mean 27.7 +/- 6.3 kg/m(2)). One hundred and seventy-six (62%) PCOS patients had normal glucose tolerance (NGT), 39 (14%) had impaired fasting glucose (IFG), 49 (17%) had impaired glucose tolerance (IGT) and 21 (7%) had type 2 diabetes mellitus (T2DM). Insulin response was higher in women with PCOS. Peak insulin was observed at 1 h. The difference between 1-h and 2-h post-glucose insulin decreased with worsening glucose tolerance. Both plasma insulin and BMI showed a rising trend from NGT to IFG to IGT. There was no further increase in either insulin or BMI from IGT to T2DM. Glucose/insulin ratio at 0, 1 and 2 h was lower (8.3 +/- 4.2, 2.0 +/- 1.6 and 3.2 +/- 3.5) than that of healthy controls. HOMA-IR was 3.1 +/- 3.0. CONCLUSION: Women with PCOS had an exaggerated insulin response to glucose. Thirty-eight percent of PCOS women had some form of abnormal glucose tolerance. Greater insulin response was seen with impairment of glucose tolerance. Obesity had no effect on fasting insulin or insulin response to oral glucose in PCOS women with NGT.
