RESUMO

The paper reports on symmetric benign lipomatosis of the head and neck--Madelung's syndrome. Over a period of 5 years, 6 men were treated for this disease at the ENT Department of the School of Medicine, Comenius University in Martin. Four of these patients had hepatopathy, two diabetes mellitus, and in two gynecomasty was observed. Surgical treatment yielded satisfactory functional and cosmetic results.

RESUMO

In 1973-1985 at the ENT Clinic of the Faculty Hospital with policlinic in Martin 559 patients with carcinoma of the larynx were treated: 23 women and 529 men. Thirteen women and 286 men were operated. Partial resection was performed in 137 patients. The best results were recorded after chordectomy, the poorest results after horizontal partial resection of the larynx. The author discusses the effectiveness of laryngectomy after previous partial surgical operation of the larynx, early diagnosis and corrected indication for surgical operation.

Poor patency of the nasal passage has a negative impact on respiration. In 52 patients with increased nasal resistance blood gases and acid-base balance were measured according to the method of Astrup before and after surgical treatment of the upper respiratory organs. We found a significant increase of paO2 (on the average by 0.656 kpa; p less than 0.05) and of the buffer bases. The other parameters did not change to a statistically significant extent. The lower paO2 value in increased nasal resistance is most likely not due to hypoventilation. Altered nasopulmonary and nasothoracis reflexes exercise a negative influence on pulmonary mechanics and the laryngotracheobronchial tonus with subsequent alteration of the intrapulmonary distribution of the inspired air, and on ventilation-perfusion conditions of the lungs.
