RESUMO

PURPOSE: To assess the impact of 1 and 2-stage palatoplasty protocol on the dental arch relationships in unilateral cleft lip and palate (UCLP) in a single center. METHODS: Our study consisted of 349 individuals divided into 2 groups according to the palatoplasty protocol. Two-stage group comprised 169 subjects with UCLP (mean age: 6.9 y, 110 male and 59 female) who underwent lip, nasal ala, and anterior palate repair with vomer flap from 3 to 6 months (first surgery stage). Soft palate repair occurred from 12 to 18 months (second surgery stage). The one-stage group comprised 180 subjects with UCLP (mean age: 7.2 y, 108 male and 72 female) who underwent 1-stage palatoplasty. Dental models were evaluated by 3 experienced orthodontists applying Goslon Yardstick and the 5-year-old index (FYOI). The influence of the palatoplasty technique and surgeon factor on the interarch relationship was evaluated. The weighted Kappa was used to assess intraexaminer and interexaminer agreements for comparisons of dental arch relationships. Intergroup comparisons were conducted using the χ 2 test ( P <0.05). RESULTS: The intraexaminer reliability was very good (0.81 to 0.98) and interexaminer reliability varied from satisfactory to very good (0.56 to 0.83). The mean occlusal index of the 2-stage and 1-stage groups was 2.77 and 3.03, respectively. The variability of the mean index between surgeons varied from 2.38 to 3.2 in the 2-stage group and 2.91 to 3.2 in the 1-stage group. There were significant differences in the frequency of Goslon 5 index ( P =0.002) between groups, with the 2-stage group presenting less cases (1.18%) than the group 1-stage (11.11%). CONCLUSION: The interarch relationship was similar for both palate repair protocols. Two-stage palatoplasty showed a decreased prevalence of Goslon index 5.

BACKGROUND: The evaluation of surgical outcomes is needed to achieve excellence in nasal reconstruction of patients with complete bilateral cleft lip and palate (BCLP). The study objective is to evaluate the quality of nasal aesthetics after the columella elongation surgery. METHODS: The sample of this study consisted of 70 patients with complete BCLP, operated on columella elongation surgery and analyzed at 6 to 12 years. The system for evaluation of nasal appearance after the columella elongation surgery was performed by objective and subjective analyses. ANTHROPOMETRIC MEASUREMENTS: Four anthropometric nose measurements were performed directly on the faces, and the same was performed in control group (60 children without oral clefts), paired by age and sex with the experimental group. The measurements were repeated 3 times (triplicate). SUBJECTIVE ANALYSIS: Scores to evaluate nasal width, nasal tip projection, and length of the columella were applied before and after the columella elongation surgery by 5 raters from the rehabilitation team. Interrater and intrarater agreement was calculated by means of the kappa test. RESULTS: Nasal width in BCLP group was higher when compared to the control group (P < 0.05). Nasal tip projection (subnasal-pronasal) and columella length (subnasal-columella) were decreased in BCLP group compared to that of the controls (P < 0.05). Columella width was similar in both study groups (P > 0.05). All scores of nasal aesthetics significantly improved after the columella elongation surgery. CONCLUSIONS: Nose of the patients with BCLP is wider, is less projected, and has the shorter columella compared to that of the noncleft subjects. After secondary columella elongation surgery in BCLP, nasal width, nasal projection, and columella length significantly improved.

RESUMO

The aim of this study is to demonstrate a different manner of treating cleft lip nose by approaching the problem at its origin, the nasolabial gap. The surgical technique we propose corrects the affected structures from the "bottom to the top." First we have to make sure that the orbicularis oris is well oriented, then we have to create space for the twisted alar cartilage, and finally we reach the tip of the nose with less asymmetry to correct, compared with previously published data. Before attempting any additions of grafts, implants, etc. for the restoration of any congenital deformity, especially clefts, the surgical team must try to reposition, reorganize, and reorient the structures affected by a lack of continuity or a gap as a result of the embryogenesis.

Introdução: Após avaliação da técnica empregada em nosso serviço para correção de fenda labial unilateral, observamos uma pontuação negativa maior com relação ao resultado final do nariz, o que nos levou a manter nossa linha de pesquisa, com o objetivo de introduzir e avaliar novo método para moldagem nasal pós-operatória imediata, por meio de molde de silicone. Método: Foram avaliados 20 pacientes operados pela mesma técnica, sendo que 10 deles utilizaram o molde nasal, e os outros 10 não. A avaliação foi feita por três cirurgiões de grande experiência, utilizando fotos pré e pós-operatórias e um filme do paciente com sua idade atual, sem o conhecimento de qual grupo utilizou o molde. Resultados: A idade de avaliação no período pós-operatório variou de 1 a 9 anos, do procedimento cirúrgico; somente 60% dos pacientes foram operados na idade preconizada pelo nosso protocolo (3 meses), o tempo médio de uso do modelador foi de 4 meses. Após a aplicação de análise estatística, observamos que não houve melhora de resultados nos pacientes que usaram o modelador nasal, em comparação ao grupo que não usou. Conclusões: O grupo de pacientes operados pela técnica de Gõteborg com a utilização do modelador nasal não apresentou resultados melhores estatisticamente significantes quando comparado ao grupo de pacientes operados pela mesma técnica sem o uso do modelador nasal.


Introduction: After evaluating the technique chosen for unilateral cleft lip surgery, we notice a negative outcome of the nose aspect, what lead us to introduce and evaluate a new method of nasal molding with a silicone strut. Methods: There were evaluate 20 patients operated by the same technique, 10 patients with immediate silicone strut molding and 10 patients without nasal molding. The results were analyzed with pre-operatory pictures, post operative pictures and a movie of the child in the present date by three expertise cleft lip surgeon, whose did not know which use or not the silicone molding. Results: The age of the patients after the surgery range from 1 to 9 years, which only 60% were operated on the pre set age (3 months), the average time using the Silicone molding strut were 4 months. After statistics analysis, it demonstrated that there was not a better outcome in the nose aspect on patients who’s used the nasal molding. Conclusions: After statistics analysis, the group of patients under went the Gõteborg Technique, using immediate silicone strut molding, shown no better results than the group without nasal molding.

Introdução: Os autores propõem um método de simples compreensão e de fácil reprodução para a abordagem e tratamento da região pudenda, geralmente realizado no final de uma dermolipectomia abdominal. Método: A técnica consiste em lipectomia aberta com tesoura, podendo ser associada a lipoaspiração convencional e pexia com tracionamento cruzado da região pudenda, devolvendo as características de jovialidade à área em questão. Resultados: Pode-se constatar pelas fotografias, que houve estreitamento do triângulo e ainda elevação da rafe mediana, posicionada mais anatomicamente. Relatos foram obtidos quanto à melhora da qualidade e desempenho do ato sexual, tanto pelas pacientes quanto por seus parceiros. Discussão: Há dez anos, o autor principal aborda esta região, inicialmente utilizava lipoaspiração como ferramenta. Os resultados tornaram-se mais promissores quando foi iniciado o uso de pexia, anteriormente paralela e atualmente cruzada. Conclusão: Conclui-se que o método descrito, propõe e atinge os seus principais. Objetivos: tornar a região pudenda mais magra, estreita e harmoniosa. Por trazer benefícios para a paciente e para seu companheiro, a técnica poderá ser adicionada ao arsenal do cirurgião plástico brasileiro que tem como intuito uma melhora global no aspecto de sua cirurgia do contorno corporal.


Introduction: The authors propose a simple, comprehensive and easily reproducible method to suspend to thin and fixate the pubic region on a better position, usually in association with abdominoplasties. Method: The technique consists in approaching the pubic region with open defating associated or not with traditional liposuction and a cross traction with absorbable sutures, anchoring them in the anterior rectus apponeurosis. Results: Pictures will show the goal was achieved, with thinner, younger and more harmonious pudenda region obtained with the method. There are some reports of better performance during intercourse, probably because of the superficialization of the clitoris. Some mates also reported a better quality intercourse after the procedure. Discussion: The main author started to address this region with liposuction ten years ago, which is also a good option. However, in his hands the results became more promising when he began to use this traction at first a parallel traction and now a crossed one. Conclusion: With these results the authors were able to show that this method can be employed usually after an abdominoplasty to reach the main proposed objectives which are: a thinner, narrower and better looking pubic region.
