Background and Aims: In a cost-benefit framework, plant carnivory is hypothesized to be an adaptation to nutrient-poor soils in sunny, wetland habitats. However, apparent exceptions to this cost-benefit model exist, although they have been rarely studied. One of these exceptions is the carnivorous subshrub Drosophyllum lusitanicum , which thrives in Mediterranean heathlands on dry sandstone soils and has relatively well-developed, xeromorphic roots. Here, the roles of leaf (carnivory) and root (soil) nutrient uptake in growth promotion of this particular species were assessed. Methods: In a greenhouse experiment, plants were fed with laboratory-reared fruit flies ( Drosophila virilis ) and received two concentrations of soil nutrients in a factorial design. Above-ground plant growth and final above- and below-ground dry biomass after 13 weeks were recorded. Nutrient uptake via roots was also evaluated, using stable nitrogen isotope analysis. Key Results: Insect feeding resulted in significantly higher growth and above- and below-ground biomass compared with soil fertilization. No additional benefits of fertilization were discernable when plants were insect-fed, indicating that roots were not efficient in nutrient absorption. Conclusions: The first evidence of strong reliance on insect prey feeding in a dry-soil carnivorous plant with well-developed roots is provided, suggesting that carnivory per se does not preclude persistence in dry habitats. Instead, the combination of carnivory and xeromorphic root features allows Drosophyllum to thrive on non-waterlogged soils. New evidence is added to recent research emphasizing the role of root systems of carnivorous plants in explaining their distribution, partly challenging the cost-benefit hypothesis.

OBJECTIVE: To assess the impact of gender upon the prognosis and medical care in a regional acute ST-elevation myocardial infarction management network. DESIGN: An observational study was made of consecutive patients entered in a prospective database. SCOPE: The Catalan acute ST-elevation myocardial infarction management network. PATIENTS: Patients treated between January 2010 and December 2011. INTERVENTIONS: Primary angioplasty, thrombolysis or conservative management. VARIABLES OF INTEREST: Time intervals, proportion and type of reperfusion, overall mortality, and in-hospital complication and overall mortality at 30 days and one year were compared in relation to gender. RESULTS: Of the 5,831 patients attended by the myocardial infarction network, 4,380 had a diagnosis of acute ST-elevation myocardial infarction, and 961 (21.9%) were women. Women were older (69.8±13.4 vs. 60.6±12.8 years; P<.001), had a higher prevalence of diabetes (27.1 vs. 18.1%, P<.001), Killip class>I (24.9 vs. 17.3%; P<.001) and no reperfusion (8.8 vs. 5.2%; P<.001) versus men. In addition, women had greater delays in medical care (first medical contact-to-balloon: 132 vs. 122min; P<.001, and symptoms onset-to-balloon: 236 vs. 210min; P<.001). Women presented higher percentages of overall in-hospital complications (20.6 vs. 17.4%; P=.031), in-hospital mortality (4.8 vs. 2.6%; P=.001), 30-day mortality (9.1 vs. 4.5%; P<.001) and one-year mortality (14.0 vs. 8.3%; P<.001) versus men. Nevertheless, after multivariate adjustment, no gender differences in 30-day and one-year mortality were observed. CONCLUSIONS: Despite a higher risk profile and poorer medical management, women present similar 30-day and one-year outcomes as their male counterparts in the context of the myocardial infarction management network.

Resprouting as a response to disturbance is now widely recognized as a key functional trait among woody plants and as the basis for the persistence niche. However, the underlying mechanisms that define resprouting responses to disturbance are poorly conceptualized. Resprouting ability is constrained by the interaction of the disturbance regime that depletes the buds and resources needed to fund resprouting, and the environment that drives growth and resource allocation. We develop a buds-protection-resources (BPR) framework for understanding resprouting in fire-prone ecosystems, based on bud bank location, bud protection, and how buds are resourced. Using this framework we go beyond earlier emphases on basal resprouting and highlight the importance of apical, epicormic and below-ground resprouting to the persistence niche. The BPR framework provides insights into: resprouting typologies that include both fire resisters (i.e. survive fire but do not resprout) and fire resprouters; the methods by which buds escape fire effects, such as thick bark; and the predictability of community assembly of resprouting types in relation to site productivity, disturbance regime and competition. Furthermore, predicting the consequences of global change is enhanced by the BPR framework because it potentially forecasts the retention or loss of above-ground biomass.

An intertidal fish assemblage was studied in central Chile for 15 years in three localities: Isla Negra, El Tabo, and Las Cruces. Analyses of their multivariate dissimilarities were carried out considering temporal and spatial factors. Temporal factors included intra-annual and year-to-year variability. Spatial factors included locality, intertidal tidepool height, and each tidepool considered as a unique unit. Complementary to this, we tested the hypothesis that the El Niño Southern Oscillation (ENSO) would help explain year-to-year dissimilarities in the multivariate structure of this fish assemblage for the 15 years of data. To this end, the ENSO was regarded as a continuous inter-annual process as well as a set of discrete events. Additionally, dissimilarities in the temporal dynamics of the fish assemblage were evaluated considering each locality and tidepool unique units. Results indicated the following: (i) The most representative species for the whole area and period of the study were Scartichthys viridis (44 %), Helcogrammoides chilensis (17 %), Girella laevifrons (10 %), Graus nigra (7 %), Auchenionchus microcirrhis (5 %), and Helcogrammoides cunninghami (4 %); (ii) there is intra-annual (i.e., seasonality) and year-to-year multivariate variability in fish assemblage dissimilarities throughout the entire area of study, including all the tidepools and localities; (iii) when considering the spatial factor specifically, each tidepool unit, as well as their heights and localities, presented their own unique temporal year-to-year dynamics. The latter can be explained by the ENSO factor, also considering the intensity of the El Niño and La Niña events. In summary, the multivariate structure of the intertidal fish assemblage was statistically dissimilar when comparing neutral periods with El Niño and La Niña events. This structure was observed for the entire area of study, for each locality, and primarily for each tidepool as a unique unit. Fish physiological mechanisms underlying the identified patterns are discussed.

BACKGROUND: An early semi-invasive diagnosis of endometriosis has the potential to allow early treatment and minimize disease progression but no such test is available at present. Our aim was to perform a combined mRNA microarray and proteomic analysis on the same eutopic endometrium sample obtained from patients with and without endometriosis. METHODS: mRNA and protein fractions were extracted from 49 endometrial biopsies obtained from women with laparoscopically proven presence (n= 31) or absence (n= 18) of endometriosis during the early luteal (n= 27) or menstrual phase (n= 22) and analyzed using microarray and proteomic surface enhanced laser desorption ionization-time of flight mass spectrometry, respectively. Proteomic data were analyzed using a least squares-support vector machines (LS-SVM) model built on 70% (training set) and 30% of the samples (test set). RESULTS: mRNA analysis of eutopic endometrium did not show any differentially expressed genes in women with endometriosis when compared with controls, regardless of endometriosis stage or cycle phase. mRNA was differentially expressed (P< 0.05) in women with (925 genes) and without endometriosis (1087 genes) during the menstrual phase when compared with the early luteal phase. Proteomic analysis based on five peptide peaks [2072 mass/charge (m/z); 2973 m/z; 3623 m/z; 3680 m/z and 21133 m/z] using an LS-SVM model applied on the luteal phase endometrium training set allowed the diagnosis of endometriosis (sensitivity, 91; 95% confidence interval (CI): 74-98; specificity, 80; 95% CI: 66-97 and positive predictive value, 87.9%; negative predictive value, 84.8%) in the test set. CONCLUSION: mRNA expression of eutopic endometrium was comparable in women with and without endometriosis but different in menstrual endometrium when compared with luteal endometrium in women with endometriosis. Proteomic analysis of luteal phase endometrium allowed the diagnosis of endometriosis with high sensitivity and specificity in training and test sets. A potential limitation of our study is the fact that our control group included women with a normal pelvis as well as women with concurrent pelvic disease (e.g. fibroids, benign ovarian cysts, hydrosalpinges), which may have contributed to the comparable mRNA expression profile in the eutopic endometrium of women with endometriosis and controls.

BACKGROUND: Type D personality is considered as an independent risk factor for morbidity and mortality in cardiovascular patients and a vulnerability factor for distress in the general population. Because representative community studies are rare, we sought to determine the prevalence of type D personality and its relationship with demographic characteristics, different features of mental disorders, cardiovascular risk factors, health behavior, endothelial function and cardiovascular biomarkers in the general population. METHODS: The prevalence of type D personality and its correlates were analyzed cross-sectionally in a population-based sample of 5,000 Mid-Europeans aged 35-74 years from the Gutenberg Health Study. RESULTS: The prevalence of type D personality was 22.2% without remarkable differences in sex distribution. Type D subjects were characterized by lower socioeconomic status, lack of a partnership, increased depression, anxiety, depersonalization and health care utilization. Despite its strong association with mental disorders, type D personality emerged as psychometrically distinct. Although type D personality was independently associated with coronary heart disease (OR = 1.54, p = 0.044), no associations with traditional cardiovascular risk factors were found independently from depression or anxiety. CONCLUSIONS: Although type D personality is strongly associated with depression, anxiety, impaired mental and somatic health status, and increased health care utilization, the type D construct seems to comprise dysfunctional personality patterns not covered by depression and anxiety scales. Beyond these associations, the pathways of the cardiotoxic impact of type D personality remain to be elucidated. There is a need for prospective population studies on potential links between type D personality and cardiac disease.

Parasites can infect larval, juvenile or adult marine fishes; however, the effects of parasites on the growth and condition of fish larvae have seldom been investigated. This study analysed the effects of a parasitic copepod on the larval growth of the Chilean triplefin Helcogrammoides chilensis (Tripterygiidae) based on the microstructure of the sagittal otoliths. Fish larvae were collected during the austral spring of 2010 off central Chile. Their body length ranged from 5.1 to 16.6 mm (2 to 57 days old). They were parasitised by a penellid larval copepod that was always externally attached to the ventral side of the fish's gut. The prevalence of the copepod ranged from 2.7% to 20.8%, with one to four parasites per fish larva. Relationships between otolith size (radius, perimeter) and larval size were equal for parasitised and unparasitised fish larvae (P > 0.05). Larval growth was also similar for unparasitised (0.21 mm/day) and parasitised fish larvae (0.19 mm/day) (P > 0.05). However, a comparison of same-aged larvae showed that the larvae with copepods were smaller in both length and estimated body volume than the larvae without copepods. The Recent Otolith Growth Index, indicated that larval H. chilensis with copepods showed a reduction in recent growth and condition compared with those without evidence of copepods (P < 0.05). Nevertheless, a higher parasite load (two vs. one pennellids) did not decrease the condition of the larval fish. The infestation of pennellids on coastal fish larvae may therefore induce an increase in the pelagic larval duration and potentially affect the settlement rates of this intertidal fish.

RESUMO

Munchausen syndrome by proxy is an illness which is very difficult to diagnose. It has a high morbidity and mortality rate. The knowledge of the characteristics of the victim and the perpetrator can be quite useful for its early recognition. The American Professional Society on the Abuse of Children, has recently brought the terms for the syndrome up to date. We look at this updating and present six cases diagnosed in our hospital, analysing their main features and comparing them with the medical literature.

RESUMO

Cardiac resynchronization improves survival and functional class of patients with advanced chronic heart failure. Placement of a stimulation electrode in the coronary sinus via the left subclavian vein is not always possible and other alternatives are required, above all when it concerns upgrading a previous device. This paper presents the case of a patient with a pacemaker/defibrillator and occlusion of both subclavian veins who had a stimulation electrode successfully placed in the coronary sinus via the right internal jugular.

This work used chemical imaging in the short-wave infrared region for analysing gunshot residues (GSR) patterns in cotton fabric targets shot with conventional and non-toxic ammunition. It presents a non-destructive, non-toxic, highly visual and hiperspectral-based approach. The method was based on classical least squares regression, and was tested with the ammunition propellants and their standard components' spectra. The propellants' spectra were satisfactorily used (R2 >0.966, and CorrCoef >0.982) for identifying the GSR irrespective of the type of ammunition used for the shooting. In a more versatile approach, nitrocellulose, the main component in the ammunition propellants, resulted an excellent standard for identifying GSR patterns (R2>0.842, and CorrCoef >0.908). In this case, the propellants' stabilizers (diphenilamine and centralite), and its nitrated derivatives as well as dinitrotoluene, showed also high spectral activity. Therefore, they could be recommended as complementary standards for confirming the GSR identification. These findings establish the proof of concept for a science-based evidence useful to support expert reports and final court rulings. This approach for obtaining GSR patterns can be an excellent alternative to the current and traditional chemical methods, which are based in presumptive and invasive colour tests.

Despite the improvement in the quality of life of patients with SLE due to scientific and technological advances, SLE remains a disease that over the years may produce irreversible damage to patients. Osteoporosis and secondary bone fractures are two of the major causes of irreparable injury in patients with SLE. Vitamin D insufficiency may play a vital role both in reduced bone mineral density (BMD) and in the appearance of fractures, although its mechanisms of action are still unclear. We performed a systematic review of the literature in order to determine the prevalence and predictors of reduced vitamin D plasma levels, bone loss and the presence of fractures in SLE patients. Our review encompassed all English-language publications using Medline and EMBase electronic databases from their inception (1966 and 1980, respectively) to December 2016. We included all intervention studies and observational studies in which vitamin D plasma levels, BMD and bone loss were measured and applied to patients with SLE. Previous studies suggested an increase in bone loss and fracture in patients with SLE compared with general population and although there is a high prevalence of vitamin D insufficiency in the general population, previous studies had demonstrated lower vitamin D levels in patients with SLE compared to age-matched controls. The etiology of reduced bone mass and reduced vitamin D plasma levels in SLE is multifactorial and includes a variety of intrinsic factors related to the disease itself and treatment side effects. SLE patients are at risk for developing these two comorbidities (reduced vitamin D plasma levels and low BMD) and it is therefore essential to study, monitor, prevent and treat bone metabolism disorders in SLE patients.

Microplastics pollution is a growing global concern that affects all aquatic ecosystems. Microplastics in the environment can be in the form of fibers and/or particles, being the former the most abundant in the marine environment, representing up to 95% of total plastics. The aim of this work was to compare the content of microplastics among intertidal fish with different feeding type. Our results show that omnivorous fish presented a higher amount of microplastic fibers than registered in herbivores and carnivores. Moreover, lower condition factors (K) were found in omnivorous specimens with higher microplastic content. We hypothesized that the type of feeding resulted in different microplastic ingestion, with species with wider range of food sources as omnivores with higher rates. Futures studies carried out to evaluate the biological impacts of microplastics on marine organisms, and microplastics cycling on the marine environment should consider the type of feeding of the studied species.

OBJECTIVE: To evaluate the results of arthroscopic reconstruction for the treatment of unstable scaphoid non-union with cancellous bone autograft. METHODS: 13 patients were treated with a mean age of 26 (18-45) years. The average time from injury until surgery was 14 (6-48) months. Preoperative and postoperative clinical and radiological parameters were evaluated. Mean follow-up was 16.8 (12-36) months. RESULTS: Consolidation was achieved in all cases at 7 (4-10 weeks), no patient had complications or reoperations. Range of motion, pain, functional assessment (DASH questionnaire) and radiological measurements improved compared to preoperative measurements. The average range of flexion improved: flexion 71.9° (55°-80°) to 81.7° (55°-90°), extension 66.3° (30°-80°) to 84.4° (70° -90°), ulnar deviation 21.5 (10°-25°) to 25.5° (20°-45°) and radial deviation 11.9° (5°-25°) to 13.3° (10th-20th). Pain (VAS 0-10) improved from 6.8 (4-10) to 0.7 (0-3). DASH functional scale improved from 36 (12-78) to 8 (0-10). The Scapho-Lunate Angle improved from 67.7° (62°-88°) to 47° (32°-55°), and the Radio-Lunate Angle improved from 30.8° (10° -45°) to 4(0°-10°). CONCLUSION: Treatment of unstable scaphoid non-union with cancellous bone graft assisted by arthroscopy presents good clinical results with a short period of consolidation and recovery.

AIMS: Monitoring disease activity in myocarditis is important for tailored therapeutic strategies. This study evaluated the ability of T1 and T2 mapping cardiovascular magnetic resonance (CMR) to monitor the course of myocardial inflammation in healing myocarditis. METHODS AND RESULTS: Forty-eight patients with strictly defined acute myocarditis underwent CMR at 1.5 T in the acute stage, at 3-months (n = 39), and at 12-months follow-up (FU) (n = 21). Normal values were obtained in a control group of 27 healthy subjects. The CMR protocol included standard ('Lake-Louise') sequences as well as T1 (modified Look-Locker inversion recovery sequence, MOLLI) and T2 (gradient- and spin-echo sequence, GraSE) mapping. T1, T2, and extracellular volume (ECV) maps were generated using an OsiriX plug-in. Native myocardial T1, T2, and ECV values were increased in the acute stage, but declined with healing of myocarditis. The performances of global native T1 and T2 to differentiate acute from healed myocarditis stages were significantly better compared with all other global CMR parameters with AUCs of 0.85 (95% CI, 0.76-0.94) and 0.83 (95% CI, 0.73-0.93). Furthermore, regional native T1 and T2 in myocarditis lesions provided AUCs of 0.97 (95% CI, 0.93-1.02) and 0.93 (95% CI, 0.85-1.01), which were significantly superior to any other global or regional CMR parameter. CONCLUSION: Healing of myocarditis can be monitored by native myocardial T1 and T2 measurements without the need for contrast media. Both native myocardial T1 and T2 provide an excellent performance for assessing the stage of myocarditis by CMR.
