RESUMO

Recent advances allow the use of Augmented Reality (AR) for many medical procedures. AR via optical navigators to aid various knee surgery techniques (e.g., femoral and tibial osteotomies, ligament reconstructions or menisci transplants) is becoming increasingly frequent. Accuracy in these procedures is essential, but evaluations of this technology still need to be made. Our study aimed to evaluate the system's accuracy using an in vitro protocol. We hypothesised that the system's accuracy was equal to or less than 1 mm and 1° for distance and angular measurements, respectively. Our research was an in vitro laboratory with a 316 L steel model. Absolute reliability was assessed according to the Hopkins criteria by seven independent evaluators. Each observer measured the thirty palpation points and the trademarks to acquire direct angular measurements on three occasions separated by at least two weeks. The system's accuracy in assessing distances had a mean error of 1.203 mm and an uncertainty of 2.062, and for the angular values, a mean error of 0.778° and an uncertainty of 1.438. The intraclass correlation coefficient was for all intra-observer and inter-observers, almost perfect or perfect. The mean error for the distance's determination was statistically larger than 1 mm (1.203 mm) but with a trivial effect size. The mean error assessing angular values was statistically less than 1°. Our results are similar to those published by other authors in accuracy analyses of AR systems.

BACKGROUND AND OBJECTIVE: Since the description of the syndrome of the second intermetatarsal space, this is a common diagnosis among foot and ankle surgeons. However, no series have been published that consider this syndrome as its own entity. The objective of this study is to evaluate the clinical and radiological results of the release of the intermetatarsal ligament and minimally invasive distal metatarsal osteotomy in patients diagnosed with second space syndrome. MATERIALS AND METHODS: An observational, longitudinal, retrospective study was carried out in patients with a clinical diagnosis of second space syndrome operated on using a minimally invasive technique. For the clinical results, the visual analog scale (VAS) for subjective pain, the Manchester-Oxford Foot Questionnaire (MOXFQ) and the scale for minor metatarsals and interphalangeals of the American Orthopedic Foot and Ankle Society (AOFAS-LMTS) were used for clinical-functional assessment. Consolidation or not of osteotomies was recorded and complications were recorded. RESULTS: Twenty-nine feet in 29 patients were included in the study. After a mean follow-up of 39 months (25-50), clinically and statistically significant improvement was obtained in the scores: pain VAS, scales and subscales of the MOXFQ and the AOFAS-LMTS (p < 0.0001). All osteotomies healed at the end of follow-up and no major complications were recorded. CONCLUSIONS: Percutaneous or minimally invasive surgery, in patients with second intermetatarsal space syndrome, obtains good clinical, functional and subjective results, with few complications. Therefore, we consider it an effective, safe and recommended technique in the hands of experienced surgeons.

Spiders are a large group of arthropods and nearly omnipresent in warm and temperate climates. They rely on tactile and visual information to hunt and breed, but compared with their mechanical senses, little is known about their visual systems. In this study, we analyzed the visual spectral sensitivity of the Chilean recluse spider Loxosceles laeta, a synanthropic species posing a significant threat to humans, using electroretinogram recordings of its three eye types and open field tests with localized chromatic illumination for behavioral analysis. The electroretinogram displayed two sensitivity peaks in the ultraviolet and green ranges, and no differences were observed between the three eye types and between male and female specimens. Selective chromatic adaptation reduced overall light sensitivity, but did not support the expression of more than one type of rhodopsin in photoreceptors. The open field tests revealed a preference for corners over side areas, and an increased exploration of open field areas illuminated by shorter wavelength (violet to green) light compared with non-illuminated areas, while no behavioral responses to red and near-infrared light were observed. These data suggest that L. laeta has monochromatic vision without spectral specializations in its three secondary eye pairs.

The speed performance is involved not only in linear sprints, but also in a wide spectrum of multi-directional movements, such as curve sprinting. Curved sprint can be defined as sprint with gradual and continuous change of direction (COD). Although ~85% of the actions performed at maximum velocity in a professional soccer league are curvilinear sprints, there is not any specific test to assess this ability. This study aimed to analyse the reliability of a new curve sprint test, and compare its results with those obtained by soccer players in linear sprint. Forty experienced soccer players performed 3 attempts of curve sprint (using the penalty arc) to right and left side (17 m), and 3 linear sprints (17 m) in two different days. The ICCs (inter-session reliability) were 0.93 for sprint curve right side (CSRS) and 0.89 for sprint curve left side (CSLS), considered "acceptable". The CVs (intra-session reliability) were 0.87% in CSRS and 1.15% in CSLS. The coefficient of determination (R2) between linear and curve sprinting was ~35%. The association between curve sides was "very large" (r = 0.878; p < 0.01). In summary, we showed that "curve sprint test" is highly reliable, and that curvilinear and linear sprints are different and independent actions.

Salmonid fishes like the rainbow trout Oncorhynchus mykiss have a highly developed olfactory sense that allows them to perceive some odorants at very low concentrations, such as certain amino acids and bile salts. Previous behavioral and electrophysiological studies in salmonids have shown strong responses to human skin odor. Because this stimulus represents a complex and heterogeneous mixture of components, we sought to determine which odorants contribute to the sensitive detection of human skin odor by salmonids. In vivo electroolfactogram recordings in O. mykiss revealed lactic acid, pyruvic acid and two B vitamins, thiamine and riboflavin, as novel, potent odorants which triggered responses at nanomolar concentrations. Two more B vitamins, nicotinic and pantothenic acid, were detected at micromolar concentrations. These compounds share important roles in cellular energy metabolism, supporting an original role in food search and feeding behavior of this species and most likely other fishes. The olfactory detection of B vitamins by salmonids represents a new paradigm in chemosensation, warranting further investigation in other teleosts.

In many fishes, the olfactory sense participates in such vital processes as feeding, reproduction, orientation, and predator avoidance. In teleosts, these tasks are fulfilled by a single type of olfactory organ for odorant and pheromone detection, containing ciliated and microvillus receptor neurons, and olfactory crypt cells. Recently, progress was made in understanding crypt cell function with the discovery of a V1R-like odorant receptor expressed in this neuron, an analysis of crypt cell odorant tuning properties, and the dissection of crypt cell connectivity within the telecephalon. Here, we review recent findings on the molecular properties, functions, and associated neural pathways of the three types of teleost olfactory receptor neurons with special emphasis on the crypt cell, and evaluate their roles in the detection of food, social and sexual odorants.

The aims of this study were (a) to assess intra-session reliability and usefulness of the soccer-specific maximum vertical jump (heading test, HT) and (b) to analyse the validity of the easy-to-use and cost-effective instrument (smartphone camera, MOB) compared with gold-standard instrument (3D motion capture system, MOCAP) to obtain the vertical jump performance during HT. Twelve semi-professional high-level and fifteen amateur soccer male players (23.9 ± 3.6 years) performed three HT attempts, and kinematic data were recorded with MOB and MOCAP. Intra-class correlation coefficient (ICC) and coefficient of variation (CV) were used as measures of intra-session reliability. T-test with Cohen's effect size (ES), Pearson's product moment and Bland-Altman analysis were used to obtain MOB validity. Regarding intra-session reliability, the CV was 1.13%, and ICC was 0.98, considered acceptable. Respecting validity criteria did not reveal significant differences (p < 0.05; effect size = 0.06, considered trivial), 'almost perfect' correlation (Pearson) (r = 0.98; p < 0.05), and strong agreement were obtained between MOB and MOCAP. This finding showed a test (HT) with a specific character, using cost-effective instrument and applicable to all soccer fields (adjusted to the standardised lines in the soccer field), all of them backed-up by reliability, usefulness and validity criteria.

Levodopa-carbidopa intestinal gel (LCIG) has shown to be efficacious in motor and non-motor symptoms (NMS). Nevertheless, studies with patient Quality of Life (QoL) as a primary endpoint are scarce. To assess the effect of LCIG on Advanced Parkinson's Disease (APD) patients QoL. Secondarily, the impact on motor symptoms and NMS, emotional well-being, treatment satisfaction, and caregiver QoL, stress, disease burden, anxiety, depression, and work impairment were also investigated. In this prospective, 6-month multicenter postmarketing observational study, LCIG was administered to 59 patients with APD. Endpoints were assessed using validated scales and questionnaires. LCIG significantly improved patient QoL (PDQ-39 mean change ± standard deviation from baseline, -12.8 ± 14.6; P < 0.0001), motor symptoms (UPDRS-III in "On," -6.5 ± 11.8; P = 0.0002), NMS (NMSS, -35.7 ± 31.1; P < 0.0001), mood (Norris/Bond-Lader VAS, -6.6 ± 21.1; P = 0.0297), fatigue (PFS-16, -0.6 ± 1.0; P = 0.0003), depression (BDI-II, -5.1 ± 9.4; P = 0.0002), anxiety (BAI, -6.2 ± 9.6; P < 0.0001), and patient treatment satisfaction (SATMED-Q, 16.1 ± 16.8; P < 0.0001). There were significant correlations between the change from baseline to 6 months between PDQ-39 and UPDRS-IV, NMSS, BAI, BDI-II, AS, and PFS-16 scores, and Norris/Bond-Lader alertness/sedation factor. Caregiver anxiety also improved (Goldberg anxiety scale, -1.1 ± 1.0; P = 0.0234), but the clinical relevance of this finding is questionable. The serious adverse events reported were similar to those previously described for LCIG. In patients with APD, LCIG improves QoL, motor symptoms and NMS, emotional well-being, and satisfaction with the treatment. Improvement in patient QoL is associated with improvements in motor complications, NMS, anxiety, depression, apathy and fatigue. Improvements in patients' QoL does not correspond with improvements in caregivers' QoL or burden.

About half of all extant vertebrates are teleost fishes. Although our knowledge about anatomy and function of their olfactory systems still lags behind that of mammals, recent advances in cellular and molecular biology have provided us with a wealth of novel information about the sense of smell in this important animal group. Its paired olfactory organs contain up to five types of olfactory receptor neurons expressing OR, TAAR, VR1- and VR2-class odorant receptors associated with individual transduction machineries. The different types of receptor neurons are preferentially tuned towards particular classes of odorants, that are associated with specific behaviors, such as feeding, mating or migration. We discuss the connections of the receptor neurons in the olfactory bulb, the differences in bulbar circuitry compared to mammals, and the characteristics of second order projections to telencephalic olfactory areas, considering the everted ontogeny of the teleost telencephalon. The review concludes with a brief overview of current theories about odor coding and the prominent neural oscillations observed in the teleost olfactory system.

Spiders of the family Sicariidae pose a serious threat to affected populations, and Loxosceles laeta (Nicolet) is considered the most venomous species. Development of nontoxic olfaction-based spider repellents or traps is hindered by a current lack of knowledge regarding olfactory system function in arachnids. In the present study, general plant odorants and conspecific odors were tested for behavioral responses in L. laeta. Although general odorants triggered neither attraction nor aversion, conspecific odor of the opposite sex caused aversion in females, and attraction in males. These results support the presence of a specific olfactory system for the detection of conspecifics in L. laeta, but suggest the absence of a broadly tuned system for general odorant detection in this species.

Sampling of agricultural soils from the Mexican northeastern region was performed to detect Trichoderma spp., genetically characterize it, and assess its potential use as a biologic control agent against Macrophomina phaseolina. M. phaseolina is a phytopathogen that attacks over 500 species of cultivated plants and causes heavy losses in the regional sorghum crop. Sampling was performed immediately after sorghum or corn harvest in an area that was approximately 170 km from the Mexico-USA border. Sixteen isolates were obtained in total. Using colony morphology and sequencing the internal transcribed spacers (ITS) 1 and 4 of 18S rDNA, 14 strains were identified as Trichoderma harzianum, T. koningiopsis and T. virens. Subsequently, their antagonistic activity against M. phaseolina was evaluated in vitro, and 11 isolates showed antagonism by competition and stopped M. phaseolina growth. In 4 of these isolates, the antibiosis phenomenon was observed through the formation of an intermediate band without growth between colonies. One strain, HTE808, was identified as Trichoderma koningiopsis and grew rapidly; when it came into contact with the M. phaseolina colony, it continued to grow and sporulated until it covered the entire petri dish. Microscopic examination confirmed that it has a high level of hyperparasitism and is thus considered to have high potential for use in the control of this phytopathogen.

Calcineurin inhibitors have reduced acute rejection rates and improved short-term graft survival, but without any improvement in long-term outcomes, since calcineurin inhibitors cause nephrotoxicity and death with a functioning graft. mTOR inhibitors have antiproliferative and anti-angiogenic effects with no nephrotoxicity. These properties could improve patient and graft long-term survival rates in select transplant recipients. In addition, monotherapy always diminishes the rate of non-compliance in chronic patients. We examined the evolution of 47 low immunological risk kidney transplant recipients with mTOR inhibitor monotherapy. The mean age was 45 ± 10 years (range: 18-69 years), with 25 males y 22 females. We performed an immunological evaluation before and at 3 and 12 months after starting monotherapy by detection of donor-specific antibodies by microsphere cytometry and the determination of lymphocyte activity with production of ATP by CD4+ T-lymphocytes activated by PHA mitogen. We considered patients to be of low immunological risk when the patient had an ATP production less than 520 ng/dl and no history of acute rejection episode or donor-specific antibodies. Initially, 5 patients received immunosuppression induction without calcineurin inhibitors (mycophenolate, prednisone, mTOR inhibitors and anti-CD25), and 42 were converted to mTOR inhibitors due to secondary effects of calcineurin inhibitors or malignancies. A total of 34 recipients had received sirolimus and 13 everolimus. Eighteen out 47 patients (38.2%) received prednisone and 29 (61.7%) mycophenolate with mTor before starting monotherapy. The mean follow-up period after starting monotherapy was 46.9 months (95% CI: 38.8-55.0 months). At the end of the follow-up, 7 out of 47 recipients (11.5%) had to change immunosuppression without losing their grafts after 1 year, due to heavy proteinuria in 2 cases, pulmonary infection in 1, acute rejection in 1, hepatotoxicity in 1, vasculitis in 1 and a temporary inclusion on dialysis after acute pyelonephritis in 1. Four out of 47 patients (8.5%) lost their grafts, as a result of chronic rejection in 3 cases, and as a result of death with a functioning graft in 1. The rate of acute rejection was 2.1%, one episode, which was solved with steroid pulses and switching from mTOR inhibitors to tacrolimus and mycophenolate. No patients developed donor-specific antibodies, and all of them maintained an ATP production less than 520 ng/dl. The rates of graft and recipient survival were both 100% at 1 year, and 88.7% and 95.7% at 5 years. The percentages of patients on monotherapy were 97.9% and 70.5 % at 1 and 5 years, respectively. At the end of the follow-up, 36 out of 47 recipients remained on mTOR inhibitor monotherapy. Serum creatinine and glomerular filtration rates improved significantly, from 2.16 ± 1.05 mg/dl to 1.49 ± 0.56 mg/dl (P=.001) and from 39.23 ± 25.23 ml/min to 52.23 ± 23.20 ml/min (P=.001), respectively. Proteinuria increased but not significantly, from 306.6 ± 400 mg/24h to 418.1 ± 514.1mg/24h (P=.17). The patients treated with mTOR inhibitors received significantly more erythropoietin and angiotensin-converting enzyme inhibitors/angiotensin receptor blockers than before starting mTOR, but there was no change in the treatment with statins or hypotensive agents. Body weight and the percentage of diabetic recipients were similar during the study. No cases of non-compliance were observed during the follow-up. The present study supports the safety and efficacy of monotherapy with mTOR inhibitors in select kidney transplant recipients.

In recent years we have witnessed a growing tendency to opt for the use of dopamine agonists (DA) as treatment for Parkinson's disease (PD), with the aim of delaying as far as possible the development of fluctuations and dyskinesias. Yet, levodopa continues to be the most effective antiparkinson drug and is probably the one that improves the greatest number of symptoms of the disease. This article reports on the results of a comprehensive review of the literature dealing with the benefits and risks of levodopa treatment in patients with PD which was conducted by a group of expert neurologists and members of the Spanish Neurology Society's Movement Disorder Group. The main conclusion reached in this article is that levodopa continues to be the most effective treatment for PD. Although the risk and incidence of developing dyskinesias remains at a lower level in the group initially treated with DA, the number of patients who develop disabling dyskinesias is very low in all the studies and is similar for DA and for levodopa. Scores on the quality of life scales are also similar in the two groups, which casts some doubt on the impact that these motor complications have on the quality of life of patients with PD. In view of these findings, we should consider whether there is any real justification for depriving patients of the good control of their symptoms offered by levodopa owing to the fear of developing dyskinesias or mild motor fluctuations that are not really going to have any negative effect on their quality of life. There is also the possibility of their developing severe side effects, which are more frequent with the use of DA.

One strain, HTE808, was identified as Trichoderma koningiopsis and grew rapidly; when it came into contact with the M. phaseolina colony, it continued to grow and sporulated until it covered the entire petri dish. Microscopic examination confirmed that it has a high level of hyperparasitism and is thus considered to have high potential for use in the control of this phytopathogen.(AU)

OBJECTIVE: To determine whether menstrual tension-type headache is a real disorder. BACKGROUND: An appendix was included in the second edition of the International Classification of Headache Disorders by the International Headache Society. Diagnostic criteria are proposed in this appendix for 2 types of menstrual migraine: pure menstrual migraine without aura and menstrually related migraine without aura. References to menstrual tension-type headache do not appear in this classification. METHODS: In a neurological outpatient clinic 165 patients were identified in whom headache was related to menstruation and the criteria of the International Classification of Headache Disorders for menstrual migraine adapted to menstrual tension-type headache were applied. RESULTS: Twenty-one patients met the criteria of menstrual tension-type headache, 6 for pure menstrual tension-type headache, and 15 for menstrually related tension-type headache. CONCLUSION: Menstrual tension-type headache is a real condition that should be recognized in the International Classification of Headache Disorders.

PURPOSE: To use transcranial Doppler (TCD) sonography to determine if patients with migraine without aura have interictal hemodynamic abnormalities compared with patients who have episodic tension-type headache (TH). METHODS: Thirty-six migraine patients without aura and 51 TH patients (age range, 16-50 years) who were diagnosed according to the criteria of the International Headache Society 1988 participated in the study. Forty-four healthy volunteers, matched for age and sex, formed the control group. Time-averaged mean velocity (TAMV), pulsatility index (PI), and breath-holding index (BHI) were measured via TCD sonography in the middle cerebral artery. RESULTS: TAMV was higher in migraine without aura than in episodic TH (p = 0.034). There were no differences between groups regarding PI or BHI. CONCLUSION: Our findings support the arteriolar vasodilatation theory in migraine without aura.

La inmunosupresión basada en los inhibidores de la calcineurina (INC) ha hecho posible la reducción del rechazo agudo en el trasplante renal y el aumento de la supervivencia del injerto a corto pero no a largo plazo, siendo la muerte del receptor con injerto funcionante y la disfunción crónica del injerto sus principales causas. Los fármacos inhibidores de la m-Tor son inmunosupresores con capacidad antiproliferativa y antimigratoria, lo que les confiere un potencial papel protector en la disfunción del injerto renal, una mejora del perfil cardiovascular y una reducción en el desarrollo de neoplasias; todo ello podría, teóricamente, preservar la función renal y la vida del paciente a largo plazo en un grupo seleccionado de pacientes. Por otro lado, el uso de monoterapia en un paciente facilita su adherencia a cualquier tratamiento crónico. El objetivo del estudio es evaluar el uso de inhibidores de la m-Tor en monoterapia a largo plazo en un grupo seleccionado de receptores de trasplante renal de bajo riesgo inmunológico mediante un estudio observacional y prospectivo, desarrollado en el período 2001-2011. Los pacientes en tratamiento con inhibidores de la m-Tor junto con micofenolato mofetilo o prednisona fueron evaluados inmunológicamente de cara a ser incluidos en el protocolo de monoterapia con inhibidores de la m-Tor. Se incluyeron los pacientes considerados de bajo riesgo inmunológico: sin antecedentes de episodios previos de rechazo agudo, ausencia de anticuerpos antidonante específico y una producción de ATP inferior a 520 ng/dl. Calcineurin inhibitors have reduced acute rejection rates and improved short-term graft survival, but without any improvement in long-term outcomes, since calcineurin inhibitors cause nephrotoxicity and death with a functioning graft. mTOR inhibitors have antiproliferative and anti-angiogenic effects with no nephrotoxicity. These properties could improve patient and graft long-term survival rates in select transplant recipients. In addition, monotherapy always diminishes the rate of non-compliance in chronic patients. We examined the evolution of 47 low immunological risk kidney transplant recipients with mTOR inhibitor monotherapy. The mean age was 45±10 years (range: 18-69 years), with 25 males y 22 females. We performed an immunological evaluation before and at 3 and 12 months after starting monotherapy by detection of donor-specific antibodies by microsphere cytometry and the determination of lymphocyte activity with production of ATP by CD4+ T-lymphocytes activated by PHA mitogen. We considered patients to be of low immunological risk when the patient had an ATP production less than 520ng/dl and no history of acute rejection episode or donor-specific antibodies. Trichoderma sp es un hongo frecuentemente usado en actividades agrícolas, pues actúa como antagonista de diversas especies de hongos fitopatógenos. En este estudio se realizó el aislamiento de cuatro cepas de Trichoderma sp nativas del noreste de México, las cuales fueron identificadas a nivel molecular mediante la secuenciación del ITS 1. Además se evaluó su capacidad antagonista en contra los hongos fitopatógenos Macrophomina phaseolina y Fusarium oxysporum, que afectan severamente cultivos de sorgo, maíz y fríjol en el noreste de México. La identificación se realizó de acuerdo al grado de concordancia con secuencias reportadas y corresponden a las especies T. hammatum (HK701); T. koningiopsis (HK702); T. asperellum (HK703) y Trichoderma sp (HK704). Por otra parte, las pruebas de antagonismo muestran que los aislados HK701, HK703 y HK704 inhiben por competencia el crecimiento de M. phaseolina y F. oxysporum, mientras que HK702 tiene la capacidad para hiperparasitar dichos fitopatógenos. Finalmente, se evaluó la promoción de crecimiento de T. asperellum HK703, en maíz (Pionner 30P49®), usando para ello concentraciones de tratamiento de 1x10e2 hasta 1x10e6 esp/mL. En estos ensayos se midió la producción de biomasa. Los resultados muestran que en concentraciones intermedias se tiene el mayor incremento en altura de plantas y mayor producción de peso seco en follaje y raíz. Entre los parámetros antes mencionados existen diferencias significativas.


Trichoderma sp is a fungus often used in agricultural activities, because it acts as an antagonist of several species of plant pathogenic fungi. In this study four strains of Trichoderma sp was isolated from the northeastern Mexico, which were identified by sequencing the ITS 1. We also evaluated its ability antagonistic against phytopathogenic fungi Macrophomina phaseolina and Fusarium oxysporum this fungi are reported affecting severely maize, sorghum and beans crops in northeastern Mexico. The identification was made according to the degree of consistency with reported sequences and the data show that the isolates belong to the species T. hammatum (HK701), T. koningiopsis (HK702), T. asperellum (HK703) and Trichoderma sp (HK704). Antagonism tests showed that the isolated, HK701, HK703 and HK704 inhibited the growth by competition to M phaseolina and F. oxysporum, while the HK702 has the ability to hyperparasites these pathogens. Finally was evaluated in maize (Pioneer 30P49®) We measured the dry weight and biomass production. The results show that at intermediate concentrations have the greatest increase in plant height and dry height of root and foliage.
