Introduction Benign paroxysmal positional vertigo (BPPV) is the peripheral vestibular dysfunction that most affects people worldwide, but its etiopathogenesis is still not fully understood. Considering the etiological diversity, some studies highlight the association between BPPV and thyroid diseases. Objective To investigate the association between thyroid diseases and BPPV. Data Synthesis Systematic review and meta-analysis of epidemiological studies searched in the PubMed, Web of Science, Embase, Cochrane Library, and Scopus databases. Studies that were fully available and investigated the association between BPPV and thyroid diseases were selected. The articles that composed the meta-analysis were analyzed using the dichotomous model, the Mantel-Haenszel statistical test, odds ratio (OR), and a 95% confidence interval (CI). Of the 67 articles retrieved from the databases, 7 met the eligibility criteria of the systematic review, and 4 had data necessary to perform the meta-analysis. Qualitative analysis revealed that the studies were conducted in the European and Asian continents. The predominant methodological design was the case-control type, and thyroid dysfunctions, hypothyroidism, and Hashimoto thyroiditis occurred more frequently. The meta-analysis showed no association between hypothyroidism and BPPV; however, there was a statistically significant relationship between Hashimoto thyroiditis and BPPV. Conclusion The meta-analysis results suggest a possible association between BPPV and Hashimoto thyroiditis. Nevertheless, we emphasize the need for further studies to elucidate the evidence obtained.

This crossover study aimed to compare the anesthetic effects of buffered 2% articaine with 1:200,000 epinephrine with that of non-buffered 4% articaine with 1:200,000 epinephrine. Forty-seven volunteers were administered two doses of anesthesia in the buccal region of the second mandibular molars in two sessions using 1.8 mL of different local anesthetic solutions. The onset time and duration of pulp anesthesia, soft tissue pressure pain threshold, and the score of pain on puncture and burning during injection were evaluated. The operator, volunteers, and statistician were blinded. There were no significant differences in the parameters: onset of soft tissue anesthesia (p = 0.80), duration of soft tissue anesthesia (p = 0.10), onset of pulpal anesthesia in the second (p = 0.28) and first molars (p = 0.45), duration of pulp anesthesia of the second (p = 0.60) and first molars (p = 0.30), pain during puncture (p = 0.82) and injection (p = 0.80). No significant adverse events were observed. Buffered 2% articaine with 1:200,000 epinephrine did not differ from non-buffered 4% articaine with 1:200,000 epinephrine considering anesthetic success, safety, onset, duration of anesthesia, and pain on injection.

INTRODUCTION: Gastric cancer is the fourth leading cause of neoplastic morbidity worldwide, and its pathogenesis has been related to genetic and epigenetic alterations in cell cycle regulatory genes, such as p16. OBJECTIVES: This systematic review was developed to investigate the association of the p16 gene promoter methylation with oncogenesis and the progression of gastric carcinomas. METHODOLOGY: MEDLINE and Scopus databases were searched for relevant subsidiary studies, with the publication until July 2021 and without language restriction. The complete protocol was registered in the PROSPERO platform under the identification 308218. The Newcastle-Ottawa scale (NOS) was used to assess the methodological quality of the included manuscripts. The meta-analysis was conducted using RevMan 5.4 ® software. The random effects model was used, Odds Ratio (OR) was calculated with 95% confidence intervals (95% CI). Heterogeneity and inferential significance were measured. RESULTS: 48 articles were aggregated in the qualitative synthesis and 47 in the meta-analysis, totaling 6599 gastric specimens evaluated. Associations of p16 methylation with the following outcomes were observed: gastric oncogenesis (p < 0.00001); intestinal metaplasia (p = 0.002); poor histological differentiation (p = 0.03); local invasion (p = 0.001); lymph node dissemination (p = 0.03); more advanced TNM staging (p = 0.01); and Epstein Barr virus infection (p < 0.00001). In contrast, no association of p16 methylation was found with Lauren's histological classification (p = 0.62); distant metastasis (p = 0.71); or Helicobacter pylori infection (p = 0.79). CONCLUSIONS: the findings described provide empirical evidence for the categorization of p16 methylation as a substantial biomolecular step in gastric carcinogenesis, and reveal a crucial role of Epstein Barr virus in triggering this epigenetic alteration.

Background: Intestinal mucositis is one of the most common and important side effects of 5-fluorouracil (5-FU). Currently, there are still no specific and effective protocols for its prevention and treatment. The aim of the present study was to evaluate the effect of oral administration of Lacticaseibacillus casei (L. casei) on the progression of 5-FU-induced intestinal mucositis. Methods: L. casei (1x109 CFU/ml) or saline was orally administered to Swiss mice, beginning 15 days before intestinal mucositis induction by single intraperitoneal 5-FU administration (450 mg/kg). Body weight, number of peripheral leukocytes and fecal lactic acid bacteria were monitored. After euthanasia, on day 18, tissue samples from colon and each small intestine segment were collected for histopathology. Jejunal tissues were collected and evaluated for iNOS and TNF-alpha immunoexpression, IL-1-beta, IL-6 and TNF-alpha levels, malonaldehyde (MDA) accumulation, invertase activity and factor nuclear kappa B (NFkB-P65) gene expression, toll like receptor-4 (TLR-4), mucin-2 (MUC-2), occludin and zonula occludens-1 (ZO-1). Results: The positive impact of L. casei on 5-FU-induced leukopenia was observed, but not on 5-FU-induced weight loss in mice. L. casei reduced 5-FU-induced inflammation in the colon and small intestine (p<0.05). Decreased TNF-α, IL-1ß, IL-6 (p<0.05) and MDA (p<0.05) levels, as well as decreased iNOS and TNF-alpha protein expressions (p<0.05) were found in the jejunum from L casei group. In addition, L-casei down-regulated NFKB-P65 (p<0.05) and TLR-4 (p<0.05) gene expressions and up-regulated MUC-2 and mucosal barrier proteins occludin and ZO-1 gene expressions (p<0.05). Furthermore, greater lactic acid bacteria population (p<0.05) was found in the L. casei group when compared to control groups. Conclusion: Oral L. casei administration can protect the intestine of Swiss mice from 5-FU-induced intestinal mucositis, thus contributing to overall health.

The aim of this study was to characterize and evaluate zirconia/hydroxyapatite in a critical size calvarial defect model in rats. Zirconia/hydroxyapatite (80/20) scaffold was characterized by X-ray diffraction (XRD) and Scanning Electron Microscopy (SEM). Critical size (8 mm) calvarial defects were created in wistar rats (n=48) and divided into four groups (90 days): G0 Group: positive control; G1 Group: hydroxyapatite; G2 Group: Zirconia; G3 Group: Zirconia/hydroxyapatite (80/20). Calvaria were subjected to Micro CT, histological and immunohistochemical analyses (RANK, RANKL, OPG, osteocalcin and FGF-2). IL-1 beta, IL-10 and TNF-alpha levels were analyzed by Elisa Immunoassay. The XRD analysis confirmed the formation of a crystalline structure and SEM showed the presence of regions corresponding to Zirconia and Hydroxyapatite. The Micro CT showed increased bone volume (BV/TV) and bone mineral density (BMD) in the G3 group (P<0.05). In addition, discrete periosteal bone formation was found at the interface of the defect edge and the external surface of the scaffold in the G3 group, showing osteocytes inside and osteoblasts (P<0.05) with scarce mononuclear inflammatory cells (P<0.01) in the central region of the defect. The immunostaining was moderate for RANKL, Osteocalcin and FGF-2 in the G3 group (P<0.5), while it was intense for OPG (P<0.001). IL-1 beta levels were decreased and IL-10 levels increased (P<0.05). Zirconia/hydroxyapatite (80/20) scaffold repair in critical size calvarial defects increased bone density, osteoblast and osteoclast cell numbers, FGF-2, osteocalcin and OPG immunostaining and IL-10 levels.

Introdução: A busca pela judicialização da saúde vem crescendo exponencialmente no país e por isso uma análise de seus efeitos e resultados no sistema de saúde é de extrema importânciapara o correto planejamento em saúde pública. Objetivos: analisar o perfil dos processos de judicialização em saúde referentes às necessidades por leitos de Unidade de Tratamento Intensivo no estado de São Paulo.Métodos: Estudo transversal constituído por todos os acórdãos no Tribunal de Justiça do Estado de São Paulo, no período compreendido entre 2009 e 2019. Os dados foram analisados de forma descritiva e expressos em frequências absolutas. Resultados: Foram selecionadas 299 ações para a análise, sendo a maior demanda tendo como autor principal no âmbito privado (n=292). Houve aumento de 0,66% no número de processos de demanda de leitos de 2009 a 2019 (n=66).Conclusões: Apesar da maioria dos pedidos de processo de judicialização advirem de pacientes com origem na esfera pública de saúde, a maioria dos processos são iniciados pela iniciativa privada. Nesse contexto, o estado necessita elaborar uma maior padronização nos acórdãos de modo a viabilizar com precisão a caracterização do perfil de autoria (AU).


Introduction: The search for healthcare judicialization has been increasing exponentially nationwide. Accordingly, an analysis of its effects and results in the health system is of utmost importance for correct planning in public health.Objectives: To analyze the profile of health lawsuits related to the need for Intensive Care Unit beds in the state of São Paulo.Methods: Crosssectional study consisting of all the judgments in the Court of Justice of the State of São Paulo from 2009 to 2019. The data were analyzed descriptively and expressed as absolute frequencies.Results: Two hundred ninety-nine (299) actions were selected for analysis, with the largest lawsuit having the private sector as the main plaintiff (n=292). There was a 0.66% increase in the number of bed cases from 2009 to 2019 (n=66).Conclusions: Although most lawsuit claims come from patients arising from the public health sphere, most lawsuits are initiated by the private sector. In this context, the state needs to elaborate a greater standardization in the judgments in order to make it possible to characterize plaintiffs' profiles accurately (AU).


Introducción: La búsqueda por la judicialización de la salud viene creciendoexponencialmente en el país y, por lo tanto, un análisis de sus efectos y resultados en el sistema de salud es de suma importancia para una correcta planificación en salud pública.Objetivos: Analizar el perfil de los procesos de judicialización de la salud con relación a las necesidades de camas de la Unidad de Terapia Intensiva en el Estado de São Paulo.Métodos: Estudio transversal compuesto por todas las sentencias del Tribunal de Justicia del Estado de São Paulo, en el período comprendido entre 2009 y2019. Los datos se analizaron descriptivamente y se expresaron en frecuencias absolutas.Resultados: Se seleccionaron para el análisis un total de 299 acciones, de las cuales la mayor demanda tenía como principal demandante el ámbito privado (n=292). Huboun aumento del 0,66% en el número de demandas de camas de 2009 a 2019 (n=66).Conclusiones: Aunque la mayoría de las solicitudes de procesos de judicialización provienen de pacientes procedentes del ámbito sanitario público, la mayoría de los procedimientos son iniciados por la iniciativa privada. En este contexto, el Estado necesita desarrollar juicios más normalizados para caracterizar con precisión el perfil de autoría (AU).

Introdução:uma vez conhecidos os mecanismos de patogênese do SARS-CoV-2, vários métodos de tratamento para a COVID-19 foram desenvolvidos, dentre eles destaca-se o uso dos anticorpos monoclonais para o contexto de pacientes em estágios graves da doença. Objetivo: compreender se o uso dos anticorpos monoclonais para tratamento da COVID-19 grave interfere nos níveis séricos da angiotensina II. Metodologia:Para a realização dessa pesquisa foram selecionados através do DeCS e MeSH os descritores "COVID-19", "Angiotensin II" e "Antibodies, Monoclonal" e seus respectivos "entry terms" sugeridos pela base MeSH. Posteriormente,utilizando-se os operadores booleanos OR e AND, foi montada uma estratégia de busca, a qual foi utilizada nas bases de dados PUBMED, EMBASE, Web of Science, Cochrane Library e Scopus, sem restrição dedata de publicação ou idioma. Resultados:ao final do processo de seleção dos artigos, 29 foram selecionados para a leitura e análise completa. Nesta revisão, foram abordados diferentes tipos de anticorpos monoclonais, os quais foram oportunamente agrupados de acordo com o seu mecanismo de ação. Conclusão: foi possível concluir que das cinco classes de anticorpos monoclonais tratadas neste trabalho, três potencialmente podem causar alterações nos níveis séricos de angiotensina II (AU).


Introduction:once the mechanisms of pathogenesis of SARS-CoV-2 are known, several methods of treatment for COVID-19 have been developed, among them the use of monoclonal antibodies for the context of patients in severe stages of the disease. Purpose:to understand whether the use of monoclonal antibodies for the treatment of severe COVID-19 interferes with serum angiotensin II levels. Methodology:For this research were selected through DeCS and MeSH the descriptors "COVID-19", "Angiotensin II" and "Antibodies, Monoclonal" and their respective entry "Terms" suggested by the MeSH database. Subsequently, using the boolean operators OR and AND, a search strategy was set up, which was used in the databases PUBMED, EMBASE, Web of Science, Cochrane Library and Scopus, without restriction of publication date or language. Results:at the end of the article selection process, 29 were selected for reading and full analysis. In this review, different types of monoclonal antibodies were addressed, which were opportunely grouped according to their mechanism of action. Conclusion:it was possible to conclude that of the five classes of monoclonal antibodies treated in this study, three potentially can cause changes in serum levels of angiotensin II (AU).

Objetivo:Comprender si el uso de anticuerpos monoclonales para el tratamiento de la COVID-19 grave interfiere en los niveles séricos de angiotensina II. Metodología:Los descriptores "COVID-19", "Angiotensina II", "Anticuerpos, Monoclonales" y sus respectivos "entry terms" (términos de entrada) sugeridos por el MeSH fueron seleccionados a través de DeCS yMeSH. Posteriormente, utilizando los operadores booleanos OR y AND, se estableció una estrategia de búsqueda que se utilizó en las bases de datos PUBMED, EMBASE, Web of Science, Cochrane Library y Scopus, sin restricción de fecha de publicación ni de idioma. Resultados:Al final del proceso de selección de artículos, se seleccionaron 29 artículos para su lectura y análisis completos. En esta revisión se han abordado diferentes tipos de anticuerpos monoclonales, que se han agrupado oportunamente según su mecanismo de acción. Conclusión:Se pudo concluir que de las cinco clases de anticuerpos monoclonales tratados en este trabajo, tres pueden potencialmente causar alteraciones en los niveles séricos de angiotensina II (AU).

ABSTRACT Introduction: Many clinical failures involving the fiberglass post result from the detachment between the retainer, the root canal and the cement. Thus, the connections that cause a probability of these failures are essential for dentistry. Objective: To compare the efficiency of intracanal disinfection between alcohol and chlorhexidine in the context of minimizing fractures. Methods: The present systematic review registered in PROSPERO under the protocol CRD42021233516 based on a Medical Subject Headings strategy: "((Endodontics OR Root Canal Filling Materials OR Dental Materials) AND Chlorhexidine AND Ethanol AND Dental Bonding)" in the search engines PubMed, Scopus, Web of Science, Cochrane Library, Embase and Lilacs. Studies in Portuguese, English or Spanish were included, without restrictions regarding their methodology and year of publication. Articles that did not report an intracanal hygiene protocol before the procedure, without full publication and orthodontic research were excluded. Results: Six studies were selected according to the inclusion and exclusion criteria. In all, chlorhexidine was used for intracanal cleaning, comparing it with the other groups, and in 4 it was compared with alcohol and other groups. Discussion: The articles used different media and storage times and most used bovine teeth. There was no significant difference between any other characteristics regarding the increase in adhesiveness of the set. Conclusion: There is no difference between the use of alcohol or chlorhexidine as a disinfection method. However, a previous protocol must be established for longer treatment longevity.


RESUMO Introdução: Muitas falhas clínicas que envolvem o pino de fibra de vidro decorrem do descolamento entre o retentor, o canal radicular e o cimento. Assim, protocolos que reduzem a probabilidade dessas falhas são essenciais para a odontologia. Objetivo: Comparar a eficiência da desinfecção intracanal entre o álcool e clorexidina no contexto de minimização de fraturas. Métodos: A presente revisão sistemática registrada no PROSPERO sob o protocolo CRD42021233516 usou a estratégia baseada nos Medical Subject Headings: "((Endodontics OR Root Canal Filling Materials OR Dental Materials) AND Chlorhexidine AND Ethanol AND Dental Bonding)" nos buscadores PubMed, Scopus, Web of Science, Cochrane Library, Embase e Lilacs. Foram incluídos estudos em português, inglês ou espanhol, sem restrições quanto a sua metodologia e ano de publicação. Excluiu-se artigos que não relataram um protocolo de higiene intracanal antes do procedimento, sem publicação na íntegra e pesquisas ortodônticas. Resultados: Foram selecionados 6 estudos de acordo com os critérios de inclusão e exclusão. Em todos utilizou-se a clorexidina para limpeza intracanal, comparando com os demais grupos, e em 4 houve a comparação dela com álcool e outros agrupamentos. Discussão: Os artigos utilizaram diferentes meios e tempos de armazenamento e a maioria fez o uso de dentes bovinos. Não verificou-se uma diferença significativa entre quaisquer substâncias quanto ao aumento da adesividade do conjunto. Conclusão: Não existe diferença entre o uso do álcool ou clorexidina como método de desinfecção, entretanto, um protocolo prévio deve ser estabelecido para uma maior longevidade de tratamento.

Introdução:Os projetos de extensão universitáriaapresentam grande importância para os estudantes e para a sociedadecomo um todo, uma vez que possibilitamuma reflexão crítica nos indivíduos, a fim de integrar os conhecimentos acadêmicos à sociedade.O projeto Formando Sorrisosda Universidade Federal do Rio Grande do Norte(UFRN), promove atendimento odontológico aos estudantes da instituição que sejam caracterizados como vulneráveis socioeconomicamente; durante as atividades os pacientes são constantemente instruídos sobre cuidados orais e incentivados a levar as práticas às suas residências, configurando-se um vetor inicial para prevenção de agravos orais.Objetivo:Relatar a contribuição do projeto na formação do estudante de Odontologia.Metodologia:Trata-se de um estudo descritivo do tipo relato de experiência com base na rotina clínica dos alunos de odontologia do segundo ao décimoperíodo de um projeto de extensão daUFRN. Os dados se referem ao período de 2018 a 2019, avaliando desde a recepção dos pacientes até a finalização de seus tratamentos odontológicos.Resultados:A experiência nas diferentes atividades doprojeto possibilitaaosestudantesum olhar mais integraldo paciente. A vivência clínica desde cedo na graduação auxiliaao aluno um maior entendimento sobre a Odontologia, motivando-oem relação ao curso, facilitando o aprendizado e contribuindo para a formação profissional.Conclusões:As atividades desenvolvidas pelo projeto contribuempara a formação acadêmica do estudante de Odontologiae proporciona aos estudantes universitários atendimento odontológico de qualidade e em horários flexíveis (AU).


Introduction:University extension projects are of great importance for students and society as a whole since they allow critical reflection on individuals, to integrate academic knowledge into society. The Formando Sorrisos project of the Federal University of Rio Grande do Norte (UFRN) promotes dental care to the institution's students who are characterized as socioeconomically vulnerable; during activities, patients are constantly instructed about oral care and encouraged to take the practices to their homes, setting up an initial vector for preventing oral diseases.Objective:To report the project's contribution to the training of dentistry students.Methodology:This is a descriptive study of the type of experience report based on the clinical routine of dentistry students from the second to the tenth period of an extension project at UFRN. The data refer to the period from 2018 to 2019, evaluating from the reception of patients to the completion of their dental treatments. Results:Experience in the different activities of the project allows students to have a more comprehensive view of the patient. The clinical experience from an early stage in graduation helps students to gain a better understanding of Dentistry, motivating them concerning the course, facilitating learning, and contributing to professional training.Conclusions:The activities developed by the project contribute to the academic education of dentistry students and provide university students with quality dental care and flexible hours (AU).


Introducción: Los proyectos de extensión universitaria son de gran importancia para los estudiantes y para la sociedad en general, ya que permiten la reflexión crítica sobre las personas, para integrar el conocimiento académico en la sociedad. El proyecto Formando Sorrisos de la Universidad Federal de Río Grande del Norte(UFRN), promueve la atención dental a los estudiantes de la institución que se caracterizan por ser socioeconómicamente vulnerables; Durante las actividades, los pacientes reciben constantemente instrucciones sobre el cuidado bucal y se les alienta a llevar las prácticas a sus hogares, estableciendo un vector inicial para laprevención de enfermedades orales.Objetivo: Informar la contribución del proyecto a la formación de estudiantes de odontología. Metodología: Este es un estudio descriptivo del tipo de informe de experiencia basado en la rutina clínica de los estudiantes de odontología del segundo al décimo período de un proyecto de extensión en la UFRN. Los datos se refieren al período de 2018 a 2019, evaluando desde la recepción de pacientes hasta la finalización de sus tratamientos dentales.Resultados: La experiencia en las diferentes actividades del proyecto permite a los estudiantes tener una visión más completa del paciente. La experiencia clínica desde una etapa temprana de la graduación ayuda a los estudiantes a comprender mejor la odontología, motivándolos en relación con el curso, facilitando el aprendizaje y contribuyendo a la formación profesional.Conclusiones: Las actividades desarrolladas por el proyecto contribuyen a la educación académica de los estudiantes de odontología y brindan a los estudiantes universitarios atención dental de calidad y horarios flexibles (AU).

O estudo verificou o nível de conhecimento do corpo docente e discente de um curso de Odontologia acerca dos aspectos éticos e legais envolvidos na obtenção e manipulação de elementos dentários humanos extraídos. Foi aplicado um questionário semiestruturado aos docentes e discentes a partir do 2º período, por se constituírem os usuários de dentes humanos nas atividades de ensino pré-clínico e de pesquisa. Os dados foram analisados utilizando-se a estatística descritiva para as perguntas fechadas, além do Discurso do Sujeito Coletivo para as perguntas abertas. Participaram 51 docentes de um total de 55 e 201 discentes do total de 288, dos quais 72,55% e 72,60%, respectivamente, reconheceram o elemento dentário como um órgão. Quanto à forma de obtenção, 39,49% dos discentes relataram obter os dentes em consultórios odontológicos e 33,76% em unidades de saúde, sendo que 91,04% destes e 80,39% dos docentes desconhecem qualquer tipo de comercialização. Em relação à biossegurança, tanto os discentes (78,61%) quanto os docentes (90,20%) reconhecem o risco biológico na manipulação desses elementos e realizam algum tipo de desinfecção. Considerando-se os aspectos legais, 68,63% dos docentes e 20,10% dos discentes afirmaram ter conhecimento da existência de uma regulamentação, o que não se refletiu de forma explícita nas respostas obtidas. Do material textual gerado pelas perguntas abertas emergiram três categorias (forma de obtenção dos dentes utilizados, comercialização de dentes e risco biológico), interpretadas à luz de discursos agregados dos segmentos (docentes e discentes) entrevistados. Pode-se concluir que tanto docentes quanto discentes apresentaram conhecimento incipiente sobre os aspectos legais que envolvem a obtenção e manipulação de elementos dentários humanos extraídos (AU)


This study assessed the level of knowledge of teachers and students of a Dentistry Course about the ethical and legal aspects involved in the acquisition and handling of extracted human dental elements. We applied a semistructured questionnaire to the teachers and students from the 2nd period, since they constitute the users of human teeth in preclinical teaching and research activities. Data were analyzed using the descriptive statistics for the closed questions, in addition to the Collective Subject Speech for the open questions. We interviewed 51 teachers from the total of 55 and 201 students from the total of 288, of which 72.55% and 72.60%, respectively, recognized the dental element as an organ. As for the way to obtain the teeth, 39.49% of the students reported that they acquired teeth in dental offices and 33.76% in health units, where 91.04% of the students and 80.39% of the teachers are unaware of any commercialization. Regarding biosafety, both students (78.61%) and teachers (90.20%) recognize the biological risk in the handling of these elements and perform some type of disinfection. When considering the legal aspects, 68.63% of the teachers and 20.10% of the students stated that they were aware of the existence of a regulation, although this statement was not explicitly reflected in the obtained responses. From the textual material generated by the open questions, we raised three categories (way to obtain the used teeth, commercialization of teeth and biological risk), which were interpreted in the light of speeches added by the interviewed segments (teachers and students). We can conclude that both teachers and students have shown little knowledge about the legal aspects involved in the acquisition and handling of extracted human dental elements (AU).
