Background: Decreased walking ability in patients with peripheral arterial disease is often a clinical problem and limits the quality of life and daily activities of these subjects. physical exercise is important in this scenario, as it improves both the daily walking distance and the ability to withstand intermittent claudication related to the limitations of the peripheral disease. Objectives: Our aim was to compare the effects of two types of exercise training (aerobic training and aerobic training combined with resistance exercises) on pain-free walking distance (PFWD) and health-related quality of life (HRQoL) in a sample composed of patients with peripheral artery disease (PAD). Methods: Twenty patients with claudication symptoms were randomized to either aerobic control (AC) N= 9, or combined training (CT) N= 8, (24 sixty-minute sessions, twice a week). The total walking distance until onset of pain due to claudication was assessed using the 6-minute walk test and HRQoL was measured using the WHOQOL-bref questionnaire (general and specific domains) at baseline and after training. We used generalized estimating equations (GEE) to assess the differences between groups for the PFWD and HRQoL domains, testing the main group and time effects and their respective interaction effects. P values < 0.05 were considered statistically significant. Results: Seventeen patients (mean age 63±9 years; 53% male) completed the study. Both groups experienced improvement in claudication, as reflected by a significant increase in PFWD: AC, 149 m to 299 m (P<0.001); CT, 156 m to 253 m (P<0.001). HRQoL domains also improved similarly in both groups (physical capacity, psychological aspects, and self-reported quality of life; P=0.001, P=0.003, and P=0.011 respectively). Conclusions: Both aerobic and combined training similarly improved PFWD and HRQoL in PAD patients. There are no advantages in adding strength training to conventional aerobic training. This study does not support the conclusion that combined training is a good strategy for these patients when compared with classic training.


Contexto: A diminuição da capacidade de marcha em pacientes com doença arterial periférica é frequentemente um problema clínico e limita a qualidade de vida e as atividades diárias desses indivíduos. O exercício físico é importante nesse cenário, pois melhora tanto a distância caminhada diária quanto a capacidade de suportar a claudicação intermitente relacionada às limitações da doença periférica. Objetivos: Comparar os efeitos do treinamento aeróbico (TA) e do treinamento aeróbico combinado com exercícios de resistência (TC) na distância percorrida livre de dor (DPLD) e na qualidade de vida relacionada à saúde (QVRS) em pacientes com doença arterial periférica (DAP). Métodos: Vinte pacientes com sintomas de claudicação foram randomizados para TA ou TC. Os treinamentos foram realizados em 24 sessões, duas vezes por semana. A DPLD foi avaliada por meio do teste de caminhada de 6 minutos, e a QVRS foi medida pelo instrumento da avaliação de qualidade de vida da Organização Mundial da Saúde (WHOQOL-BREF), no início e após o treinamento. Para avaliar as diferenças entre os grupos para DPLD e os domínios da QVRS, foi utilizado o modelo de equações de estimativa generalizada, testando os efeitos principais do grupo e tempo, bem como os respectivos efeitos de interação. Valores de p < 0,05 foram considerados estatisticamente significativos. Resultados: Dezessete pacientes (idade média: 63±9 anos; 53% do sexo masculino) completaram o estudo. Ambos os grupos apresentaram melhora na claudicação, refletida por um aumento significativo na DPLD: grupo controle aeróbico - de 149 m para 299 m (P < 0,001); grupo de treinamento combinado - de 156 m para 253 m (P < 0,001). Os domínios da QVRS também melhoraram de forma semelhante em ambos os grupos (capacidade física, aspectos psicológicos e qualidade de vida autorreferida; P = 0,001, P = 0,003 e P = 0,011, respectivamente). Conclusões: Ambos os treinamentos melhoraram de forma semelhante a DPLD e a QVRS em pacientes com DAP. Não há vantagens em associar o treinamento de força ao treinamento aeróbico convencional. O estudo não permite concluir que o TC é uma boa estratégia para esses pacientes quando comparado ao treinamento clássico.

The Brazilian Society of Angiology and Vascular Surgery, through the Guidelines Project, presents new Abdominal Aortic Aneurysm Guidelines, on the subject of care for abdominal aortic aneurysm patients. Its development prioritized descriptive guidelines, using the EMBASE, LILACS, and PubMed databases. References include randomized controlled trials, systematic reviews, meta-analyses, and cohort studies. Quality of evidence was evaluated by a pair of coordinators, aided by the RoB 2 Cochrane tool and the Newcastle Ottawa Scale forms. The subjects include juxtarenal aneurysms, infected aneurysms, and new therapeutic techniques, especially endovascular procedures. The current version of the guidelines include important recommendations for the primary topics involving diagnosis, treatment, and follow-up for abdominal aortic aneurysm patients, providing an objective guide for medical practice, based on scientific evidence and widely available throughout Brazil.


A Sociedade Brasileira de Angiologia e Cirurgia Vascular, por meio do projeto Diretrizes, apresenta as novas Diretrizes de Aorta Abdominal, referentes aos cuidados de pacientes com aneurisma de aorta abdominal. Para sua elaboração, foram priorizadas diretrizes descritivas, utilizando as bases EMBASE, LILACS e PubMed. As referências incluem ensaios clínicos randomizados, revisões sistemáticas, metanálises e estudos de coorte. A qualidade das evidências foi examinada por uma dupla de coordenadores, com auxílio da ferramenta RoB 2 da Colaboração Cochrane e dos formulários da Newcastle Ottawa Scale. Aneurismas justarrenais, infectados e novas técnicas terapêuticas, principalmente no âmbito endovascular, estão entre os temas estudados. A versão atual das Diretrizes apresenta importantes recomendações para os principais itens que envolvem o diagnóstico, tratamento e acompanhamento de pacientes com aneurisma de aorta abdominal, oferecendo um guia objetivo para prática médica, construído a partir de evidências científicas e amplamente acessível em todo o território nacional.

BACKGROUND: Diagnosis of the etiologic agent of endoprosthesis infections is essential to enable treatment, since these infections constitute important complications of endovascular procedures. Sonication of explanted tissue and materials is a technique that can be used to facilitate detection of biofilm-producing bacteria. OBJECTIVES: To evaluate infection of pigs' aortas after implantation of nitinol stents coated with polytetrafluoroethylene (ePTFE) or Dacron, previously infected with biofilm-producing Staphylococcus epidermidis. Intimal thickening and the inflammatory response in the aortic wall were also evaluated. METHODS: 11 ePTFE-coated nitinol stents and 10 Dacron stents infected with S. epidermidis strains were implanted in the infrarenal aorta of 21 8-week-old pigs. After 2 weeks, the aorta containing the stents was removed. A vortex mixer and ultrasound were used to homogenize the samples and remove the biofilm. Subsequently, the number of colony-forming units was counted. RESULTS: There were no significant differences between the two groups in terms of the number of colony-forming units or of inflammation in the arterial wall. With the exception of one specimen from the Dacron group, all aortic stent cultures were positive for S. epidermidis. CONCLUSIONS: There were no significant differences in the inflammatory response or infection rate between ePTFE and Dacron-coated stents actively infected with biofilm-producing S. epidermidis. Intimal thickening and the inflammatory response to infection of endoprostheses were similar. These results suggest that the two most widely used stent lining materials have a similar infection rate.


CONTEXTO: O diagnóstico do agente etiológico é essencial para o tratamento das infecções de endoprótese, pois representam uma importante complicação do tratamento endovascular. A sonificação dos tecidos pode ser uma técnica usada para auxiliar na detecção de bactérias produtoras de biofilme. OBJETIVOS: Avaliar a infecção da aorta dos porcos após o implante de stents de nitinol revestidos com politetrafluoretileno (ePTFE) ou Dacron, infectados com Staphylococcus epidermidis, produtor de biofilme. O espessamento intimal e a resposta inflamatória na parede aórtica também foram avaliados. MÉTODOS: Onze stents de nitinol revestidos com ePTFE e 10 stents de Dacron infectados com cepas de S. epidermidis foram implantados na aorta infrarrenal de 21 porcos com 8 semanas de idade. Após duas semanas, a aorta contendo os stents foi removida. Um misturador de vórtice e ultrassom foram utilizados para homogeneizar as amostras e remover o biofilme. Posteriormente, o número de unidades formadoras de colônias foi contado. RESULTADOS: Não houve diferenças significativas no número de unidades formadoras de colônias ou inflamação na parede arterial entre os dois grupos. Todas as culturas de stent aórtico foram positivas para S. epidermidis, exceto uma no grupo Dacron. CONCLUSÕES: Não houve diferenças significativas na resposta inflamatória ou na taxa de infecção entre os stents revestidos de ePTFE e Dacron, infectados ativamente pelo S. epidermidis produtor de biofilme. O espessamento intimal e a resposta inflamatória à infecção das endopróteses foram semelhantes. Esses resultados sugerem que os dois materiais de revestimento de stent mais amplamente utilizados têm uma taxa de infecção semelhante.

BACKGROUND: Aneurysm repair using endovascular techniques has grown in importance as materials have improved. Studies of endovascular prostheses require experimental models that reproduce anatomic and pathophysiological characteristics of human aneurysms. OBJECTIVES: To describe a porcine model of abdominal aortic aneurysm. METHODS: This prospective cohort study used eleven Large White female pigs with a mean age of 12 weeks in two study phases. In phase I, the aneurysm was produced with a bovine pericardium patch by retroperitoneal surgery conducted under general anesthesia. In phase II, 15 days later, the animals underwent arteriography and were then euthanized before specimens were removed for histological analysis. RESULTS: Formation of parietal thrombus was observed in all animals. Microscopic analysis showed calcifications around thrombus in 82% of the animals. There was lymphoplasmacytic infiltration in the graft and adjacent area, with fibrosis in nine animals. Three pigs had substantial myointimal thickening, and eight had microcalcifications. Mortality was zero, and there were no ruptures, ischemia, or surgery site infections. CONCLUSIONS: This is a unique model, using inexpensive, biocompatible material. Bovine pericardium is easy for the surgeon to handle and has very similar characteristics to autologous tissue in terms of integration with the cell wall.


CONTEXTO: A correção de um aneurisma por meio de técnicas endovasculares tem ganhado importância com o aprimoramento dos materiais. O estudo das próteses endovasculares requer modelos experimentais que reproduzam as características anatômicas e fisiopatológicas dos aneurismas humanos. OBJETIVOS: Descrever um modelo suíno de aneurisma de aorta abdominal. MÉTODOS: Este estudo de coorte prospectivo utilizou 11 porcas da raça Large White com idade média de 12 semanas nas duas fases do estudo. Na fase I, o aneurisma foi produzido com um saco de pericárdio bovino através de cirurgia por via retroperitoneal sob anestesia geral; na fase II, 15 dias após a fase I, os animais foram submetidos a arteriografia e, posteriormente, a eutanásia, sendo os espécimes retirados para análise histológica. RESULTADOS: A formação de trombo parietal foi observada em todos os animais. A análise microscópica mostrou calcificações ao redor do trombo em 82% dos animais. Havia infiltrado linfoplasmocitário no enxerto e na área adjacente, com fibrose em nove animais. Três porcos tinham espessamento miointimal substancial, e oito tinham microcalcificações. A mortalidade foi zero, e não houve rupturas, isquemia ou infecção no local da cirurgia. CONCLUSÕES: É um modelo único, com material biocompatível e de baixo custo. O pericárdio bovino é de fácil manuseio pelo cirurgião e possui características muito semelhantes ao tecido autólogo em sua integração com a parede celular.

BACKGROUND: Physical training is a well-established strategy for rehabilitation of the functional capacity of individuals with chronic peripheral arterial occlusive disease (PAOD). However, some individuals quit training after participating in a physical training program, undergoing detraining. There is scant literature on the effects of physical detraining in individuals with PAOD and it is therefore important to investigate the effects of this phenomenon. OBJECTIVES: The objective of this article was to evaluate the effects of physical detraining on functional capacity in individuals with PAOD. METHODS: Cross-sectional study with 22 individuals. Participants were divided into two groups: a detraining group (DG) and a control group (CG). The distance covered in the 6-minute walk test (6MWTD) and the pain-free walking distance (PFWD) were evaluated. The PFWD is the distance covered until claudication begins, i.e., the distance covered without pain. RESULTS: Mean age was 66 ± 8 in the DG and 67 ± 7 in the CG. There were no differences between the groups in either the 6MWTD or the PFWD (p = 0.428; p = 0.537, respectively). CONCLUSIONS: The present pilot study allows us to conclude that the functional capacity of individuals with PAOD who participated in a physical training program and subsequently underwent detraining was not superior in relation to individuals who did not participate in a physical training program. The results of the present study serve to encourage maintenance of physical exercise, since physical training is no longer effective if detraining occurs.

Physical training can increase peak oxygen uptake (VO2peak) in people who have suffered acute myocardial infarction (AMI). However, there is still a gap in the literature in relation to the effectiveness of different types of interventions. Therefore, the aim of the present study was to evaluate the effects of different physical training modalities on VO2peak in post-AMI patients. The following databases were used: PubMed (MEDLINE), Cochrane Library, Scopus, and Pedro. Studies that evaluated aerobic exercise, strength exercise, or combined exercise were included. Six studies met eligibility criteria. Aerobic exercise increased VO2peak by 6.07 ml.kg-1.min-1 when compared to the control group (CG) (p = 0.013). The comparison between combined exercise and control group detected a difference of 1.84 ml.kg-1.min-1, but this was not significant (p = 0.312). We therefore conclude that aerobic exercise is the only modality that is effective for increasing VO2peak compared to a control group.

The natural histories of intramural hematoma (IMH) and penetrating atherosclerotic ulcer (PAU) are highly variable as they may progress to aneurysm formation, rupture, or dissection, or even resolve, in the specific case of IMH. Imaging plays an increasingly important role in clinical and surgical management of IMH and PAU. In contrast to ulcer-like projections, images of intramural blood pools have not been widely reported in CT studies of patients with IMH. Understanding the imaging characteristics and the natural course of each of these entities would help clinicians and surgeons to identify patients at greatest risk for bad prognosis and may improve outcomes. This paper discusses the pathophysiology of these entities, the controversies regarding their natural history, and the prognostic factors that should be identified in CT scans.

BACKGROUND: Anti-angiogenic regulators may have therapeutic implications for onset and progression of atherosclerosis. OBJECTIVES: To demonstrate histological changes secondary to the use of bevacizumab in the aorta of pigs after interruption of flow in the vasa vasorum. METHODS: Twelve pigs were divided into two groups. The intercostal arteries of the descending aorta were dissected and ligated and wrapped with a polyvinyl chloride membrane. The treatment group received an intravenous dose of bevacizumab. After 15 days, the animals were euthanized and the aorta removed. Histological slides were prepared for control and treatment groups and for non-manipulated areas and analyzed for degree of angiogenesis, injury, inflammation, and intimal thickening. Data were expressed as mean (SD) of scores and groups were compared using the Mann-Whitney test. The Poisson distribution was used to calculate 95% confidence intervals for mean scores, in order to determine effect statistics. RESULTS: Bevacizumab had adverse effects on all treated pigs. The analysis using a Scale of Magnitudes for Effect Statistics showed a trend toward a decrease in angiogenesis [0.58 (1.79/-0.63)] and injury [0.55 (1.76/-0.66)] and an increase in inflammation [0.67 (1.89/-0.55)] with threshold moderate effects. There was no difference in intimal thickening [0 (1.19/-1.19)]. CONCLUSIONS: The medication exhibited a trend toward reduced angiogenesis and injury, but no reduction in the inflammatory process or intimal thickening of the aortic wall. These findings are in disagreement with studies that correlate neovascularization with increased migration of inflammatory cells. Bevacizumab exhibited toxicity in the porcine model.


CONTEXTO: Agentes antiangiogênicos podem ter implicações terapêuticas na progressão e manifestação da aterosclerose. OBJETIVOS: Demonstrar a alteração histológica secundária ao uso de bevacizumabe na aorta descendente de suínos submetida à interrupção dos vasa vasorum. MÉTODOS: Em doze suínos, divididos em dois grupos, foi realizada dissecção da aorta torácica, além de ligadura das artérias intercostais e proteção com polivinil. O grupo tratamento recebeu dose endovenosa de bevacizumabe. Após 15 dias, os animais foram sacrificados para retirada da artéria e preparo das lâminas histológicas dos grupos tratamento, controle e áreas não manipuladas para análise quanto aos graus de angiogênese, injúria, inflamação e espessamento intimal. A análise estatística foi conduzida através da média e do desvio padrão dos escores. As comparações entre os grupos foram realizadas pelo teste de Mann-Whitney. A distribuição de Poisson calculou os intervalos de confiança de 95% para as médias, a fim de determinar o efeito estatístico. RESULTADOS: O bevacizumabe causou efeitos adversos em todos os suínos tratados. As variáveis analisadas através da Escala de Magnitude para Efeito Estatístico demonstraram tendência de redução da angiogênese [0,58 (1,79/-0,63)] e da injúria [0,55 (1,76/-0,66)] e aumento da inflamação [0,67 (1,89/-0,55)] no limite do moderado. Não ocorreu diferença no espessamento intimal [0 (1,19/-1,19)]. CONCLUSÕES: A medicação utilizada mostrou tendência de redução da angiogênese e da injúria, mas não reduziu o processo inflamatório ou o espessamento intimal da parede arterial. Esses achados contrariam estudos que correlacionam a neovascularização com o aumento da migração de células inflamatórias. O bevacizumabe mostrou toxicidade no modelo suíno.

BACKGROUND: Physical exercise (PE) may improve plasma concentration of interleukin- 6 (IL-6), tumor necrosis factor-alpha (TNF-alpha), and adiponectin (adpN) in heart transplant (HT) patients. However, no consistent data is available on this population. AIM: Thus, we aimed to conduct a systematic review and meta-analysis on the effects of PE over these pro- and anti-inflammatory biomarkers in HT patients. METHODS: Following the guidelines established by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 statement, we conducted a systematic literature search in the PubMed, Cochrane, and Scopus databases. Outcomes included IL-6, TNF-alpha, and adpN. Effect size (ES) was calculated using the standardized mean difference with a 95% confidence interval (CI). RESULTS: The PE group (aerobic modality) was associated with reduced IL-6 compared to the control group (ES: -0.53; 95% CI: -0.99 to -0.06 pg/mL; P = 0.026). However, the PE group did not show a significant effect on TNF-alpha and adpN levels (ES: -0.33; 95% CI: -0.79 to 0.13; P = 0.16 and ES: -0.20; 95% CI: -0.70 to 0.30 pg/mL; P = 0.444, respectively). CONCLUSION: PE is associated with IL-6 reductions, although TNF alpha and adpN did not change after this intervention in HT patients. Therefore, PE is an effective intervention to downregulate IL-6 in post-HT patients.

PURPOSE: To describe an unpublished experimental model of descending thoracic aortic aneurysm in pigs. METHODS: Ten Landrace female pigs aged 10 to 12 weeks old and with initial weights from 17 to 25 kg were anesthetized and their descending thoracic aortas exposed by fifth intercostal space left thoracotomy. The thoracic aorta was isolated. A 2-cm wide × 2-cm long patch of ready-made bovine pericardium was sewn onto the left anterolateral side of the aorta. After three weeks' follow-up, a control aortography was taken, and the animals were euthanized. The segment of thoracic aorta containing the aneurysm and the adherent tissues were explanted en bloc. The specimens were stained for histological examination. RESULTS: One hundred percent of the animals survived the procedure, and after sacrifice a patent aneurysm was observed in all of them. There were no defects on the suture lines. Weight gain during follow-up was normal. All specimens exhibited intense adventitial reaction with myofibroblasts. There were no complications related to the thoracotomy. CONCLUSIONS: The descending thoracic aortic aneurysms induced experimentally appear to be stable, were of easy execution, with null mortality and no influence on the animals' normal development. Furthermore, they have similar characteristics to those observed in human degenerative aneurysms.

PURPOSE: To compare aortic intimal thickening of normal and hyperhomocysteinemic pigs (induced with a methionine-rich diet) following placement of a self-expanding nitinol stent. METHODS: Eighteen Macau pigs were used. They were older than eight weeks in age and had an average weight of 30 kg. Pigs were randomly divided into two groups. The first, Group C (control), was fed a regular diet, and the second group, Group M, was fed a methionine-rich diet for 30 days to induce hyperhomocysteinemia. The self-expandable nitinol stents were 25mm in length and 8 mm in diameter after expansion. Blood samples were collected to measure total cholesterol, triglycerides, HDL and homocysteine concentrations. All animals were subjected to angiography. Thirty days after the procedure, the animals were sacrificed, and the abdominal aorta was removed for histological and digital morphometry analysis. RESULTS: Under microscopic evaluation, the intima was significantly thicker in Group C than in Group M. When groups were compared by digital morphometric analysis, intimal thickening of the vessel wall was higher in Group C than in Group M. There was no significant change in total cholesterol, triglycerides or HDL concentrations in either group. In group C the levels of plasma homocysteine ranged from 14,40 to 16,73 micromol/l; in Group M, plasma homocysteine levels ranged from 17.47 to 59.80 micromol/l after 30 days of a methionine-rich diet. CONCLUSION: Compared to normal pigs, less intimal hyperplasia was observed in the abdominal aortas of hyperhomocysteinemic pigs thirty days after the insertion of a self-expandable nitinol stent.

OBJECTIVE: To evaluate intimal thickening secondary to stent placement or to insertion of the delivery system without stent placement in pig carotid arteries. METHODS: Stents were placed in the right common carotid arteries of 7 pigs, and 7 other control pigs underwent only insertion of the delivery system without stent placement. Uninjured contralateral common carotid arteries of the two groups were also used as controls. Samples of arterial tissue, obtained from the area adjacent to the distal segment of the stent four weeks after placement, underwent morphometric analysis. Morphometric data were compared with findings for arterial samples from injured arteries of the control group and uninjured contralateral arteries of the two groups. The unpaired Mann-Whitney U test and the Wilcoxon t test for nonparametric samples were used for statistical analysis. RESULTS: Greater intimal thickening was found in the group in which stents were placed (p=0.008). Changes in luminal area and media layer were not significantly different between the two groups. The comparison of contralateral carotid arteries of the two groups revealed significant changes in intimal and luminal areas. No statistically significant changes were found in medial layer area. CONCLUSIONS: All arteries that underwent stent placement showed intimal thickening without changes in the tunica media at four weeks. Dissection and insertion of the delivery system without stent placement was associated with a lesser degree of intimal thickening.

Abstract Background Decreased walking ability in patients with peripheral arterial disease is often a clinical problem and limits the quality of life and daily activities of these subjects. physical exercise is important in this scenario, as it improves both the daily walking distance and the ability to withstand intermittent claudication related to the limitations of the peripheral disease. Objectives Our aim was to compare the effects of two types of exercise training (aerobic training and aerobic training combined with resistance exercises) on pain-free walking distance (PFWD) and health-related quality of life (HRQoL) in a sample composed of patients with peripheral artery disease (PAD). Methods Twenty patients with claudication symptoms were randomized to either aerobic control (AC) N= 9, or combined training (CT) N= 8, (24 sixty-minute sessions, twice a week). The total walking distance until onset of pain due to claudication was assessed using the 6-minute walk test and HRQoL was measured using the WHOQOL-bref questionnaire (general and specific domains) at baseline and after training. We used generalized estimating equations (GEE) to assess the differences between groups for the PFWD and HRQoL domains, testing the main group and time effects and their respective interaction effects. P values < 0.05 were considered statistically significant. Results Seventeen patients (mean age 63±9 years; 53% male) completed the study. Both groups experienced improvement in claudication, as reflected by a significant increase in PFWD: AC, 149 m to 299 m (P<0.001); CT, 156 m to 253 m (P<0.001). HRQoL domains also improved similarly in both groups (physical capacity, psychological aspects, and self-reported quality of life; P=0.001, P=0.003, and P=0.011 respectively). Conclusions Both aerobic and combined training similarly improved PFWD and HRQoL in PAD patients. There are no advantages in adding strength training to conventional aerobic training. This study does not support the conclusion that combined training is a good strategy for these patients when compared with classic training.


Resumo Contexto A diminuição da capacidade de marcha em pacientes com doença arterial periférica é frequentemente um problema clínico e limita a qualidade de vida e as atividades diárias desses indivíduos. O exercício físico é importante nesse cenário, pois melhora tanto a distância caminhada diária quanto a capacidade de suportar a claudicação intermitente relacionada às limitações da doença periférica. Objetivos Comparar os efeitos do treinamento aeróbico (TA) e do treinamento aeróbico combinado com exercícios de resistência (TC) na distância percorrida livre de dor (DPLD) e na qualidade de vida relacionada à saúde (QVRS) em pacientes com doença arterial periférica (DAP). Métodos Vinte pacientes com sintomas de claudicação foram randomizados para TA ou TC. Os treinamentos foram realizados em 24 sessões, duas vezes por semana. A DPLD foi avaliada por meio do teste de caminhada de 6 minutos, e a QVRS foi medida pelo instrumento da avaliação de qualidade de vida da Organização Mundial da Saúde (WHOQOL-BREF), no início e após o treinamento. Para avaliar as diferenças entre os grupos para DPLD e os domínios da QVRS, foi utilizado o modelo de equações de estimativa generalizada, testando os efeitos principais do grupo e tempo, bem como os respectivos efeitos de interação. Valores de p < 0,05 foram considerados estatisticamente significativos. Resultados Dezessete pacientes (idade média: 63±9 anos; 53% do sexo masculino) completaram o estudo. Ambos os grupos apresentaram melhora na claudicação, refletida por um aumento significativo na DPLD: grupo controle aeróbico - de 149 m para 299 m (P < 0,001); grupo de treinamento combinado - de 156 m para 253 m (P < 0,001). Os domínios da QVRS também melhoraram de forma semelhante em ambos os grupos (capacidade física, aspectos psicológicos e qualidade de vida autorreferida; P = 0,001, P = 0,003 e P = 0,011, respectivamente). Conclusões Ambos os treinamentos melhoraram de forma semelhante a DPLD e a QVRS em pacientes com DAP. Não há vantagens em associar o treinamento de força ao treinamento aeróbico convencional. O estudo não permite concluir que o TC é uma boa estratégia para esses pacientes quando comparado ao treinamento clássico.

Abstract The Brazilian Society of Angiology and Vascular Surgery, through the Guidelines Project, presents new Abdominal Aortic Aneurysm Guidelines, on the subject of care for abdominal aortic aneurysm patients. Its development prioritized descriptive guidelines, using the EMBASE, LILACS, and PubMed databases. References include randomized controlled trials, systematic reviews, meta-analyses, and cohort studies. Quality of evidence was evaluated by a pair of coordinators, aided by the RoB 2 Cochrane tool and the Newcastle Ottawa Scale forms. The subjects include juxtarenal aneurysms, infected aneurysms, and new therapeutic techniques, especially endovascular procedures. The current version of the guidelines include important recommendations for the primary topics involving diagnosis, treatment, and follow-up for abdominal aortic aneurysm patients, providing an objective guide for medical practice, based on scientific evidence and widely available throughout Brazil.


Resumo A Sociedade Brasileira de Angiologia e Cirurgia Vascular, por meio do projeto Diretrizes, apresenta as novas Diretrizes de Aorta Abdominal, referentes aos cuidados de pacientes com aneurisma de aorta abdominal. Para sua elaboração, foram priorizadas diretrizes descritivas, utilizando as bases EMBASE, LILACS e PubMed. As referências incluem ensaios clínicos randomizados, revisões sistemáticas, metanálises e estudos de coorte. A qualidade das evidências foi examinada por uma dupla de coordenadores, com auxílio da ferramenta RoB 2 da Colaboração Cochrane e dos formulários da Newcastle Ottawa Scale. Aneurismas justarrenais, infectados e novas técnicas terapêuticas, principalmente no âmbito endovascular, estão entre os temas estudados. A versão atual das Diretrizes apresenta importantes recomendações para os principais itens que envolvem o diagnóstico, tratamento e acompanhamento de pacientes com aneurisma de aorta abdominal, oferecendo um guia objetivo para prática médica, construído a partir de evidências científicas e amplamente acessível em todo o território nacional.

PURPOSE: To compare through digital morphometry, the intimal thickening of the common ilíac arteries (CIA) in pigs, submitted to angioplasty and angioplasty followed by stent implantation. METHODS: Angioplasty was performed in 10 pigs in both CIA, followed by a stent implantation in the left CIA. After four weeks, the aorto-iliac segment was dissected. Histologic slices where divided in three groups: left CIA in the area of the proximal (group 1) and distal (group 2) implantation site of the stent and the region of angioplasty in the right CIA (group 3). The histological slices were digitalized to be analyzed by morphometry with calculation of the luminal, intimal and media layers areas. Descriptive statistics analysis was performed through average and standard deviation of areas on each group, using ANOVA, with LSD Post-Hoc test (p<0.05). RESULTS: A significant statistic difference was observed when the median area of intimal layer of the groups 1 and 2 where compared with group 3 and in the media layer area of the groups 1 and 2 when compared with group 3. No difference was observed in luminal areas among the three groups. CONCLUSION: Angioplasty followed by stent insertion produces an intimal thickening larger than that observed after simple angioplasty. However, the area of the media layer is smaller in the "angioplasty plus stent" group and there is no significant change in the luminal area among the three groups.

OBJECTIVE: to determine the blood flow pattern changes after endovascular treatment of saccular abdominal aortic aneurysm with triple stent. METHODS: we conducted a hemodynamic study of seven Landrace and Large White pigs with saccular aneurysms of the infrarenal abdominal aorta artificially produced according to the technique described. The animals were subjected to triple stenting for endovascular aneurysm. We evaluated the pattern of blood flow by duplex scan before and after stent implantation. We used the non-paired Mann-Whitney test for statistical analysis. RESULTS: there was a significant decrease in the average systolic velocity, from 127.4cm/s in the pre-stent period to 69.81cm/s in the post-stent phase. There was also change in the flow pattern from turbulent in the aneurysmal sac to laminate intra-stent. CONCLUSION: there were changes in the blood flow pattern of saccular abdominal aortic aneurysm after endovascular treatment with triple stent. OBJETIVO: determinar as alterações do padrão do fluxo sanguíneo após tratamento endovascular do aneurisma sacular de aorta abdominal com triplo stent. MÉTODOS: estudo hemodinâmico de sete suínos das raças Landrace e Large White portadores de aneurismas saculares de aorta abdominal infrarrenal artificialmente produzidos segundo técnica descrita. Os animais foram submetidos a implante de triplo stent para correção endovascular do aneurisma e reavaliados por duplex scan quanto ao padrão do fluxo sanguíneo antes e após o implante dos stents. A análise estatística foi realizada com o teste Mann-Whitney não pareado. RESULTADOS: verificou-se uma queda significativa da velocidade sistólica média de 127,4cm/s na fase pré-stent para 69,81cm/s na fase pós-stent. Houve ainda mudança no padrão do fluxo de turbilhonar no saco aneurismático para laminar intrastent. CONCLUSÃO: o estudo demonstrou alterações do padrão do fluxo sanguíneo do aneurisma sacular de aorta abdominal após tratamento endovascular com triplo stent.

OBJECTIVE: To consider modifications in an experimental model of saccular aortic aneurysm, aiming at better reproducibility, to be used in the development of vascular prostheses. METHODS: Experimental study in two phases, developed in the Center of Experimental Surgery and Bioterium (CCEB) of the University of Health Sciences of Alagoas (UNCISAL), with 11 hybrid swine, female, mean weight of 20 ± 5 kg, according to modifications in the Perini technique was performed. In the first phase, the aneurysm was confectioned with bovine pericardial patch. In the second phase, fifteen days later, the patency of the aneurysms was confirmed by Doppler ultrasonography. The described variables were aortic and aneurysm sac patency, incidence of rupture, morbidity and mortality. The statistical analysis program used was STATA v.8. RESULTS: All animals survived to the procedures. Surgical mean time was 73 minutes. Aneurysm rupture, proximal or distal aortic thrombosis, visceral or legs ischemia weren't observed. Parietal thrombus formation was observed in all of the aneurysms, two of which (18%; IC 95% = 3.98 - 48.84) were occluded and nine (82%; IC 95% = 51.15 - 96.01) were patent. CONCLUSION: In this series, the modifications carried out in the technique related to the surgical approach, race, anesthesia, and imaging exams reproduced the experimental model, reducing its costs, without hindering the analysis of the variables. The satisfactory patency ratio allows the method to be used in experimental models for the development of vascular prostheses.

Resumo Contexto O treinamento físico é uma estratégia bem estabelecida para a reabilitação da capacidade funcional de indivíduos com doença arterial obstrutiva crônica periférica (DAOP). No entanto, após um programa de treinamento físico, alguns indivíduos podem descontinuá-lo, causando destreinamento. A literatura é escassa sobre os efeitos do destreinamento físico em indivíduos com DAOP; portanto, torna-se importante investigar os efeitos nessa temática. Objetivos Avaliar os efeitos do destreinamento físico sobre a capacidade funcional em indivíduos com DAOP. Métodos Estudo transversal com 22 indivíduos. Os participantes foram divididos em dois grupos: grupo destreinamento (GD), grupo controle (GC). Foram avaliadas a distância percorrida no teste de caminhada de 6 minutos (DTC6M) e a distância livre de dor claudicante (DLDC), a qual é referida pela distância percorrida até o início da claudicação, ou seja, sem dor. Resultados A média da idade foi de 66±8 para o GD e de 67±7 para o GC. Tanto a DTC6M como a DLDC não apresentaram diferenças entre os grupos (p = 0,428 e p = 0,537, respectivamente). Conclusões O presente estudo piloto permite concluir que indivíduos com DAOP que participaram de um programa de treinamento físico e posterior destreinamento não tiveram efeitos superiores na sua capacidade funcional em relação a indivíduos que não participaram de programa de treinamento físico. O resultado do presente estudo serve para incentivar a manutenção da prática de exercício físico, visto que o treinamento físico deixa de ser efetivo se ocorrer destreinamento.


Abstract Background Physical training is a well-established strategy for rehabilitation of the functional capacity of individuals with chronic peripheral arterial occlusive disease (PAOD). However, some individuals quit training after participating in a physical training program, undergoing detraining. There is scant literature on the effects of physical detraining in individuals with PAOD and it is therefore important to investigate the effects of this phenomenon. Objectives The objective of this article was to evaluate the effects of physical detraining on functional capacity in individuals with PAOD. Methods Cross-sectional study with 22 individuals. Participants were divided into two groups: a detraining group (DG) and a control group (CG). The distance covered in the 6-minute walk test (6MWTD) and the pain-free walking distance (PFWD) were evaluated. The PFWD is the distance covered until claudication begins, i.e., the distance covered without pain. Results Mean age was 66 ± 8 in the DG and 67 ± 7 in the CG. There were no differences between the groups in either the 6MWTD or the PFWD (p = 0.428; p = 0.537, respectively). Conclusions The present pilot study allows us to conclude that the functional capacity of individuals with PAOD who participated in a physical training program and subsequently underwent detraining was not superior in relation to individuals who did not participate in a physical training program. The results of the present study serve to encourage maintenance of physical exercise, since physical training is no longer effective if detraining occurs.

Abstract Background Diagnosis of the etiologic agent of endoprosthesis infections is essential to enable treatment, since these infections constitute important complications of endovascular procedures. Sonication of explanted tissue and materials is a technique that can be used to facilitate detection of biofilm-producing bacteria. Objectives To evaluate infection of pigs' aortas after implantation of nitinol stents coated with polytetrafluoroethylene (ePTFE) or Dacron, previously infected with biofilm-producing Staphylococcus epidermidis. Intimal thickening and the inflammatory response in the aortic wall were also evaluated. Methods 11 ePTFE-coated nitinol stents and 10 Dacron stents infected with S. epidermidis strains were implanted in the infrarenal aorta of 21 8-week-old pigs. After 2 weeks, the aorta containing the stents was removed. A vortex mixer and ultrasound were used to homogenize the samples and remove the biofilm. Subsequently, the number of colony-forming units was counted. Results There were no significant differences between the two groups in terms of the number of colony-forming units or of inflammation in the arterial wall. With the exception of one specimen from the Dacron group, all aortic stent cultures were positive for S. epidermidis. Conclusions There were no significant differences in the inflammatory response or infection rate between ePTFE and Dacron-coated stents actively infected with biofilm-producing S. epidermidis. Intimal thickening and the inflammatory response to infection of endoprostheses were similar. These results suggest that the two most widely used stent lining materials have a similar infection rate.


Resumo Contexto O diagnóstico do agente etiológico é essencial para o tratamento das infecções de endoprótese, pois representam uma importante complicação do tratamento endovascular. A sonificação dos tecidos pode ser uma técnica usada para auxiliar na detecção de bactérias produtoras de biofilme. Objetivos Avaliar a infecção da aorta dos porcos após o implante de stents de nitinol revestidos com politetrafluoretileno (ePTFE) ou Dacron, infectados com Staphylococcus epidermidis, produtor de biofilme. O espessamento intimal e a resposta inflamatória na parede aórtica também foram avaliados. Métodos Onze stents de nitinol revestidos com ePTFE e 10 stents de Dacron infectados com cepas de S. epidermidis foram implantados na aorta infrarrenal de 21 porcos com 8 semanas de idade. Após duas semanas, a aorta contendo os stents foi removida. Um misturador de vórtice e ultrassom foram utilizados para homogeneizar as amostras e remover o biofilme. Posteriormente, o número de unidades formadoras de colônias foi contado. Resultados Não houve diferenças significativas no número de unidades formadoras de colônias ou inflamação na parede arterial entre os dois grupos. Todas as culturas de stent aórtico foram positivas para S. epidermidis, exceto uma no grupo Dacron. Conclusões Não houve diferenças significativas na resposta inflamatória ou na taxa de infecção entre os stents revestidos de ePTFE e Dacron, infectados ativamente pelo S. epidermidis produtor de biofilme. O espessamento intimal e a resposta inflamatória à infecção das endopróteses foram semelhantes. Esses resultados sugerem que os dois materiais de revestimento de stent mais amplamente utilizados têm uma taxa de infecção semelhante.

Resumo O treinamento físico é capaz de aumentar o consumo de oxigênio de pico em indivíduos que sofreram infarto agudo do miocárdio. No entanto, em relação à eficácia de diferentes tipos de intervenções ainda há uma lacuna na literatura. O objetivo do presente estudo foi avaliar os efeitos de diferentes modalidades de treinamento físico no consumo de oxigênio de pico de pacientes pós-infarto agudo do miocárdio. Foram utilizadas as seguintes bases de dados: PubMed (MEDLINE), Cochrane Library, Scopus e PEDro. Foram incluídos estudos que avaliassem exercícios aeróbicos, de força ou combinados. Seis estudos preencheram elegibilidade. O exercício aeróbico aumentou 6,07 mL.kg-1.min-1 quando comparado ao grupo controle (p = 0,013). Na comparação entre exercício combinado e grupo controle, foi observada uma diferença de 1,84 mL.kg-1.min-1, no entanto, sem significância (p = 0,312). Portanto, concluímos que o exercício aeróbico é a única modalidade que apresenta eficácia para aumentar o consumo de oxigênio de pico em comparação a um grupo controle.


Abstract Physical training can increase peak oxygen uptake (VO2peak) in people who have suffered acute myocardial infarction (AMI). However, there is still a gap in the literature in relation to the effectiveness of different types of interventions. Therefore, the aim of the present study was to evaluate the effects of different physical training modalities on VO2peak in post-AMI patients. The following databases were used: PubMed (MEDLINE), Cochrane Library, Scopus, and Pedro. Studies that evaluated aerobic exercise, strength exercise, or combined exercise were included. Six studies met eligibility criteria. Aerobic exercise increased VO2peak by 6.07 ml.kg-1.min-1 when compared to the control group (CG) (p = 0.013). The comparison between combined exercise and control group detected a difference of 1.84 ml.kg-1.min-1, but this was not significant (p = 0.312). We therefore conclude that aerobic exercise is the only modality that is effective for increasing VO2peak compared to a control group.
