RESUMO

BACKGROUND: Postoperative cognitive dysfunction (POCD) may be related to the systemic inflammatory response and an increase in serum markers of brain injury such as S100B protein and neuron-specific enolase (NSE). OBJECTIVE: The study aims to evaluate the association between POCD and serum levels of S100B and NSE after coronary artery bypass grafting surgery (CABG). DESIGN: Prospective observational study. SETTING: Single university teaching hospital. PATIENTS: We investigated 88 patients undergoing CABG. MAIN OUTCOMES MEASURES: Cognitive function was measured preoperatively, and at the 21st and 180th postoperative days (i.e. 6 months after surgery). S100B protein and NSE serum levels were evaluated preoperatively, after induction of anaesthesia, at the end of surgery and at 6 and 24 h after surgery. RESULTS: The incidence of POCD was 26.1% at 21 days after surgery and 22.7% at 6 months after surgery. Increased serum levels of S100B protein and NSE were observed postoperatively and may indicate brain damage. CONCLUSION: Although serum levels of S100B protein and NSE are both significantly increased postoperatively, our findings indicate that serum levels of S100B protein may be more accurate than NSE in the detection of POCD after CABG. RESUMO

Trata-se de um estudo que visa verificar como pacientes não idosos (menores de sessenta anos), atendidos por um serviço de assistência domiciliar e seus cuidadores vivenciam a situação de doença e tratamento, haja visto que há poucos estudos sobre a população não idosa. Foram investigados aspectos como mudanças trazidas pelo adoecimento, repercussões emocionais, significado de cuidar e ser cuidado, a existência de projetos de vida e a influência da assistência domiciliar em suas vidas. Foi realizada entrevista semidirigida com sete pacientes e treze cuidadores; as respostas foram avaliadas qualitativamente e quantitativamente em relação à freqüência de respostas. Concluiu-se que, apesar da vivência ser individual, o adoecimento impõe limitações à vida de pacientes e cuidadores, trazendo, entre outras conseqüências, a restrição social e a dificuldade do exercício profissional; as mudanças no estilo de vida entristecem pacientes e preocupam cuidadores, sendo a assistência domiciliar percebida como um recurso para o enfrentamento da doença. It’s about a research that aims check how not elderly patients (below sixty years), being served by a home care service and their caregivers experience the disease and the treatment, noticing that there is no much studies about not elderly population. It was investigated points like changes that happen because of the disease, emotive changes, the meaning of take care and being took care, the existence of life projects and the influence of home care in their lives. It was aplicated a half-directed interview with open questions to seven patients and to thirteen caregivers; the answers were evaluated in their frequency of the quantities and their qualities. The conclusion was that besides living is individual, the disease brings limitations to the patient and to the caregivers´ lives, bringing besides other consequences, the limitations to the social’s life and the difficulty at the professional life; the changes in the way of life bring more sadness to the patients and more worries to the caregivers, so the home care service is noticed like a tool to face the disease.
