OBJECTIVES: We assessed the timeliness of contact tracing following rapid-positive COVID-19 test result at point-of-care testing (POCT) sites in New York City (NYC). DESIGN: Interviewed case-patients to elicit exposed contacts and conducted COVID-19 exposure notifications. SETTINGS: Twenty-two COVID-19 POCT sites in NYC, the 2 NYC international airports, and 1 ferry terminal. PARTICIPANTS: Case-patients with rapid-positive COVID-19 test results and their named contacts. MAIN OUTCOME MEASURES: We quantified the proportions of interviewed individuals with COVID-19 and notified contacts and assessed the timeliness between the dates of the rapid-positive COVID-19 test results and the interviews or notifications. RESULTS: In total, 11 683 individuals with rapid-positive COVID-19 test results were referred for contact tracing on the day of their diagnosis; 8878 (76) of whom were interviewed within 1 day of diagnosis, of whom 5499 (62%) named 11 486 contacts. A median of 1.24 contacts were identified from each interview. The odds of eliciting contacts were significantly higher among individuals reporting COVID-19 symptoms than among persons with no symptoms (51% vs 36%; adjusted odds ratio [aOR] = 1.37; 95% confidence interval [CI], 1.11-1.70) or living with 1 or more persons than living alone (89% vs 38%; aOR = 12.11; 95% CI, 10.73-13.68). Among the 8878 interviewed case-patients, 8317 (94%) were interviewed within 1 day of their rapid-positive COVID-19 test results and 91% of contact notifications were completed within 1 day of contact identification. The median interval from test result to interview date and from case investigation interview to contact notification were both 0 days (IQR = 0). CONCLUSIONS: The integration of contact tracers into COVID-19 POCT workflow achieved timely case investigation and contact notification. Accelerated contact tracing can be used to curb COVID-19 transmission during local outbreaks.

Exposure to PM2.5 is associated with hundreds of premature mortalities every year in New York City (NYC). Current air quality and health impact assessment tools provide county-wide estimates but are inadequate for assessing health benefits at neighborhood scales, especially for evaluating policy options related to energy efficiency or climate goals. We developed a new ZIP Code-Level Air Pollution Policy Assessment (ZAPPA) tool for NYC by integrating two reduced form models─Community Air Quality Tools (C-TOOLS) and the Co-Benefits Risk Assessment Health Impacts Screening and Mapping Tool (COBRA)─that propagate emissions changes to estimate air pollution exposures and health benefits. ZAPPA leverages custom higher resolution inputs for emissions, health incidences, and population. It, then, enables rapid policy evaluation with localized ZIP code tabulation area (ZCTA)-level analysis of potential health and monetary benefits stemming from air quality management decisions. We evaluated the modeled 2016 PM2.5 values against observed values at EPA and NYCCAS monitors, finding good model performance (FAC2, 1; NMSE, 0.05). We, then, applied ZAPPA to assess PM2.5 reduction-related health benefits from five illustrative policy scenarios in NYC focused on (1) commercial cooking, (2) residential and commercial building fuel regulations, (3) fleet electrification, (4) congestion pricing in Manhattan, and (5) these four combined as a "citywide sustainable policy implementation" scenario. The citywide scenario estimates an average reduction in PM2.5 of 0.9 µg/m3. This change translates to avoiding 210-475 deaths, 340 asthma emergency department visits, and monetized health benefits worth $2B to $5B annually, with significant variation across NYC's 192 ZCTAs. ZCTA-level assessments can help prioritize interventions in neighborhoods that would see the most health benefits from air pollution reduction. ZAPPA can provide quantitative insights on health and monetary benefits for future sustainability policy development in NYC.

BACKGROUND: Airborne polycyclic aromatic hydrocarbons (PAH) possess carcinogenic and endocrine disrupting properties linked to mammary tumorigenesis. These effects may be initiated during a prenatal period of susceptibility to PAH activation of the aryl hydrocarbon receptor (Ahr) and through downstream effects on estrogen receptor (Er) α. PURPOSE: We hypothesized prenatal airborne PAH exposure induces sustained effects in female adult wild type BALB/cByj mice detected in the offspring (F1) and grandoffspring (F2) generation. We hypothesized these effects would include altered expression and epigenetic regulation of Erα and altered expression of aryl hydrocarbon receptor repressor (Ahrr, Ahrr/aryl hydrocarbon receptor nuclear translocator (Arnt), and breast cancer type 1 susceptibility (Brca1). Further, we hypothesized that PAH would induce precancerous outcomes such as epithelial cell proliferation and epithelial cell hyperplasia in mammary glands of adult female offspring and grandoffspring. RESULTS: Prenatal ambient PAH exposure lowered Erα mRNA expression (F1 and F2: p<0.001 for each) and induced methylation in the Erα promoter in mammary tissue in offspring and grandoffspring mice on postnatal day (PND) 60. Prenatal PAH lowered Brca1 mRNA (F1: p=0.002, F2: p=0.02); Erα mRNA was correlated with Brca1 (F1: r=0.42, p=0.02; F2: r=0.53, p=0.005). Prenatal PAH lowered Ahrr (F1: p=0.03, F2: p=0.009) and raised Arnt mRNA expression (F1: p=0.01, F2: p=0.03). Alterations in Erα mRNA (F2: p<0.0001) and Ahrr (F2: p=0.02) in the grandoffspring mice also occured by PND 28, and similarly occurred in the dam on postpartum day (PPD) 28. Finally, prenatal PAH was associated with higher mammary epithelial cell proliferation in the offspring (p=0.02), but not grandoffspring mice, without differences in the frequency of mammary cell hyperplasia. These results did not differ after adjustment by each candidate gene expression level. CONCLUSIONS: Prenatal PAH exposure induces DNA methylation and alters gene expression in the Erα-mediated pathway across generations, and suggests that functional outcomes such as mammary cell proliferation also may occur in offspring as a result.

Although short-duration elevated exposures (peak exposures) to pollutants may trigger adverse acute effects, epidemiological studies to understand their influence on different health effects are hampered by lack of methods for objectively identifying peaks. Secondhand smoke from cigarettes (SHS) in the residential environment can lead to peak exposures. The aim of this study was to explore whether peaks in continuous PM2.5 data can indicate SHS exposure. A total of 41 children (21 with and 20 without SHS exposure based on self-report) from 28 families in New York City (NY, USA) were recruited. Both personal and residential continuous PM2.5 monitoring were performed for five consecutive days using MicroPEM sensors (RTI International, USA). A threshold detection method based on cumulative distribution function was developed to identify peaks. When children were home, the mean accumulated peak area (APA) for peak exposures was 297 ± 325 hour*µg/m3 for children from smoking families and six times that of the APA from non-smoking families (~50 ± 54 hour*µg/m3 ). Average PM2.5 mass concentrations for SHS exposed and unexposed children were 24 ± 15 µg/m3 and 15 ± 9 µg/m3 , respectively. The average SHS exposure duration represents ~5% of total exposure time, but ~13% of children's total PM2.5 exposure dose, equivalent to an additional 2.6 µg/m3 per day. This study demonstrated the feasibility of peak analysis for quantifying SHS exposure. The developed method can be adopted more widely to support epidemiology studies on impacts of short-term exposures.

The 2010 Deepwater Horizon oil spill resulted in 1.6-2.6 × 10(10) grams of petrocarbon accumulation on the seafloor. Data from a deep sediment trap, deployed 7.4 km SW of the well between August 2010 and October 2011, disclose that the sinking of spill-associated substances, mediated by marine particles, especially phytoplankton, continued at least 5 mo following the capping of the well. In August/September 2010, an exceptionally large diatom bloom sedimentation event coincided with elevated sinking rates of oil-derived hydrocarbons, black carbon, and two key components of drilling mud, barium and olefins. Barium remained in the water column for months and even entered pelagic food webs. Both saturated and polycyclic aromatic hydrocarbon source indicators corroborate a predominant contribution of crude oil to the sinking hydrocarbons. Cosedimentation with diatoms accumulated contaminants that were dispersed in the water column and transported them downward, where they were concentrated into the upper centimeters of the seafloor, potentially leading to sustained impact on benthic ecosystems.

BACKGROUND: Fine particulate matter (PM2.5) is associated with various adverse health outcomes. The MicroPEM (RTI, NC), a miniaturized real-time portable particulate sensor with an integrated filter for collecting particles, has been widely used for personal PM2.5 exposure assessment. Five-day deployments were targeted on a total of 142 deployments (personal or residential) to obtain real-time PM2.5 levels from children living in New York City and Baltimore. Among these 142 deployments, 79 applied high-efficiency particulate air (HEPA) filters in the field at the beginning and end of each deployment to adjust the zero level of the nephelometer. However, unacceptable baseline drift was observed in a large fraction (> 40%) of acquisitions in this study even after HEPA correction. This drift issue has been observed in several other studies as well. The purpose of the present study is to develop an algorithm to correct the baseline drift in MicroPEM based on central site ambient data during inactive time periods. METHOD: A running baseline & gravimetric correction (RBGC) method was developed based on the comparison of MicroPEM readings during inactive periods to ambient PM2.5 levels provided by fixed monitoring sites and the gravimetric weight of PM2.5 collected on the MicroPEM filters. The results after RBGC correction were compared with those using HEPA approach and gravimetric correction alone. Seven pairs of duplicate acquisitions were used to validate the RBGC method. RESULTS: The percentages of acquisitions with baseline drift problems were 42%, 53% and 10% for raw, HEPA corrected, and RBGC corrected data, respectively. Pearson correlation analysis of duplicates showed an increase in the coefficient of determination from 0.75 for raw data to 0.97 after RBGC correction. In addition, the slope of the regression line increased from 0.60 for raw data to 1.00 after RBGC correction. CONCLUSIONS: The RBGC approach corrected the baseline drift issue associated with MicroPEM data. The algorithm developed has the potential for use with data generated from other types of PM sensors that contain a filter for weighing as well. In addition, this approach can be applied in many other regions, given widely available ambient PM data from monitoring networks, especially in urban areas.

Efficient and reproducible measurements of multiple polycyclic aromatic hydrocarbon (PAH) metabolites in urinary samples are required to evaluate the complex health effects of PAH exposure. Here, we demonstrate a highly practical, automated off-line solid-phase extraction (SPE) of deconjugated hydroxylated PAHs followed by LC-MS/MS to simultaneously measure eight mono-hydroxylated PAH compounds: 1-hydroxynaphthalene, 2-hydroxynaphthalene, 2-hydroxyfluorene, 1-hydroxyphenanthrene, 2&3-hydroxyphenanthrene, 4-hydroxyphenanthrene and 1-hydroxypyrene. Initially, we observed low recovery rates (e.g., 16% for 1-hydroxypyrene) when using previously published methods. We optimized the procedure by choosing polymeric absorbent-based cartridges, automating the sample loading step by diluting samples with 15% methanol/sodium acetate, and most importantly, replacing acetonitrile with methanol as the eluting solvent. Optimized sample preparation has improved the recovery rates to more than 69% for analytes of interest. This improvement led to higher method sensitivity and detection frequency, especially for 1-hydroxypyrene, in all of 100 urine samples collected in the New York City site of the Legacy Girls Study. The limits of detection ranged from 7.6 pg/mL to 20.3 pg/mL using 1 mL of urine, compared to the 2 mL required in CDC, method 09-OD. The average coefficients of variance of quality control samples (n = 60) ranged between 7 and 21%; variance of repeated measurements (n = 45) was less than 10%. This efficient and reliable method for measuring PAH metabolites will greatly benefit epidemiology studies and biomonitoring programs.

BACKGROUND: Contact tracing is an important public health tool for curbing the spread of infectious diseases. Effective and efficient contact tracing involves the rapid identification of individuals with infection and their exposed contacts and ensuring their isolation or quarantine, respectively. Manual contact tracing via telephone call and digital proximity app technology have been key strategies in mitigating the spread of COVID-19. However, many people are not reached for COVID-19 contact tracing due to missing telephone numbers or nonresponse to telephone calls. The New York City COVID-19 Trace program augmented the efforts of telephone-based contact tracers with information gatherers (IGs) to search and obtain telephone numbers or residential addresses, and community engagement specialists (CESs) made home visits to individuals that were not contacted via telephone calls. OBJECTIVE: The aim of this study was to assess the contribution of information gathering and home visits to the yields of COVID-19 contact tracing in New York City. METHODS: IGs looked for phone numbers or addresses when records were missing phone numbers to locate case-patients or contacts. CESs made home visits to case-patients and contacts with no phone numbers or those who were not reached by telephone-based tracers. Contact tracing management software was used to triage and queue assignments for the telephone-based tracers, IGs, and CESs. We measured the outcomes of contact tracing-related tasks performed by the IGs and CESs from July 2020 to June 2021. RESULTS: Of 659,484 cases and 861,566 contact records in the Trace system, 28% (185,485) of cases and 35% (303,550) of contacts were referred to IGs. IGs obtained new phone numbers for 33% (61,804) of case-patients and 11% (31,951) of contacts; 50% (31,019) of the case-patients and 46% (14,604) of the contacts with new phone numbers completed interviews; 25% (167,815) of case-patients and 8% (72,437) of contacts were referred to CESs. CESs attempted 80% (132,781) of case and 69% (49,846) of contact investigations, of which 47% (62,733) and 50% (25,015) respectively, completed interviews. An additional 12,192 contacts were identified following IG investigations and 13,507 following CES interventions. CONCLUSIONS: Gathering new or missing locating information and making home visits increased the number of case-patients and contacts interviewed for contact tracing and resulted in additional contacts. When possible, contact tracing programs should add information gathering and home visiting strategies to increase COVID-19 contact tracing coverage and yields as well as promote equity in the delivery of this public health intervention.

OBJECTIVES: High rates of hospitalization and death disproportionately affected Black, Latino, and Asian residents of New York City at the beginning of the COVID-19 pandemic. To suppress COVID-19 transmission, New York City implemented a workforce of community engagement specialists (CESs) to conduct home-based contact tracing when telephone numbers were lacking or telephone-based efforts were unsuccessful and to disseminate COVID-19 information and sanitary supplies. MATERIALS AND METHODS: We describe the recruitment, training, and deployment of a multilingual CES workforce with diverse sociodemographic backgrounds during July-December 2020 in New York City. We developed standard operating procedures for infection control and safety measures, procured supplies and means of transportation, and developed protocols and algorithms to efficiently distribute workload. RESULTS: From July through December 2020, 519 CESs were trained to conduct in-person contact tracing and activities in community settings, including homes, schools, and businesses, where they disseminated educational materials, face masks, hand sanitizer, and home-based specimen collection kits. During the study period, 94 704 records of people with COVID-19 and 61 246 contacts not reached by telephone-based contact tracers were referred to CESs. CESs attempted home visits or telephone calls with 84 230 people with COVID-19 and 49 303 contacts, reaching approximately 55 592 (66%) and 35 005 (71%), respectively. Other CES activities included monitoring recently arrived travelers under quarantine, eliciting contacts at point-of-care testing sites, and advising schools on school-based COVID-19 mitigation strategies. PRACTICE IMPLICATIONS: This diverse CES workforce allowed for safe, in-person implementation of contact tracing and other prevention services for individuals and communities impacted by COVID-19. This approach prioritized equitable delivery of community-based support services and resources.

Adsorption of organic pollutants onto microplastics has been reported in prior studies, indicating the potential of these particles to serve as vectors of pollutants. Most prior investigations, however, have been conducted in laboratories under conditions with relatively little environmental relevance. Here we report the results of in-situ experiments to investigate the adsorption of pharmaceuticals (atenolol, sulfamethoxazole, and ibuprofen) on to eight types of test materials (pellets from five types of widely-used polymers, small pieces of straws, fragments of bags, and glass beads for control). Three sample sets survived 28 days of deployment in New York City waterways. Concentrations of each analyte in water samples taken at these sites were also measured. Adsorption coefficients were calculated based on mass and surface area for each type. Mass-based coefficients showed much higher values for straw and bag samples than other types, consistent with their greater surface area to mass ratios. The surface area-based coefficients were similar among the plastic materials tested as well as the glass beads, indicating that surface area is a major determinant of the pharmaceutical adsorption, regardless of material type. Rapid biofouling, which was observed on all samples, appeared to be the predominant factor controlling the sorption capacity of the plastics. Our observations suggest that extensive biofouling and the formation of biofilms in nutrient-enriched waters can significantly impact the adsorption of pharmaceuticals onto plastics.

BACKGROUND: Reducing cadmium (Cd) exposure in Cd-polluted areas in Asia is urgently needed given the toxic effects of Cd. The short-term and long-term benefits of lowering Cd exposure are unknown because of its long half-life in the body. OBJECTIVES: We aimed to investigate whether an intervention with low-Cd rice in a contaminated area of China reduced urinary Cd (UCd) levels and improved blood pressure and kidney function outcomes compared to no-intervention in consumers of high-Cd rice in the same region. METHODS: 106 non-smoking subjects were divided into three treatment groups: the intervention group (replacing homegrown high-Cd rice with market low-Cd rice, n = 34), the non-intervention group (continue eating high-Cd rice, n = 36) and the control group (continued eating low-Cd rice they have been eating for years, n = 36). The intervention period lasted for almost 8 months, during which participants were visited on up to 4 occasions and UCd, systolic and diastolic blood pressure (SBP, DBP), kidney function biomarkers (ß2-microglobulin and N-acetyl-ß-D-glucosaminidase) were measured. RESULTS: After 3 months, the geometric mean UCd in the intervention (Int) group decreased significantly by 0.32 µg/g (p = 0.007), while changes were not significant in the non-intervention (non-Int) group (0.13 µg/g, p = 0.95) or the control group (-0.01 µg/g, p = 0.52). UCd in the Int group remained lower than in the non-Int group but higher than in the Control group through the end of follow up. DBP in the Int group decreased significantly from 80 mm Hg at month three (p = 0.03) and stayed around 74 mm Hg for the remainder of the study. SBP also decreased in the Int group but with variations similar to those observed in the other two groups. The two kidney biomarkers showed variations without a clear pattern. CONCLUSION: This study suggested that UCd reflected both short-term (<3 months) and long-term Cd exposure. In addition, the low-Cd rice intervention showed initial benefits in lowering blood pressure levels, especially DBP, but not kidney biomarkers.

Oxalic acid potentially enhances pump-and-treat for groundwater As remediation by accelerating mobilization. This study examines how oxalic acid mobilizes As from Fe(III)-oxide coated sand under hydrodynamic conditions. Four columns were packed with metal-substituted ferrihydrite or goethite to 1% Fe, presorbed to 50% As surface coverage, and reacted with pH = 2.2 artificial groundwater amended with 10 mM oxalic acid at 1 m day-1. Arsenic elution was affected by both As and Fe speciation. Although the As(V) columns experienced faster substrate dissolution, As(V) elution was delayed by re-adsorption, whereas As(III) elution was rapid due to pH decrease that prevented re-adsorption. Cr-ferrihydrite and Ni-goethite dissolved both effectively initially but then diverged. The Cr-ferrihydrite columns experienced continuous stoichiometric Fe and Cr release, and As release could be sustained. The Ni-goethite columns initially experienced nonstoichiometric Fe and Ni release, and As release was extensive. Such release, however, was not sustained. We hypothesized that Ni-goethite contained sites with distinct reactivity, and oxalic acid targeted readily-dissolved, sorption-dense sites. Our data indicate that oxalic acid-enhanced pump-and-treat methods would be easier to apply to aquifers dominated by As(III), requiring less amendment to be injected; such oxalic acid-enhanced methods remove reactive sediment Fe and As, potentially preventing future groundwater contamination.

This study validates the analysis of polycyclic aromatic hydrocarbons (PAHs) in microgram levels of particulate matter (PM) collected on filters by two low-flow rate, real-time monitors, microPEM™ and microAeth®. Particle-associated PAHs were analyzed by a coupling of a gas chromatograph to a sensitive, atmospheric-pressure laser ionization-mass spectrometer. Air particulate samples were collected over the course of one or two days in the living room of a fourth-floor apartment in New York City. Three types of samplers, the two aforementioned personal samplers and a high-flow rate pump (4 liters per minute), were operated side by side, and three samples of each type were collected during each sampling period. Intrasampler agreement as measured by relative standard deviation (RSD) was within 1% to 18%. After background subtraction, total PAH measured by all three sampler types had good agreement (R=0.99). This ability to accurately characterize personal PAH exposure in archived filters collected by these real-time samplers could provide additional important PAH exposure information that can benefit many environmental health studies using these monitors.

Fine particulate matter (PM2.5), levels of which are about 6 times the 2014 WHO air quality guidelines for 190 cities in China, has been found to be associated with various adverse health outcomes. In this study, personal PM2.5 exposures were monitored along a fixed routine that included 19 types of non-residential micro-environments (MEs) on 4 hazy days (ambient PM2.5 292 ± 70 µg m-3) and 2 non-hazy days (55 ± 16 µg m-3) in Nanjing, China using miniaturized real-time portable particulate sensors that also collect integrated filters of PM2.5 (MicroPEMs, Research Triangle Institute (RTI), NC). Gravimetric correction is necessary for nephelometer devices in calculating real-time PM levels. During both hazy and non-hazy days, personal PM2.5 levels were generally higher in MEs with noticeable PM2.5 sources than MEs serving as receptor sites, higher in open MEs than indoor MEs, and higher in densely populated MEs than MEs with few people. Personal PM2.5 levels measured during hazy and non-hazy days were 242 ± 91 µg m-3 and 103 ± 147 µg m-3, respectively. The ratio of personal exposure to ambient PM2.5 levels (rp/a ) was less than 1.0 and less variable on hazy days (0.85 ± 0.31); while it was larger than 1.0 and more variable on non-hazy days (1.71 ± 1.93), confirming the importance of local sources other than ambient during non-hazy days. Air handling methods (e.g., ventilation/filtration) impacted personal exposures in enclosed locations on both types of days. Street food vendors with cooking emissions were MEs with the highest personal PM2.5 levels while subway cars in Nanjing were relatively clean due to good air filtration on both hazy and non-hazy days. In summary, on hazy days, personal exposure was mainly affected by the regional ambient levels, while on non-hazy days, local sources together with ambient levels determined personal exposure levels.

Exhaled breath condensate (EBC) provides a relatively easy, non-invasive method for measuring biomarkers of inflammation and oxidative stress in the airways. However, the levels of these biomarkers in EBC are influenced, not only by their levels in lung lining fluid but also by the volume of water vapor that also condenses during EBC collection. For this reason, the use of a biomarker of dilution has been recommended. Urea has been proposed and utilized as a promising dilution biomarker due to its even distribution throughout the body and relatively low volatility. Current EBC urea analytical methods either are not sensitive enough, necessitating large volumes of EBC, or are labor intensive, requiring a derivatization step or other pretreatment. We report here a straightforward and reliable LC-MS approach that we developed that does not require derivatization or large sample volume (∼36 µL). An Acclaim mixed-mode hydrophilic interaction chromatography column was selected because it can produce good peak symmetry and efficiently separate urea from other polar and nonpolar compounds. To achieve a high recovery rate, a slow and incomplete evaporation method was used followed by a solvent-phase exchange. Among EBC samples collected from 28 children, urea levels were found to be highly variable, with a relative standard deviation of 234%, suggesting high variability in dilution of the lung lining fluid component of EBC. The limit of detection was found to be 0.036 µg/mL.
