RESUMO

High resolution esophageal manometry (HRM) is currently under development as can be seen in the various Chicago classifications. In order to standardize criteria in certain practical aspects with limited scientific evidence, the First National Meeting for Consensus in High Resolution Manometry of the Spanish Digestive Motility Group took place, bringing together a wide group of experts. The proposals were based on a prior survey composed of 47 questions, an exhaustive review of the available literature and the experience of the participants. Methodological aspects relating to the poorly defined analysis criteria of certain new high resolution parameters were discussed, as well as other issues previously overlooked such as spontaneous activity or secondary waves. Final conclusions were drawn with practical applications.

RESUMO

Gastroesophageal reflux disease is a highly frequent disorder classically characterized by the presence of heartburn and/or acid regurgitation that improves with drug therapy that reduces acid content in the stomach. However, especially in patients with non-erosive disease, response to proton pump inhibitors is unsatisfactory in approximately 1 out of 3 patients, and consequently, in these patients, it is important to establish a definitive diagnosis and an alternative therapeutic strategy. In the last few years, advances have been made in knowledge of the physiopathology of reflux, such as identification of the role of the acid pocket in producing reflux, technological advances that allow differentiation among acid reflux, non-acid reflux and slightly acid reflux, and advances in the treatment of reflux with drugs that attempt to act on the barrier function of the esophagogastric junction.

La manometría esofágica de alta resolución (MAR) está en fase de desarrollo, como se evidencia por las diferentes clasificaciones de Chicago. Con el fin de unificar criterios en algunos aspectos prácticos con limitada evidencia científica se llevó a cabo la Primera Reunión Nacional de Consenso en Manometría de Alta Resolución del Grupo Español de Motilidad Digestiva, en la que participaron un amplio grupo de expertos. Las propuestas se basaron en una encuesta previa con 47 preguntas, la exhaustiva revisión de la bibliografía disponible y la experiencia de los participantes. Se plantearon aspectos metodológicos sobre criterios de análisis poco definidos de algunos nuevos parámetros de alta resolución y otros aspectos no considerados, como la actividad espontánea o las ondas secundarias, elaborándose conclusiones finales con utilidad práctica (AU)

El reflujo gastroesofágico es un trastorno muy frecuente que se caracteriza clásicamente por la presencia de pirosis retroesternal y/o regurgitación ácida que mejora con fármacos que disminuyen el contenido ácido del estómago. Sin embargo, y especialmente en los pacientes con enfermedad no erosiva, la respuesta a los inhibidores de la bomba de protones es insatisfactoria en aproximadamente uno de cada 3 pacientes, por lo que en estos casos se deberá intentar establecer un diagnóstico de certeza, y una estrategia terapéutica diferenciada. En los últimos años se han producido avances en el conocimiento de la fisiopatología del reflujo, como es la identificación del papel que el sobrenadante ácido posprandial (acid pocket) desempeña en la producción de reflujo, avances tecnológicos que permiten diferenciar el reflujo ácido, no ácido y débilmente ácido y avances en el tratamiento del reflujo con fármacos que intentan actuar sobre la función barrera de la unión esofagogástrica (AU)
