AIM: To assess correlations between the characteristics of the nursing practice environment, job outcomes and safety climate. BACKGROUND: The nursing practice environment is critical to the well-being of professionals and to patient safety, as highlighted by national and international studies; however, there is a lack of evidence regarding this theme in paediatric units. METHODS: A cross-sectional study, in two paediatric hospitals in Brazil, was conducted from December 2013 to February 2014. For data collection, we used the Nursing Work Index - Revised, Safety Attitudes Questionnaire - Short Form 2006 and the Maslach Burnout Inventory, and for analysis Spearman's correlation coefficient and structural equation modelling were used. RESULTS: Two hundred and sixty-seven professional nurses participated in the study. Autonomy, control over the work environment and the relationship between nursing and medical staff are factors associated with job outcomes and safety climate and can be considered their predictors. CONCLUSIONS: Professional nurses with greater autonomy, good working relationships and control over their work environment have lower levels of emotional exhaustion, higher job satisfaction, less intention of leaving the job and the safety climate is positive. IMPLICATIONS FOR NURSING MANAGEMENT: Initiatives to improve the professional practice environment can improve the safety of paediatric patients and the well-being of professional nurses.

BACKGROUND: Insulin resistance is a common risk factor in chronic kidney disease patients contributing to the high cardiovascular burden, even in the absence of diabetes. Glucose-based peritoneal dialysis (PD) solutions are thought to intensify insulin resistance due to the continuous glucose absorption from the peritoneal cavity. The aim of our study was to analyse the effect of the substitution of glucose for icodextrin on insulin resistance in non-diabetic PD patients in a multicentric randomized clinical trial. METHODS: This was a multicenter, open-label study with balanced randomization (1:1) and two parallel-groups. Inclusion criteria were non-diabetic adult patients on automated peritoneal dialysis (APD) for at least 3 months on therapy prior to randomization. Patients assigned to the intervention group were treated with 2L of icodextrin 7.5%, and the control group with glucose 2.5% during the long dwell and, at night in the cycler, with a prescription of standard glucose-based PD solution only in both groups. The primary end-point was the change in insulin resistance measured by homeostatic model assessment (HOMA) index at 90 days. RESULTS: Sixty patients were included in the intervention (n = 33) or the control (n = 27) groups. There was no difference between groups at baseline. After adjustment for pre-intervention HOMA index levels, the group treated with icodextrin had the lower post-intervention levels at 90 days in both intention to treat [1.49 (95% CI: 1.23-1.74) versus 1.89 (95% CI: 1.62-2.17)], (F = 4.643, P = 0.03, partial η(2) = 0.078); and the treated analysis [1.47 (95% CI: 1.01-1.84) versus 2.18 (95% CI: 1.81-2.55)], (F = 7.488, P = 0.01, partial η(2) = 0.195). CONCLUSIONS: The substitution of glucose for icodextrin for the long dwell improved insulin resistance measured by HOMA index in non-diabetic APD patients.

BACKGROUND AND PURPOSE: Assessing patient satisfaction requires reliable and valid instruments. This study evaluated the reliability and validity of the Brazilian version of the Newcastle Satisfaction with Nursing Scales (B-NSNS). METHODS: This cross-sectional study enrolled 351 adult inpatients. Partial least squares path modeling (PLS-PM) approach was used to evaluate the reliability and validity of this instrument. RESULTS: After the adjustments of model, both constructs exhibited adequate convergent and discriminant validity, showing that experiences with nursing care have a significant effect on patient satisfaction (r = .70) and account for 49.1% of the variance in patient satisfaction with the nursing care (R2 = 0.49). CONCLUSIONS: The B-NSNS result in a valid and reliable measure for use in managing nursing care.

OBJECTIVE: To build and validate an instrument to evaluate Lean Healthcare in healthcare institutions. METHOD: Methodological study conducted in three stages: 1) Instrument construction; 2) Content validity using the Delphi technique with 14 experts; and 3) Construct validation using Structural Equation Modeling with sample consisted of 113 professionals with experience in Lean Healthcare. Data collection carried out from October/2020 to January/2021 using a digital form. Data analysis performed with the SmartPLS2.0/M3 software. RESULTS: Items were developed after an integrative review and divided into the dimensions Structure, Process and Outcome, according to Donabedian's theoretical framework. Content validation in two rounds of the Delphi technique. Final instrument, after model adjustment, containing 16 items with Cronbach's alpha of 0.77 in Structure, 0.71 in Process and 0.83 in Outcome. CONCLUSION: The instrument presented evidence of validity and reliability, enabling its use in healthcare institutions to evaluate Lean Healthcare.

BACKGROUND: Remote work was evidenced in the pandemic and studies in this area increased. Most studies focus on professionals of companies or professors/students in the academic environment. At the same time, non-academic staff, that provide all the support required for the core activities of the institutions (research/teaching/extension activities) have been neglected. OBJECTIVE: This article aims to exploratory analyse which variables (interruptions when working remotely (1), health concerns (2) and fear of contracting coronavirus (3), anxiety and concern about professional career (4), frustration to have cancelled plans and missed opportunities (5) and gender (6)) can impact feelings of sadness and depression experienced by non-academic staff of a university working remotely. METHODS: Using a database on behaviour and feelings of non-academic staff from a Brazilian university working remotely during the COVID-19 pandemic, a binary logistic regression model was structured. In an exploratory manner, six independent variables (presented in the previous item) were analysed in terms of their ability to predict the dependent variable (feelings of sadness and depression). RESULTS: The results presented the prediction power of the independent variables for the dependent variable. The variables regarding concern with their health, increased anxiety and concern about their career presented Odds Ratios of 3.6 (1.4-8.5 -95% C.I.) and 3.3 (2.2-5.0 -95% C.I.), respectively, standing out from the other variables. CONCLUSIONS: These results focus on staff at one institution, but they can contribute to better understand feelings and behaviours experienced by professionals working remotely and provide information for debates on the field of COVID-19-related changes of work.

The objective of this study was to evaluate the moderating effect of Occupational Health and Safety actions based on the World Health Organization (WHO) recommendations to mitigate the negative effect of COVID-19 on the operational, logistical, marketing (OLMP), and health and safety performance (OHSP) of workers in multinational industries. The development of surveys in companies was the method adopted, which had confirmatory evaluations through Structural Equations Modelling (SEM). As a result, it was confirmed that this is one of the few scientific studies that expectedly validates that the COVID-19 pandemic has severely impacted operational, logistical, market, and Occupational Health and Safety (OHS) performance. This is also one of the few research projects to assess the moderating effect of OHS practices based on WHO to mitigate the effects of COVID-19. According to our findings, those practices were able to reduce by at least 50% the effect of the COVID-19 crisis on operational, logistical, and marketing performance. However, they minimize by only 1.8% the negative effects of health and safety performance for the worker, generating absenteeism increasingly due to physical and mental problems. This number could be higher if the social distance could be provided in public transportation and if employees were more aware of the risks of COVID-19 contamination during their social activities.

INTRODUCTION: The coronavirus-19 pandemic threatens the lives of all people, but results in higher mortality rates for patients with end-stage kidney disease (ESKD) including those on peritoneal dialysis (PD). Telemedicine was the main alternative to reduce exposure to the virus, but it was introduced in the Brazil without proper training. OBJECTIVE: To investigate the impact of telemedicine on metabolic control, peritonitis rates, and hospitalization in PD patients during the pandemic. METHODS: This was a retrospective multicenter cohort study. We included all adult patients on chronic PD from 9 clinics selected by convenience during the pandemic. The outcomes of interest were measured and compared between before and after switching to telemedicine using repeated measure analysis and multilevel Poisson regression. RESULTS: The study included 747 patients with a mean age of 59.7±16.6 years, of whom 53.7% were male and 40.8% had diabetes. Biochemical parameters including hemoglobin, potassium, phosphate, calcium, and urea serum levels did not change significantly after transition to telemedicine. There was no association between telemedicine and peritonitis rates. In contrast, hospitalization rates increased significantly in the telemedicine period. The incidence rate ratio (IRR) for hospitalization in the telemedicine period was 1.54 (95%CI 1.10-2.17; p 0.012) and 1.57 (95%CI 1.12-2.21; p 0.009) in the mixed-effects Poisson regression before and after adjustment for the presence of confounders. Admissions for hypervolemia and infections not related to PD doubled after transition to telemedicine. CONCLUSION: The implementation of telemedicine without proper training may lead to an increase in adverse events in PD patients.

OBJECTIVE: To analyze the scientific evidences in the literature about the evaluation of Lean Healthcare after its implementation. METHOD: An integrative review conducted in the CINAHL, Scopus, WOS, and Embase databases, and in the PubMed portal, resulting in 18 articles published in English, Spanish and Portuguese, from 2008 to 2019. RESULTS: The findings were categorized into results for the institution, professional, and patient. The predominant category was institutional, with cost analysis and increased productivity, followed by the professional, with job satisfaction and leadership, and finally the patient, with satisfaction, attitudes, and behaviors. CONCLUSION: This study reinforces the need to establish, for management, a systematic method of monitoring the results achieved in the Lean Healthcare implementation phase. Since the value in this method is defined by the patient, further research in this aspect may lead to new evidence.

RESUMO

Chloroquine and hydroxychloroquine have shown promising preliminary results and have been discussed as therapeutic options for patients with Covid-19. Despite the lack of robust evidence demonstrating the benefits and justifying the use of one of these drugs, the final decision is the responsibility of the attending physician and should be individualized and shared, whenever possible. This position statement recommends dosage adjustment for these drugs in the context of renal impairment.

RESUMO

The Covid-19 pandemic brought several challenges to the healthcare system: diagnosis, treatment and measures to prevent the spread of the disease. With the greater availability and variety of diagnostic tests, it is essential to properly interpret them. This paper intends to help dialysis units concerning the use of clinical criteria and diagnostic tests for decision making regarding the discontinuation of isolation of patients with suspected or confirmed Covid-19, as well as the return to work activities for employees with suspected or confirmed Covid-19.

RESUMO

Dialysis units are environments potentially prone to the spread of Covid-19. Patients cannot suspend treatment, and they often have comorbidities, which assigns them a higher risk and worse prognosis. The Brazilian Society of Nephrology prepared this document of good practices, whose technical recommendations deal with general measures that can be implemented to reduce the risk of transmission and prevent the spread of the disease in the unit.

BACKGROUND AND PURPOSE: To assess the psychometric properties of the PAM13 Brazilian-Portuguese (PAM13-B) among outpatients with chronic diseases. METHODS: 513 adults participated, diagnosed with a chronic disease for more than 6 months, under outpatient monitoring. Reliability was tested using internal consistency and test-retest reliability. Construct validity was verified through different techniques (correlation between activation and self-esteem measures, anxiety, depression and health status), known-groups validity and dimensionality by means of confirmatory factor analysis. Significance was set at 0.05. RESULTS: The PAM13-B presented appropriate results for internal consistency (α = 0.83) and test-retest reliability (intraclass correlation coefficient [ICC] = 0.81). A moderate correlation was found between activation and self-esteem only (r = 0.43, p < 0.001). The one-dimensional structure was not confirmed in the sample analyzed. CONCLUSIONS: In the study sample, the version PAM13-B demonstrated its reliability and validity, but with a two-factor structure.

BACKGROUND: Differences in small solutes transport rate (SSTR) during peritoneal dialysis (PD) may affect water and solutes removal. Patients with high SSTR must rely on shorter dwell times and increased dialysate glucose concentrations to keep fluid balance. Glucose absorption during peritoneal dialysis (PD), besides affecting glucose and insulin metabolism, may induce weight gain. The study aimed at examining acute glucose and insulin serum level changes and other potential relationships in PD patients with diverse SSTR. METHODS: This cross-sectional study used a modified peritoneal equilibration test (PET) that enrolled 34 prevalent PD patients. Zero, 15, 30, 60, 120, 180, and 240-minute glucose and insulin serum levels were measured. Insulin resistance index was assessed by the homeostasis model assessment (HOMA-IR) formula. SSTR categories were classified by quartiles of the four-hour dialysate/serum creatinine ratio (D(4)/P(Cr)). Demographic and clinical variables were evaluated, and the body mass index (BMI) was estimated. Correlations among variables of interest and categories of SSTR were explored. RESULTS: Glucose serum levels were significantly different at 15, 30, and 60 minutes between high and low SSTR categories (p = 0.014, 0.009, and 0.022). Increased BMI (25.5 +/- 5.1) and insulin resistance [HOMA-IR = 2.60 (1.40-4.23)] were evidenced overall. Very strong to moderate correlations between insulin levels along the PET and HOMA-IR (r = 0.973, 0.834, 0.766, 0.728, 0.843, 0.857, 0.882) and BMI (r = 0.562, 0.459, 0.417, 0.370, 0.508, 0.514, 0.483) were disclosed. CONCLUSIONS; Early glucose serum levels were associated with SSTR during a PET. Overweight or obesity and insulin resistance were prevalent. An association between insulin serum levels and BMI was demonstrated.

Resumo O objetivo foi analisar a produção científica do Programa Segundo Tempo (PST) por meio de uma revisão sistemática. Os resultados indicaram que pesquisadores de universidades do Sul, Nordeste e Sudeste impulsionaram as publicações sobre o programa, entre 2007 e 2021, com concentração da produção em periódicos da área da educação física e educação. Houve uma concentração de pesquisas sobre a gestão, formação/acompanhamento pedagógico e avaliação, com ênfase na descentralização, formação dos recursos humanos e análise do impacto do programa. Com menor incidência, reportou-se pesquisas sobre as práticas corporais, educação física e aula, bem como a percepção de diferentes sujeitos e a relação do programa com os megaeventos esportivos. Por fim, destaca-se a necessidade de pesquisas com diferentes referenciais teórico-metodológicos, que ultrapassem as pesquisas descritivas, com abordagem qualitativa. (AU)


Abstract The aim was to analyze the scientific production of the "Programa Segundo Tempo (PST)" through a systematic review. The results indicated that researchers from universities in the South, Northeast and Southeast promoted publications about the program, between 2007 and 2021, with a concentration of production in journals in the area of physical education and education. There was a concentration of research on management, professional training/monitoring and evaluation, with an emphasis on decentralization, training of human resources and analysis of the impact of the program. With less incidence, research was reported on body practices, physical education and classes, as well as the perception of different subjects and the program's relationship with sports mega-events. Finally, the need for investigations with different theoretical-methodological frameworks stands out, which goes beyond the descriptive investigation, with a qualitative approach. (AU)


Resumen El objetivo fue analizar la producción científica del "Programa Segundo Tempo" (PST) a través de una revisión sistemática. Los resultados indicaron que investigadores de universidades del Sur, Nordeste y Sudeste promovieron publicaciones sobre el programa, entre 2007 y 2021, con concentración de la producción en revistas del área de educación física y educación. Se concentró la investigación en gestión, formación/acompañamiento pedagógico y evaluación, con énfasis en la descentralización, formación de recursos humanos y análisis de impacto del programa. Con menor incidencia se reportaron investigaciones sobre prácticas corporales, educación física y clases, así como la percepción de diferentes temas y la relación del programa con los megaeventos deportivos. Finalmente, se destaca la necesidad de investigaciones con diferentes referenciales teórico-metodológicos, que vayan más allá de la investigación descriptiva, con un enfoque cualitativo. (AU)

OBJECTIVE: To adapt and validate the Patient Activation Measure (PAM22) in a sample of Brazilians with chronic diseases under outpatient monitoring. METHOD: Adaptation process comprises translation, back translation, analysis by a committee of judges, semantic analysis, and pre-test. Psychometric validation was performed with 513 individuals. Construct validity was analyzed through Pearson's correlation, Student's t-test and Structural Equation Modeling; reliability was assessed by the intraclass correlation coefficient and Cronbach's Alpha. RESULTS: The internal consistency was adequate and test-retest reliability was low to moderate (p < 0.05). Validity evidence was found on the convergent construct, with statistically significant correlations between measures of activation, self-esteem, anxiety, depression, and health status. The one-dimensionality of the theoretical model was not confirmed in the adapted version. CONCLUSION: Results have shown that the adapted version is reliable and valid, although the theoretical model cannot be explained in a single dimension.

ABSTRACT Objective: to analyze the factors associated with physical activity levels in adolescents. Method: cross-sectional, quantitative study, conducted with adolescents enrolled in two educational institutions in a capital of the midwest region of Brazil. Data collection occurred from April to October 2021, through a self-administered questionnaire composed of validated scales. The association between the variables was verified by logistic regression. Results: 219 adolescents participated in the study, with a mean age of 15.7 years, more than half were girls (52.5%) from medium economic class (51.1%); 55.3% were very active/active; 39.3% had low self-esteem; 17.8% were at risk for developing an eating disorder; 12.3% at risk for severe anxiety symptoms; and 13.3% at risk for moderate depression symptoms. The factors associated with the level of physical activity were self-esteem and risk for depression. Conclusion: Early screening of adolescents with low self-esteem and risk for symptoms of depression is proposed as an initial strategy to direct actions that encourage the practice of physical activity.


RESUMEN Objetivo: analizar los factores asociados a los niveles de actividad física en adolescentes. Método: estudio transversal, cuantitativo, realizado con adolescentes matriculados en dos instituciones educativas de una capital de la región Centro-Oeste de Brasil. La recolección de datos ocurrió de abril a octubre de 2021, a través de un cuestionario autoadministrado compuesto por escalas validadas. La asociación entre las variables se verificó por regresión logística. Resultados: Participaron del estudio 219 adolescentes, con una edad media de 15,7 años, más de la mitad eran niñas (52,5%) de clase económica media (51,1%); el 55,3% eran muy activos/activos; el 39,3% tenía baja autoestima; el 17,8% tenían riesgo de desarrollar un trastorno alimentario; 12,3% en riesgo de síntomas de ansiedad severa; y 13,3% en riesgo de síntomas de depresión moderada. Los factores asociados al nivel de actividad física fueron la autoestima y el riesgo de depresión. Conclusión: El cribado precoz de adolescentes con baja autoestima y riesgo de síntomas depresivos puede constituir una estrategia inicial para encaminar acciones que incentiven la práctica de actividad física.

Método: estudo transversal, de abordagem quantitativa, realizado com adolescentes matriculados em duas instituições de ensino de uma capital da região Centro-Oeste brasileira. A coleta de dados ocorreu no período de abril a outubro de 2021, por meio de questionário autoaplicado composto por escalas validadas. A associação entre as variáveis foi verificada por meio de regressão logística. Resultados: participaram do estudo 219 adolescentes, com média de idade de 15,7 anos, mais da metade eram meninas (52,5%) e de classe econômica média (51,1%); 55,3% eram muito ativo/ativo; 39,3%, com autoestima baixa; 17,8% apresentaram risco para desenvolver transtorno alimentar; 12,3% risco para sintomas de ansiedade grave; e 13,3%, risco para sintomas de depressão moderada. Os fatores associados ao nível de atividade física foram a autoestima e o risco para depressão. Conclusão: O rastreamento precoce de adolescentes com baixa autoestima e risco para sintomas de depressão pode constituir estratégia inicial para direcionar ações de estímulo à prática de atividade física.

O objetivo foi analisar o Programa Bolsa-Atleta paralímpico no estado de Mato Grosso do Sul, a partir do número de bolsas, modalidades e categorias. Configurou-se como um estudo descritivo-exploratório, com análise de dados na Plataforma de Dados do Inteligência Esportiva, entre os anos 2004-2020. Foram identificados 114 atletas paralímpicos, 77 do gênero masculino e 37 do feminino, contemplados em 12 modalidades paralímpicas, com destaque para o atletismo. Somaram-se 484 bolsas distribuídas entre as categorias do programa, com o predomínio da categoria nacional (61,98%) e estudantil (12,39%). Do total, houve modificação dos atletas entre as categorias, 45,61% mantiveram a bolsa por quatro ou mais anos e 54,39% só tiveram a bolsa entre um e três anos. Conclui-se que o Programa Bolsa-Atleta paralímpico acompanhou os investimentos do governo federal, com o pico de bolsas durante o Plano Brasil Medalhas 2016 e baixa em 2020, em decorrência da pandemia e prioridade governamental. The main goal was to analyze the Paralympic Athletes Scholarship Program at Mato Grosso do Sul state, Brazil, based on the number of scholarships, sports modalities and categories. The research was descriptive exploratory, with data from website Sport Intelligence, between the years 2004-2020. We identified 114 Paralympic athletes, 77 male and 37 female, contemplated in 12 Paralympic modalities, with emphasis on athletics. There were 484 scholarships distributed among the categories, predominance the national category (61.98%) and student category (12.39%). From these athletes, there were modifications from categories, 45.61% kept the scholarship for four years or more and 54.39% had the scholarship between one and three years. We concluded that the Paralympic Athletes Scholarship Program followed the Federal Government budget, with high investment with the Brazilian Medal Plan 2016, and the lower investment in 2020, due to the pandemic and government priority. El objetivo fue analizar el Programa Beca-Atleta paralímpico en el Estado de Mato Grosso do Sul/Brasil, con base en el número de becas, modalidades y categorías. Se configuró como un estudio descriptivo-exploratorio, con análisis de datos en la Plataforma de Datos de Inteligencia Deportiva, entre los años 2004 y 2020. Se identificaron 114 atletas paralímpicos, 77 del género masculino y 37 del femenino, contemplados en 12 modalidades paralímpicas, donde destaca el atletismo. Se encontraron 484 becas distribuidas entre las categorías del programa, con predominio de la categoría nacional (61,98%) y estudiantil (12,39%). El el total, hubo modificación de los atletas entre las categorías, 45,61% mantuvieron la beca por cuatro o más años y 54,39% solo tuvieron la beca por entre uno y tres años. Se concluyó que el Programa Beca-Atleta paralímpico siguió la inversión del Gobierno Federal, con un pico de becas durante el Plan Brasil Medallas 2016 y el menor número de becas en 2020, como consecuencia del período pandémico y de la prioridad gubernamental.
