RESUMO

The authors analyze twenty cases of vascular reconstructions, completed upon management of 118 vascular traumas, over a 10-year period. The traumas were, predominantly, of iatrogenic origin. The following trunks were treated: v. cava inferior, v. mesenterica superior, pelvic, femoral and popliteal veins. In most cases, the reconstruction included a direct vascular suture technique. Furthermore, in addition to the direct vascular suture technique, analogous vein-graft patches or interponates were used. In a single case, a thin-walled PTFE prosthesis was applied. Out of the patient study group, one patient exited as a result of septic complications of his traumas, connected to the v. cava inferior injury. In three cases, femoral and popliteal reconstructions were affected with thrombosis. Non of the reconstructions resulted in a lower extremity amputation.

RESUMO

An isolated aneurysm of the iliac artery is a rare disease occurring 10 times less frequently than an aneurysm of the aorta. Aneurysms of the iliac artery are yet 10 times less frequent. The rare incidence of the condition is the cause why it is not taken into account in the differential diagnosis. Electively treated aneurysms have a minimal mortality and morbidity, while symptomatic aneurysms with subsequent complications due to haemorrhage, penetration or embolization calling for urgent surgery, have a mortality of cca 50%. In the patient described by the authors embolization and ischaemia of the lower extremity developed during the immediate postoperative period after prostatectomy. Only the peroperative finding explained the cause of the embolization.

RESUMO

The authors describe the case of a 73 year old man with obstruction jaundice, who was examined before the operation by PTC with resulting diagnosis of choledochocoele connected with cholelithiasis. The preoperation diagnosis of this rare anomaly is not frequent and is due to the development of radiodiagnostic methods. A surgical solution is based either on discision or removal of the whole formation.

RESUMO

The authors demonstrate in a group of 10 patients treated in the course of 15 years the rare incidence of aggressive fibromatosis of the extremities and the large number of relapses after surgery. They also emphasize the necessity of detailed preoperative examination, priority of a surgical approach, as compared with radiotherapeutic procedures, and the possibility of skeletal and vascular reconstruction after radical surgery.
