[CHANGES OF HISTOSTRUCTURE OF CHRONIC ANAL FISSURE, DEPENDING ON DURATION OF THE DISEASE].

The results of histological investigation of the chronic anal fissure (AF) edges in 95 patients with the disease duration from 6 mo to 4.5 yrs were analyzed. Basing on estimation of severity of an acute inflammation signs and the granulation and cicatricial tissue presence there was established, that duration of an acute inflammation in the AF may constitute up to 6 - 7 mo; subacute--from 8 to 11 mo, chronic cicatricial process was observed in terms from 12 mo and more.

[SURGICAL TREATMENT OF VARICOSE DISEASE, COMPLICATED BY VARICOTHROMBOPHLEBITIS, COMBINED WITH TROPHIC ULCERS].

The comparative analysis of treatment results of 40 patients over the varicose disease of the lower extremities, complicated varicothrombophlebitis, in conjunction with trophic ulcers was conducted. In 22 (55%) patients (I group) performed radical surgery with minimally invasive techniques, in 18 (45%) patients (11 group)--crossectomy followed by conservative therapy. The active tactics and radical surgery varicose disease of the lower extremities, trophic ulcers, and complicated by the varicothrombophlebitis, have led to effective results and improve of patients quality of life.

[Experimental substantiation of possibility of sanation for purulent enteritis in mechanical acute ileus].

In experiment on the model of mechanical acute ileus (AI) in accordance to morphological and microbiological investigations data there was established, that in intestinal wall, situated above the obstacle place, purulent enteritis occurs, what may constitute the origin of purulent-septic complications after elimination of the Al cause. Application of antibacterial preparations permits to slow down the progression of destructive processes in the affected intestinal wall.

[Prophylaxis of purulent complications in mechanical ileus].

In experiment, while simulating an acute ileus, the possibility of antibacterial preparations for prophylaxis of purulent--septic complications was studied. There was established, that while progressing purulent intestinal inflammation its wall already in 12 h losses a capacity to cumulate penicillines and aminoglycosides. In a phlegmon-like changed intestine during 48 h cephalosporins and fluorochinolons are accumulated in bactericidal concentration, making a destruction of intestinal wall and occurrence of purulent peritonitis by 6-12 h slower.

[Hepatic dysfunction correction in patients with obturation jaundice].

Comparative analysis of results of examination and treatment of 54 patients, suffering obturation jaundice syndrome, is presented. The presence and severity of hepatic dysfunction was determined in accordance to indices of cytolysis and cholestasis syndromes, and its severity--in accordance to V. P. Zinevich criteria (1986). There was established, that timely elimination of obturation jaundice, using miniinvasive interventions and subsequent conduction of complex hepatotropic therapy, using L-ornitin L-aspartate, permits to correct hepatic dysfunction effectively.

[Local treatment of operation wounds in patients with acute paraproctitis using preparation distreptase].

The results of local treatment of an acute paraproctitis in 37 patients, using suppositoria Distreptase (Biomed Sara and Vassinas Production Ltd., Lyublin, Poland) were analyzed. There were noted sufficiently high efficacy of treatment, in particular, the necrolysis resolution approaching, the wounds granulation and epithelization beginning as well as reduction of the patients treatment period.

[Characteristics of diagnosis and treatment of patients with an acute intestinal obstruction].

Results of treatment of 1035 patients with an acute intestinal impassability (AII) were analyzed. There were determined advantages of the ultrasonic investigation (USI) in comparison with roentgenological investigation. Diagnostic validity of USI, not depending on the All origin, had constituted 100% and of roentgenological investigation--33-78%. Modern diagnosis and adequate volume of operative intervention had permitted to improve the results of the patients treatment with All. The mortality rate had constituted 3.3% (43 patients died).

[The current possibilities for optimizing the diagnosis of acute cholecystitis]. / Sovremennye vozmozhnosti optimizatsii diagnostiki ostrogo kholetsistita.

Based on an analysis of the material of clinical observations of 1523 patients with acute cholecystitis the authors make a conclusion that findings of repeated echosonography with photometry and endoscopic intraduodenal electrothermometry of the duodenal mucosa allow reliable diagnosis of acute cholecystitis, estimation of the character of morphological alterations of the gallbladder wall, determination of the morphofunctional state of the liver. These methods allow to reliably control the course of the inflammatory process in acute cholecystitis and to timely change the curative tactics.

[Application of ultrasonic investigation in diagnosis of "high" forms of an acute paraproctitis]. / Primenenie ul'trazvukovogo issledovaniia v diagnostike "vysokikh" form ostrogo paraproktita.

Possibilities of ultrasonic investigation (USI) in diagnosis of "high" forms of an acute paraproctitis, ischiorectal and pelviorectal one, were studied. Of 20 patients examined, in 14 the diagnosis difficulties were noted, for correction of which the USI method was applied. The presence of purulent formations in pararectal cellular tissue was confirmed, their localization and spread were determined, permitting to substantiate the rational method of surgical intervention choice.

[The efficacy of the clinical use of the preparation Cifran in treating surgical infection]. / Effektivnost' klinicheskogo primeneniia preparata "Tsifran" v lechenii khirurgicheskoi infektsii.

The results of the use of antibacterial preparation "Cifran" in patients with purulent-inflammatory processes of different genesis and location were studied. A good tolerance of the patients to the preparation was established. High chemotherapeutic effectiveness of the preparation in detection of gram-positive, gram-negative and anaerobic microflora was noted.
