PURPOSE: To identify gestational dietary patterns and evaluate the association between these patterns and the blood pressure (BP) rate of change during pregnancy and the postpartum. METHODS: Prospective cohort study composed of 191 healthy pregnant women. Systolic BP (SBP) and diastolic BP (DBP) were obtained at the 5th-13th, 20th-26th, 30th-36th gestational weeks, and with 30-45 days postpartum. A food frequency questionnaire administered at the 30th-36th gestational week was used to measure dietary intake during pregnancy. Principal component analysis was performed to identify the dietary patterns. A longitudinal linear mixed-effects regression model was used to evaluate the association between the dietary patterns and BP (adjusted for time elapsed after conception and the women's age, education, parity, body mass index and total energy intake). RESULTS: Three gestational dietary patterns were identified: healthy, common-Brazilian and processed. SBP/DBP mean values (SD) were 110.1 (9.0)/66.9 (7.5), 108.7 (9.0)/64.9 (6.7), 111.3 (9.2)/67.0 (6.9) and 115.0 (10.7)/73.7 (8.6) mmHg at the first, second and third gestational trimesters and postpartum, respectively. Women with higher/lower adherence to the processed pattern presented SBP of 117.9 and 113.0 mmHg (P = 0.037), respectively, during postpartum. No association was found between any of the three dietary patterns and SBP in the multiple longitudinal linear regression models, whereas 1 SD increase in the common-Brazilian pattern was associated with a small change of DBP (ß = 0.0006; 95% CI 4.66e-06, 0.001; P = 0.048). CONCLUSION: The three dietary patterns identified revealed no association with changes of SBP and DBP levels during pregnancy and at early postpartum in this sample of healthy Brazilian women.

PURPOSE: The objective of the present study was to investigate whether dietary patterns are associated with excess weight and abdominal obesity among young adults (23-25 years). METHODS: A cross-sectional study was conducted on 2061 participants of a birth cohort from Ribeirão Preto, Brazil, started in 1978-1979. Twenty-seven subjects with caloric intake outside ±3 standard deviation range were excluded, leaving 2034 individuals. Excess weight was defined as body mass index (BMI ≥ 25 kg/m(2)), abdominal obesity as waist circumference (WC > 80 cm for women; >90 cm for men) and waist/hip ratio (WHR > 0.85 for women; >0.90 for men). Poisson regression with robust variance adjustment was used to estimate the prevalence ratio (PR) adjusted for socio-demographic and lifestyle variables. Four dietary patterns were identified by principal component analysis: healthy, traditional Brazilian, bar and energy dense. RESULTS: In the adjusted analysis, the bar pattern was associated with a higher prevalence of excess weight (PR 1.46; 95 % CI 1.23-1.73) and abdominal obesity based on WHR (PR 2.19; 95 % CI 1.59-3.01). The energy-dense pattern was associated with a lower prevalence of excess weight (PR 0.73; 95 % CI 0.61-0.88). Men with greater adherence to the traditional Brazilian pattern showed a lower prevalence of excess weight (PR 0.65; 95 % CI 0.51-0.82), but no association was found for women. There was no association between the healthy pattern and excess weight/abdominal obesity. CONCLUSIONS: In this sample, the bar pattern was associated with higher prevalences of excess weight and abdominal obesity, while the energy-dense (for both genders) and traditional Brazilian (only for men) patterns were associated with lower prevalences of excess weight.

Dietary patterns before pregnancy may be associated with depressive symptomatology during pregnancy. The aim of this study was to identify dietary patterns before pregnancy and to examine the association between these dietary patterns and depressive symptoms during pregnancy. A prospective cohort of 248 healthy pregnant women were followed at 5-13, 20-26, and 30-36 gestational weeks. Dietary intake was obtained by using a food-frequency questionnaire administered between 5 and 13 gestational weeks, which referred to the 6 mo preceding gestation, and factor analysis (principal components) was applied to identify dietary patterns. The Edinburgh Postnatal Depressive Scale (EPDS) was used to evaluate depressive symptoms during 3 follow-up pregnancy points. A multiple linear mixed-effects model was applied to verify the association between dietary patterns and depressive symptoms adjusted for obstetric factors, socioeconomic status, and energy intake. Three prepregnancy dietary patterns were identified: common-Brazilian, healthy, and processed. Together, these patterns explained 36.1% of the total percentage of variance; the eigenvalues were 2.88, 2.12, and 1.86, respectively. Mean depressive symptom scores were 9.0 (95% CI: 8.4, 9.6), 7.2 (95% CI: 6.5, 7.8), and 7.0 (95% CI: 6.4, 7.7) for trimesters 1, 2, and 3, respectively. The rate of decrease in depressive symptoms was -0.088/wk (95% CI: -0.115, -0.061; P < 0.001). In the multiple longitudinal linear regression model, the healthy dietary pattern before pregnancy was inversely associated with depressive symptoms (ß:-0.723; 95% CI: -1.277, -0.169; P = 0.011). High adherence to the healthy pattern before pregnancy was associated with lower EPDS scores during pregnancy in women from Rio de Janeiro, Brazil.

Objetivo: Avaliar a associação entre sintomas de ansiedade gestacional e pós-parto e intenção de amamentar exclusivamente até os seis meses. Métodos Coorte prospectiva com uma população de mulheres atendidas no serviço de obstetrícia de um centro de saúde do município do Rio de Janeiro. Os sintomas de ansiedade foram avaliados pelo Inventário de Ansiedade Traço/Estado no primeiro trimestre da gestação (ansiedade-traço) e 30-45 dias pós-parto (ansiedade-estado). A intenção de amamentar exclusivamente até seis meses foi avaliada por questionário aplicado 30-45 dias pós-parto. Variáveis independentes foram obtidas de questionários padronizados com dados socioeconômicos, obstétricos e nutricionais. As associações brutas e ajustadas foram testadas por regressão de Poisson com variâncias robustas. Resultados: Foram avaliadas 195 mulheres no primeiro trimestre gestacional e 185 no pós-parto. A prevalência de sintomas de ansiedade-traço ao início da gestação foi de 53%, e ansiedade-estado no pós-parto 29%. Cinquenta e nove por cento das mulheres pretendiam amamentar exclusivamente até seis meses. Nas análises ajustadas, a presença de sintomas de ansiedade-traço associou-se com a intenção de não amamentar exclusivamente até os seis meses (RP=1,70; IC95%=1,07­2,72). A presença de sintomas de ansiedade-estado no pós-parto perdeu a associação com o desfecho após os ajustes (RP=1,54; IC95%=0,97­2,44). Conclusões: A presença de sintomas de ansiedade-traço influencia negativamente na intenção de amamentar exclusivamente até os seis meses. Ressalta-se a importância de avaliar a saúde mental da mulher na gestação e pós-parto visando proteger a manutenção do aleitamento materno exclusivo. (AU)


Objective: To evaluate association between symptoms of gestational and postpartum anxiety and intention to breastfeed exclusively until six months of age. Methods: This was a prospective cohort study with a population of women treated at the obstetrics service of a health center in the municipality of Rio de Janeiro. Anxiety symptoms were assessed by the State-Trait Anxiety Inventory in the first trimester of pregnancy (trait anxiety) and 30-45 days postpartum (state anxiety). Intention to breastfeed exclusively up to six months was assessed by a questionnaire administered 30-45 days postpartum. Independent variables were obtained from standardized questionnaires with socioeconomic, obstetric and nutritional data. Crude and adjusted associations were tested by Poisson regression with robust variance. Results: A total of 195 women were evaluated in the first trimester of pregnancy, and 185 were evaluated in the postpartum period. Prevalence of trait anxiety symptoms at the beginning of pregnancy was 53%, while postpartum state anxiety was 29%. Fifty-nine percent of women intended to breastfeed exclusively for up to six months. In the adjusted analyses, presence of trait anxiety symptoms was associated with the intention not to breastfeed exclusively until six months (PR=1.70; 95% CI = 1.07-2.72). Presence of state anxiety symptoms in the postpartum period lost its association with the outcome after adjustments (PR=1.54; 95% CI = 0.97-2.44). Conclusions: Presence of trait anxiety symptoms negatively influences intention to breastfeed exclusively up to six months. Emphasis is placed on the importance of evaluating the mental health of women during pregnancy and postpartum to ensure exclusive breastfeeding is maintained. (AU)

This study aimed to identify dietary patterns and assess their association with abdominal fat. A cross-sectional study was conducted in the Central West Region of Brazil with a probabilistic sample of 208 adults of both sexes. Data on food intake was obtained using a food frequency questionnaire and factor analysis (principal components) was conducted to identify dietary patterns. Waist circumference (WC) and waist-to-hip ratio (WHR) were used to assess abdominal fat. The association between dietary patterns and body fat distribution was examined using multiple linear regression analysis adjusted for confounders. Three dietary patterns were identified: Western, regional traditional, and prudent. A positive association was found between the Western pattern and WC (p = 0.04) and WHR (p = 0.001) and between the regional traditional pattern and WHR (p = 0.05) among women. A slight association was also found between the latter pattern and WC (p = 0.07) also among women. An association was found between the Western and regional traditional dietary patterns and a larger concentration of abdominal fat among women.

Serum lipids have been associated with depression in the adult population; however, this association during pregnancy remains unclear. The aim of this study was to evaluate the association between serum lipids and depressive symptom scores during pregnancy. A prospective cohort of 238 pregnant women was followed at the 5th-13th, 20th-26th and 30th-36th weeks of gestation. Depressive symptoms were assessed using the Edinburgh Postnatal Depression Scale (EPDS). Serum concentrations (mg/dL) of triglycerides, total cholesterol, and low- and high-density lipoproteins (LDL-c; HDL-c) were the main exposures. Marital status (married/single), physical activity (active or very active/low or very low active), unplanned pregnancy (no/yes), pre-pregnancy BMI (<25/≥ 25 kg/m(2)), generalized anxiety disorder (no/yes) and current suicidal ideation (no/yes) were considered as potential confounders. Analyses were performed using linear mixed-effects models. The results showed that the EPDS mean score (95%CI) decreased with time during pregnancy trimesters [1st: 8.89 (95%CI = 8.28-9.51), 2nd: 7.32 (95%CI = 6.67-7.97) and 3rd: 7.08 (95%CI = 6.41-7.74)]. Suicidal ideation frequency at baseline was 18%. HDL-c concentrations were inversely associated with changes in EPDS score (ß = -0.080, 95%CI = -0.157 to -0.002), while low or very low active women (ß = 1.288, 95%CI = 0.630-1.946), with single marital status (ß = 1.348, 95%CI = 0.163-2.534), unplanned pregnancy (ß = 1.922, 95%CI = 0.714-3.131), generalized anxiety disorder (ß = 2.139, 95%CI = 0.410-3.868) and current suicidal ideation (ß = 1.927, 95%CI = 0.596-3.258) tended to have higher EPDS scores. No relationship was observed between other lipids and EPDS scores. HDL-c concentration was inversely associated with changes in depressive symptom scores during pregnancy after adjusting for socio-economic, demographic, behavioral, nutritional, biochemical and mental health disorders.

This study aimed to describe the prevalence of psychiatric disorders and to identify the factors associated with Current Suicide Risk (CSR) in the first trimester of pregnancy. The Mini-International Neuropsychiatric Interview (M.I.N.I.) was employed to diagnose mental disorders in 239 women enrolled in a prospective cohort in Rio de Janeiro, Brazil. Serum lipids, leptin and socio-economic status were the independent variables. CSR, the dependent variable, was entered as binary (yes/no) variable into crude and adjusted Poisson regression models with robust variances. CSR was found to be the main psychiatric syndrome (18.4%), followed by agoraphobia (17.2%), major depressive disorder (15.1%) and generalized anxiety disorder (10.5%). Women with CSR showed higher mean levels of cholesterol (169.2 vs. 159.2; p=0.017), high density lipoprotein (50.4 vs. 47.7; p=0.031) and low density lipoprotein (102.8 vs. 95.6; p=0.022) when compared to women without CSR. The adjusted regression model showed a higher prevalence ratio (PR) of CSR among pregnant women with generalized anxiety disorder (PR=2.70, 95% CI: 1.36-5.37), with ≥ two parturitions (PR=2.46, 95% CI: 1.22-4.93), and with major depressive disorder (PR=2.11, 95% CI: 1.08-4.12). We have shown that generalized anxiety disorder, major depressive disorder and higher parity are associated with CSR in the first trimester of pregnancy.

OBJECTIVE: To identify dietary patterns among adolescents and to assess their association with socioeconomic and lifestyle characteristics and nutritional status. METHODS: A probabilistic clustered two-stage sample of students between 14 and 19 years old from public and private high schools from Cuiabá, Mato Grosso, Brazil, was investigated (n = 1.139). A self-administered food frequency questionnaire was used to obtain information on food intake. Nutritional status was classified according to the Body Mass Index (BMI = weight/height²) z-score. Socioeconomic status was inferred by the education level of head of household and by the presence of electrical appliances in the household. Dietary patterns were identified by applying exploratory factor analysis (extraction method: principal component analysis) and their relationship with socioeconomic conditions, lifestyle variables, and nutritional status was estimated by mutually adjusted multilinear regression models. RESULTS: "Western", "traditional" and "mixed" dietary patterns were identified. Studying in the morning hours and reporting the intake of alcoholic beverages were associated with greater adherence to the "Western" pattern. Male low-income students from public schools who have a BMI in the normal range preferred the "traditional" pattern. The "mixed" pattern was adopted by boys from public schools reporting physical activity. CONCLUSION: Several factors were associated with dietary patterns of adolescents and the Brazilian traditional diet was associated with normal weight.

OBJECTIVE: To investigate the association between socio-demographic factors and dietary patterns in pregnancy. METHODS: Cross-sectional study with baseline data from a cohort of 421 postpartum women aged 18 and 45 years resident in Rio de Janeiro, Brazil. Dietary intake was evaluated with a validated food frequency questionnaire at 15 days following delivery, having as time frame the second and third pregnancy trimesters. Dietary patterns were identified using factor analysis for principal components analysis. The association between socio-demographic factors and the identified dietary patterns was assessed with multiple linear regression analysis. RESULTS: Two dietary patterns were identified: i) healthy: fruits; green vegetables; vegetables; fish; roots, corn and potato; milk and dairy and herbal tea mate, and negatively loadings for alcohol and coffee and ii) mixed: rice; bean; flour and pasta; breads; cake and cookies; soda and juice; sugar and sweets; fatty foods; meats; chicken; and eggs. The linear regression showed that the income (β=0.0002; 95%CI: 0.0002-0.0004) and schooling (β=0.0491; 95%CI: 0.0264-0.0718) were positively associated with healthy pattern, and parity (β=-0.1044; 95%CI: -0.1665- -0.0423) and skin color (β=-0.3102; 95%CI: -0.5256- -0.0947) were negatively associated. Skin color (β=0.1647; 95%CI: 0.0378- 0.2916) and marital status (β=0.1065; 95%CI: 0.0062- 0.2067) were positively associated with mixed pattern and income (β=-0.0001; 95%CI:-0.0002- -0.0001) and schooling (β=-0.0281; 95%CI: -0.0417- -0.0146) were negatively associated. CONCLUSION: Socio-demographic factors such as income, schooling, skin color, marital status and parity were associated with dietary patterns in this sample of postpartum women residents in Rio de Janeiro. .


OBJETIVO: Investigar a associação entre fatores sociodemográficos e padrões de consumo alimentar gestacional. MÉTODOS: Estudo transversal, com dados da linha de base de uma coorte com 421 puérperas entre 18 e 45 anos, residentes no Rio de Janeiro. O consumo alimentar foi avaliado com um questionário de frequência alimentar validado, aplicado aos 15 dias do pós-parto, e teve como referência o segundo e o terceiro trimestres gestacionais. Os padrões alimentares foram identificados a partir da análise fatorial por meio de análise de componentes principais. A associação entre os fatores sociodemográficos e os padrões alimentares foi avaliada com regressão linear múltipla. RESULTADOS: Foram identificados dois padrões alimentares: i) saudável: frutas; folhosos; hortaliças; peixe; raízes, milho e batata; leite e derivados e mate, e, negativamente para o consumo de álcool e café; e ii) misto: arroz; feijão; farinhas e macarrão; pães; bolo e biscoito; refrigerante e suco; açúcares e doces; alimentos gordurosos; carnes; frango e ovos. Na regressão linear, a renda (β=0,0002, IC95%: 0,0002-0,0004) e a escolaridade (β=0,0491, IC95%: 0,0264-0,0718) associaram-se positivamente ao padrão saudável, e a paridade (β=-0,1044, IC95%: -0,1665- -0,0423) e a cor da pele (β=-0,3102, IC95%: -0,5256- -0,0947) associaram-se negativamente. A cor da pele (β=0,1647, IC95%: 0,0378-0,2916) e o estado marital (β=0,1065, IC95%: 0,0062-0,2067) associaram positivamente ao padrão misto; e a renda (β=-0,0001, IC95%: -0,0002- -0,0001) e a escolaridade (β=-0,0281, IC95%: -0,0417- -0,0146) associaram-se negativamente. Este estudo teve o objetivo identificar padrões alimentares e analisar a associação com a adiposidade abdominal. Estudo transversal, realizado na Região Centro-Oeste do Brasil, com amostra probabilística de 208 adultos, de ambos os sexos. O consumo alimentar foi obtido por questionário de frequência alimentar e aplicada análise fatorial (componentes principais) para identificar padrões alimentares. A circunferência da cintura (CC) e a relação cintura quadril (RCQ) foram indicadores de adiposidade abdominal. A associação dos padrões alimentares com adiposidade abdominal foi analisada em modelos de regressão linear múltipla ajustados por fatores de confusão. Identificaram-se três padrões alimentares: ocidental, tradicional regional e prudente. O padrão ocidental associou-se positivamente com CC (p = 0,04) e RCQ (p = 0,001) e o padrão tradicional regional associou-se também com RCQ (p = 0,05), e marginal a associação com a CC (p = 0,07) somente em mulheres. Os padrões ocidental e tradicional regional associaram à maior adiposidade abdominal em mulheres.


Este estudio tuvo como objetivo determinar los patrones alimentarios y analizar su asociación con la adiposidad abdominal. Estudio transversal, realizado en la región centro-oeste de Brasil, con una muestra probabilística de 208 adultos de ambos sexos. El consumo alimentario se obtuvo en un cuestionario de frecuencia alimentaria y para determinar los patrones alimentarios se utilizó un análisis factorial (componentes principales). El perímetro de cintura (PC) y la relación cintura/cadera (RCC) indicaron adiposidad abdominal. La asociación de patrones alimentarios y adiposidad abdominal se analizó en modelos de regresión lineal múltiple, ajustados por factores de confusión. Se identificaron tres patrones alimentarios: occidental, tradicional regional y prudente. This study aimed to identify dietary patterns and assess their association with abdominal fat. A cross-sectional study was conducted in the Central West Region of Brazil with a probabilistic sample of 208 adults of both sexes. Data on food intake was obtained using a food frequency questionnaire and factor analysis (principal components) was conducted to identify dietary patterns. Waist circumference (WC) and waist-to-hip ratio (WHR) were used to assess abdominal fat. The association between dietary patterns and body fat distribution was examined using multiple linear regression analysis adjusted for confounders. Three dietary patterns were identified: Western, regional traditional, and prudent. A positive association was found between the Western pattern and WC (p = 0.04) and WHR (p = 0.001) and between the regional traditional pattern and WHR (p = 0.05) among women. A slight association was also found between the latter pattern and WC (p = 0.07) also among women. An association was found between the Western and regional traditional dietary patterns and a larger concentration of abdominal fat among women.(AU)


Este estudo teve o objetivo identificar padrões alimentares e analisar a associação com a adiposidade abdominal. Estudo transversal, realizado na Região Centro-Oeste do Brasil, com amostra probabilística de 208 adultos, de ambos os sexos. O consumo alimentar foi obtido por questionário de frequência alimentar e aplicada análise fatorial (componentes principais) para identificar padrões alimentares. A circunferência da cintura (CC) e a relação cintura quadril (RCQ) foram indicadores de adiposidade abdominal. A associação dos padrões alimentares com adiposidade abdominal foi analisada em modelos de regressão linear múltipla ajustados por fatores de confusão. Identificaram-se três padrões alimentares: ocidental, tradicional regional e prudente. O padrão ocidental associou-se positivamente com CC (p = 0,04) e RCQ (p = 0,001) e o padrão tradicional regional associou-se também com RCQ (p = 0,05), e marginal a associação com a CC (p = 0,07) somente em mulheres. Os padrões ocidental e tradicional regional associaram à maior adiposidade abdominal em mulheres.(AU)

Estudio transversal, realizado en la región centro-oeste de Brasil, con una muestra probabilística de 208 adultos de ambos sexos. El consumo alimentario se obtuvo en un cuestionario de frecuencia alimentaria y para determinar los patrones alimentarios se utilizó un análisis factorial (componentes principales). El perímetro de cintura (PC) y la relación cintura/cadera (RCC) indicaron adiposidad abdominal. La asociación de patrones alimentarios y adiposidad abdominal se analizó en modelos de regresión lineal múltiple, ajustados por factores de confusión. Se identificaron tres patrones alimentarios: occidental, tradicional regional y prudente. El occidental se asoció positivamente al PC (p = 0,04) y RCC (p = 0,001), el tradicional regional se asoció también a RCC (p = 0,05) y marginal la asociación al PC (p = 0,07), sólo en mujeres. OBJETIVO: Identificar padrões alimentares de adolescentes e sua associação com características socioeconômicas, estilo de vida e estado nutricional. MÉTODOS: Investigou-se amostra probabilística de estudantes entre 14 a 19 anos de idade do ensino médio público e privado de Cuiabá/Mato Grosso (n = 1.139). Os dados foram coletados em sala de aula utilizando-se questionário autoaplicado. Para a obtenção de dados de consumo alimentar, aplicou-se questionário de frequência alimentar semiquantitativo. O estado nutricional foi classificado pelo escore z do Índice de Massa Corporal (IMC = peso/estatura²). Para a classificação socioeconômica considerou-se a escolaridade do chefe da família e a presença de bens e equipamentos no domicílio. Os padrões alimentares foram identificados por meio da análise fatorial exploratória (método de extração: análise de componentes principais) e sua associação com as características socioeconômicas, estilo de vida e estado nutricional foi estimada por modelos de regressão linear múltipla mutuamente ajustados para os outros padrões. RESULTADOS: Foram identificados três padrões alimentares: "ocidental", "tradicional" e "misto". Estudar no turno vespertino e consumir bebidas alcoólicas foram associados (p < 0,05) a maior aderência ao padrão "ocidental". Meninos de escolas públicas, com menor poder aquisitivo e IMC na faixa de normalidade aderiam ao padrão "tradicional". O padrão "misto" foi adotado pelos meninos, estudantes de escolas públicas e que relataram prática de atividade física. CONCLUSÃO: Em adolescentes, vários fatores se associam aos padrões alimentares, mas destacamos que o consumo dos alimentos tradicionais da dieta brasileira associou-se ao IMC na faixa de normalidade.


OBJECTIVE: To identify dietary patterns among adolescents and to assess their association with socioeconomic and lifestyle characteristics and nutritional status. METHODS: A probabilistic clustered two-stage sample of students between 14 and 19 years old from public and private high schools from Cuiabá, Mato Grosso, Brazil, was investigated (n = 1.139). A self-administered food frequency questionnaire was used to obtain information on food intake. Nutritional status was classified according to the Body Mass Index (BMI = weight/height²) z-score. Socioeconomic status was inferred by the education level of head of household and by the presence of electrical appliances in the household. Dietary patterns were identified by applying exploratory factor analysis (extraction method: principal component analysis) and their relationship with socioeconomic conditions, lifestyle variables, and nutritional status was estimated by mutually adjusted multilinear regression models. RESULTS: "Western", "traditional" and "mixed" dietary patterns were identified. Studying in the morning hours and reporting the intake of alcoholic beverages were associated with greater adherence to the "Western" pattern. Male low-income students from public schools who have a BMI in the normal range preferred the "traditional" pattern. The "mixed" pattern was adopted by boys from public schools reporting physical activity. CONCLUSION: Several factors were associated with dietary patterns of adolescents and the Brazilian traditional diet was associated with normal weight.
