RESUMO

Background: Goiter is a common public health issue in Ethiopia; however, there is not much evidence of it among adolescent girls, especially in Northwest Ethiopia. Objective: This study aimed to determine the prevalence and associated factors of goiter in adolescent girls living in the highlands of Northwest Ethiopia. Methods: From 5 January to 15 February 2022, 560 adolescent girls participated in this study. A multistage sampling method was used. Participants were checked for goiter using World Health Organization guidelines. Using a quick test kit, the amount of iodine in samples of table salt was determined. After being entered into Epi Info version 7, the data were exported to the SPSS version 22 for statistical analysis. Binary logistic regression was applied. It was decided to set the statistically significant level at p < 0.05. Results: In this study, the total prevalence of goiter among adolescent girls was 33.4% (95% CI: 26.3%-39.4%). The odds of goiter were 5.3 times higher among adolescents whose families had a history of goiter (AOR = 5.34, 95% CI: 3.65-9.74). The likelihood of developing goiter was 3.7 times greater among adolescent girls who consumed cabbage at least once per week (AOR = 3.74, 95% CI: 2.46-8.31). In addition, adolescent girls who did not consume meat at all were 3.3 times more likely to develop goiter (AOR = 3.32, 95% CI: 1.95-6.78). Similarly, the odds of getting goiter among adolescent girls from families who use salt with inadequate levels of iodine (0-14) were 9.84 times greater compared to their counterparts (AOR = 9.84, 95% CI: 5.61-26.24). Conclusion: In the study area, goiter was very common. Therefore, the district's health sector should make an effort to raise community knowledge by spreading important messages regarding the proper use of iodized salt and food sources.

INTRODUCTION: Helicobacter pylori is one of the most common bacterial infections of humankind that affects more than 50% of the world's population. It has been implicated as an important agent in the pathogenesis of peptic ulcer disease and gastric cancer. Data regarding its prevalence using stool antigen test is scarce in Ethiopia. Hence, the main aim of this study is to determine the prevalence of Helicobacter pylori infection among dyspeptic patients using stool antigen test and assessing the potential risk factors. METHODS: Institution based cross-sectional study was conducted on 373 dyspepsia patients. Data were collected using a pre-tested interviewer administered questionnaire. SPSS Version 23 for Windows software was used for summarization and analyses of data. Bivariate analysis was conducted to detect the association between dependent and independent variables, and all candidate variables were entered into multivariate logistic regression. Statistical significance was set at p-value <0.05. RESULT: More than one-third (34%) of dyspepsia patients were positive for H. pylori stool antigen test. Having greater than or equal to four children in the house [AOR = 7.5 95% CI (1.7, 33.6) p = 0.008)], absence of latrine for the house hold [AOR = 4.3 95% CI (1, 17.8), p = 0.043 and drinking of river water [AOR = 12.5 95% CI (1.5, 105), p = 0.021] were predictors of H-pylori infection. CONCLUSION: More than one-third of dyspepsia patients were positive for H-pylori infection. Overcrowding and poor hygienic conditions are the main risk factors of H-pylori infection.

INTRODUCTION: Lack of sanitation is a major global problem mainly for the poor and disadvantaged. According to the 2016 Ethiopian Demographic and Health Surveys (EDHS) report, one out of every three households lack a toilet in Ethiopia and about 56% of rural households use unimproved toilet facilities. We aimed to determine the magnitude of improved household latrine utilization and its determinants in Ethiopia using the mini-Ethiopian Demographic Health Survey (EDHS) 2019 data set. METHOD: A secondary data analysis was conducted based on the mini 2019 EDHS data set. A total weighted sample of 8663 households were involved in analysis. After selecting the relevant variables for the outcome variable, we have fitted four different models. The null (empty) model with no independent variables and the second model contained the effects of the individual-level factors on the outcome variable. The third model included the influence of the community-level factors on the response variable, and the final multilevel multivariable logistic regression model examined the effects of individual-level and community-level variables on the outcome variable. The measure of variation was quantified using Intra-Class Correlation (ICC), Median Odds Ratio, and Proportional Change in Variance (PCV). The Adjusted Odds Ratio (AOR) with a 95% Confidence Interval (CI) was used to show the strength of association and statistical significance was declared at p value < 0.05. RESULTS: The magnitude of improved latrine utilization in Ethiopia was 19.5% with 95% CI (18.6%, 20.3%). The factors: educational status (AOR = 1.67; 95% CI: (1.10, 2.55), highest wealth index (AOR = 3.73; 95% CI: (2.73, 5.12), urban residence (AOR = 3.09; 95% CI: (1.68, 5.67), living in Addis Ababa (AOR = 4.08; 95% CI: (1.03, 16.2) and Dire Dawa (AOR = 8.22; 95% CI: (2.46, 27.42) and Somali regions (AOR = 3.11; 95% CI: (1.15, 8.42) were significantly associated to improved latrine utilization in Ethiopia. CONCLUSION: The magnitude of improved latrine utilization was quite low in Ethiopia. Higher wealth index, living in more urbanized areas, and the household head's educational status were all significant predictors of improved latrine utilization. The finding implies a need to increase household's access to latrine facilities and improve latrine utilization, particularly for rural households in the country.

INTRODUCTION: Globally, the prevalence of refractive error was 12%, and visual impairment due to refractive error was 2.1%. In sub-Saharan Africa, the prevalence of refractive error and visual impairment due to refractive error was 12.6% and 3.4%, respectively. In Ethiopia, the prevalence of visual impairment due to refractive error varies from 2.5% in the Gurage zone to 12.3% in Hawassa city. Hence, this Meta-analysis aimed to summarize the pooled prevalence of visual impairment due to refractive error in Ethiopia. METHODS: A systematic search of the literature was conducted by the authors to identify all relevant primary studies. All articles on the prevalence of visual impairment due to refractive error in Ethiopia were identified through a literature search. The databases used to search for studies were PubMed, Science Direct, POPLINE, HENARI, Google Scholar, and grey literature was searched on Google until December 15, 2021. In this meta-analysis, the presence of publication bias was evaluated using funnel plots and Begg's tests at a significance level of less than 0.05. The sensitivity analysis was conducted to check for a single study's effect on the overall prevalence of refractive error. RESULT: About 1664 studies were retrieved from initial electronic searches using international databases and google searches. A total number of 20,088 children and adolescents were included in this meta-analysis. The pooled prevalence of visual impairment due to refractive error in Ethiopia using the random effects model was estimated to be 6% (95% CI, 5-7) with a significant level of heterogeneity (I2 = 94.4%; p < 0.001). The pooled prevalence of visual impairment due to refractive was analyzed by subtypes, and pooled prevalence was estimated to be 4%, 5.2%, and 1% for myopia, hyperopia, and astigmatism, respectively. CONCLUSION: The pooled prevalence of visual impairment due to refractive error was high in Ethiopia. About one in twenty-five Ethiopian children and adolescents are affected by visual impairment due to myopia.

Introduction: Musculoskeletal disorders (MSDs), which are the leading cause of workplace health problems; affect workers, employers, and society as a whole following direct and indirect cost. Bankers are expected to perform at computer workstations for long periods of time. As a result, they are at risk of developing musculoskeletal disorders. Hence, this study intends to assess magnitude of work related musculoskeletal disorders and its predictors among computer user bankers in this study area. Material and methods: Institutional based cross-sectional study was conducted from September to December 2021. A total of 422 private and Public own computer user bankers were participated. Simple random sampling technique was used to select the study participants. Binary logistic regression model was used to investigate the relationship between dependent and independent variables. P value less than .05 at 95% CI was considered as a significant association between dependent and independent variables. Result: Overall, 245 (58.8%) of computer-user bankers were suffered work-related musculoskeletal disorders in the previous 12 months. Aged 30 and above [AOR:6.5 (1.7-24.6)], no regular physical exercise [AOR: 6.03 (2.2-16.3)], not received ergonomics training [AOR: 5.46 (2.2-13.1)] and working in awkward posture [AOR: 8.76 (2.9-25.9)] were significantly associated to work related musculoskeletal disorders in the previous 12 months. Conclusion: The magnitude of musculoskeletal disorders among computer user bankers was high. Implementing proper working posture, regular physical exercise, avoiding prolonged use of computer and receiving ergonomics training will reduce the burden of musculoskeletal disorders among computer user bankers. The implications of the findings enhance the need for training supervision, cascading tasks taking the age of workers in to account, sustained supply of work station materials and promoting physical exercise by banks to their workers uninterruptedly.

Background: The exact cause of brain tumours is still unknown, but disruptions of redox balance are thought to play a significant role in all stages of brain tumour development. However, the roles of free radical imbalance at different grades of brain tumour and degree of oxidative stress before and after surgery have not been addressed in prior studies. Aim: A comparative cross-sectional study was conducted to assess the redox imbalance among confirmed brain tumour patients. Methods and results: An institution-based comparative cross-sectional study was conducted on a total of 100 participants (50 brain tumour patients and 50 controls) at referral hospitals in Addis Ababa, Ethiopia. Descriptive statistics, t-test and analysis of variance (ANOVA) (post-hoc) analysis were used and statistical significance was declared at p ≤ 0.05. The serum oxidised glutathione and total oxidative stress were significantly higher in the serum of brain tumour patients (0.72 ± 0.03 µM/µg and 9.66 ± 1.76 µmol H2O2 Eq/L, respectively) compared to the control group (0.21 ± 0.07 µM/µg and 6.59 ± 0.81 µmol H2O2 Eq/L, respectively) (p ≤ 0.05). The serum total oxidant status gradually increased as the tumour grade increased, being higher in grade four (11.96 ± 0.72) and lower in grade one (8.43 ± 1.56), and the mean differences were statistically significant (p ≤ 0 05). A statistically significantly higher total antioxidant capacity (116.78 ± 5.03 Trolox Eq/L) was obtained in the post-surgery than pre-surgery level (79.65 ± 17.914 Trolox Eq/L) (p ≤ 0 05). Conclusion: Higher oxidant and lower antioxidant levels were found in the serum of brain tumour patients than in the control group. The post-surgery oxidant level was lower than the pre-surgery state. The findings of this study could suggest that redox imbalance may have a role in the pathophysiology of brain tumours, but further experimental studies are needed.

BACKGROUND: Sleep is a physiologic necessity for all humankind. Pregnant women, in particular, need adequate sleep to develop their fetuses as well as save energy required for delivery. A change in sleep quality and quantity is the most common phenomena during pregnancy due to mechanical and hormonal factors. However, there is a scarcity of data about poor sleep quality and its associated factors among pregnant mothers in Ethiopia. Therefore, this study aims to determine the prevalence of poor sleep quality and its associated factors among pregnant mothers at Wadila primary hospital, Ethiopia. METHODS: Institution based cross-sectional study design was employed on 411 pregnant mothers. Data were collected using a pre-tested interviewer administered questionnaire. SPSS Version 23 for Windows software was used for data analyses. Bivariate analysis was conducted to detect the association between dependent and independent variables, and to choose candidate variables (p < 0.25) for multivariate logistic regression. Statistical significance was set at p-value <0.05. RESULTS: A total of 411 participants were included in the study making a response rate of 97.4%. Overall, 68.4% of participants found to have poor sleep quality (PSQI>5). Age of the mother [age 20-30 years; AOR = 4.3 CI (1.8, 9.9), p = 0.001, and age >30 years; AOR = 4.7 CI (1.6, 13.9) p = 0.005], gestational age [second trimester, AOR = 2.46, CI (1.2, 4.9), p = 0.01 and third trimester, AOR = 7.5, CI (3.2, 17.8), p = 0.000] and parity [multiparous women; AOR = 2.1(1.24, 3.6) p = 0.006] were predictor variables for poor sleep quality among pregnant mothers. CONCLUSION: More than two-third of pregnant mothers had poor sleep quality. Advanced maternal age, increased gestational age and multiparty are found to be predictors of poor sleep quality in pregnant women.

BACKGROUND: Late presentation for HIV/AIDS care is defined as individuals newly presenting for HIV/AIDS care with a CD4 count below 350 cells/µl or presenting for care with WHO clinical staging of stage III or IV. Globally, around 21.7 million people living with HIV/AIDS were receiving ART in 2017, with an increase of 2.3 million since 2016. Despite this progress, most people start ART late in their disease progression. OBJECTIVE: This study aims to identify predictors of late presentation for HIV/AIDS among people living with HIV and attending ART clinics in West Arsi Zone public health institutions, South Ethiopia, 2019. METHODOLOGY: A facility-based unmatched case-control study was conducted among people living with HIV attending ART clinics in West Arsi Zone public health institutions, with a total sample size of 500 (167 cases and 333 controls). The sample size was calculated using Epi info version 7 and participants were selected using the case-based control selection sampling technique. Descriptive statistics were carried out to summarize the data. Bi-variate binary logistic regression analysis was carried for selecting candidate variables for multivariate binary logistic regression. A p-value of <0.05 was taken to declare the presence of a statistical association between outcomes and explanatory variables. RESULTS: Rural residence (AOR=7.74 95% CI (3.4-17.6)), being single (AOR=0.18 95% CI (0.06-0.49)) symptom(s) at first HIV diagnosis (AOR=7.69 95% CI (4.09-14.4)), no private house (AOR=5.09 95% CI (2.47-10.45)), fear of losing job (AOR=4.12 95% CI (2.04-8.31)), alcohol consumption (AOR=4.35 95% CI (2.18-8.69), and having chronic medical illness (AOR=5.04 95% CI (2.48-10.24)) were identified as having significant associations with late presentation of HIV/AIDS care. CONCLUSION: Rural residence, fear of losing a job, and chronic medical illness were potential risk factors for late presentation of HIV/AIDS care. Being single is the only protective factor for the late presentation of HIV/AIDS care.

BACKGROUND: Coronavirus disease is a highly transmittable and pathogenic viral infection caused by severe acute respiratory syndrome coronavirus 2, which poses therapeutic dilemmas. Some suggestions for drug treatment seem problematic. Beliefs about the causes of health problems are determinants of treatment seeking decisions. Hence, one of the alternatives for the solution of health problems is employing traditional medicine to prevent coronavirus disease 19. OBJECTIVE: To assess the knowledge, attitude, and utilization of drugs toward the coronavirus disease 19 pandemic among Bale zone residents. METHODS: Community-based cross-sectional study was conducted among the Bale Zone population from May 30 to June 30, 2020. Eight hundred fifty-four participants were selected using a single population formula, and a multistage sampling technique was employed. Data were analyzed using SPSS version 25.0. Descriptive and binary logistic regression was used to analyze the outcomes. A p value ⩽ 0.05 was considered statistically significant. RESULTS: Out of the total 854 respondents, about 534 (62.5%) got information about coronavirus disease 19 from TV/radio. More than two-thirds (71.9%) of the respondents have known that severe acute respiratory syndrome coronavirus 2 has no curative drugs, and 65.4% had a more appropriate attitude toward overall drug use in coronavirus disease 19. Level of education and, attitude towards drug use were singnificantly associated with knowledge of drugs, source of information about coronavirus disease 19 and having good practice were significantly associated with atittude toward drug utilization. CONCLUSION: One out of two study participants have good knowledge regarding drugs to the coronavirus disease 19 pandemic. Illiterate people in the community were identified to have poor knowledge about drugs, hence community educuation program is important to improve the knowledge about drugs towards the coronavirus disease 19 pandemic.
