RESUMO

PURPOSE: To compare early changes in the corneal biomechanical parameters after photorefractive keratectomy (PRK) and small incision lenticule extraction (SMILE) and their correlations with corneal shape parameters. METHODS: One hundred twenty four eyes received myopic PRK and SMILE for similar amounts of myopia. Corneal tomography with Pentacam HR, biomechanical parameters using Corvis ST, and Ocular Response Analyzer (ORA) were evaluated before and 2 weeks after surgery. The change in each parameter was compared between groups, while the difference in central corneal thickness and cornea-compensated intraocular pressure measured before and after surgery were considered as covariates. RESULTS: A significant reduction was seen in the corneal stiffness parameter at first applanation, and an increase in deformation amplitude ratio (DAR), and integrated inverse radius (IIR) in both groups after surgery (p < 0.001) Changes in DAR, and IIR were significantly greater in the SMILE than in the PRK group (p < 0.001) Corneal hysteresis (CH) and corneal resistance factor (CRF) decreased in both SMILE and PRK groups after surgery, (p < 0.001) with no statistically significant difference between groups (p > 0.05) Among new Corvis ST parameters, DAR showed a significant correlation with changes in Ambrosio relational thickness in both groups (p < 0.05). CONCLUSIONS: Both techniques caused significant changes in corneal biomechanics in the early postoperative period, with greater elastic changes in the SMILE group compared to the PRK group, likely due to lower tension in the SMILE cap and thinner residual stromal bed in SMILE. There were no differences in viscoelastic changes between them, so the lower CH may reflect the volume of tissue removed.

PURPOSE: The aim of this study was evaluate the effect of governmental support in the form of Health Transformation Plan (HTP) on increasing the cataract surgical rate. METHODS: The number of cataract surgeries was collected from Iranian cataract surgery clinics during 2019. HTP was implemented in 2014. Forty-seven major and forty-five minor surgery centers were selected from all provinces. In each center, sampling was done from 2012, 2013, 2015, and 2016. RESULTS: On average, 6202 and 7134 cataract surgery rate were performed before and after HTP, respectively. The cataract surgical rate rose by 15.03% after the HTP. After HTP, the proportion of cataract surgery increased by 21.32% in governmental centers and decreased by 17.56%, 24.45%, and 14.89% in private, insurance, and charity centers, respectively. The cataract surgical rate was 4093 and 6026 in the first economic quartile (the poorest), 3669 and 4595 in the second quartile, 5884 and 5928 in the third quartile, and 8427 and 9681 in the fourth quartile (the richest) before and after HTP, respectively. The highest growth in the cataract surgical rate was seen in the first quartile (47.24%) followed by the second (25.26%), fourth (14.88%), and third quartiles (0.74%). CONCLUSION: The Health Transformation Plan has been successful in increasing the cataract surgical rate in the low-income group and identifying differences in the services as well as the economic groups within the population.

AIM: To determine the prevalence of ocular trauma in Iranian children aged 6-12 years. METHODS: This population-based cross-sectional study, comprised the first phase of the Shahroud Schoolchildren Eye Cohort Study on primary school children using cluster sampling in urban areas and census in rural areas. The students underwent the measurement of uncorrected and corrected visual acuity as well as non-cycloplegic, cycloplegic, and subjective refraction. The history of trauma, hospitalization, and surgery due to trauma was collected from parents using a questionnaire. RESULTS: The data of the trauma history was recorded for 5267 out of 5620 students. The mean age of the students was 9.7 ± 1.7 years (range: 6-12 years), 53.7% of them were boys, and 79.3% were from urban areas. A positive history of ocular trauma was found in 285 participants, and the lifetime prevalence of ocular injury (95% CI) was 5.2% (4.6-5.9). Blunt trauma was the most common ocular injury with a prevalence of 66.2%. There was a significant positive assocation between ocular trauma and living in rural areas (OR: 1.49, p: 0.012), older age (OR: 1.17, p < 0.001), and male sex (OR: 1.62, p: 0.002). Furthermore, 9.3% and 4.7% of the traumas required hospitalization and surgical intervention, respectively. CONCLUSION: This study found a marked prevalence of ocular trauma compared to previous studies. Male sex, older age, and living in rural areas were associated with ocular trauma, which could be due to differences in lifestyle preference, outdoor exposure, and dangerous situations. Educational programs and safety instructions should be encouraged.

PURPOSE: To determine the age and sex-standardized prevalence of corneal arcus and its associated factors in a geriatric population. METHODS: This population-based cross-sectional study was conducted in 2019 in Tehran; the capital of Iran, using a multi-stage stratified random cluster sampling method. All participants underwent a detailed interview, blood pressure measurement, laboratory blood tests, and a complete ocular examination. RESULTS: Three thousand three hundred ten of 3791 invitees participated in the study (response rate: 87.31%). The mean age of the participants was 69.35 ± 7.62 years (60-97 years) and 1912 (57.76%) were female. Overall, the age and sex-standardized prevalence (95% CI) of corneal arcus was 44.28% (41.21-47.39). Based on the multiple logistic regression, the odds of corneal arcus were higher in men than in women (OR: 1.51; 95% CI: 1.14-2.00); in the age group ≥ 80 years compared to the age group 60-64 years (OR: 2.44; 95% CI: 1.68-3.53), and in retired people compared to employed individuals (OR: 2.05; 95% CI: 1.31-3.21). CONCLUSION: The present study showed a high prevalence of corneal arcus in the geriatric population. Although various studies have reported a significant relationship between corneal arcus with blood lipid and glucose levels as well as blood pressure, these relationships were not found in the present study.

PURPOSE: To determine the distribution of corneal-compensated intraocular pressure (IOPcc), Goldmann-correlated intraocular pressure (IOPg) and their associated factors in a geriatric population. METHODS: The present cross-sectional study was performed in individuals above 60 years of age in Tehran, the capital of Iran. The sampling was performed using multi-stage random cluster sampling method. All participants underwent preliminary ocular examinations, and then imaging by Pentacam HR and IOL master 500. Measurement of IOPg, IOPcc, and corneal biomechanical indices including corneal hysteresis (CH) and corneal resistant factor (CRF) was performed in a random sub-sample using ocular response analyzer (ORA). RESULTS: The mean IOPg and IOPcc were 16.76 ± 4.71 mmHg and 19.05 ± 4.67 mmHg, respectively. There were no statistically significant differences in both IOPg (p = 0.891) and IOPcc (p = 0.248) between males and females. Based on the multiple linear regression models, both IOPg and IOPcc showed a statistically significant direct relationship with CRF (P < 0.001) and a significant inverse relationship with CH (P < 0.001 for IOPg and IOPcc), anterior chamber angle (ACA) (p = 0.006 for IOPg and p = 0.017 for IOPcc), and spherical equivalent refractive error (p = 0.032 for IOPg and p = 0.046 for IOPcc). CONCLUSION: Mean IOPg and IOPcc in the present study were higher compared to most previous studies. Corneal biomechanical indices including CH and CRF, refractive error and anterior chamber angle were independent associated factors of IOPg and IOPcc in the present study. There was no significant relationship between ORA-derived IOP values and CCT.

SIGNIFICANCE: The present study is the first population-based study to examine the prevalence of convergence insufficiency and its associations specifically in the geriatric population. Knowledge of the population-based determination of prevalence of this disorder in the elderly is necessary to support proper clinical diagnosis and management. PURPOSE: This study aimed to determine the prevalence of convergence insufficiency and its associated factors in a geriatric population. METHODS: In this study, all residents older than 60 years in Tehran city were selected through random stratified cluster sampling. All participants underwent a complete ocular examination including the measurement of uncorrected and best-corrected visual acuity, objective and subjective refraction, binocular vision assessments including unilateral and alternating cover tests, measurement of the near point of convergence, the positive fusional vergence, and finally ocular health examination. RESULTS: In this population-based sample of 1793 participants, the overall prevalences of two-sign and three-sign convergence insufficiency were 29.6% (95% confidence interval, 27.2 to 32.0%) and 21.5% (95% confidence interval, 19.5 to 23.6%), respectively. There were no statistically significant differences in the prevalence of both two-sign (P = .19) and three-sign (P = .41) convergence insufficiency between men and women. The highest and lowest prevalences of two-sign and three-sign convergence insufficiency were in the age groups 70 to 74 and 75 to 79 years, respectively. The prevalence showed no significant trend with age (P = .26 for two-sign convergence insufficiency, P = .33 for three-sign convergence insufficiency). In the multiple logistic regression model, none of the variables, including age, sex, and refractive errors, showed a significant relationship with convergence insufficiency (all, P > .05). CONCLUSIONS: The results of the present study showed a high prevalence of convergence insufficiency in the geriatric population. Clinicians should give special attention to this binocular vision disorder in this age group.

SIGNIFICANCE: Fixation disparity tests with various characteristics of fusional stimulus are very important for assessment of decompensated heterophoria. The results suggest that there was no reasonable agreement between the fixation disparity curve's parameters of the modified near Mallett unit and the Sheedy disparometer. PURPOSE: The purpose of this study was to determine the agreement of the fixation disparity curve parameters between the modified near Mallett unit and the Sheedy disparometer in patients with decompensated near heterophoria. METHODS: A total of 147 young adults (mean age, 22.7 ± 4.8 years) participated in this cross-sectional study. After applying the exclusion criteria, the statistical analysis was done on the data of 134 individuals. All participants underwent preliminary optometric examinations including the measurement of uncorrected and best-corrected visual acuity, objective and subjective refraction, and unilateral and alternating cover tests. The fixation disparity was evaluated using the modified near Mallett unit and the Sheedy disparometer at 40 cm, and forced-vergence fixation disparity curves were generated. RESULTS: There were statistically significant differences in the fixation disparity curve parameters (except the center of symmetry) between the two devices. The median fixation disparity measured by the Sheedy disparometer was more positive compared with the modified near Mallett unit (toward more esodisparity or less exodisparity). The median associated phoria measured by the Sheedy disparometer was more positive compared with the Mallett unit. Also, the slope of the curve obtained by the Mallett unit was steeper. The wide limits of agreement indicated the poor agreement of all fixation disparity curve parameters between the two instruments.

PURPOSE: To determine the distribution of the corneal asphericity coefficient (Q value) and related factors in an Iranian geriatric population. METHODS: This population-based study was conducted in 2019 in Tehran, using stratified multistage random cluster sampling. The study population was ≥60 years of age. Participants underwent corneal imaging using a Pentacam HR. Mean keratometry, corneal astigmatism, central corneal thickness, anterior chamber depth and the overall anterior and posterior Q values (for 8 mm chord diameter) were recorded. Axial length measurements were performed using the IOL Master 500. RESULTS: 2457 eyes of 2457 individuals were analysed. The mean age was 67.3 ± 5.82 years and 1479 (60.2%) were female. The mean Q value for the anterior corneal surface was -0.35 ± 0.17 (95% CI: -0.35 to -0.34). The anterior Q value showed a statistically significant inverse relationship with axial length and mean keratometry, and a significant direct association with anterior chamber depth and corneal astigmatism. The mean posterior Q value was -0.41 ± 0.15 (95% CI: -0.42 to -0.40). The posterior Q value had a significant direct relationship with age, anterior chamber depth, mean keratometry and corneal astigmatism. CONCLUSION: The corneal Q values in this geriatric Iranian population were more negative than the values reported in most previous studies. Corneal asphericity was greater affected by ocular biometry and corneal curvature than demographic factors and refractive status.

PURPOSE: To determine the prevalence of Meibomian gland dysfunction (MGD) and its risk factors in an elderly Iranian population METHODS: This cross-sectional study was conducted in 2019. The target population was the subjects aged 60 and over living in Tehran. Multistage cluster sampling was done to select the subjects from all Tehran districts. After an initial interview, all subjects underwent complete ophthalmic examinations including the measurement of visual acuity and refraction as well as slit lamp biomicroscopy to evaluate Meibomian glands. RESULTS: Of 3791 selected subjects, 3310 participated in the study. The data of 3284 participants were analyzed. The mean age of the subjects was 68.24 ± 6.53 years (range: 60-97 years) and 57.8% of them were female. The total prevalence of MGD was 71.2% (68.3-74.1), and 38.1% (8.35-40.4), 30.3% (27.4-33.2), and 2.8% (2-3.6) of the subjects had MGD stage 2, 3, and 4, respectively. The prevalence of MGD was significantly higher in men (p < 0.001) and increased with age from 64.4% in the age group 60-64 years to 82.4% in subjects aged 80 years and over. There was no significant difference in the prevalence of MGD between smokers and nonsmokers; however, severe MGD was more common in smokers. The prevalence of MGD was 76.3% and 68.52% in subjects with and without a history of ocular surgery, respectively. The prevalence of MGD stage 3 and 4 was higher in subjects with a history of ocular surgery. CONCLUSIONS: In line with the results of other studies in Asian countries, this study found a high prevalence of MGD in an elderly population of Iran. It was found that male gender and advanced age were risk factors of MGD and smoking and history of ocular surgery might worsen this disease in MGD patients.

SIGNIFICANCE: Fitting specialty contact lenses (hybrid and miniscleral) can be a useful option in keratoconus patients to decrease higher-order aberrations (HOAs) and increase the quality of vision. PURPOSE: The aim of the present study was to compare corneal HOAs between miniscleral and hybrid lenses in keratoconus patients. METHODS: The target population of this study was 37 patients with bilateral keratoconus aged 20 to 35 years who were referred to a specialized contact lens clinic by a corneal specialist. Pre-fitting examinations included objective and subjective refraction, measurement of uncorrected and best spectacle-corrected visual acuity, and measurement of corneal HOAs. Lens fitting was performed in the next step. Finally, post-fitting measurements including contact lens-corrected visual acuity and corneal HOAs were performed. RESULTS: The root mean square of the total HOAs significantly decreased after fitting both miniscleral and hybrid lens designs. There was a significant change in the third-order vertical coma and spherical aberration after fitting the miniscleral lens. In the hybrid lens group, a significant change was found only in vertical coma after fitting. There was no significant difference in the post-fitting HOA change between the two groups. CONCLUSIONS: The results of this study showed the effectiveness of both miniscleral and hybrid lenses in decreasing HOAs in keratoconus patients. No significant difference was observed between these two lenses in this regard.

SIGNIFICANCE: It is apparent that a variety of biometric changes are caused by different types of cycloplegic eye drops. However, these effects are inconsistent and have not been reported in different refractive groups. PURPOSE: The purpose of this study was to determine the effect of cyclopentolate 1% on ocular biometric components in different types of refractive errors in children. METHODS: This cross-sectional study was conducted on 226 eyes of 113 schoolchildren in Shahroud, northeast Iran, with a mean ± standard deviation age of 9.20 ± 1.65 years. All participants had noncycloplegic and cycloplegic objective refraction using an autorefractometer. Cycloplegia was induced using cyclopentolate 1% eye drops. Biometric measurements were made with Allegro Biograph (WaveLight AG, Erlangen, Germany) before and after administering cycloplegic drops. Mixed-effect model regression was used to analyze the data. RESULTS: After cycloplegia, the vitreous chamber depth (VCD) (-0.043; 95% confidence interval [CI], -0.067 to -0.019 mm), lens thickness (-0.146; 95% CI, -0.175 to -0.117 mm), axial length (-0.009; 95% CI, -0.012 to -0.006 mm), and lens power (-0.335; 95% CI, -0.463 to -0.208 D) decreased significantly, whereas the anterior chamber depth (ACD) (0.183; 95% CI, 0.164 to 0.202 mm), anterior segment length (0.036; 95% CI, 0.014 to 0.058) mm), lens central point (0.109; 95% CI, 0.094 to 0.124 mm), and pupil diameter (1.599; 95% CI, 1.482 to 1.716 mm) increased (P value for all tests, <.001). For changes in VCD and ACD, a significant interaction was observed between different types of refractive errors and cycloplegia, such that the adjusted mean change for ACD was significantly lower and for VCD was significantly higher in hyperopes compared with emmetropes. Lens center moves backward in myopes (0.17 mm) and stays the same in hyperopes under cycloplegia. CONCLUSIONS: According to the findings of this study, cycloplegia reduces the thickness of the crystalline lens and subsequently causes an increase in the ACD. Cycloplegia-related ocular biometric changes were different by type of refractive error.

OBJECTIVES: To determine the distribution of keratoconus indices in normal children 6 to 12 years of age. METHODS: This cross-sectional study was conducted in children living in urban and rural areas of Shahroud, northeast Iran in 2015. After careful optometric examinations, the Pentacam was used for corneal imaging and measurement of keratoconus indices. RESULTS: Of 5,620 students who participated in the study, the data of the right eye of 4,947 children were analyzed after applying the exclusion criteria. The mean age of all children was 9.23±1.71 years (range: 6-12 years). The mean and 95% confidence intervals (in the parentheses) of index of surface variance (ISV), index of vertical asymmetry, keratoconus index (KI), central keratoconus index (CKI), index of height asymmetry, index of height decentration (IHD), and average pachymetric progression index (PPIAve) was 17.770 (17.610-17.930), 0.137 (0.135-0.139), 1.022 (1.022-1.023), 1.010 (1.010-1.010), 3.299 (3.230-3.368), 0.007 (0.007-0.008), and 0.948 (0.943-0.952), respectively. The results of multilevel mixed-effects linear regression analysis showed that ISV, KI, CKI, IHD, and PPIAve were significantly higher in girls. Index of surface variance and CKI were higher in urban students, and PPIAve was higher in rural students. Except for PPIAve, which was increased with increasing age, the mean values of other keratoconus indices were significantly lower in 12-year-old students compared with 6-year-old ones. CONCLUSION: The results provide valuable information about normal distribution of keratoconus indices in children aged 6 to 12 years. These findings can be used in future research and detection of abnormal cases in the clinical setting.

OBJECTIVES: To determine the distribution of corneal topography patterns (CTPs) in children aged 6 to 12 years and its association with the central corneal thickness (CCT), axial length (AL), mean corneal power (MCP), and corneal astigmatism (CA). METHODS: In this population-based study, 5,620 children selected through random stratified cluster sampling were examined. After applying the inclusion criteria, 4,655 right eyes were analyzed. The Pentacam was used to determine the CTP and measure CA and MCP, and the BioGraph was used to measure the AL. Corneal topography patterns were classified based on the 10 patterns proposed by Rabinowitz et al. RESULTS:: The most common pattern in the right eye was the oval pattern [32.7%, 95% confidence interval (CI): 30.9-34.5] followed by asymmetric bowtie with superior steepening (AB-SS) (14.5%, 95% CI: 13.3-15.7), whereas the rarest patterns were AB with skewed radial axis (AB-SRAX) (0.1%, 95% CI: 0.0-0.2) and irregular pattern (0.2%, 95% CI: 0.1-0.4). The similarity between the two eyes was 23.2% (95% CI: 20.9-25.7). The mean CCT was significantly higher in AB-SS than inferior steepening (IS) (P=0.023), symmetric bowtie with SRAX (SB-SRAX) (P=0.030), and AB with IS (AB-IS) (P=0.015). Corneal astigmatism was higher in SB-SRAX compared with round and oval patterns (P value for both <0.001). The AL was greater, and the MCP was lower in the oval versus the SB-SRAX pattern (P value for both <0.001). CONCLUSION: The distribution of CTP was different in children and adults, and the oval and AB-SS patterns were the most common CTPs in the current study.

PURPOSE: The purpose of the present study was to evaluate the prevalence of refractive errors in the Middle East region. METHODS: In this meta-analysis, a structured strategy was applied to search databases PubMed, Web of Science, Scopus, and Google Scholar, databases as well as the reference lists of the selected articles to identify cross-sectional studies assessing the prevalence of refractive errors in the Middle East region until September 2019. The outcome measure was the prevalence of refractive errors, including myopia, hyperopia, and astigmatism, in two age groups of ≤ 15 years and > 15 years. The study results were combined using a random effects model at a confidence level of 95%. RESULTS: The prevalence of myopia, hyperopia, and astigmatism was 4% (95% CI 4, 5), 8% (95% CI 6, 10), and 15% (95% CI 10, 19) in people less than or equal to 15 years and 30% (95% CI 25, 34), 21% (95% CI 15, 28), and 24% (95% CI 16, 31) in subjects over 15 years, respectively. The prevalence of myopia, hyperopia, and astigmatism was 3.5%, 12.4%, and 9.0% in male and 4.2%, 13.1%, and 9.9% in female subjects aged ≤ 15 years, respectively. In subjects aged > 15 years, the prevalence was 31.7%, 14.5%, and 31.5% in males and 31.9%, 11.2%, and 31% in females, respectively. CONCLUSION: The prevalence of hyperopia is relatively high in Middle Eastern children, while the prevalence of myopia is higher in adults in this region. It seems that astigmatism is not a serious refractive problem in this region compared to the rest of the world.

BACKGROUND: Beta thalassemia (ß-thalassemia) is a hereditary disease caused by defective globin synthesis and can be classified into three categories of minor (ß-TMi), intermedia (ß-TI), and major (ß-TM) thalassemia. The aim of our study is to investigate the effects of ß-thalassemia and its treatment methods on different parts of the eye and how early-diagnostic methods of ocular complications in this disorder would prevent further ocular complications in these patients by immediate treatment and diet change. METHODS: We developed a search strategy using a combination of the words Beta thalassemia, Ocular abnormalities, Iron overload, chelation therapy to identify all articles from PubMed, Web of Science, Scopus, and Google Scholar up to December 2018. To find more articles and to ensure that databases were thoroughly searched, the reference lists of selected articles were also reviewed. RESULTS: Complications such as retinopathy, crystalline lens opacification, color vision deficiency, nyctalopia, depressed visual field, reduced visual acuity, reduced contrast sensitivity, amplitude reduction in a-wave and b-wave in Electroretinography (ERG), and decrease in the Arden ratio in Electrooculography (EOG) have all been reported in ß-thalassemia patients undergoing chelation therapy. CONCLUSION: Ocular problems due to ß-thalassemia may be a result of anemia, iron overload in the body tissue, side effects of iron chelators, and the complications of orbital bone marrow expansion.

PURPOSE: To investigate familial aggregation of anterior and posterior corneal curvature, corneal astigmatism, and some corneal topometric indices using the Pentacam. METHODS: Of 3,851 eligible individuals who resided in the selected areas, 2,779 met the conditions for analysis. However, analysis was limited to families whose Pentacam measures were available for at least 2 family members (father or mother and a child), resulting in 1,383 individuals in 382 families. All selected subjects underwent a set of examinations, including refraction, uncorrected and corrected visual acuity measurement, slitlamp biomicroscopy, and Pentacam imaging. Heritability estimation was used to calculate familial aggregation. RESULTS: The results of our study showed a fairly high corneal curvature heritability. The heritability of K2 and K1 in the anterior surface was 58.61% (95% confidence interval [CI], 54.99-62.23) and 55.82% (95% CI, 52.66-58.98), respectively. The heritability of posterior corneal curvature was slightly higher than that of the anterior corneal curvature. The heritability of K2 and K1 in the posterior surface was 63.42% (95% CI, 60.07-66.77) and 59.67% (95% CI, 55.85-63.49), respectively. Investigation of the quantitative topographic corneal indices showed that index of surface variance (ISV), central keratoconus index, and index of vertical asymmetry (IVA) had the highest levels of heritability (81.2% [95% CI, 73.64-88.76], 75.21% [95% CI, 67.19-82.28], and 66.46% [95% CI, 61.99-70.93], respectively). However, keratometric power deviation and index of height asymmetry had the lowest heritability levels (7.48% 95% CI, [4.94-10.02] and 18.31% [95% CI, 16.07-20.55], respectively). CONCLUSION: The familial aggregation and relatively high heritability of the corneal curvature and some keratoconus-related indices, such as ISV and IVA, confirm a high correlation between these phenotypes and genetic factors and warrant further investigation of the genetic mechanisms in keratoconus.

PURPOSE: This study compares the corneal biomechanical properties, as measured with Corvis ST, in three groups of cases with delayed-onset mustard gas keratopathy (DMGK), chronic corneal scarring (CCS), and those with normal corneas. METHODS: Forty-five eyes were enrolled in the DMGK, CCS, and normal groups (15 eyes each). All patients were examined with the slitlamp and Corvis ST. Extracted corneal biomechanical parameters included first and second applanation time (AT1, AT2), first and second applanation length (AL1, AL2), first and second applanation velocity (AV1, AV2), deformation amplitude (DA), highest concavity time, peak distance, highest concavity radius (HCR), central corneal thickness, corrected intraocular pressure (IOP), and noncorrected IOP. RESULTS: The mean age of the participants in the DMGK, CCS, and normal groups was 49.20±5.16, 60.40±22.92, and 44.60±8.32 years, respectively. Comparison between DMGK and CCS groups showed significant differences in AT1, AT2, corrected IOP, and noncorrected IOP. There were also significant differences in AT1, AT2, HCR, DA, and noncorrected IOP between the DMGK and normal groups. None of the parameters were significantly different between the CCS and normal groups. CONCLUSION: There seems to be reduced corneal rigidity and stiffness in cases exposed to mustard gas, which causes the cornea in the DMGK group to become applanated more easily and take longer to rebound compared with the other two groups. Also, IOP was lower than normal in the DMGK group, and thus, misdiagnosis of glaucoma is likely for these patients.

OBJECTIVES: This study aims to determine the diagnostic ability of Pentacam indices for keratoconus and identifying the best index for differentiating diseased from normal cases. METHOD: In this study, 150 keratoconus patients and 150 refractive surgery candidates with a definitive diagnosis of normal healthy corneas were enrolled. Initially, the placido disk topography imaging was performed. The keratoconus and normal corneas were defined based on placido disk topographic data from Rabinowitz-McDonnell. After complete eye examinations for all participants, they underwent Pentacam imaging, and corneal surface topographic indices were extracted. Multiple logistic regression was used to determine the best indices for differentiating diseased from healthy corneas, and the receiver operating curve was calculated to determine the diagnostic capability of each index. RESULTS: Among the studied indices, the keratoconus index (KI), index of vertical asymmetry (IVA), thinnest point (TP), and maximum keratometry (Kmax) were found capable of detecting keratoconus. Among these, IVA was the best index, with an area under curve (AUC) of 95.24%. The best cutoff point for IVA was 0.20 µm, and its sensitivity and specificity were 87.50% and 96.30%, respectively. Comparison of the AUC of different indices showed that only TP and IVA significantly differed (P=0.002). The combination of KI, IVA, Kmax, and TP indices leads to correct detection in 78% of cases. CONCLUSION: Measuring corneal topographic indices using Pentacam can be helpful in the diagnosis of keratoconus. According to the results of this study, IVA is the best diagnostic index for keratoconus. However, it is recommended to use a combination of Pentacam indices for more accurate differentiation of keratoconus from normal cases.

OBJECTIVES: To evaluate the repeatability of anterior chamber depth (ACD) measurements by Orbscan and Pentacam imaging devices in different grades of keratoconus. METHODS: One examiner performed 3 consecutive ACD measurements with both devices on 74 eyes of 42 keratoconus patient. Repeatability was assessed using intrasession test-retest variability. Within-subject SD was determined for repeatability, and the coefficient of variation was calculated for each measurement. The intraclass correlation coefficient (ICC) was also determined to assess the variance of repeated data. RESULTS: Overall, the three ACD measurements were not significantly different either with Pentacam or Orbscan. The ICC index values were greater than 90% with both devices, and it significantly reduced at higher grades of keratoconus with Pentacam. Interdevice differences were statistically significant. The interdevice agreement with ACD measurements was 0.981, and the agreement was lower at higher grades of keratoconus. The 95% limits of agreement between the 2 devices for ACD was from -0.19 to 0.04. CONCLUSIONS: Our results indicate acceptable repeatability for ACD measurements with both Orbscan and Pentacam in keratoconus patients. The progression of keratoconus has no significant effect on repeatability results of these devices. The good agreement between them allows their interchangeable use.

PURPOSE: To determine the distribution of anterior chamber depth (ACD), anterior chamber volume (ACV), and anterior chamber angle (ACA) values in a rural population over 5 years of age using the Pentacam. METHODS: In this cross-sectional study, samples were selected from over 1-year-old inhabitants of two villages in Iran using a multistage cluster sampling approach. All participants underwent the measurement of uncorrected and corrected visual acuity and auto-refraction, retinoscopy, subjective refraction, and slit lamp examination. Finally, corneal imaging was done for all subjects over 5 years of age using the Pentacam. RESULTS: Of 3851 selected individuals, 3314 participated in the study, of whom 2681 met the inclusion criteria. The mean age of the sample was 36.03 ± 18.5 years (range 6-90 years). The mean ACD, ACA, and ACV values were 3.37 mm (95% CI: 3.37-3.39), 34.82° (95% CI: 34.45-35.2), and 159.17 µL (95% CI: 156-161.36), respectively. The results of multiple linear regression models showed that ACD, ACA, and ACV values reduced with age, and ACD and ACV values were significantly higher in males. ACA and ACV values correlated inversely with central corneal thickness, while the ACA value correlated directly with keratometry and inversely with the ACV value. CONCLUSION: This study is one of the few studies in the world showing changes in ACD values in different age groups using the Pentacam. According to the results, aging was associated with a decline in the mean ACD, ACA, and ACV values. These parameters were the largest in patients with emmetropia and smallest in hyperopic subjects.
