RESUMO

INTRODUCTION: Childhood alopecia is a relative rare event in general paediatric dermatology practice. Hair loss in children may have multiple causes, and there are different types of alopecia according to age groups. The aim of the study was to describe the clinical and epidemiological profile of alopecia in children from two Chilean paediatric hospitals. PATIENTS AND METHOD: Descriptive analysis of clinical records of patients from the Dermatology Department of Roberto del Rio and Luis Calvo Mackenna Hospitals between January 2007 and June 2010. Patients with clinical diagnosis of alopecia were included. RESULTS: A total of 345 clinical records were analysed, with 179 males (51.9%). The median age was 72 months. Overall, the most common diagnoses were: alopecia areata (AA), (36.8%), tinea capitis (TC), (21%), nevus sebaceous (13.2%), and tellogen effluvium (8.7%). According to age groups, in newborns, the most common causes were aplasia cutis and nevus sebaceous. In toddlers, pre-school and school children, the principal causes were nevus sebaceous, AA and TC. Trichotillomania was also significant in school children. In adolescents, nevus sebaceous, AA and tellogen effluvium were the most frequent diagnoses. AA was statistically associated with autoimmune disease, thyroid disease, nail disorder, psychiatric disease, and Down's syndrome. The most common aetiological agent in TC was M. canis (86.6%). Trichotillomania was also statistically associated to psychiatric disorders. CONCLUSIONS: In this study, the main causes of alopecia in children were acquired and non-scarring alopecia. In our results, the type of alopecia varies according to age group. Some types of childhood alopecia showed a close correlation to psychiatric disorders.

RESUMO

The use of facemask has become a key element to prevent the infection of Covid-19. Its prolonged use, both, by health personnel and the general population, has caused the appearance of multiple adverse effects at the facial level; highlighting the report of symptoms such as discomfort with the mask, facial itching, redness, erythema, among others. The appearance of dermatological diseases such as acne, allergic and irritative contact dermatitis, and exacerbation of pre-existing pathologies such as acne, rosacea and seborrheic dermatitis are also frequent. Knowledge of these complications is important for their prevention and proper management. (AU)

RESUMO

The first case of infection by the virus SARS-CoV-2 was reported in China in the late 2019. The disease caused by this virus was called COVID-19 and was declared as a global pandemic by World Health Organization in March 2020. Among the consequences caused by the virus, some dermatological pathologies have been reported, such as Telogen effluvium (TE). In this review we will address the relation between Telogen effluvium and COVID-19. Material and methods: All literature related to Telogen effluvium and COVID-19 was searched in PubMed. Results: Since the beginning of the pandemic, the number of patients diagnosed with TE has increased. This is explained on one hand by the infection itself of the virus, generating a proinflammatory and procoagulant answer that lead to TE. On the other hand, it can be explained due to the stress caused by the lockdowns and the psychological consequences of a global pandemic. Also, the therapies used to treat the infection may increase the severity of the TE. The relation between TE and Covid-19 is not totally clear yet, but it is important due to the great impact that generates in the lives of the patients. (AU)

RESUMO

Introduction: Obesity is a growing global health problem; it may even be one of the worst public health issues. In Chile, 34.4% of the population is obese, therefore, is it important for clinicians to be aware of all the consequences of obesity. In this review, we will address the relation of four main dermatologic conditions with obesity: psoriasis, hidradenitis suppurativa, acanthosis nigricans and malignant melanoma. Material and methods: Search in pubmed for obesity and psoriasis, hidradenitis suppurativa, acanthosis nigricans and malignant melanoma. Results: Obesity has a direct impact in the prevalence and severity of psoriasis, hidradenitis suppurativa, acanthosis nigricans. The reduction on body weight has proven to reduce severity of psoriasis and hidradenitis suppurativa. Obese patients with psoriasis have a higher risk on adverse effects due to medication and less effectiveness of biological medications. Acanthosis nigricans is a clinical indicator of insulin resistance and a risk predictor for those with greater risk to develop diabetes in the future. The relationship between obesity and malignant melanoma is not clear and needs further research. Conclusion: Obesity is increasing, dermatologist will face this condition more frequently, it has a great impact over psoriasis, hidradenitis suppurativa, acanthosis nigricans and malignant melanoma, thus it should be considered in treatment decisions. RESUMO

We present the case of a 52 year old woman with a history of bilateral ovarian cancer operated. The patient attended the dermatology unit for a sudden onset of a left plantar nodule of three weeks of evolution, painful on walking, with no other history in particular. At physical examination, the patient presented a surface nodule slightly hyperpigmented, with ill-defined edges, round shape, firm consistency, mobile and painful to compression, one cm of diameter, in the middle third of the left plant. No concomitant scars are seen. Doppler soft tissue ultrasound is requested, showing a granulomatous nodule on the surface of the plantar aponeurosis. The biopsy lesion revealed non-caseating granulomas in cell tissue, PAS and Ziehl-Neelsen staining are negative A left plantar subcutaneous sarcoidosis is diagnosed, systemic involvement is ruled out. Within the first year of follow-up, there are no new lesions or findings compatible with systemic disease. (AU)

RESUMO

Skin cancer rates have increased worldwide despite the current prevention measures. It’s crucial to evaluate people’s knowledge, attitudes, and behaviour regarding sun exposure. Objective: To assess the knowledge and attitudes regarding sun exposure in the workers from primary health care. Method: 223 workers from three Family Care Centres(CESFAM) and from one Primary Emergency Service (SAPU) in the district of Recoleta were surveyed between the months of October and November 2015. Fisher’s exact test and Chi square test were used for statistic analysis. The survey included questions about demographic variables, knowledge of the association between sun radiation/skin cancer and attitudes towards sun protection. Results: the sample consists of 74 men and 149 women, with a mean age of 37 years ±SD 7.9. Phototype III was the most common (41.3percent). The majority of the surveyed declared to be aware of the association between sun exposure and skin cancer (93.3percent). As photoprotecting measures, sunscreens (87.4percent) and seeking shade (71.3percent) were the most frequently measures used. Regarding sunburns, 91.9percent declared to have at least one during their lifetime, 21.5percent occurring during the previous summer. As to the use of sunscreen, 42.1percent referred only using while at the pool / beach and 12.6percent declared not to use it at all. As for health promotion, 29.1percent declared they never recommend the use of sun protection measures in their working practice. Discussion: Although there is adequate knowledge regarding the effects of sun exposure, it doesn’t always reflect on favourable attitudes and photoprotecting measures. There was a significantly greater amount of females with adequate knowledge and use of sun protection measures. This study is one of the first to assess photoprotection behaviour in health care workers.

RESUMO

Phototherapy is the use of specific wavelenghts of light that have shown effectiveness in the treatment of several cutaneous diseases. Current indications for phototherapy in Dermatology include multiple pathologies and it is considered to be costeffective. Literature about its use in children is rare, there are no studies on phototherapy in Chilean children. Patients and methods: Observational, descriptive, retrospective study. Clinical records of pediatric patients treated with phototherapy at University of Chile Clinical Hospital between 2007 and 2015, were studied. For each patient, the following parameters were analyzed: age of starting treatment, sex, skin phenotype, diagnosis, prescribed phototherapy and weekly frecuency. Results: 146 patients, 85 girls and 51 boys, average age 11.7 +/- 4.0 years. Predominant skin phenotype, was Fitzpatrick type III. Vitiligo was identified as the most common indication for phototherapy (52,2 percent), followed by psoriasis (22,6 percent). By a large margin, the most used type of phototherapy was nb UVB (96.6 percent). Most of the patients were treated twice per week (69.2 percent). Discussion: The obtained results in this group are similar to those results described in the international literature. Further studies are required for a better knowledge about efficacy and possible long-term effects of phototherapy on Chilean children.

In this study we analyze the clinical and histopathological profile of acral MM in the population attending at state hospitals, during the periods (1992-2001) and (2005-2009). We analyzed 70 cases of acral MM, 51.4 percent were females, with average age of 66.6 years old. The majority of acral MM were located in soles (97.1 percent). The main clinical diagnosis was malignant melanoma (78.6 percent); however, a 17.1 percent of acral MM showed a different clinical diagnosis. The main clinical type of MM was Nodular Melanoma (31.4 percent). The most frequent histopathological type was invasive melanoma (62.9 percent). The most frequent Breslow thickness was > 1.7 mm (52,9 percent). This study is one of the first analyses of acral MM in the Chilean population, with similar results to the observed in the international literature; however, our patients presented an older average age of diagnosis and showed an important percentage of clinical diagnosis different to Malignant Melanoma.

INTRODUCTION: Skin disorders associated with Parkinson’s disease (PD) are a poorly studied area. OBJECTIVES: To describe skin changes in a group of PD patients and its association with cutaneous functional parameters (sebum and humidity). PATIENTS AND METHODS: Descriptive prospective observational study of PD patients evaluated between February and October 2010. RESULTS: Seborrheic dermatitis (SD) was observed in 9.58 percent and not correlated with seborrhea (p = 0.415), duration (p = 0.338) and severity of PD (p = 0.787). In addition, a 6.84 percent of non-melanoma skin cancer was observed. Sebometry was inversely associated with age and higher doses of L-dopa. Humidity was not associated with time of evolution, severity of PD or motor fluctuations. CONCLUSIONS: The evaluated patients with PD have a higher frequency of non-melanoma skin cancer and SD. However, more studies are needed to establish their real association.

The aging population is a worldwide phenomenon which implies all functions including skin, with pathological prevalence of xerosis, itching, infection, and neoplasia. This study establish the dermatological pathologies most frequently seen in patients 65 years and older in three health centers with distinct demographic, social, and administrative characteristics - and the possible differences between them – through the descriptive analysis of their medical records from April 2010 to April 2011. The results showed a greater prevalence of dermatological pathologies in the first health center, with greater frequency in female patients. No matter the health center, infectious diseases of the skin are the prevalent group. The results concur with other Latin American studies, but differ from studies in developed countries.

BACKGROUND: There is an increase in the incidence of malignant melanoma in Chile. Previous studies have shown that the density and size of acquired melanocytic nevi (AMN) are a risk factor for developing malignant melanoma. AIM: To assess the number and anatomical distribution of acquired melanocytic nevi in Chilean adolescents. MATERIAL AND METHODS: The number of AMN was counted in 30 anatomical sites in 201 Chilean school children (111 females), aged from 11 to 15 years. The number of small AMN (2-4.9 mm) and large AMN (>5 mm of diameter) was determined. RESULTS: Mean AMN number per person was 32.7 (95% confidence interval (CI) 30.7-42.3) in males and 29 (95% CI 271-38.6) in females (p =NS). Males had a higher number of AMN in the face, neck and trunk. Females had a higher number of AMN in the upper and lower limbs. Older children, adolescents with history of sunburns and with lighter skin had a higher number of total, small and large nevi. CONCLUSIONS: The average number of AMN found in this study is similar to that reported in the literature. The differences in site distribution between males and females may be explained by a different sun exposure pattern.

RESUMO

Acral melanoma is an uncommon cutaneous malignant tumour, which may arise on the foot. A 73 years old female patient presented with an ulcerated lesion on the sole of the left foot for one year, associated to an older pigmented lesion. The biopsy of both lesions, showed “in situ cutaneous melanoma” in the pigmented lesion and “infiltrating cutaneous melanoma” in the ulcerated lesion. In this case report is discussed the epidemiology of acral melanoma around the world, showing the importance in early recognition and proper management...

Introduction: Skin cancer incidence rates have increased in the last decades. The purpose of this investigation was to study the incidence rates and the clinical and histopathological pattern of Basal Cell Carcinoma (BCC), Squamous cell Carcinoma (SCC) and Malignant Melanoma (MM) in the National Health Service Hospital of Iquique, Chile. Methodology: 25.338 histopathological reports from the Anatomical Pathology Department were analyzed during the period 2006-2010. 257 skin tumors were found. The variables: sex, age, race, anatomical localization and histopathological types of each tumour were also studied. The statistical analysis was performed by STATA®. Results: 65 percent of BCC, 28 percent SCC, 7 percent of MM were found. The skin cancer incidence rates obtained in our study changed from year to year. BCC (changed between 11.07 and 22.1 / 100.000 inhabitants), SCC ( changed between 5.03 and 9.33/ inhabitants), MM ( changed between 5 and 9 /100.000 inhabitants). Regarding the age, the average age of BCC was 69.8+/- 14.9, of SCC was 76.9+/-10.3 and of MM was 64.7+/-15 years old. Concerning the anatomical location of the tumours, 75 percent of BCC were located in the head; 14 percent in trunk, 3% in lower and upper extremities. In the case of SCC, 32 percent were located in the head, 30 percent in the trunk, and 34 percent in lower and upper extremities. Furthermore, a 38.8 percent of MM percent were located in the lower and upper extremities, 33.3 percent of MM in the head and 11 percent in the trunk. Discussion: In this study, the clinical and histopathological pattern of the cutaneous tumours were similar to those found in other cities of Chile (Santiago, Concepción); however; we found a different anatomical location of SCC in this study, different of national and international results. We observed a higher location of SCC in trunk and extremities. Also, it is important to establish that in this study, we observed a higher percentage of SCC...

Introduction: Basal cell carcinoma (BCC) of the skin is now the most common malignancy in the human population. Its incidence rates are increasing worldwide. Objective: The purpose of this research was to analyze the anatomical location of facial and scalp BCC, per gender and histological aggressiveness. Methodology: A total of 1732 (958 females, 774 males), BCC from 4 state hospitals from Santiago, Chile (2005-2009) was studied. The analyzed subsides were: scalp, forehead, eyelid, eye zone, ears, left cheek, right cheek, middle face and chin. Micronodular, Morpheiform and Metatipical histological subtypes were classified as aggressive and Superficial, Keratotic, Adenoid, Nodular were classified as non-aggressive histological subtypes. The statistical analysis was performed by the Chi square test. Results: We observed a highest frequency of BCC in middle face (42.8 percent) followed by forehead (11.3 percent), eyelid (10 percent), ears (7.5 percent), scalp (4.21 percent) and chin (1.85 percent). Discussion: These results are mainly in agreement with those obtained in other populations reported in the international literature; the BCC distribution in the head is related to ultraviolet radiation exposure of each anatomical site. Regarding the gender, the frequency of BCC was higher in females than in males in all anatomical locations within the face. In the scalp, males showed a higher proportion of BCC. Regarding the aggressitivity of the tumours, no differences within the locations were observed. Also, non-aggressive histological tumors were higher in frequency in all locations within the head. This investigation helps to identify the possible risk zones of BCC within the face and scalp in the Chilean population.

RESUMO

BACKGROUND: A possible relationship has been reported between psoriasis and celiac disease, with common pathogenic mechanisms that may need further investigation. AIM: To investigate the presence of clinical and serological markers for celiac disease in a group of Chilean psoriatic patients. MATERIAL AND METHODS: We included 80 psoriatic patients (42 males) aged 16 to 79 years, whose serum was tested for antitransglutaminase antibodies (ATGA) and antiendomysial antibodies (AEMA). Patients with weakly positive AEMA tests were also tested for antigliadin antibodies (AGA). RESULTS: In six patients (7.5%), AEMA and AGA were positive and one patient was positive for ATGA. An upper gastrointestinal endoscopy and duodenal biopsy was offered to these six patients and five accepted the procedure. Only one had a pathological diagnosis of celiac disease. CONCLUSIONS: Only one of 80 patients with psoriasis had celiac disease (1.2%). Other four patients with positive serologic markers had a normal duodenal biopsy. This group of patients may have latent celiac disease and they should be followed up.

RESUMO

Cutaneous squamous cell carcinoma (SCC) is the second most common skin cancer; its incidence is increasing in the last decades worldwide. The purpose of this study was to analyze 1320SCC (716 males and 604 females) from five hospitals of Santiago. The tumours were studied by gender, anatomical localization and age of the patient. For this, 600.000 histopathological reports were reviewed (1992-2001). SCC was more frequent in males than females and also more frequent in older people. The average age was 72.1 +/-14.3 years old in females and 69.5+/-13.2 years old in males. The most frequent anatomical locations in females were the face (51.2 percent), lower extremities (13.9 per cent) and genitals (12.1 percent). The most frequent anatomical locations in males were the face (62.4 percent); genitals (9.8 percent) and lower extremities (8.1 percent ). More number of facial SCC was observed in males than in females; however; more SCC in lower extremities was observed in females than in males. In relation to age, CEC was more frequent in males in the 50-59 and 70-79 years old groups, however; CEC was more frequent in females older than 80 years old. The study of 1320 SCC tumours allows us to analyze risk groups in the Chilean population that help to direct future skin cancer prevention campaigns in Chile.

RESUMO

At the beginning of the twentieth century, Dermatology’s teaching was more complex than today, heavily based on fine semiology. During the last centuries, the art of moulage preparation evolved from 2-dimensional images in black and white to 3-dimensional figures with color andrelief, realistic representations of diseased parts of the human body, essential in a morphological and descriptive discipline. The technique of wax figures in Dermatology began in the nineteenth century in France, at St. Louis Hospital. Subsequently, Olavide created a wax museum at San Juan de Dios Hospital in Spain. In Chile, wax moulages of syphilis and other sexually transmitted diseases were used for the teaching of Dermatology by the National Health Service. The moulages were carved by Mrs. Anita Lagarrigue and designed by Professor Dr Luis Prunés (Chair of Dermatology at Saint Vincent de Paul Hospital) in the 1940’s. Until today, we have found 15 wax moulages, partially preserved in wooden and glass boxes in the Dermatology Department at the Clinical Hospital of the University of Chile (former Saint Vincent de Paul Hospital). This wax figures collection represents a significant work of art and reflects a veryimportant part of the history of Chilean Dermatology.
