This study aims to assess the evidence on adverse pregnancy outcome associated with living close to polluted industrial sites, and identify the strengths and weaknesses of published epidemiological studies. A systematic literature search has been performed on all epidemiological studies published in developed countries since 1990, on the association between residential proximity to industrial sites (hazardous waste sites, industrial facilities and landfill sites) and adverse pregnancy outcome (low birth weight, preterm birth, small for gestational age, intrauterine growth retardation, infant mortality, congenital malformation). Based on 41 papers, our review reveals an excess risk of reproductive morbidity. However, no studies show significant excess risk of mortality including fetal death, neonatal or infant mortality and stillbirth. All published studies tend to show an increased risk of congenital abnormalities, yet not all are statistically significant. All but two of these studies revealed an excess risk of low birth weight. Results for preterm birth, small for gestational age and intrauterine growth retardation show the same pattern. There is suggestive evidence from the post-1990 literature that residential proximity to polluted sites (including landfills, hazardous waste sites and industrial facilities) might contribute to adverse reproductive outcomes, especially congenital malformations and low birth weight-though not mortality. This body of evidence has limitations that impede the formulation of firm conclusions, and new, well-focused studies are called for. The review findings suggest that continued strengthening of rules governing industrial emissions as well as industrial waste management and improved land use planning are needed.

In the Russian Federation anthrax epizootics are still being registered among animals as well as epidemic foci of the population. This situation is linked to natural reservoirs of the pathogen - numerous anthrax burial sites which belong to class I of dangerous objects. In this connection, a one-kilometer sanitary protective zone is required according to current Russian Federation legislation. As a result, a significant land of the country is unsuitable for any agricultural use. Meanwhile, epizootologo-epidemiological observations indicate to that different anthrax burial sites differ in their characteristics and represent varying degrees of the risk. In connection with the development of the agricultural sector, intensive construction and the development of new and abandoned areas there is a need of creating unified approaches to assess the risk of anthrax burial sites, as well as to determine the size of sanitary protection zones based on the risk assessment. This article represents an original methodology to assess the actual danger of anthrax burial sites. It is based on a comprehensive multi-factor quantity-related risk assessment, described by a model that accounting the importance of each study for natural, social and biological factors. Undertaking this methodology allowed to reveal a degree of danger of anthrax burial sites located in different territories of the Russian Federation, and helped to substantiate the dimensions of their sanitary protection zones.

Over the past few decades, zinc smelting activities in Guizhou, China have produced numerous slag dumps, which are often dispersed on roadsides and hill slopes throughout the region. During periods of acid rain, these exposed slags release heavy metals into surface water bodies. A column leaching study was designed to test the potential release of the heavy metals cadmium (Cd), chromium (Cr), copper (Cu), lead (Pb), and zinc (Zn) under simulated acid rain events. Two slags with varying environmental exposure periods were packed in columns and subjected to leaching solutions of pH 3.5, 5.5, or DI H2O at intervals of 1, 7, 14, 28, 56d. Pulse concentrations of Cd in leachate were found above 5µg/L, Cr, Pb, and Zn >10µg/L, whereas, Cu reached 10µg/L. After five leaching events, the leachability (percentage of cumulative heavy metal leached after five leaching events as in its respective total concentration in slags) of Cd was 0.05 percent and 0.035 percent from the old and young slag, respectively. Cr (0.035 percent and 0.05 percent) was greater than Cu (0.002 percent and 0.005 percent) and Zn (0.006 percent and 0.003 percent), while the lowest leachability was observed for Pb (0.0005 percent and 0.0002 percent) from the old and young slags, respectively. Reaction rates (release amount of heavy metals in certain period of leaching) of heavy metals in the leachates demonstrated the sequence of Zn>Cr>Cd, Cu>Pb. Leaching release of heavy metals was jointly affected by the pH of leaching solution and mineral composition of slags (including chemical forms of Cd, Cr, Cu, Pb, and Zn). Environmental exposure period of slags, resulting in the alteration of minerals, could affect the release process of heavy metals in leaching as well.

The Waste Management Department of Nuclear and Energy Research Institute (IPEN) is responsible for the safety management of the waste generated at all internal research centers and that of other waste producers such as industry, medical facilities, and universities in Brazil. These waste materials, after treatment, are placed in an interim storage facility. Among them are (226)Ra needles used in radiotherapy, siliceous cake arising from conversion processes, and several other classes of waste from the nuclear fuel cycle, which contain Ra-226 producing (222)Rn gas daughter.In order to estimate the effective dose for workers due to radon inhalation, the radon concentration at the storage facility has been assessed within this study. Radon measurements have been carried out through the passive method with solid-state nuclear track detectors (CR-39) over a period of nine months, changing detectors every month in order to determine the long-term average levels of indoor radon concentrations. The radon concentration results, covering the period from June 2012 to March 2013, varied from 0.55 ± 0.05 to 5.19 ± 0.45 kBq m(-3). The effective dose due to (222)Rn inhalation was further assessed following ICRP Publication 65.

Radioactive waste containing a few grams of plutonium (Pu) was disposed between 1960 and 1968 in trenches at the Little Forest Burial Ground (LFBG), near Sydney, Australia. A water sampling point installed in a former trench has enabled the radionuclide content of trench water and the response of the water level to rainfall to be studied. The trench water contains readily measurable Pu activity (~12 Bq/L of (239+240)Pu in 0.45 µm-filtered water), and there is an associated contamination of Pu in surface soils. The highest (239+240)Pu soil activity was 829 Bq/kg in a shallow sample (0-1 cm depth) near the trench sampling point. Away from the trenches, the elevated concentrations of Pu in surface soils extend for tens of meters down-slope. The broader contamination may be partly attributable to dispersion events in the first decade after disposal, after which a layer of soil was added above the trenched area. Since this time, further Pu contamination has occurred near the trench-sampler within this added layer. The water level in the trench-sampler responds quickly to rainfall and intermittently reaches the surface, hence the Pu dispersion is attributed to saturation and overflow of the trenches during extreme rainfall events, referred to as the 'bathtub' effect.

RESUMO

This paper brings understanding to the ethical dimensions of human health research conducted in the context of contaminated sites. Principle-based ethical analysis, complemented with virtuous traits of character, aid in bringing clarity to recommendations for actions following research. Epidemiology is the discipline for conducting health research not only because of its methodological foundations, but also because of its social justice focus. Because contaminated sites include communities that have been exposed to excessive concentrations of hazardous substances, extra care is needed when using epidemiology. For instance, vigilance over potential influence and engagement with affected communities are needed. Community engagement not only aids in understanding the contextual framework, but also demonstrates respect for both community and individual autonomy. Ethical analysis makes transparent the rationale for decisions against which researchers can be held accountable and provides a basis for evaluating observed outcomes as a function of the rationale provided for past actions.

BACKGROUND: Exposure to heavy metals at contaminated industrial and mining sites, known also as hot spots, is a significant source of toxic exposure and adverse health outcomes in countries around the world. The Toxic Sites Identification Program (TSIP) developed by Pure Earth, a New York-based nongovernmental organization, is the only systematic effort to catalogue contaminated sites globally. To date, TSIP has identified and catalogued 3282 sites in low- and middle-income countries. The TSIP methodology is not designed to survey all contaminated sites in a country. Rather sites are prioritized based on their perceived impact on human health, and only a limited number of the most highly hazardous sites are surveyed. The total number of contaminated sites globally and the fraction of contaminated sites captured by TSIP is not known. OBJECTIVE: To determine the TSIP site capture rate, the fraction of contaminated sites in a country catalogued by TSIP. METHODS: Ghana was selected for this analysis because it is a rapidly industrializing lower middle income country with a heterogeneous industrial base, a highly urban population (51%), and good public records systems. To develop an estimate of the fraction of sites in Ghana captured by TSIP, assessors targeted randomly selected geographic quadrats for comprehensive assessment using area and population statistics from the Ghana Statistical Service. Investigators physically walked all accessible streets in each quadrat to visually identify all sites. Visual identification was supplemented by field-based confirmation with portable x-ray fluorescence instruments to test soils for metals. To extrapolate from survey findings to develop a range of estimates for the entire country, the investigators used 2 methodologies: a "bottom-up" approach that first estimated the number of waste sites in each region and then summed these regional subtotals to develop a total national estimate; and a "top-down" method that estimated the total number of sites in Ghana and then allocated these sites to each region. Both methods used cluster random sampling principles. FINDINGS: The investigators identified 72 sites in the sampled quadrats. Extrapolating from these findings to the entire country, the first methodology estimated that there are 1561 sites contaminated by heavy metals in Ghana (confidence interval [CI]: 1134-1987), whereas the second estimated 1944 sites (CI: 812-3075). The estimated total number of contaminated sites in Ghana is thus 7-9 times the number of sites captured through TSIP. On a population basis, it was estimated that there are between 31 and 115 contaminated sites per million inhabitants in Ghana. CONCLUSIONS: The findings of this study indicate that the TSIP methodology provides a sound statistical basis for policy formulation. The statistical approaches used in this study can be replicated in other countries to improve estimates of the prevalence of contaminated sites. This information provides important input to calculations of the global burden of disease attributable to hazardous exposures at contaminated sites.
