RESUMEN

AIM: To evaluate bone tissue reactions in rats to an MTA-based endodontic sealer with and without the addition of various concentrations of C3A or C3A + Ag. METHODOLOGY: Bone tissue reactions were evaluated in 45 Wistar rats after 7, 30 and 90 days (n = 5 per period). Three surgical cavities were prepared on the right femur and filled with 0.2 mL MTA Fillapex, MTA Fillapex + C3A and C3A + Ag at various concentrations: AH Plus (Dentsply DeTrey GmbH, Konstanz, Germany), EndoSequence BC (Brasseler USA, Savannah, GA, USA) or no sealer (negative control). By the end of each experimental period, animals were randomly euthanized. The samples were histologically processed and analysed using a light microscope. The presence of inflammatory cells, fibres and hard tissue barrier formation was evaluated. Data were analysed statistically using nonparametric tests to compare the differences between groups. Multiple groups were compared using the Kruskal-Wallis and Mann-Whitney U-tests with a Bonferroni correction at P = 0.05. RESULTS: The inflammatory response significantly decreased from 30 to 90 days (P < 0.05). Fibre condensation was similar amongst the groups at 07 and 30 days after intervention (P > 0.05). At 90 days, however, fibres were absent in most specimens of EndoSequence BC Sealer, AH Plus, MTA Fillapex and the control group, whilst they were still observed in samples of the modified sealers (P < 0.05). At 90 days, all specimens of AH Plus, EndoSequence BC Sealer and control group had complete formation of hard tissue barrier. In the MTA Fillapex group, as well as in the modified sealers groups, partial deposition of mineralized tissue was noticed. CONCLUSION: The hypothesis tested that the incorporation of C3A and C3A + Ag particles to MTA Fillapex would improve bone tissue repair was partially accepted, since modified MTA Fillapex did not have the same repair potential as the commercial bioceramic material.

OBJECTIVE: To evaluate the presence of x ray changes in the proximal humerus growth plate of 21 young pitchers. DESIGN: Case series. SETTING: This study was conducted at the Orthopaedic and Traumatology Department of the ABC College of Medicine, Santo André, São Paulo, Brazil, tertiary institution. PATIENTS: 21 male adolescent baseball pitchers, with a mean age of 14.5 years and selected from the Brazilian National Team, were studied. INTERVENTIONS: The patients underwent x ray examinations of the shoulders. MAIN OUTCOME MEASUREMENTS: The x rays of the pitchers' shoulders were studied to determine any changes in the proximal humeral growth plate and correlate them with clinical findings. RESULTS: Fourteen athletes (66%) had radiographic changes in the humerus growth plate but only 5 (36%) had pain complaints. Nine of these 14 cases (64%) whom the observers found radiographic changes did not complain of pain. It was also observed that there was a correlation in only 11 cases between clinical and radiographic evaluations. CONCLUSION: Radiographic changes in the proximal humerus growth plate were found in 66% of the cases, but 64% were asymptomatic. These changes indicate possible evolution, in the future, to a greater degree of retroversion in pitchers' dominant shoulders.

OBJECTIVES: To investigate the long term setting reaction in the glass ionomer restorative, Fuji IX, by monitoring the structural evolution of aluminium and fluorine species using (27)Al and (19)F MAS-NMR spectroscopy. METHODS: Fuji IX cements aged from 5min up to 3 months were prepared according to the manufacturer instructions from the commercial material. The (27)Al and (19)F MAS-NMR studies were carried out on powders made after terminating the setting reaction. RESULTS: (27)Al MAS-NMR results show conversion of aluminium from the glass phase, where it has coordination number four, Al(IV), into the cement matrix where it has a coordination number of six, Al(VI). At least two different Al(VI) species were detected at short ageing time cements. Assignment of these species is discussed and compared with the data from other sources. The possibility for a condensed aluminium species [Al(13)(OH)(24)(H(2)O)(12)](7+) to form is considered. The ratio of aluminium in the cement, Al(VI), to the remaining unreacted in glass has been evaluated by deconvolution of the spectra. Various theoretical ratios of aluminium species in the cement matrix to the unreacted ones remaining in glass have been estimated. The (19)F MAS-NMR spectra are identical for the glass and cements at the early times and contain a dominant signal assigned to Al-F-Sr(n). CONCLUSIONS: The data confirms that the conversion of aluminium is a diffusion-controlled process at early stage less than 1h and it is largely complete between 1 and 6h. The comparison with the experimental data shows that the majority of aluminium cations do not form tricarboxylates but are coordinated with one or two carboxylic groups and other ligands. Insufficient amount of water and excess of glass in this cement formulation affect glass degradation mechanism.
