RESUMEN

An orbital teratoma with extension into the maxillary sinus, pterygopalatine fossa nasal cavity, and middle cranial fossa is presented. Through combined efforts of the services of Ophthalmology, Otolaryngology, and Neurosurgery, a tumor that was 4.5 cm in greatest diameter was removed, leaving the globe and optic nerve within the orbit. Within the 2 1/2-year period since surgery, the condition of the patient has been satisfactory.

Among the factors that determine the behavior of an intracranial aneurysm is the relationship between its volume and the size of the orifice. The investigative method described herein is the means being used to define that relationship in humans. It is a postmortem study that focuses on unruptured aneurysms. Central to the protocol was a synthetic rubber cast of the aneurysm's interior. The cast was made under normal arterial pressure so that unruptured aneurysms were reexpanded to lifelike size and shape. After the cast was removed intact from the specimen, the lumenal features recorded upon it were verified by comparison with the opened aneurysm. Working now only with the cast, the chamber was cut from the artery through its neck. The orifice area was determined by dividing it, mathematically, into many smaller, measurable forms. Chamber volume was ascertained by a fluid displacement technique. Both measurements were made with magnification and engineering instruments. Casts of ruptured or thrombosed aneurysms gave helpful morphological information, but were of limited value for measurement. The techniques are described, and examples of the initial results are presented.

Saccular aneurysm associated with segmental duplication (also called "fenestration") of the basilar artery is an anomaly that results from an embryonic fault. Reports of the treatment of the aneurysmal component have only recently appeared in the neurosurgical literature, and little has been written on the morphology of this anomaly. This study answers the need for information about its structure to the extent permitted by the examination of a single specimen. The specimen was obtained at postmortem examination. A cast of its interior features was made with a synthetic rubber. After the cast was removed, the entire anomaly was serially sectioned for histological study. Defects in the wall of the basilar artery were seen microscopically at each end of the fenestration. At the extensive proximal defect, a saccular aneurysm arose that bulged into the window between the two limbs of the segmental duplication and also presented dorsally and ventrally. It had fatally ruptured. The manner in which the fenestration was formed by intraluminal septa was also revealed.

RESUMEN

This is a report of an atypical fibrous histiocytoma in the skull of an infant, who at the age of 3 months was noted to have a "lump" beneath the scalp in the right parietal region. It was about 2 cm in diameter, and the scalp was movable over it. Physical examination was otherwise normal. Radiographs showed erosion of the scalp deep on the palpable mass. At operation a neoplasm was found, which had destroyed the bone and invaded the adjacent temporal muscle and dura mater. The tumor was removed en bloc after the surrounding bone had been excised. Histological examination using light and electron microscopy revealed the tumor to be an atypical fibrous histiocytoma. Radiation therapy was not given. There has been no recurrence for 7 years.

RESUMEN

Aluminum was used to reconstruct the supraorbital ridge and adjacent frontal bone in six patients. Its malleability makes it particularly useful for this purpose. Each cranioplasty was done several months after injury. The longest follow-up is 19 years and the shortest three years. There were no complications. The results are satisfactory.

RESUMEN

The mixed-lymphocyte reaction reactivity of normal and tumor-bearing host (TBH) T-cell subsets was examined in response to normal and TBH macrophage (M phi) supernatants. Both inhibiting and enhancing activities were identified in normal and TBH M phi supernatants. The present data suggest that TBH M phi supernatants contained more inhibitory activity than normal host M phi supernatants and that enhancing activity of M phi supernatants was restricted to the Lyt 2,3+ population of cells. TBH Lyt 2,3+ cells were more responsive to the enhancing molecule(s) than their normal counterparts. These data were consistent with studies which implicate M phi as being partially responsible for the immune dysfunction seen in TBH, and extends previous findings on the ability of M phi to regulate the immune response in an attempt to achieve homeostasis.
