RESUMEN

Management of parturients with a history of Fontan procedure requires careful monitoring of cardiovascular parameters and anticipation of potential complications. We describe potential pitfalls in a parturient with atriopulmonary Fontan circulation, who received epidural analgesia for labor and who later required emergency cesarean section. Low-dose local anesthetic in combination with meperidine provided excellent perioperative epidural analgesia and cardiovascular stability. Epidural analgesia during labor offered optimal pain relief and facilitated conversion to surgical anesthesia for an emergency cesarean section.

RESUMEN

Examination of 20 individuals without any pathological conditions of the cardiovascular system and 8 patients with myocardial hypertrophy has shown that the hypocalciemic hormone of the thyroid gland thyrocalcitonin has a certain negative inotropic effect and does not possess chrono- and dromotropic affects. The mechanism of the cardiac effect of thyrocalcitonin is evidently due to its effect on the metabolism of calcium and its transportation in the myocardial structures.

In male rats, i. m. administration of thyrocalcitonin (TCT)--2 and 10 units/kg for 1, 5, 10 and 15 days, reduced calcium level in the blood serum. In the TCT-treated rats the glucose-tolerance curves after i. p. injection of glucose became diabetoid-like. TCT inhibited the increase of insulin-like activity of the serum after glucose injection. The data suggest that TCT, lowering the calcium content of blood serum, inhibited the glucose-induced secretion of insulin.

RESUMEN

Injection of parathormone and thyrocalcitonin into rats with DMBA induced sarcomas resulted in a considerable stimulation of the metastasization process. It was found that the level of total Ca++ in tumor tissue was reduced under the influence of PTH. Under conditions of chronic exogenous hyperparathyroidism autoimmunereactions were suppressed. A prolonged administration of TCT did not alter considerable the total Ca++ content in tumor and markedly activated the autoantibody production in immunocompetent cells.

RESUMEN

A distinct effect on all the stages of regenerating bone mineralisation was observed after daily (within the experimental period) intramuscular administration of thyrocalcitonine with prolonged action (a complex with polyvinyl pyrrolidone) into male rat at a dose 2.5 un/kg of body weight. The drug caused an inhibitory effect on metabolism within the first 10 days of the experiment. Within 2-3 weeks after the fracture the intensity of metabolic processes was increased. Contents of calcium, phosphorus, sodium and potassium were markedly increased in liquid phase of the bone rissue. The synthesis of phosphoproteins, organic non-protein phosphates and the inorganic phosphate was also increased. The data obtained suggest that thyrocalcitonine caused a stimulating effect on aerobic resynthesis of macroergic phosphate containing substances and the increase in the functional activity of osteogenic cells.
