RESUMEN

Data are presented on a series of 39 stomach cancer patients subjected to preoperative assays of blood CEA and GICA. High levels of both markers were found in the advanced (stage III and IV) cases. The use of both markers together improved the sensitivity of the test but only to 30%. Nevertheless preoperative CEA and GICA assays are recommended because of the information they provide about tumor stage.

The Authors report a series of 386 non-selected cases of cervical adenopathies with systemic (23.1%) and secondary (76.9%) tumors, treated using both simple biopsy and radical surgery. The predominant sites were right (29.0%) and left (23.6%) laterocervical. Histological tests revealed that in relation to metastatic adenopathies the predominant histotype in the series as a whole was squamous carcinoma (29.5%). Within the scope of metastatic adenopathies, tumors of the oral cavity or salivary glands, if taken together, account for the highest percentage of incidence: 14.2%. Three points emerge from an analysis of the data: 1) in the presence of lymph node tumors of the cervical district, taken as a whole, approximately 21.8% may be attributed to tumors of the oral cavity and salivary glands; 2) in the context of metastatic adenopathies, the predominant histotype is squamous carcinoma, 38.4%; 3) the presence of a high percentage of metastatic adenopathies of the neck due to cancer of the oral cavity and adjacent salivary glands always requires the greatest care to commence integrated chemo-radio-surgical treatment of prevent efficacy.

The paper report 592 non-selected cases of cervical adenopathies treated using both simple biopsy and more radical surgery. The predominant sites were right (29.7%) and left (25.2%) laterocervical. Histological tests showed that in 206 cases (34.8%) the pathology was benign and/or aspecific, whereas in 386 cases (65.2%) the presence of metastatic or systemic cancer was identified. Within the scope of metastatic adenopathies, tumors of the oral cavity or salivary glands, if taken together, account for the highest percentage of incidence: 14.2%. For this reason, the presence of cervical adenopathies must be given priority in the search for possible primary lesions affecting all the anatomical structures forming the oral cavity and the adjacent salivary glands.

RESUMEN

A careful examination of the mouth should be a routine check in patients affected by neoplasms before commencing chemotherapy. This allows an appropriate programme of odontostomatologic treatment to be drawn up and put into practice before, during and after the administration of anti-blastic drugs. Together with instructions for the maintenance of oral hygiene which should be given to the patient, this treatment will reduce the morbidity and mortality which is induced, either directly or indirectly by chemotherapy.

The paper discusses current views on the subject of the treatment of salivary gland tumours, based on the largest and most important studies reported in the literature. In particular, it takes into consideration the role of preoperative biopsy, the extension to be given to operations, the opportunity of using surgical and/or radiotherapeutic treatment for the neck, and factors which influence prognosis.

RESUMEN

The authors present a protocol personally adopted in hospital activity. They explain their extractive surgical behaviour in relation to thrombotest values found in patients treated with oral anticoagulants and conclude that also in the presence of thrombotest values a little below 20% they have not noticed intra- or postoperative complications.

The authors report their actual behaviour in dental extractive surgery with regard to patients subjected to oral anticoagulant therapy. They analyze the hospital ambulatory casistry treated, formulate some considerations and conclude confirming their behaviour supported by the results obtained.

RESUMEN

Data from a retrospective study conducted on 20 patients with parotid malignancies are reported. Males were more often affected than females and adult-elderly than the young. A large percentage of the cases were histologically undifferentiated. Eleven total parotidectomies were performed. A further 9 patients only required surgical biopsies for the purpose of histological confirmation. Two of the twenty patients are still alive over 10 years after diagnosis of the tumour.

Data are reported on 2 cases of primary non-Hodgkin lymphomas of the parotid gland considered interesting in view of the rarity of such tumours. The patients were treated surgically and are currently alive and well over 10 years later.

RESUMEN

A series of 7 patients (4 males, 3 females) suffering from benign, non-pleiomorphous neoplasia of the parotid observed in the period 1963-1983 is reported. The most commonly adopted surgery was extracapsular enucleo-resection. No post-operative complications were observed. No recurrences were encountered during follow-up.

Data relating to a series of 23 pleiomorphous adenomas of the parotid (19 primary forms and 4 previously operated recurrences) are reported. The ages most affected are the 4th, 5th and 6th decades with a higher incidence of females. The most commonly adopted surgery was extracapsular enucleoresection of the tumour (17 cases) followed by superficial parotidectomy (4 cases) and total parotidectomy (2 cases). No significant post-operative complications were observed. Anesthetic results were excellent. No recurrences were observed during follow-up.

The authors study a group of 25 patients treated for tonsillar carcinomas mainly in stage IV at the Istituto di Oncologia di Torino. The most part of the deaths are in the first 2 years after treatment, mainly due to distant metastases; a good stabilization of survival follows, obtained by radiation therapy alone or associated with surgery and/or chemotherapy.

The authors study a group of 108 patients treated for lip carcinoma by surgery alone or associated with radiotherapy and/or chemotherapy. They point out that the stage at the moment of the first treatment influences the prognosis and they confirm the survival after surgical therapy around 60% at 5 years.

The authors study a group of 61 patients treated for floor of the mouth neoplasms at the Istituto di Oncologia di Torino. They confirm a high incidence of deaths in the first 2 years after treatment in patients T greater than 2 N+, and better survival in patients with stage I-II tumours. The survival does not tend to fall between 5 and 10 years after treatment.

RESUMEN

The paper reports a series of 29 patients treated for advanced neoplasias of the palate at the Oncology Institute in Turin. The study confirms the proportional deterioration of the survival rate in correlation with the advanced stage of the carcinoma. The majority of deaths due to insufficient control occur during the second year following treatment but, after this period, the survival rate tends to stabilise around 40%.
