RESUMEN

The aim of this study was to elucidate more clearly the role of interleukin (IL)-18 in modulating the IL-22 pathway in primary Sjögren's syndrome (pSS) patients and in pSS-associated lymphomas. Minor salivary glands (MSGs) from patients with pSS and non-specific chronic sialoadenitis (nSCS), parotid glands biopsies from non-Hodgkin lymphomas (NHL) developed in pSS patients, were evaluated for IL-18, IL-22, IL-22 receptor 1 (IL-22R1), IL-22 binding protein (IL-22BP) and signal transducer and activator of transcription-3 (STAT-3) expression. MSGs IL-22R1-expressing cells were characterized by confocal microscopy and flow cytometry in pSS, nSCS and healthy controls . The effect of recombinant IL-18 and IL-22 on peripheral blood mononuclear cells (PBMCs) from pSS and nSCS was studied by flow cytometry and reverse transcription-polymerase chain reaction (RT-PCR). MSGs of pSS and NHL were characterized by an imbalance between IL-22 and IL-22BP protein expression, with IL-18 and IL-22BP being expressed in a mutually exclusive manner and IL-18 and IL-22R1 being correlated directly. Aberrant expression of IL-22R1, induced by IL-18, was observed only among tissue and circulating myeloid cells of pSS patients and macrophages of NHL tissues of pSS patients, but not nSCS. IL-22R1 expression on PBMC of pSS was functional, as its stimulation with recombinant IL-22 significantly up-regulated the expression of STAT-3, IL-17 and IL-22. An IL-18-dependent aberrant expression of IL-22R1 on cells of haematopoietic origin seems to be a specific immunological signature of patients with pSS and pSS-associated lymphomas.

Now among asbestos substitutes, the man-made organic fibers (MMOFs) find interesting applications on the industrial side. This class includes aramidic, polyacrylic, polyamides, polyolefins and polyvinylic fibers and it has been listed in category 3 of IARC classification (not classifiable as to carcinogenicity to humans). The aim of this study is to provide a panoramic regarding the toxicity of MMOFs present on the market according to their chemical-physical properties. Three materials have been taken in to consideration: the polyacrylonitrile (PAN), the polyvinyl alcohol (PVA) and the Kevlar. Each of them has been observed by the Scanning Electron Microscopy and Energy Dispersive Spectroscopy to estimate the morphology and the mean diameter. The obtained mean diameter of fibers is 10.50 microm for the PAN, 16.60 microm for the PVA and 11.61 microm for the Kevlar. The last one presents fibrils having a diameters in the range 0.2-1.6 microm. The ability of the Kevlar to produce fibrils with dimensions less than 1 microm, after abrasion or mechanical stress of other kind, is the cause of numerous studies based on the evaluation of their biosolubility. Finally we show some spectra obtained from the analysis of these fibers through Fourier Transform Infrared Spectroscopy (FTIR).

The aim of this study is to evaluate the wood dust exposures characterizing the working more to risk. Two selectors for inhalable fraction were been used: IOM sampler (Institute Occupational Medicine) and conical sampler. The sampling time is choosen by environmental airborne dust and it has varied around three and four hours. The surveys involved some companies of the Lazio that carried out the second working of wood that predict the transformation in ultimate products. The woodworking processes investigated have been grouped in three different classes: wood dissection; planning and moulding; sanding. The results have shown that the medium concentration of wood dusts, obtained in three processes investigated ranges from 2 mg/m3 to 4 mg/m3. The higher amounts is of 16 mg/m3 and it have been obtained during the sanding. Moreover the dust collected by IOM sampler was always higher than by conical sampler probably it is due to large particulates that were projected into IOM causing an overestimate of the amount of wood dust particulate that was inhalable. This study need to of more personal sampling for being able to carry out an exhaustive statistical analysis.

RESUMEN

For teaching-learning operative procedures the authors propose a new method ("KTE" method) in three progressive phases (step "K" or "basic knowledge"; step "T" or "training"; step "E" or "execution") that allow the student to achieve gradually the psychomotorial adaptation needed for the correct execution of surgical procedures. After preliminary study of the general features of the procedure (step B1) the student learns its operative features (step B2) on appositely constructed didactical tools ("Self-learning and learning maintenance boxes") that, together with the "observational" training (step A1), allow him to create in his mind an "operation image" and then, following an "operative" simulators training (step A2), to pass from that image to its carrying out (psychomotorial adaptation) that in the last phase (steps E1 and E2) concludes in the partial or complete execution of the surgical procedure in the patient. This didactic method can be apply to classic or endoscopic surgery and even to a lot of not surgical practical procedures.

RESUMEN

The authors have demonstrated a significant decrease of the plasmatic ionized calcium level in 84 patients during retinal fluorangiography likely due to a chemical bond between calcium and fluorescein. The side effects noticed during the procedure were similar to that quoted in the literature; their frequency, however, was not correlated with the decrease of the plasmatic ionized calcium level, even though the magnitude of the decrease was twice as great in the patients who experienced some trouble as in those who did not. This lack of correlation may be related to the too small patients sample. A greater frequency of side effects has been noticed in patients treated by calcium inhibitors.

RESUMEN

A case of mediastinal perfusion by a left internal jugular vein catheter is reported. The catheter was of Shaldon type with six lateral side-holes. The patient displayed progressive pulmonary oedema and peripheral vasoconstriction, whereas clinically the catheter seemed to work quite well. Only radiological control with injection of radiopaque dye demonstrated mediastinal penetration of the catheter tip including the most distal side-hole. The five more proximally located holes remained in intravascular position. The authors advocate radiographic catheter control with radiopaque dye in every catheterized patient demonstrating cardiorespiratory manifestations of unknown origin.

RESUMEN

A case is reported of bronchial stenosis due to a vascular cause in a patient with chronic obstructive lung disease, cor pulmonale and pulmonary arterial hypertension. This led to right lower lobe atelectasis and acute respiratory failure (pHa 7.24; PaCO2 85 mmHg; PaO2 44 mmHg) with important right-to-left shunting. This diagnosis was only suggested on day 7 by fibreoptic bronchoscopy and confirmed a week later by tomography and digital angiography. Nifedipine, used to reduce the pulmonary arterial hypertension, increased the cardiac index (31.min-1.m-2 to 3.3.1.min-1.m-2) and oxygen transport (488 ml.min-1.m-2 to 554 ml.min-1.m-2), despite increasing the shunt effect (Qs/QT: 26% to 31%). This and the antiinflammatory action of methylprednisolone were probably responsible for the favourable outcome.

During an acute decompensation of chronic obstructive pulmonary disease (COPD) associated with pulmonary arterial hypertension (PAHP), the right cardiac ventricle is unable to tolerate the increased workload required to overcome the increase in pulmonary arterial pressure (Ppa). As a result, the cardiac (CI) and systolic (SI) indices decrease much more than in those patients without PAHP. This study aimed to evaluate the effects of nifedipine on haemodynamic parameters and oxygen availability (QO2) when given to mechanically ventilated COPD patients with PAHP during an acute decompensation. The series included 14 patients aged 68 +/- 8 yr, admitted to an intensive care ward for an acute decompensation of COPD, with Ppa greater than 20 mmHg. They remained haemodynamically stable throughout the study period. The measurements were made 20 min after the Swan-Ganz and radial artery catheters were set up (t0), and one hour after administration of 10 mg sublingual nifedipine (t1). Thereafter this agent was given three times a day. A further set of measurements were carried out in seven patients, 24 h after the first dose of nifedipine (t2). At t1, there was a significant increase in CI (+12.3%) and QO2 (+14.1%), whereas Ppa, indexed pulmonary vascular resistances, indexed systemic vascular resistances (SVRI) and PaO2 decreased significantly (-9.2%; -20%; -12.8% and -6.4% respectively). At t2, QO2 was significantly higher (+18.4%), whereas Ppa and SVRI were significantly lower, than at t0. PaO2 and the shunt fraction (Qs/Qt) returned to basal values, with a significant decrease in Qs/Qt when compared with t1.(ABSTRACT TRUNCATED AT 250 WORDS)

A case is reported of a 46-year-old male patient who sustained a blunt thoracic injury with an anterior flail chest, and right haemopneumothorax. He was intubated and ventilated because of acute respiratory failure. There were initially no signs suggesting any myocardial injury. It was not before day 20 that the electrocardiogram showed a QS wave in leads V2 and V3. The hypothesis of an antero-septal myocardial infarct was not confirmed by echocardiography, which only revealed slight thickening of the posterior pericardium. From day 50 on, the patient had tachycardia, raised jugular venous pressure, and effort dyspnoea. Echocardiography (day 59) showed an anterior and posterior pericardial effusion (about 500 ml), marked pericardial thickening, and inferior vena caval collapse during inspiration, with normal myocardial wall movements. Drainage pericardiocentesis was therefore carried out, followed by, four days later, a pericardiectomy. A small ecchymosis was found on the anterior aspect of the right ventricle. The pericardium was thickened, fibrous, hyperhaemic, Case is y stuck to the epicardium. Eight months later, echocardiography showed that the posterior pericardium remained thickened, and there was a very small residual effusion. Movements of the septum had returned to normal.

RESUMEN

The chronic obstruction of the aortic bifurcation and iliac axis was definide by the French surgeon Rene Leriche, whose name it bears today. The advancing age of the population move the definition from a syndrome, as reported from Leriche, to complex aortoiliac vascular lesions current multidisciplinary guidelines recommend to treat extensive aortoiliac occlusive disease by surgical revascularization. Surgery provides good long-term patency, but at the cost of substantial perioperative morbidity. Development of new technologies and techniques has led to increased use of endovascular therapy for extensive aortoiliac disease. This review article summarized current multidisciplinary guidelines indication in endovascular treatment of extensive aorto-iliac disease and the various worldwide single centre experience published on endovascular treatment of Leriche Syndrome.

In search for innovative therapeutic agents for children neuroblastoma, the oxygen therapy could be considered an alternative anti-tumoral treatment. Given the physiochemical properties of O(2/3) gas mixture including fairly low aqueous solubility and spreading, and the interesting perspective of hyperoxia, we analyzed the inhibitory effect of O(2/3) treatment on two human neuroblastoma cell lines (SK-N-SH and SK-N-DZ). In this study, we demonstrated that O(2/3) treatment was able to induce cell growth inhibition and cell cycle perturbation in both cell lines. We observed an arrest at G(2) phase, accompanied by an alteration in the expression and localization of cyclin B1/cdk1 complex and a reduction in its activity in SK-N-SH cells. This reduction was consistent with the increase in both Wee1 and chk1 protein levels. On the contrary, O(2/3) induced apoptosis in SK-N-DZ cells via caspase 3 activation and Poly ADP-ribose polymerase-1 (PARP) cleavage, associated with an increase in the pro-apoptotic Bax protein. Consequently, we considered the possibility of improving the responsiveness to chemotherapeutic agents such as Cisplatin, Etoposide, and Gemcitabine in combination with O(2/3) treatment. The combined treatments produced a stronger cell inhibitory effect than Cisplatin and Etoposide used alone in SK-N-SH cells. On the contrary, the combination data were not significantly different from O(2/3) treatment alone in SK-N-DZ cells, thus suggesting that the obtained changes in cell growth inhibition were due to the effect of O(2/3) alone.

Mammary glands were excised from female C3H mice at various stages of their estrus cycle. Homogenates incubated at 37 degrees C +/- 10(5) M indomethacin synthesized prostaglandins (PG) E and f2 alpha at rates that varied as a function of the stage at estrus. The rate of PGF2 alpha synthesis was maximal at 1330 pg per mg protein per 2 hr in early diestrus and fell to undetectable levels in early estrus. PGE synthesis exhibited a similar pattern, being maximal at 83 pg per mg protein per 2 hr in early diestrus. These observations suggest that prostaglandins play a role in the cyclic changes observed within the mammary gland.
