RESUMEN

The problem of prematurity in industrial region has been presented. The rate of prematurity in the time of 1972 to 1996 has been estimated and it equaled 13.2%. The highest rate of these newborn reached 16.7% in 1996. Systematical correlation in falling of perinatal mortality has been observed in time of research. Some attempts to compare the way of delivery and perinatal mortality have been made. It has been found that the frequency of caesarean section has grown significantly in the cases of low birth weight newborn.

OBJECTIVES: Approximately one-half of women with cervical cancer are premenopausal. Preservation of ovarian function in these women is highly desirable. The safety and efficacy of ovarian retention include risk of ovarian metastasis, risk of surgery, psychological impact of surgical castration, expense of hormone supplementation. DESIGN: This report compares intra- and postoperative surgical complications in 2 groups of women with cancer in stage Ib and IIa treated by radical hysterectomy (Wertheim-Meigs operation). MATERIALS AND METHODS: Two groups were compared: 72 patients with ovarian transposition and 55 women without transposition, but after brachy-HDR therapy. The surgery time was mean 30 minute longer in case of ovarian transposition. RESULTS: There was no difference in mean time of hospital stay after the surgery, the frequency of relaparotomy, deep vein thrombosis, urological complications and blood transfusion between the groups. There was confirmed the higher number of lymphatic pseudocysts (p < 0.0005) and fever (p < 0.001) in women without transposition, but after brachy-HDR therapy. We linked this with the previously applied radiotherapy. CONCLUSIONS: We conclude that ovarian transposition is the procedure which can safe the endogenous ovarian function in young women with cervical cancer without the higher risk of complications.

The 16 afterbirths samples obtained from pregnancies complicated by idiopathic fetal hypotrophy was examined. The criteria of hypotrophy were established: the duration of gestation above 36 weeks, the birth-weight was less than 2500 g. The control group included 8 afterbirth samples from physiological full-term pregnancies. It was performed macroscopic examination, light microscopy and transmission electron microscopy. Macroscopically, no changes were found in the study group apart from the small differences of the weight. Light microscopy showed the retardation of placenta maturation from fetus hypotrophy group, congestion of the villi and presence of the numerous calcifications. Transmission electron microscopy showed the changes of placental barrier elements: adaptation processes and chronic progressive damage of the cells (exfoliation of the endothelial cells from the basal membrane).

The purpose of this study was to assess the hormonal function of transposed ovaries in young women treated for cervical cancer. Between 1992 and 1998, in the Silesian Medical Academy in Bytom, 101 women underwent radical hysterectomy with ovarian transposition by the Wertheim-Meigs method. Concentrations of follicle-stimulating hormone (FSH), luteinizing hormone (LH), prolactin, estradiol and testosterone in blood serum and cytologic indices (maturation index, karyopyknotic index and maturation value) were assessed before surgery and on the ninth day after surgery. Patients were subsequently requested to return in July 1999 to have the hormonal activity of their ovaries assessed from the perspective of a few years after the operation, and the effect of supplementary radiotherapy soon after surgery. There was a statistically significant difference between those who had radiotherapy and those who did not with respect to climacteric complaints and hormonal parameters. A correlation was found between serum estradiol levels and maturation value. Proper ovarian hormonal function was still present in 69.8% of patients, even 60 months after surgery, but a lower bone mineral density was also observed. Ovarian transposition is a procedure that allows ovarian function to be preserved in young women treated for cervical cancer; it also helps to obviate the necessity for long-term hormone-replacement therapy. However attention must be paid to the concomitant possibility that women with cervical cancer may have a lower bone mineral density, and routine follow-up measurement of FSH levels or cytologic indices is advocated.
