Background: The average length of stay is often used to indicate health system efficiency; shorter stays are associated with reduced costs. In South Africa, mental healthcare expenditure is spent on inpatient care. Aim: To identify factors associated with a long stay in an acute psychiatric unit. Setting: A tertiary hospital. Methods: A case-control study review of inpatients diagnosed with psychotic symptoms was used. Sample was divided into two groups, length of stay (LOS) (LOS greater than 21 days, LOS less than 14 days). Total of 82 patients were divided into short stay group (SSG, n = 23) and long stay group (LSG) (n = 59). A comparison of demographic, clinical and system variables was conducted. Results: In demographics, LSG had fewer men compared to SSG (78.3%) and differed statistically from LSG with p = 0.05. Long stay groups were older in comparison to SSG with a p = 0.02. Illicit substance use in LSG was 44.1% and statistically less than SSG (73.91%; p = 0.02). A high proportion of LSG had medical or surgical and psychiatric comorbidities (67.8%) compared to SSG (43.5%) (p = 0.04). A total of 95% patients in SSG had family support. Conclusion: Longer stay was found to be associated with older females with primary psychotic disorders. Comorbidities with less availability of family support were associated with younger males presenting with psychotic symptoms that may be related to illicit substances that respond to rapid stabilisation. Contribution: Active surveillance of medical comorbidities amongst older female patients is necessary for early liaison services to reduce their length of stay.

Arachidonic acid 15-lipoxygenases (ALOX15) play a role in mammalian erythropoiesis but they have also been implicated in inflammatory processes. Seven intact Alox genes have been detected in the mouse reference genome and the mouse Alox15 gene is structurally similar to the orthologous genes of other mammals. However, mouse and human ALOX15 orthologs have different functional characteristics. Human ALOX15 converts C20 polyenoic fatty acids like arachidonic acid mainly to the n-6 hydroperoxide. In contrast, the n-9 hydroperoxide is the major oxygenation product formed by mouse Alox15. Previous experiments indicated that Leu353Phe exchange in recombinant mouse Alox15 humanized the catalytic properties of the enzyme. To investigate whether this functional humanization might also work in vivo and to characterize the functional consequences of mouse Alox15 humanization we generated Alox15 knock-in mice (Alox15-KI), in which the Alox15 gene was modified in such a way that the animals express the arachidonic acid 15-lipoxygenating Leu353Phe mutant instead of the arachidonic acid 12-lipoxygenating wildtype enzyme. These mice develop normally, they are fully fertile but display modified plasma oxylipidomes. In young individuals, the basic hematological parameters were not different when Alox15-KI mice and outbred wildtype controls were compared. However, when growing older male Alox15-KI mice develop signs of dysfunctional erythropoiesis such as reduced hematocrit, lower erythrocyte counts and attenuated hemoglobin concentration. These differences were paralleled by an improved ex vivo osmotic resistance of the peripheral red blood cells. Interestingly, such differences were not observed in female individuals suggesting gender specific effects. In summary, these data indicated that functional humanization of mouse Alox15 induces defective erythropoiesis in aged male individuals.

Mammalian arachidonic acid lipoxygenases (ALOXs) have been implicated in the pathogenesis of inflammatory diseases, and its pro- and anti-inflammatory effects have been reported for different ALOX-isoforms. Human ALOX15B oxygenates arachidonic acid to its 15-hydroperoxy derivative, whereas the corresponding 8-hydroperoxide is formed by mouse Alox15b (Alox8). This functional difference impacts the biosynthetic capacity of the two enzymes for creating pro- and anti-inflammatory eicosanoids. To explore the functional consequences of the humanization of the reaction specificity of mouse Alox15b in vivo, we tested Alox15b knock-in mice that express the arachidonic acid 15-lipoxygenating Tyr603Asp and His604Val double mutant of Alox15b, instead of the arachidonic acid 8-lipoxygenating wildtype enzyme, in two different animal inflammation models. In the dextran sodium sulfate-induced colitis model, female Alox15b-KI mice lost significantly more bodyweight during the acute phase of inflammation and recovered less rapidly during the resolution phase. Although we observed significant differences in the colonic levels of selected pro- and anti-inflammatory eicosanoids during the time-course of inflammation, there were no differences between the two genotypes at any time-point of the disease. In Freund's complete adjuvant-induced paw edema model, Alox15b-KI mice were less susceptible than outbred wildtype controls, though we did not observe significant differences in pain perception (Hargreaves-test, von Frey-test) when the two genotypes were compared. our data indicate that humanization of the reaction specificity of mouse Alox15b (Alox8) sensitizes mice for dextran sodium sulfate-induced experimental colitis, but partly protects the animals in the complete Freund's adjuvant-induced paw edema model.

Group communication enables Internet of Things (IoT) devices to communicate in an efficient and fast manner. In most instances, a group message needs to be encrypted using a cryptographic key that only devices in the group know. In this paper, we address the problem of establishing such a key using a lattice-based one-way function, which can easily be inverted using a suitably designed lattice trapdoor. Using the notion of a bad/good basis, we present a new method of coupling multiple private keys into a single public key, which is then used for encrypting a group message. The protocol has the apparent advantage of having a conjectured resistance against potential quantum-computer-based attacks. All functions-key establishment, session key update, node addition, encryption, and decryption-are effected in constant time, using simple linear-algebra operations, making the protocol suitable for resource-constrained IoT networks. We show how a cryptographic session group key can be constructed on the fly by a user with legitimate credentials, making node-capture-type attacks impractical. The protocol also incorporates a mechanism for node addition and session-key generation in a forward- and backward-secrecy-preserving manner.

The Internet of Things (IoT) is an emerging paradigm that enables many beneficial and prospective application areas, such as smart metering, smart homes, smart industries, and smart city architectures, to name but a few. These application areas typically comprise end nodes and gateways that are often interconnected by low power wide area network (LPWAN) technologies, which provide low power consumption rates to elongate the battery lifetimes of end nodes, low IoT device development/purchasing costs, long transmission range, and increased scalability, albeit at low data rates. However, most LPWAN technologies are often confronted with a number of physical (PHY) layer challenges, including increased interference, spectral inefficiency, and/or low data rates for which cognitive radio (CR), being a predominantly PHY layer solution, suffices as a potential solution. Consequently, in this article, we survey the potentials of integrating CR in LPWAN for IoT-based applications. First, we present and discuss a detailed list of different state-of-the-art LPWAN technologies; we summarize the most recent LPWAN standardization bodies, alliances, and consortia while emphasizing their disposition towards the integration of CR in LPWAN. We then highlight the concept of CR in LPWAN via a PHY-layer front-end model and discuss the benefits of CR-LPWAN for IoT applications. A number of research challenges and future directions are also presented. This article aims to provide a unique and holistic overview of CR in LPWAN with the intention of emphasizing its potential benefits.

Long-Range Wide Area Network (LoRaWAN) is a fast-growing communication system for Low Power Wide Area Networks (LPWAN) in the Internet of Things (IoTs) deployments. LoRaWAN is built to optimize LPWANs for battery lifetime, capacity, range, and cost. LoRaWAN employs an Adaptive Data Rate (ADR) scheme that dynamically optimizes data rate, airtime, and energy consumption. The major challenge in LoRaWAN is that the LoRa specification does not state how the network server must command end nodes pertaining rate adaptation. As a result, numerous ADR schemes have been proposed to cater for the many applications of IoT technology, the quality of service requirements, different metrics, and radio frequency (RF) conditions. This offers a challenge for the reliability and suitability of these schemes. This paper presents a comprehensive review of the research on ADR algorithms for LoRaWAN technology. First, we provide an overview of LoRaWAN network performance that has been explored and documented in the literature and then focus on recent solutions for ADR as an optimization approach to improve throughput, energy efficiency and scalability. We then distinguish the approaches used, highlight their strengths and drawbacks, and provide a comparison of these approaches. Finally, we identify some research gaps and future directions.

OBJECTIVE: To determine the prevalence of self-induced vomiting (SIV) in a sample of patients with Alcohol Use Disorder (AUD), and to explore whether such behavior is associated with a comorbid eating disorder psychopathology and other clinical symptomatology, such as impulsivity. Methods: This cross-sectional descriptive study was carried out at the Alcohol Rehabilitation Unit at Stikland Psychiatric Hospital, South Africa. All consecutive inpatient admissions between April and June 2017 were invited to participate in an hour-long data interview. The Alcohol Use Disorders Identification Test (AUDIT), Bulimic Investigatory Test, Edinburgh (BITE), and the Barratt Impulsiveness Scale (BIS-11) were used to collect data from 104 adults. Results: A total of 29% of patients with AUD reported SIV. The main reason stated for SIV was to prevent hangover (66%). There were no significant differences in BITE and BIS110 scores between participants who reported SIV and those who did not. Conclusions: Almost a third of patients with AUD reported SIV. BITE scores indicated no relationship between eating disorders and alcohol-related SIV in this sample. Our findings also did not support an etiological role for impulsivity in alcohol-related SIV.

BACKGROUND: Eating disorders (EDs) and substance-related disorders pose a challenge when they co-occur and have implications for patient management. Clinical information on EDs and substance-related disorders as independent disorders is fairly well established in South Africa, but our understanding of the coexistence of these disorders is limited. AIM: To determine the prevalence, the concurrent nature and the possible trends of substance use among patients diagnosed with EDs at a South African tertiary hospital over a 21-year period. SETTING: The ED unit at Tygerberg Hospital, Cape Town, South Africa. METHODS: We performed a retrospective chart review of 162 patients who were treated for EDs between January 1993 and December 2014. RESULTS: The prevalence of ED subtypes was 40.1% bulimia nervosa (BN), 33.3% EDs not otherwise specified (EDNOS) and 26.5% anorexia nervosa. Most participants (71.0%) used at least one substance. Alcohol was the most prevalent substance of choice (54.8%). Most patients had an additional psychiatric disorder (62.3%), of which major depressive disorder was the most prevalent (46.3%). Apart from the use of alcohol and cannabis, which remained consistent, the use of most other substances as well as the prevalence of BN declined during the study period. CONCLUSION: Understanding the prevalence and trends of EDs and the corresponding patterns of substance misuse is essential to improve service provision. This study emphasises the need to better understand the ongoing and changing behavioural trends in EDs to improve patient management.

The overcrowding of the wireless space has triggered a strict competition for scare network resources. Therefore, there is a need for a dynamic spectrum access (DSA) technique that will ensure fair allocation of the available network resources for diverse network elements competing for the network resources. Spectrum handoff (SH) is a DSA technique through which cognitive radio (CR) promises to provide effective channel utilization, fair resource allocation, as well as reliable and uninterrupted real-time connection. However, SH may consume extra network resources, increase latency, and degrade network performance if the spectrum sensing technique used is ineffective and the channel selection strategy (CSS) is poorly implemented. Therefore, it is necessary to develop an SH policy that holistically considers the implementation of effective CSS, and spectrum sensing technique, as well as minimizes communication delays. In this work, two reinforcement learning (RL) algorithms are integrated into the CSS to perform channel selection. The first algorithm is used to evaluate the channel future occupancy, whereas the second algorithm is used to determine the channel quality in order to sort and rank the channels in candidate channel list (CCL). A method of masking linearly dependent and useless state elements is implemented to improve the convergence of the learning. Our approach showed a significant reduction in terms of latency and a remarkable improvement in throughput performance in comparison to conventional approaches.

Reduced expression of the Indy ("I am Not Dead, Yet") gene in lower organisms promotes longevity in a manner akin to caloric restriction. Deletion of the mammalian homolog of Indy (mIndy, Slc13a5) encoding for a plasma membrane-associated citrate transporter expressed highly in the liver, protects mice from high-fat diet-induced and aging-induced obesity and hepatic fat accumulation through a mechanism resembling caloric restriction. We studied a possible role of mIndy in human hepatic fat metabolism. In obese, insulin-resistant patients with nonalcoholic fatty liver disease, hepatic mIndy expression was increased and mIndy expression was also independently associated with hepatic steatosis. In nonhuman primates, a 2-year high-fat, high-sucrose diet increased hepatic mIndy expression. Liver microarray analysis showed that high mIndy expression was associated with pathways involved in hepatic lipid metabolism and immunological processes. Interleukin-6 (IL-6) was identified as a regulator of mIndy by binding to its cognate receptor. Studies in human primary hepatocytes confirmed that IL-6 markedly induced mIndy transcription through the IL-6 receptor and activation of the transcription factor signal transducer and activator of transcription 3, and a putative start site of the human mIndy promoter was determined. Activation of the IL-6-signal transducer and activator of transcription 3 pathway stimulated mIndy expression, enhanced cytoplasmic citrate influx, and augmented hepatic lipogenesis in vivo. In contrast, deletion of mIndy completely prevented the stimulating effect of IL-6 on citrate uptake and reduced hepatic lipogenesis. These data show that mIndy is increased in liver of obese humans and nonhuman primates with NALFD. Moreover, our data identify mIndy as a target gene of IL-6 and determine novel functions of IL-6 through mINDY. CONCLUSION: Targeting human mINDY may have therapeutic potential in obese patients with nonalcoholic fatty liver disease. German Clinical Trials Register: DRKS00005450. BACKGROUND: The classification of sexuality-related conditions and conditions relating to transgender identity has generated controversy. Growing evidence suggests that the distress and dysfunction reported by transgender individuals is more likely associated with social exclusion, stigmatization, and violence than as a result of gender incongruence per se. Our study aimed to explore the experiences of South African transgender individuals through: 1) their self-reported accounts of gender incongruence, and 2) associations between their experiences of social exclusion and violence, and their reports of psychological distress and dysfunction during adolescence. METHODS: Our sample of 57 South African transgender adults completed a structured interview, in English, including questions related to experiences of gender incongruence duration, distress, dysfunction, social exclusion, and violence. RESULTS: Many transgender individuals reported having experienced an intense desire to be a different gender, with all noting discomfort with several aspects of their bodies. Importantly, psychological distress and dysfunction were significantly associated with social exclusion (most commonly perpetuated by family and friends) and not with gender incongruence per se. CONCLUSIONS: This study adds to the growing evidence that experiences of social exclusion play a significant role in the psychological distress and dysfunction reported by transgender persons.

Wireless Sensor Networks (WSNs), in recent times, have become one of the most promising network solutions with a wide variety of applications in the areas of agriculture, environment, healthcare and the military. Notwithstanding these promising applications, sensor nodes in WSNs are vulnerable to different security attacks due to their deployment in hostile and unattended areas and their resource constraints. One of such attacks is the DoS jamming attack that interferes and disrupts the normal functions of sensor nodes in a WSN by emitting radio frequency signals to jam legitimate signals to cause a denial of service. In this work we propose a step-wise approach using a statistical process control technique to detect these attacks. We deploy an exponentially weighted moving average (EWMA) to detect anomalous changes in the intensity of a jamming attack event by using the packet inter-arrival feature of the received packets from the sensor nodes. Results obtained from a trace-driven simulation show that the proposed solution can efficiently and accurately detect jamming attacks in WSNs with little or no overhead.

In recent times, Wireless Sensor Networks (WSNs) are broadly applied in the Industrial Internet of Things (IIoT) in order to enhance the productivity and efficiency of existing and prospective manufacturing industries. In particular, an area of interest that concerns the use of WSNs in IIoT is the concept of sensor network virtualization and overlay networks. Both network virtualization and overlay networks are considered contemporary because they provide the capacity to create services and applications at the edge of existing virtual networks without changing the underlying infrastructure. This capability makes both network virtualization and overlay network services highly beneficial, particularly for the dynamic needs of IIoT based applications such as in smart industry applications, smart city, and smart home applications. Consequently, the study of both WSN virtualization and overlay networks has become highly patronized in the literature, leading to the growth and maturity of the research area. In line with this growth, this paper provides a review of the development made thus far concerning virtualized sensor networks, with emphasis on the application of overlay networks in IIoT. Principally, the process of virtualization in WSN is discussed along with its importance in IIoT applications. Different challenges in WSN are also presented along with possible solutions given by the use of virtualized WSNs. Further details are also presented concerning the use of overlay networks as the next step to supporting virtualization in shared sensor networks. Our discussion closes with an exposition of the existing challenges in the use of virtualized WSN for IIoT applications. In general, because overlay networks will be contributory to the future development and advancement of smart industrial and smart city applications, this review may be considered by researchers as a reference point for those particularly interested in the study of this growing field.

Using a single sensor to determine the pose estimation of a device cannot give accurate results. This paper presents a fusion of an inertial sensor of six degrees of freedom (6-DoF) which comprises the 3-axis of an accelerometer and the 3-axis of a gyroscope, and a vision to determine a low-cost and accurate position for an autonomous mobile robot. For vision, a monocular vision-based object detection algorithm speeded-up robust feature (SURF) and random sample consensus (RANSAC) algorithms were integrated and used to recognize a sample object in several images taken. As against the conventional method that depend on point-tracking, RANSAC uses an iterative method to estimate the parameters of a mathematical model from a set of captured data which contains outliers. With SURF and RANSAC, improved accuracy is certain; this is because of their ability to find interest points (features) under different viewing conditions using a Hessain matrix. This approach is proposed because of its simple implementation, low cost, and improved accuracy. With an extended Kalman filter (EKF), data from inertial sensors and a camera were fused to estimate the position and orientation of the mobile robot. All these sensors were mounted on the mobile robot to obtain an accurate localization. An indoor experiment was carried out to validate and evaluate the performance. Experimental results show that the proposed method is fast in computation, reliable and robust, and can be considered for practical applications. The performance of the experiments was verified by the ground truth data and root mean square errors (RMSEs).

Wireless sensor networks (WSNs) are becoming increasingly popular with the advent of the Internet of things (IoT). Various real-world applications of WSNs such as in smart grids, smart farming and smart health would require a potential deployment of thousands or maybe hundreds of thousands of sensor nodes/actuators. To ensure proper working order and network efficiency of such a network of sensor nodes, an effective WSN management system has to be integrated. However, the inherent challenges of WSNs such as sensor/actuator heterogeneity, application dependency and resource constraints have led to challenges in implementing effective traditional WSN management. This difficulty in management increases as the WSN becomes larger. Software Defined Networking (SDN) provides a promising solution in flexible management WSNs by allowing the separation of the control logic from the sensor nodes/actuators. The advantage with this SDN-based management in WSNs is that it enables centralized control of the entire WSN making it simpler to deploy network-wide management protocols and applications on demand. This paper highlights some of the recent work on traditional WSN management in brief and reviews SDN-based management techniques for WSNs in greater detail while drawing attention to the advantages that SDN brings to traditional WSN management. This paper also investigates open research challenges in coming up with mechanisms for flexible and easier SDN-based WSN configuration and management.

Overweight and obesity are associated with hyperinsulinemia, insulin resistance, and a low-grade inflammation. Although hyperinsulinemia is generally thought to result from an attempt of the ß-cell to compensate for insulin resistance, there is evidence that hyperinsulinaemia itself may contribute to the development of insulin resistance and possibly the low-grade inflammation. To test this hypothesis, U937 macrophages were exposed to insulin. In these cells, insulin induced expression of the proinflammatory cytokines IL-1ß, IL-8, CCL2, and OSM. The insulin-elicited induction of IL-1ß was independent of the presence of endotoxin and most likely mediated by an insulin-dependent activation of NF-κB. Supernatants of the insulin-treated U937 macrophages rendered primary cultures of rat hepatocytes insulin resistant; they attenuated the insulin-dependent induction of glucokinase by 50%. The cytokines contained in the supernatants of insulin-treated U937 macrophages activated ERK1/2 and IKKß, resulting in an inhibitory serine phosphorylation of the insulin receptor substrate. In addition, STAT3 was activated and SOCS3 induced, further contributing to the interruption of the insulin receptor signal chain in hepatocytes. These results indicate that hyperinsulinemia per se might contribute to the low-grade inflammation prevailing in overweight and obese patients and thereby promote the development of insulin resistance particularly in the liver, because the insulin concentration in the portal circulation is much higher than in all other tissues.

RESUMEN

We present the most comprehensive genetic characterization to date of five Fijian island populations: Viti Levu, Vanua Levu, Kadavu, the Lau Islands, and Rotuma, including nonrecombinant Y (NRY) chromosome and mitochondrial DNA (mtDNA) haplotypes and haplogroups. As a whole, Fijians are genetically intermediate between Melanesians and Polynesians, but the individual Fijian island populations exhibit significant genetic structure reflecting different settlement experiences in which the Rotumans and the Lau Islanders were more influenced by Polynesians, and the other Fijian island populations were more influenced by Melanesians. In particular, Rotuman and Lau Islander NRY chromosomal and mtDNA haplogroup frequencies and Rotuman mtDNA hypervariable segment 1 region haplotypes more closely resemble those of Polynesians, while genetic markers of the other populations more closely resemble those of the Near Oceanic Melanesians. Our findings provide genetic evidence supportive of modifying regional boundaries relative to Fiji, as has been suggested by others based on a variety of nongenetic evidence. Specifically, for the traditional Melanesia/Polynesia/Micronesia scheme, our findings support moving the Melanesia-Polynesia boundary to include Rotuma and the Lau Islands in Polynesia. For the newer Near/Remote Oceania scheme, our findings support keeping Rotuma and the Lau Islands in Remote Oceania and locating the other Fijian island populations in an intermediate or "Central Oceania" region to better reflect the great diversity of Oceania.
