RESUMEN

The physical examinations at school enrollment of the years 1999 to 2004 were subjected to comparative analysis to help clarify whether children's language performance at enrollment has significantly decreased in recent years or not. This question has indeed been a matter of public debate for some time. Furthermore, potential influencing factors, in particular social environment, were to be examined as regards their effects on language performance. The results indicate that the children's performance level has not significantly decreased over the six-year period of observation. Instead, rather an increase can be observed over this period of time. If, nevertheless, more and more and, in particular, more significant language deficits continue to be observed, one explanation for this may be seen in a changing assessment of children's needs for therapy or remedial education. The performance level of children who were recommended therapy or means of remedial education in 2004 is significantly higher than that of children for whom such recommendations were given in 1999 or 2000. Perhaps an increased sensitivity for those performance areas where performance levels are found to be deficient should be postulated, and, consequently, a more differentiated assessment of diagnostic findings should be assumed. The results of the examinations once again demonstrate the enormous influence that social factors have on language performance. Children growing up in a less advantaged social environment tend to achieve significantly lower performance marks in language tests than children from more privileged social environments. Also the duration of kindergarten education appears to be relevant to children's language performance: children who have visited institutions of preschool-education for three or more years achieve higher performance marks than children who have spent no ore only little time in these institutions. When seen against the background of circa one in four children with a migration background (around 8% of persons of the same age in Münster's population) having unsatisfactory knowledge of German at school enrollment, there appear to be efficient prevention measures available here.

Antibodies directed against the extracellular immunoglobulin (Ig)-like domain of the myelin oligodendrocyte glycoprotein (MOG(Igd)) mediate demyelination in experimental autoimmune encephalomyelitis (EAE) and are implicated in the immunopathogenesis of multiple sclerosis (MS). In this study we investigated the epitope specificity of MOG(Igd)-specific autoantibodies immunopurified from MS patients (n=17) and normal healthy controls (HD; n=9). ELISA, using a panel of synthetic MOG(Igd) peptides, revealed that the epitope specificity of this response was heterogeneous in both groups. The most frequently recognised epitopes were located in amino acid sequences (a.a.) 1-26 (13/17) and 63-87 (15/17) in MS patients, and 14-39 (6/9) and 63-87 (6/9) in HDs, but there was no association between MS and any particular peptide specificity. We therefore investigated the ability of the immunopurified antibodies to recognise native MOG(Igd) expressed on at the membrane surface by FACS. Unexpectedly, antibodies fulfilling this essential criterion for a demyelinating antibody response were detected only in one of the MS samples. These results indicate that the epitope specificity of the human B cell response to MOG is not only heterogeneous, but may only mediate demyelination in a limited subset of MS patients.

To obtain true half lives, glycoside elimination from six healthy subjects was studied for 14 days after multiple intravenous doses or oral administration of a daily maintenance dose of beta-methyldigoxin 0.3 mg. After oral or intravenous administration of beta-methyldigoxin ceased, the plasma concentrations declined from the 14th to the 16th days with a half life of 1.7 days. From the 16th to the 20th day a change from a shorter to a longer half life of 2.8 and 2.9 days was observed. Similar half lives were found in urine: after the last dose the initial slope from the 14th to the 16th day had a half life of 1.8 days, and the terminal slope had one of 3.2 days. The results indicate release of the glycoside from slowly equilibrating tissues.

To evaluate and possibly improve the hearing aid fittings of children attending the Westphalian School for the Hearing Impaired or the Westphalian School for the Deaf, regular pedaudiologic consulting hours were established at both schools. During a 2-year period, 115 children were examined once, 35 children twice, and 5 children three times. The examinations comprised ear microscopy, audiometry, and a check of the hearing aids with a 0.6-cm3 coupler (children up to 7 years) or 2-cm3 coupler, respectively. The following criteria were used to assess the quality of the hearing aid setting: status of the external auditory canal and middle ear, acceptance of wearing the hearing aid, status of the ear mold, technical status of the hearing aid, and its setting. The results were related to four variables: gender, type of school, age, and mean hearing loss. Overall, just 40.9% of all children showed satisfactory hearing aid performance at the first examination and just 37.1% at the second. A significant influence of the variables on the hearing aid performance was documented for hearing loss only. The higher the hearing loss, the more likely the children were to have good hearing aid status. Analysis of the different parameters revealed that an incorrect setting was the main problem, with a rate of 20.9%; the rate of the other parameters varied from 6.1% to 15.7%. Thus, no parameter was of major relevance to the results. The results of the second examination were poorer in most parameters than those of the first. These alarming results, which are probably not only of regional significance, demonstrate that the hearing aid status of children attending schools for the hearing impaired or for the deaf is in urgent need of improvement.
