RESUMEN
A comparative diagnostic study was carried out on 40 patients with pathologically proven renal malignancies which were staged both by angiography and CT. CT is more accurate than angiography for the detection of both perirenal and pararenal extension of the primary tumour (T-staging), and angiography is only slightly superior to CT in the evaluation of intravascular extension of the malignancy. The diagnostic performance of CT is clearly superior to angiography in detecting malignant lymphatic spread (L-staging) and distant metastases (M-staging). CT is, therefore, becoming the primary diagnostic approach for the staging of renal malignant tumours.

The computer tomographic appearances of cavernous haemangiomas of the liver in five patients are described. The typical time-density relationship of these benign liver tumours after intravenous contrast enhancement is stressed and the differential diagnosis is discussed.

The morphology of gynaecological cancers was studied by computer tomography in 220 ovarian and uterine carcinomas. The value of the method with regard to staging and diagnosis is discussed. Non-invasive computer tomography for gynaecological cancers is indicated. 1. In order to obtain significant information regarding the extent of the tumour and its regional metastases. 2. In order to reduce the need for other invasive diagnostic measures. 3. For improving treatment by interdisciplinary procedures, particularly with respect to planning of radiation therapy. 4. For the early recognition of recurrences and for objective evaluation of the result of treatment.

The computer tomographic morphology of the larynx was studied in 19 patients during various phases of respiration, such as during quiet breathing and in expiratory phonation. The diagnostic information which can be obtained by computer tomographic functional studies of laryngeal neoplasms is discussed.

The angiographic morphology for various arterial injuries is described and illustrated by means of 46 patients examined by us; these consisted of 13 socclusions, 21 arterial rupture and 12 arterial lacerations. The main angiographic feature is thrombotic occlusion of the damaged artery. Consequently, the pathology of the arterial lesion could be defined in only one third of the patients with arterial injuries. This, however, is not of major importance in planning vascular surgery.

The angiographic appearances of biliary neoplasms and their differential diagnosis is discussed in relation to 15 patients with annular carcinomas of the gall bladder or bile ducts. The outstanding angiographic features of biliary carcinomas are irregularities of the arterial contours and vascular occlusion, followed in importance by neo-vascularity of the malignant tumour.

The differential diagnosis between hepatoblastomas and hepatocellular carcinoma is discussed with reference to ten patients seen by the authors, the difference depending on the maturity of the liver tissue undergoing malignant change.

The normal appearances of the pancreas as seen on computerised tomograms (EMI) has been demonstrated in fifty normal patients and correlated with fifteen post mortem studies on patients with a normal pancreas. The problems of optimal demonstration of the pancreas on computerised tomograms and possible sources of error in the interpretation of transverse pancreas tomograms are discussed.

The sonographic-radiological diagnosis of abdominal abscesses is discussed; these are divided into intraparenchymal, extra-parenchymal-intraperitoneal and extraparenchymal-extraperitoneal lesions. The results of grey scle sonography are most informative and are correlated with the radiological and scintigraphic findings in 25 patients. In addition to the advantages of economy and of a non-invasive method, ultrasound permits aimed fine-needle aspiration of pathological abdominal processes such as abscesses.

Thoracic computer tomography was performed in 135 patients with bronchial carcinoma. 1) In 90% a precise estimate of the extent of the primary tumour was possible. The computer tomogram was markedly superior to traditional radiological methods in showing invasion of the chest wall and mediastinum. 2) Regional lymph node metastases were demonstrated correctly in 82%. Lymph node enlargement must be differentiated from that due to benign causes. 3) In 92%, the extent of distal metastases could be correctly established. The computer tomogram makes it possible to avoid exploratory thoracotomy if there is transpleural invasion, extensive mediastinal lymph node metastasation or distant metastasation. Where there is doubt, computer tomography-guided needle aspiration should be employed.

Twenty-four patients with dissecting aortic aneurysms have been examined by CT and possible causes of mistaken diagnosis are discussed. Failure to diagnose a dissection by CT may be due to atypical localisation and thrombosis of the aortic dissection; dissections of small volume, particularly in the ascending aorta; failure to demonstrate an intimal flap and inadequate contrast enhancement. A false positive diagnosis may be due to fibrotic, inflammatory or neoplastic periaortic changes, aneurysm of the sinus of Valsalva, or artifacts.
