RESUMEN

BACKGROUND: To report a case of interface fluid syndrome (IFS) following traumatic corneal perforation repair after small incision lenticule extraction (SMILE). CASE PRESENTATION: A 23-year-old woman, with a past history of SMILE, was struck in the left eye with a barbecue prod and subsequently underwent corneal perforation repair at local hospital. Primary wound repaired with a single 10 - 0 nylon suture at the area of leakage. After the surgery, her best corrected visual acuity (BCVA) was 20/30. Four days later, she presented at our hospital with blurred vision, and interface fluid syndrome (IFS) was diagnosed. Intraoperative optical coherence tomography (iOCT) was used to guide the resuturing of the corneal perforation in the left eye, followed by anterior chamber gas injection. At the first postoperative month, the BCVA was 20/25. The corneal cap adhered closely to the stroma, the surface became smooth. CONCLUSIONS: This case illustrates that any corneal perforation following lamellar surgery, including SMILE, may lead to IFS. It is crucial to consider the depth of corneal perforation, and intraoperative optical coherence tomography (iOCT) plays a unique role in the repair procedure.

RESUMEN

PURPOSE: The purpose of this study was to quantify thickness, vessel density (VD) of retina and choroid in young adults (18-24 years old) using OCTA. METHODS: This observational, cross-sectional study included 154 eyes from 77 young myopic adults. En-face angiogram OCTA was performed on a 3.00 × 3.00 mm region centered on the macula. Automated thickness calculations and macular maps were measured. Spherical equivalent refraction (SER) and AL were examined to determine associations with thickness, vessel density (VD) of retina and choroid. RESULTS: A total of 148 healthy eyes from 77 young myopic adults (29 males and 48 females) with a mean age of 21.80 ± 1.32 years (range: 18-24 years) were included. The mean SER and AL were - 4.06 ± 2.26D and 25.25 ± 1.28 mm, respectively. The mean retinal thickness (RT, ILM-RPE layer) was 240.91 ± 13.36 µm, the retinal superficial (SVD) and deep vessel density (DVD) in fovea region were 18.35 ± 4.77% and 32.99 ± 6.01%, respectively. The foveal avascular zone (FAZ) area was 0.31 ± 0.10 mm2. The mean subfoveal choroidal thickness (SFCT) and choriocapillaris (CC) perfusion area were 232.16 ± 56.65 µm and 2.17 ± 0.10 mm2, respectively. By Pearson's correlation analysis, SER was revealed to be negatively correlated with RT (r = -0.180, p = 0.028) and DVD (r = -0.185, p = 0.025) in fovea region. SER was revealed to be positively correlated with RT in nasal (r = 0.224, p = 0.006) and inferior (r = 0.217, p = 0.008) regions. AL was revealed to be positively correlated with RT (r = 0.250, p = 0.002) and DVD (r = 0.284, p < 0.001) in fovea region. SER was revealed to be positively correlated with SFCT (r = 0.486, p < 0.001). AL was revealed to be negatively correlated with FAZ area (r = -0.232, p = 0.005) and SFCT (r = -0.407). RT was revealed to be negatively correlated with FAZ area (r = -0.645, p < 0.001). SER (r = -0.079), AL (r = 0.071) and SFCT (r = 0.089) did not correlate significantly with the CC perfusion area (all p > 0.05). CONCLUSION: Myopic eyes present increased RT, DVD and thinned SFCT in fovea, while no significant correlation could be found between SER, AL, SFCT and CC perfusion area. It may indicate that the SFCT thinning may be secondary to ocular elongation, while the CC perfusion area may be a factor independent of AL growth.

RESUMEN

Depressive disorders are both prevalent and debilitating, and a proportion of patients have treatment resistance to classic antidepressants. Recent evidence has implicated the intracellular WNT signaling pathway as having a key role in the pathogenesis of major depressive disorder. In the present study, we investigated the role of ß-catenin and transcription factor-4 (TCF4) in the depression-like and anxiety-like behaviors exhibited by mice exposed to maternal separation, or chronic mild stress. Both rodent models of childhood and adulthood stress showed depression and anxiety-like behaviors. During the last three weeks of medication, we applied AMBMP (2-Amino-4-[3,4- (methylenedioxy)benzylamino]-6-(3-methoxyphenyl)pyrimidine) to the maternal separation and chronic stress model for the first time. The drug alleviated the depression-like index in saccharin preference test (SPT) and forced swim test (FST), and anxiety-like index in open field test (OFT) and elevated-plus maze (EPM), and reversed the disruption of ß-catenin and TCF4 in stressed mice by upregulating the WNT pathway specifically. Therefore, the WNT pathway may be involved in the mediation of patient recovery and could be a target for novel antidepressants.

RESUMEN

With the wide application of phacoemulsification and intraocular lens implantation, many cataract patients have effectively recovered vision. Postoperative intraocular lens opacification is an important indicator for evaluating biocompatibility, as it affects the visual quality of patients. The manifestation and risk factors of opacification vary among different materials used for intraocular lenses. However, better visual quality after surgery is not determined by a single factor. The material of the intraocular lens and the patient's response to the intraocular lens are all factors that affect the postoperative visual quality. With the continuous advancement of technology, an increasing number of new materials are being applied in the field of intraocular lenses. Fully understanding the characteristics of intraocular lens materials, selecting suitable intraocular lens for patients and reducing complications caused by materials will be beneficial to patients. The characteristics of different intraocular lens materials and the risk factors of opacification after intraocular lens implantation were discussed in this paper.

RESUMEN

In medical research, predictive models have been widely used to predict disease progression and identify high-risk populations in advance, especially in the prevention and diagnosis of chronic diseases. In ophthalmology, the predictive and diagnostic models for fundus diseases such as age-related macular degeneration and diabetic retinopathy have demonstrated expert-level accuracy. However, the application of predictive models is still in the exploratory stage as for myopia prevention and control. The establishment of a predictive model is helpful to identify the high-risk myopic children in advance, so that preventive measures such as adequate outdoor activities and reducing near work can be taken in time, which is of great significance to prevent or slow down the occurrence and development of myopia. Because the mechanism of myopia has not been fully elucidated, there are still challenges and limitations in the selection of application objects, predictors and predictive outcomes. This paper reviews the research progress of different types of myopia predictive models in order to provide reference for further development and improvement.

RESUMEN

The visual signal generated by the eye receiving external light stimulation reaches the visual center through the visual pathway and is processed and integrated by the visual center to form a subjective sensation called cerebral vision.Cerebral vision reflects the function of the whole visual nervous system from the retina to the visual cortex.Abnormal cerebral vision has been found in myopes using electrophysiology, magnetic resonance imaging (MRI) and functional near-infrared spectroscopy (fNIRS). The abnormal electrophysiological results are mainly found in patients with high myopia and pathological myopia.MRI shows abnormal changes in the structure and function of several vision-related brain regions in patients with high myopia.Recently, by applying near-infrared brain function imaging technology combined with self-developed equipment, our team conducted a series of studies on the brain function of myopic patients and found that adult patients with moderate myopia and patients with pathological myopia have the abnormal cerebral vision. The mechanism of neural regulation of eye accommodation in myopic children needs further investigation. In this article, we reviewed the current status and outlook of myopic brain vision research in terms of myopia classification, myopic brain vision research methods, and myopic visual electrophysiology, MRI, and fNIRS studies.

RESUMEN

Purpose: The retina has an important role in the signal transmission related to eye development and myopia. Glutamate (Glu) and γ-aminobutyric acid (GABA) are the major excitatory and inhibitory neurotransmitters in the retina, which can affect the development of both the eye and myopia. Nevertheless, change in the balance between the excitatory and inhibitory neurotransmitters still is unclear during development of eyes and myopia. The purpose of this study was to explore the alterations of the ratio of Glu to GABA (RGG), which mediates the balance between the excitatory and inhibitory neurotransmitters in retina in the development of the eyes and lens-induced myopia (LIM) in a guinea pig model. Method: An LIM guinea pig model was established using a -10 diopter (D) negative lens. The levels of Glu, GABA, and the dynamic change of RGG were measured in the retina in normal and myopia guinea pigs at four time points (i.e., 0, 2, 4, and 6 weeks after onset of LIM). Considering that Glu and GABA are related closely to the occurrence of myopia, we further studied the changes of RGG in the retina in LIM guinea pigs. Result: Our results showed that the RGG was upregulated and was well correlated with diopter and axial length than either Glu or GABA during the development of normal eyes. Besides, we observed that the content of the RGG in the retina in myopia eyes was higher than that of Glu and GABA in normal subjects and was an obviously positive correlation. Conclusions: Taken together, our findings suggest that the RGG has a pivotal role in eye development and myopia. The abnormal retina signal induced by the unbalanced ratio between Glu and GABA is related closely to the occurrence of myopia.

RESUMEN

Objective To investigate the changes of corneal biomechanics of intrastromal correction (INTRACOR) femtosecond technique to treat presbyopia. Methods A prospective, clinical self-control clinical trial was designed.Twenty-four presbyopic patients with emmetropia or mild hyperopia were enrolled in this study. The INTRACOR procedure was performed using the Technolas femtosecond laser in the nondominant eye.Uncorrected distance visual acuity (UCDVA),uncorrected near visual acuity (UCNVA) and spherical equivalent (SE) were recorded in preoperation and postoperative 12 months, and the quality of life of postoperative patients was evaluated. The corneal deformation parameters including highest concavity deformation amplitude (HC-DA),highest concavity peak distance (HC-PD),highest concavity radius (HC-R),non-contact intraocular pressure and the central corneal thickness (CCT) were measured using the Corvis ST visualization biomechanical analyzer in preoperation and postoperative 1 month,3,6 and 12 months, respectively. This study followed the Declaration of Helsinki.Written informed consent was obtained from each subject prior to entering study cohort. This study protocol was approved by Ethic Committee of Affiliated Eye Hospital of Shandong University of Traditional Chinese Medicine (No.SDZYYDXYKYYLL2011.06).Results Compared with preoperation,UCNVA of all 24 (100%) eyes was improved at 12 months postoperatively with minimal or no change in UCDVA.The mean spherical equivalent was (+0.35± 0.29)D and (-0.37 ± 0.29)D before and 12 months after operation, with a significant difference beteeen them (t=-7.39,P<0.01).No significant differences were seen in the intraocular pressure,CCT and HC-PD values between preoperation and postoperative 1 month,3,6,12 months (F =1.273,1.347,2.434;all at P > 0.05).Compared with preoperation, the postoperative 1 month,3,6,12 months HC-R values were significantly declined,HC-DA values were significantly increased, with significant differences between them (all at P<0.05),and no significant differences were found in HC-R and HC-DA between the postoperative adjacent time points (all at P>0.05).Corneal aspherical index (Q Value) was-0.28±t0.10 at 12 months postoperatively, which was increased compared with the preoperation, and the maximum value added value (diff-K) of the central corneal curvature was (2.55±0.81)D.Conclusions INTRACOR treatment of presbyopia can effectively improve near vision, increase postoperative corneal biomechanical maximum HC-R and HC-DA,negatively increase the corneal central curvature increased aspheric index, which suggests that corneal biomechanics of central cornea is weakened after intrastromal femtosecond presbyopic treatment, and the hyperprolate mutifocal corneal shape is formed under normal intraocular pressure.

RESUMEN

Background Previous studies showed that the pathogenesis of uveitis is related to γδ T cells. However,it remains unclear that how these cells are involved in experimental autoimmune uveitis (EAU).Objective This study aimed to observe the dynamic changes of γδ T cells in EAU and explore the role of γδ T cells in the pathological process of EAU.Methods Forty-five C57BL/6(B6) mice were assigned to the normal control group (six mice) and EAU model group (thirty-nine mice). The mice were immunized subcutaneously at 6 spots on the footpads, tail base, and flank with emulsion containing human interphotoreceptor retinoid binding protein1-20 (IRBP1-20) emulsified in complete Freund's adjuvant. After immunization, the mice were examined for clinical signs of EAU by using a Genesis-D camera. The changes of histopathology were compared by hematoxylin and eosin staining.Mouse lymphocytes were isolated and purified from the spleens of IRBP1-20-immunized or normal B6 mice by using a γδ T-cell isolation kit.Flow cytometry was used to detect the changes of intracellular expression of interleukin-17A (IL-17A),and then transferred the activated γδ T cells into EAU models to analyze the changes of clinical signs and histopathology of EAU.Experimental study program as well as the use and feeding of the animals were authorized by the Animal Management and Use Committee of Shandong Traditional Chinese Medicine University. Results The inflammatory symptoms in mouse eyes appeared on day 12 after modeling. The initial changes were fundal blood vessel thickening and minimal inflammatory cell infiltration. Then, multifocal chorioretinal lesions, serious vasculitis and linear lesions were observed on days 16-20,along with abundant lymphocyte infiltration in the vitreous and retinal disorganization. The inflammation symptom scores and the pathological inflammation scores at different time points after modeling had statistically significant differences (F =51.399,P =0.000;F =47.342,P =0.000).The inflammatory symptoms in the eyes began to abate from day 28 onwards. The number of γδ T cells was obviously increased during the inflammation phase of EAU at day 16-20 after modeling, with the number of γδ T cells was (5.67 ±-0.49) % and (5.78 ±±0.55) %,respectively, which was significantly higher than (1.53 ± 0.14) % before modeling, with significant differences between them (both at P<0.05),meanwhile CD69 levels and the integrin lymphocyte function-associated antigen-1 (LFA-1) and secreted IL-17A were elavated. The secretion level of IL-17A was (13.40±0.50)% and (17.80±2.37)% on day 16 and day 20 after modeling, respectively, which was significantly higher than (1.53 ± 0.19) % before modeling, with significant differences between them (P =0.000,0.001).The activated γδ T cells were transferred into EAU model, the inflammation symptom scores were 1.00 (1.00,2.00) after activated γδ T cells were transferred into EAU model, which was significantly higher than 0.75 (0.05,1.00) of the untransferred group (Z =27.00,P =0.03),and the symptoms of EAU were aggravated. Conclusions The proportion of γδ T cells reaches peak in inflammation of EAU,and the cells are activated. The activated γδ T cells in the EAU model play a immune regulation role by secreting IL-17A.

RESUMEN

Objective To investigate the regulatory role of rno-miR-30b-5p in the expressions of interleukin-10 (IL-10) and toll-like receptor 4 (TLR4) in uveitis. Methods Both IL-10 and TLR4 gene 3'UTR lucfferase vectors and relevant binding site mutant vectors were constructed.Further, both rno-miR-30b-5p mimics and reporter gene vector were co-transferred into 293 T cells to validate the fluorescent alterations of the reporter gene expression to detect the interactions between rno-miR-30b-5p and the related target genes. Moreover,an experimental autoimmune uveitis (EAU) model was induced with IRBP peptide emulsion in rats, and both lymph node and spleen were isolated on day 12 after EAU induction. In order to measure rno-miR-30b-5p levels and IL-1 0,TLR4 expressions in spleen and lymph node, quantitative PCR and ELISA techniques were applied. Results The results of double lucfferase reporter gene expression analysis showed rno-miR-30b-5p mimic apparently down-regulated the fluorescence intensity of both IL-10 and TLR4 in wild type cells. After the mutation of the target site, the fluorescence intensity of the mutant vector was significantly reduced, accompanied by a significantly statistical difference (all P < 0.01).Moreover,animal results revealed the expressions of rno-miR-30b-5p were apparently decreased, whereas IL-10 and TLR4 were markedly increased in both lymph node and spleen (all P < 0.05).Conclusion Target identification shows that rno-miR-30b-5p can obviously regulate the expressions of 3'UTR gene with either IL-10 or TLR4 gene fragment, though its regulation might not be through the predicted site. The down-regulated expression of rno-miR-30b-5p in both spleen and lymph node in EAU rats result in the up-regulated expressions of both IL-10 and TLR4,further influence the development of uveitis. This study paves a way for the modulation of microRNA on the occurrence and development of uveitis, and will provide a new insight on treating uveitis.

Background Studies show that regulatory T cells (Treg) are a kind of T cell subsets to negatively regulate immune response, and play an important role in maintaining immune homeostasis and immune tolerance.Autoimmune uveitis is an autoimmune disease, the regulation of Treg cells in pathogenesis and progression of autoimmune uveitis is not fully unelucidated. Objective This study was to observe the dynamic changes of Treg in experimental autoimmune uveitis (EAU) rats and explore the role of Treg cells in the pathological process of EAUrats. Methods Eighty four 6-8 week-old SPF Lewis rats were randomly divided into model group and control group. The mixed emulsifier of interphotoreceptor retinoid-binding protein (IRBP)1177-1191,tuberculin (TB),complete Freund adjuvant (CFA) and PBS (300 μl) was subcutaneously injected in double rear foot pad, abdominal side and back, and only equal amount of TB,CFA and PBS emulsifier was used in the same way in the control group.Ocular inflammation symptoms was examined at 9,13,18,23,28,35 and 48 days after modeling and scored based on the severity of the inflammatory.Six rats of each group were sacrificed in above time points respectively for the histopathological examination of iris, ciliary body and retinas by haematoxylin-eosin staining. The lymphocytes were isolated and cultured from rat spleens, and the proportion of Foxp3-labelled cells,a specific marker of Treg cells, was assayed by flow cytometry. The relative expression level of Foxp3 mRNA in the lymphocytes detected by using realtime quantitative PCR (RT-PCR).The use and care of the rats complied with the ARVO Statement. Results Eye inflammatory response appeared at 8 days after immunization, showing vasodilation and hyperemia of rat iris in the model group, and the response peaked at 13 days, with exudation and hypopyon in the anterior chamber. The highest inflammatory scores were 3.75±0.42 at day 13,and the ocular inflammation reaction was gradually relieved after that and disappeared at 23 days after immunity.A significant difference in ocular inflammatory scores of model rats was found among different time points (F =81.709,P < 0.001);while no inflammatory symptom was observed in the control group.Histopathology examination showed obvious infiltration of inflammatory cells in the iris, ciliary body and retinas in model rats, including neutrophils, lymphocytes and mononuclear cells. The proportion of Foxp3-labelled cells in spleen lymphocytes was (5.50 ± 0.64)%,(13.36 ± 0.98)%,(10.34 ± 0.79)%,(9.58 ± 1.02)%,(6.73 ±0.81)% and (5.58 ± 0.47) % in the model group on day 13,18,23,28,35,48 respectively, with statistically significant differences in comparison with (2.80 ± 0.38) %,(3.36 ± 0.53) %,(3.65 ± 0.57) %,(3.37 ± 0.43) %,(3.33±0.50)% and (3.13±0.61)% in the control group (t=-6.272,-15.556,-11.910,-9.753,-6.154,-5.491,all at P<0.01).The change trend of Foxp3 mRNA expression was consistent to the dynamic change of the proportion of Foxp3-labelled cells. Conclusions The pathogenesis and development is closely associated with the dynamic changes of CD4+ CD25 + Foxp3+ Treg cells in EAU rats.

RESUMEN

Background It has not been reported that if the visual cortex M receptor changed during the development of myopia and how it changed if given acupuncture treatment. Objective The aim of this study was to observe the effect of electroacupuncture stimulation on the expression of acetylcholine receptors M1 (AchRM1) in visual cortex of guinea with lens-induced myopia (LIM).Methods Forty-eight three-week-old healthy guinea pigs were randomized into the normal control group, the LIM model group and the LIM electroacupuncture group. The right eyes of the guinea pigs were selected as the experimental eyes.LIM was created by monocularly wearing of-10 D lens for 4 weeks in the right eyes in the LIM model group and LIM electroacupuncture group, and then the acupuncture at the temple and hegu point was performed for 30 minutes per day for consequent 4 weeks,in the LIM electroacupuncture group. The fellow eyes of the guinea pigs were used as the self-control eyes. The refractive power and axial length were examined with retinoscopy and A-type sonography before and 4 weeks after modeling, respectively. The animals were sacrificed by excessive anesthesia at the fourth week after acupuncture and visual vertex tissue was obtained. The expression of M1 receptor mRNA in visual vertex was detected by fluorescence quantitative PCR,and the content of M1 receptor protein in visual vertex was assyed by ELISA.The study protocal was approved by Animal Ethic Committee of Shandong University of Traditional Chinese Medicine, and the use and care complied with Statement of the Association for Research in Vision and Ophthalmology. Results At the fourth week after modeling, the mean diopters were (-3.24±0.28) D and (-3.30±0.45) D in the LIM model group and the LIM eleetroacupuncture group, which were significantly higher than (0.83 ±0.86)D in the normal control group (both at P=0.000),and there was no significant difference in the diopter between the LIM model group and the LIM electroacupuncture group (t =0.200,P =0.659).The mean axial lengths were (8.67 ±0.14) mm and (8.60±0.06) mm in the LIM model group and the LIM electroacupuncture group, which were considerably increased in comparison with (8.33±0.08)mm in the normal control group (both at P<0.05).The relative expression levels of AchRM1 mRNA in visual cortex were 0.79±0.18,1.36±0.23 and 1.13±0.13 in the normal control group,LIM model group and LIM electroacupuncture group, and the relative expression level of AchRM1 mRNA in the LIM electroacupuncture group was significantly higher than that of the normal control group and lower than that of the LIM model group (both at P<0.05).In addition, the contents of AchRM1 receptor protein in the visual cortex were 248.00±33.31,455.17±42.40 and 396.17±47.57 in the normal control group,LIM model group and the LIM electroacupuncture group, with a similar pattern among the groups (both at P<0.05).Conclusions A electroacupuncture stimmulation do not affect the myopic diopter and axial length in LIM model. The AchRM1 and AchRM1 receptor in the visual cortex up-regulate in LIM eyes, infering that electroacupuncture stimmulation can improve vision by decreasing the level of AchRM1 receptor in visual cortex in LIM eyes in guinea pigs.
