From 1972 to 1991, 50 Chiari procedures were performed in 41 patients. The follow-up period ranged from 3 years to 24 years and 3 months, with an average of 7 years. Age of the patients at operation averaged 10 years and 6 months (3 years and 1 month to 28 years and 6 months). The best results were obtained in patients older than 10 years of age. There was no statistical relationship between the technical details of Chiari osteotomy (angle of Chiari, displacement and distance to the femoral head) and the results.

The authors report avascular necrosis of the talus, after a Dennyson-Fulford subtalar arthrodesis. Vascular and neuropathic causes are discussed.

This study entails the follow-up of 28 patients (33 hips) with congenital hip dislocation. The primary treatment was one-stage open reduction and Salter type osteotomy. The mean follow-up period was 5 years and 6 months (range = 3 years to 7 years 4 months). At last follow-up, 9 hips (27%) were classified as excellent, 22 hips (67%) as good, and 2 hips (6%) as fair. Avascular necrosis developed in two hips (6%) postoperatively. The difference between the acetabular index at immediate postoperative time and at last follow-up was statistically significant (p < 0.01). The Salter osteotomy by itself may have a biologic stimulating effect on the acetabulum, which accentuates reversal of the dysplastic changes.

We report a case of Dyggve-Melchior-Clausen syndrome, a skeletal dysplasia with short trunk dwarfism and mental retardation. A Chiari pelvic osteotomy was performed to halt hip subluxation on both sides. The patient was evaluated after 9 years and 10 months. The progressive lateral migration of the femoral head seemed to have been unaffected by this type of osteotomy.

A fracture of the femur in a 7-year-old boy who was treated with retrograde Küntscher nailing is described. The follow-up period was 8 1/2 years. Deep infection, physeal injury of the femoral head, and trochanteric epiphysiodesis were serious consequences of the surgery.

We report a case of osteopetrosis with bilateral nonunited femoral neck and coxa vara in a 7-year-old boy. There was a history of traumatic transcervical left femoral neck fracture unsuccessfully treated by internal fixation. We performed a bilateral subtrochanteric valgus osteotomy. K-Wire fixation failed on the right side due to hardness of the bone. Finally, internal fixation with an angular plate after predrilling of the femoral neck offered stability. Bone union was achieved on both sides, resulting in full recovery of normal physical activity.
